Microvascular decompression (MVD) surgery is traditionally performed via a retrosigmoid craniotomy. Recently, neurosurgeons have increasingly adopted smaller approaches; however, the available evidence has not yet been systematically compiled. This is the first systematic review and meta-analysis on the efficacy and safety of ≤ 2-cm-keyhole retrosigmoid MVD. PubMed and Embase databases were searched for studies involving adults with trigeminal neuralgia (TN), hemifacial spasm (HFS), or glossopharyngeal neuralgia (GPN), who underwent first-time retrosigmoid keyhole MVD (diameter ≤ 2 cm). BACKGROUND: Although transcervical styloidectomy is an established treatment for Eagle syndrome, the narrow operative corridor can make deep exposure challenging, particularly in patients at higher risk of complications. We report two cases in which neurosurgical microsurgical techniques provided wider and clearer deep visualization than conventional methods.
METHODS: Two patients underwent transcervical styloidectomy using microsurgical techniques commonly employed in neurosurgery: one with Eagle syndrome-related internal carotid artery dissection on antiplatelet therapy, and the other with

When a neurovascular conflict is demonstrated on high-resolution MRI in patients with drug-refractory Vago-Glossopharyngeal neuralgia microvascular decompression is regarded as the most effective therapeutic option. The aim of surgery is to relieve the conflict, most often produced by the posterior inferior cerebellar artery, not infrequently in association with a compressive megadolicho-vertebrobasilar artery. This can be safely achieved by exposure of the Vago-Glossopharyngeal nerve complex through a retromastoid-retrosigmoid infrafloccular approach through the cisterna magna, allowing expos

INTRODUCTION: Glossopharyngeal neuralgia (GPN) is a rare neuropathy characterized by paroxysmal, unilateral, and severe pain episodes affecting the throat, tonsils, base of the tongue, and pharynx. Its management remains challenging due to the absence of standardized treatment protocols and the limited availability of high-quality comparative studies.
OBJECTIVE: To review and compare the effectiveness and safety of available treatments for GPN.
METHODS: A systematic review was conducted, analyzing studies published in the last decade that evaluated GPN treatments. Searches were performed in

PURPOSE: Building on the pioneering observations of Dandy and Gardner and on Jannetta's establishment of microvascular decompression (MVD) through microsurgical demonstration of vascular compression, MVD has continuously evolved into a safe and durable treatment for neurovascular compression syndromes. Despite advances in radiosurgery and pharmacotherapy, MVD remains widely used as a first-line surgical option for appropriately selected patients with trigeminal neuralgia, hemifacial spasm, and glossopharyngeal neuralgia.
METHODS: This selective narrative historical review traces MVD's modern

With this review, we aim to provide information about microvascular decompression in various forms of neurovascular conflicts between blood vessels and cranial nerves 7, 9, and 10 in the posterior fossa. Specifically, we will address the treatment of hemifacial spasms, glossopharyngeal neuralgia, and, in rare cases, vagus nerve involvement. We intend to disseminate this information widely to doctors in Denmark, as it is our impression that the diseases mentioned in the article are not well known to all doctors who encounter these patients, and the same applies to the treatment options.

BACKGROUND: Trigeminal neuralgia (TN) typically presents with paroxysmal, shock-like, or stabbing pain in the unilateral facial region corresponding to the trigeminal nerve distribution. However, cases manifesting solely as isolated tongue pain are exceedingly rare.
CASE PRESENTATION: A 63-year-old female presented with a two-year history of left-sided tongue pain. She was initially suspected of having glossopharyngeal neuralgia (GPN) and treated with oxcarbazepine at the local hospital, which provided suboptimal relief. Over the past two months, her pain intensified significantly. Upon refer

INTRODUCTION: Lower cranial nerve (LCN) palsy may develop following tumor resection in the cerebellopontine angle or jugular foramen, often resulting in dysphagia and dysphonia. Although many patients recover with rehabilitation, some exhibit persistent functional deficits. In such cases, detailed pathophysiologic evaluation may assist in guiding surgical intervention to improve outcomes.
CASE PRESENTATION: A 77-year-old woman presented with severe dysphagia and hoarseness after resection of a right cerebellopontine angle meningioma, which caused glossopharyngeal, vagus, and accessory nerve p

BACKGROUND: Cala Trio is a wrist-worn transcutaneous afferent patterned stimulation (TAPS) device for essential tremor (ET), delivering high-frequency stimulation to the median and radial nerves.
CASE REPORT: We report the first case of glossopharyngeal neuralgia (GPN) associated with Cala Trio initiation in a 69-year-old male with ET and Parkinson's disease, with prior GPN. Throat spasms triggered by swallowing and coughing began after initiation on two trials and resolved with discontinuation; evaluation was unrevealing.
DISCUSSION: We review TAPS mechanisms, GPN neuroanatomy, and circuit-

To explore the clinical characteristics and long-term prognosis of nervus intermedius sectioning via a retrosigmoid approach for the treatment of nervus intermedius neuralgia (NIN). A total of 10 NIN patients treated at the First Affiliated Hospital of Fujian Medical University between May 2016 and October 2024 were included retrospectively. All patients underwent nervus intermedius sectioning through the retrosigmoid approach, with combined microvascular decompression (MVD) performed for those with concomitant trigeminal neuralgia (TN) or glossopharyngeal neuralgia (GN) based on intraoperativ

Chronic pain is inherently multifactorial, with biological, psychological, and social factors contributing to neuropathic pain (NP) and central sensitization (CS) syndromes. Comorbidity between functional disorders and the lack of clinical biomarkers adds to the challenge of diagnosis and treatment, leading to frustration for healthcare professionals and patients. Available treatments are limited, increasing patient suffering with personal and financial costs. This systematic review examined multisensory processing alterations in chronic pain and reviewed current pharmacological and non-pharma

BACKGROUND: Cranial nerve neuropathies represent a rare manifestation of cat scratch disease (CSD). Only a few case reports have been published, and the full clinical spectrum remains poorly characterized. We aimed to describe the clinical presentation, diagnostic approach, and prognosis of cat scratch disease (CSD)-associated cranial nerve neuropathies, a manifestation that is poorly characterized. METHODS: Using data from a national CSD surveillance study, we identified patients with CSD-associated cranial neuropathies confirmed by serology and/or PCR for Bartonella henselae. Clinical, epide

Cranial nerve (CN) functional diseases, comprising trigeminal neuralgia (TN), hemifacial spasm (HFS), and glossopharyngeal neuralgia (GN), are frequently caused by neurovascular conflicts (NVCs) in the posterior fossa. This chapter examines surgical and non-surgical procedures for these conditions, emphasizing minimally invasive retrosigmoid approach (RA) for microvascular decompression (MVD) as the gold standard for definitive treatment for all of them. MVD achieves long-term pain relief in more than 80% of classical TN cases, though efficacy decreases significantly in multiple sclerosis (MS)

BACKGROUND: Renal cell carcinoma is a rare pediatric solid tumor that typically presents with hematuria, abdominal mass, or flank pain. It is uncommon for renal cell carcinoma to manifest with headache and isolated extra-urogenital symptoms. We present, to our knowledge, the first case of renal cell carcinoma with bony metastases, presenting initially as isolated cranial nerve twelve palsy. Although bony metastases can occur in renal cell carcinoma, skull-based metastases and cranial neuropathies are exceedingly rare, especially in the pediatric population.
CASE PRESENTATION: We describe the

BACKGROUND: Neurovascular compression syndromes (NVCSs) are a series of vascular compression diseases wherein there is usually entrapment or distortion of cranial nerves due to redundant or aberrant culprit vessels. Microvascular decompression (MVD) is an effective treatment for NVCS. However, the global adoption of fully endoscopic MVD (E-MVD) remains limited.
METHODS: This retrospective study reviewed 105 patients with NVCS (102 cases for initial operation and 3 cases of disease recurrence) who underwent E-MVD. The cohort comprised of 59 hemifacial spasm (HFS), 34 trigeminal neuralgia (TN),
