Impact of intermittent exotropia and surgical realignment on myopia progression in myopic children.

PURPOSE: To determine whether untreated intermittent exotropia (IXT) accelerates pediatric myopia and whether strabismus surgery modifies this risk. METHODS: This retrospective study enrolled 259 children aged 6-14 years with spherical equivalent refraction (SER) ≤ -0.75 D; 90 orthotropic controls, 86 untreated IXT, and 83 post-surgery IXT (PS-IXT) with successful alignment. Exclusion criteria included prior ocular surgery, trauma, or myopia-control interventions. After standardized cycloplegia, certified optometrists measured SER and axial length (AL) at baseline, 6 and 12 months. RESULTS:

Double-seronegative neuromyelitis optica: it is possible to interrupt treatment after 10 years of stability.

Double-seronegative neuromyelitis optica spectrum disorder (DS-NMOSD) encompasses a heterogeneous spectrum, including monophasic and relapsing phenotypes. While 1/3 patients may follow a monophasic course, lifelong immunotherapy remains common practice due to the fear of relapse. However, this strategy may unnecessarily expose stable patients to long-term adverse effects and economic burden. The condition lacks the relapse-associated mechanisms observed in aquaporin 4 (AQP4)-positive disease, questioning the generalizability of prior treatment-withdrawal studies. Although predictive markers fo

The history and evolution of pediatric ophthalmology training: celebrating 50 years of the American Association for Pediatric Ophthalmology and Strabismus.

In recognition of the 50th anniversary of the American Association for Pediatric Ophthalmology and Strabismus (AAPOS), we present the history and evolution of pediatric ophthalmology and strabismus training in the United States and Canada. Using historical records, AAPOS charter member interviews, and a program director survey, we present the timeline of key events and our training history. Pioneers in Canada and the United States established the first pediatric ophthalmology preceptorships. These preceptorships and the programs founded by their trainees evolved into modern fellowship programs

Recession-resection combined with vertical offset of horizontal rectus muscles for management of combined head turn and head tilt in idiopathic infantile nystagmus syndrome.

A male child presented with complex abnormal head posture (AHP), characterised by a 35° left head turn and a 45° left head tilt. Orthoptic evaluation revealed orthotropia with full ocular motility and a high-frequency, moderate-amplitude horizontal pendular nystagmus, which dampened in the preferred AHP. The remainder of ocular examination was unremarkable.Surgical correction was performed using Park's modification of the horizontal Kestenbaum procedure to shift both eyes to the left. To address the head tilt, extorsion of the left eye and intorsion of the right eye were achieved by vertically

The coordinate systems in the Hess screen test and the Harms tangent screen test for ocular motility analysis of strabismus: their metrics and relationship.

In the diagnosis of strabismus due to paralysis of the eye muscles, the Hess screen test and the Harms tangent screen test are used to measure the misalignment of the eyes in different gaze directions (motility analysis). Both examination methods use different procedures to define the gaze direction and coordinate systems. Coordinate systems with scales in angular degrees are used to determine the direction of gaze and simplify reading the squint angle in the event of a misalignment (horizontal and vertical deviation). In the measurement results, however, their numerical values differ due to t

Horizontal nystagmus identification with joint SAM segmentation and time series classification.

PURPOSE: Nystagmus is an involuntary jerky eye movement. It is an oculomotor sign of asymmetry in the vestibular pathway. With the rapid advancement of deep learning and its widespread applications, numerous deep learning-based methods for nystagmus detection have been proposed based on eye movement videos. This would enhance diagnostic efficiency. We propose a horizontal nystagmus detection model with joint SAM segmentation and time series classification. METHODS: In this study, a convolutional neural network is employed to eliminate the invalid interference frames in the video. Subsequently

Real-world evaluation of Armstrong's criteria in corticobasal degeneration: Phenotypic overlap and diagnostic challenges.

BACKGROUND: Corticobasal degeneration (CBD) is a four-repeat tauopathy with heterogeneous clinical manifestations. Armstrong's criteria involve a two-step diagnostic approach: first, classifying patients into five clinical phenotypes-probable/possible corticobasal syndrome (CBS), frontal behavioral-spatial syndrome (FBS), non-fluent/agrammatic variant primary progressive aphasia (naPPA), and progressive supranuclear palsy syndrome (PSPS); second, determining whether they meet the clinical research criteria for probable CBD (cr-CBD) or the clinical criteria for possible CBD (p-CBD), which are d

Progressive Supranuclear Palsy PERK Haplotype B Selectively Translates DLX1 Promoting Tau Toxicity.

The unfolded protein response (UPR) sensor PERK exists in haplotypes A and B. PERK-B confers increased risk for tauopathies like progressive supranuclear palsy (PSP), but the mechanisms distinguishing its function from PERK-A and contributing to its association with tauopathy remain unknown. Here, we developed a controlled cellular model for a pair-wise comparison of the two PERK haplotypes, finding their UPR functions nearly indistinguishable. Puromycin-based proteomics highlighted a subset of mRNA translation events that was permissible under the PERK-B-dependent, but not the PERK-A-dependen
