Mindfulness-based self-compassion and vagal nerve stimulation impact on heart rate variability, psychological resilience, and sleep quality in earthquake survivors.

Earthquakes pose significant risks to both psychological and physiological health by disrupting autonomic regulation and increasing stress-related disorders. This study aims to evaluate the combined effects of Mindful Self-Compassion (MSC) and vagus nerve stimulation (VNS) on sleep quality, psychological resilience, and heart rate variability (HRV) in survivors of the February 2023 Turkey-Syria earthquakes. A randomized experimental study was conducted with 26 earthquake survivors, assigned to either a combined MSC+VNS group or a VNS-only group. The intervention lasted three weeks. Pre- and po

Transcutaneous auricular vagus nerve stimulation may improve cognitive deficits in neuropsychiatric diseases-a systematic review.

BACKGROUND: Transcutaneous auricular vagus nerve stimulation (taVNS) is a safe, effective, and non-invasive therapeutic approach for various neuropsychiatric disorders, including depression, headache disorders, and epilepsy. Cognitive impairment is a common and clinically relevant feature across these conditions, often contributing to poor functional outcomes. While improvements in cognitive performance have been reported in healthy individuals undergoing taVNS, it remains unclear whether taVNS can also alleviate cognitive deficits in individuals with neuropsychiatric disorders. METHODS: A co

Transcutaneous vagus nerve stimulation in breast cancer: a neuroimmune model to improve quality of life.

Breast cancer care has shifted beyond remission toward optimizing long-term physiological, emotional, and functional recovery. Many survivors continue, however, to experience persistent symptom clusters, such as insomnia, fatigue, anxiety, pain, depression, and cognitive impairment. These poor quality of life outcomes reflect underlying dysregulation of autonomic, neuroendocrine, and immune systems. Autonomic imbalance characterized by vagal withdrawal and sympathetic hyperactivation is linked to increased inflammatory load, impaired stress regulation, disrupted sleep, and poorer survival outc

Deceleration capacity as a predictor of vasovagal syncope subtypes.

BACKGROUND: Autonomic function may play a crucial role in the pathogenesis of vasovagal syncope (VVS). However, the characteristics of autonomic function across different VVS subtypes remain unclear. Deceleration capacity (DC), a novel vagal indicator, may offer improved discriminatory value. OBJECTIVE: To evaluate the ability of DC to distinguish between different subtypes of VVS. METHODS: Patients with VVS confirmed by a positive head-up tilt test were included. All heart rate variability (HRV) and DC data were obtained from 24-h Holter monitoring and analyzed among different subtypes usin

EIF2AK2-related hypomyelinating leukoencephalopathy presenting with congenital bilateral vocal cord paralysis and a spinal cord lesion.

Heterozygous EIF2AK2 variants reportedly cause leukoencephalopathy, developmental delay, and episodic neurologic regression syndrome. We present, to our knowledge, the first reported patient with EIF2AK2-related hypomyelinating leukoencephalopathy involving congenital bilateral vocal cord paralysis. She had respiratory distress soon after birth and underwent tracheostomy. Neonatal brain magnetic resonance imaging showed normal myelination, but severe delayed myelination, accompanied by the rare finding of dorsal medulla and cervical spinal cord signal changes, was apparent at 8 months after tr

Colitis-associated gut-vagus-brain signaling integrates tumorigenesis and neuroinflammation: comparative regulation by vagotomy and atropine.

BACKGROUND: Colitis-associated cancer (CAC) is frequently accompanied by neuropsychiatric comorbidities, yet the underlying gut-brain signaling pathways remain elusive. Emerging evidence highlights the gut-brain axis, particularly the vagus nerve, as a pivotal mediator linking gastrointestinal pathology with neuropsychiatric dysfunction. This study aimed to dissect the dual roles of vagal signaling in a murine CAC model and to evaluate the therapeutic potential of the muscarinic antagonist atropine on CAC. METHODS: The azoxymethane/dextran sulfate sodium (AOM/DSS) was adopted to induce CAC mic

Standardizing Management of Pediatric Vocal Fold Immobility After Cardiothoracic Surgery.

IMPORTANCE: Unilateral vocal fold immobility (UVFI) is a common complication of cardiothoracic surgery in pediatric populations, yet no standardized treatment guidelines exist. The current literature supporting pediatric injection laryngoplasty (IL) has been limited by heterogeneity in patient populations, treatment approaches, and outcomes analysis. OBJECTIVE: To implement a standardized, multidisciplinary protocol for managing pediatric UVFI after cardiothoracic surgery and describe recovery patterns and outcomes observed among patients treated with IL vs conservative approaches. The impact of inflammation, neuromodulation, and gut microbiota on developing cardiac fibrosis and hypertension.

Cardiovascular diseases (CVD) are the leading cause of premature mortality worldwide. Due to pressure overload and cardiac fibrosis, CVD often begin with hypertension and gradually progress to heart failure. Cardiac fibrosis reduces the number of functional cardiomyocytes and the force of contraction while increasing oxygen demand. It has been noted that myofibroblasts, which produce excessive amounts of extracellular matrix in the failing heart, express specific proteins such as periostin, tenascin C, thrombospondin, and osteopontin. Their activation involves immune cells that have a well-doc

Vagus nerve stimulation enhances memory and long-term potentiation via the hippocampal NE/β-AR signaling pathway in pilocarpine rats.

BACKGROUND: Memory deficits in temporal lobe epilepsy severely impair quality of life, yet effective therapeutic strategies remain limited. Vagus nerve stimulation (VNS), an FDA approved treatment for drug resistant epilepsy, has demonstrated beneficial effects on cognition, but the underlying mechanisms are unclear. VNS activates the locus coeruleus norepinephrine (NE) system, and hippocampal β-adrenergic receptors (β-AR) are critical for memory formation and synaptic plasticity. In this study, we used a pilocarpine (PILO) induced memory impaired rodent model to investigate whether VNS improv

Why The Polyvagal Theory Is Untenable: An international expert evaluation of the polyvagal theory and commentary upon Porges, S.W. (2025). Polyvagal theory: current status, clinical applications, and future directions. Clin. Neuropsychiatry, 22(3), 169-184.

Thirty-nine highly acknowledged experts in the areas of the physiology and the evolution of the vagus nerve and of vertebrate social behavior (many whose works have been cited in the polyvagal theory [PVT] literature as supporting the theory) were invited by the first author to participate as co-authors of this article. They were asked to evaluate the PVT and comment upon an overview of the theory written by its author (Porges, 2025a). All those invited, save one, accepted and co-authored the paper. The dissenting scholar was "unfamiliar with the PVT." This article specifically appraises--base

Protocol of the randomized double blind sham controlled AddVNS study of transcutaneous vagus nerve stimulation mechanisms in depression.

Depression is among the most prevalent mental disorders worldwide, carrying one of the highest burden of disease among all mental disorders. While invasive vagus nerve stimulation has been approved for treatment-resistant depression for decades, its clinical use is limited by surgical risks and heterogeneous clinical efficacy. Transcutaneous auricular VNS (tVNS) may offer a non-invasive alternative, but to date it remains experimental due to limited high-quality evidence, unclear biological mechanisms of action, and rudimentary knowledge on optimal stimulation parameters. To address these gaps

Anesthetic Management of Patients for Vagal Nerve Stimulator Placement: A Narrative Review.

Vagal nerve stimulation (VNS) is an established neuromodulation therapy used in the treatment of drug-resistant epilepsy and treatment-resistant depression. This therapy is expanding to avenues in autonomic disorders, heart failure, and chronic migraine. Utilization of this therapy is continuing to grow, and anesthesiologists will increasingly encounter patients undergoing primary implantation or revision of VNS devices. Understanding of device mechanisms, implantation techniques, and physiologic responses to vagal stimulation is imperative to safer perioperative management. This narrative rev

Computational modeling of human vagus nerve stimulation with three-dimensional fascicular morphology.

Implanted vagus nerve stimulation is FDA-approved to treat epilepsy, depression, and stroke sequelae and is under development for other disorders such as heart failure and rheumatoid arthritis. Anatomically realistic computational models enable the design of electrodes and stimulation parameters that activate nerve fibers that mediate therapeutic responses, and avoid activating fibers that cause side effects. Conventional modeling techniques assume constant longitudinal morphology, extruding a single cross section to define the three-dimensional nerve geometry. However, recent imaging data sho

Transcutaneous vagus nerve stimulation reduces total striatal GABA+ content, increases DLPFC Glu content, and facilitates early-phase motor learning.

OBJECTIVES: Transcutaneous vagus nerve stimulation (tVNS) has emerged as a promising non-invasive technique for modulating neuroplasticity. Previous studies have suggested that changes in regional brain GABA+ signaling contribute to these effects, but empirical neurophysiological evidence remains limited. MATERIALS AND METHODS: We investigated the neurophysiological and behavioral effects of tVNS (200-μs pulses at 20 Hz, alternating 30 s ON-1 s OFF cycles, 30 min total duration) in healthy adults using two experimental paradigms. In Experiment 1, GABA+ and Glutamate (Glu) levels were measured
