Hypoglossal Nerve Stimulation and Hypoxic Burden in Patients With Obstructive Sleep Apnea: A Secondary Analysis of the STAR Trial.

IMPORTANCE: Hypoglossal nerve stimulation has been shown to reduce Apnea-Hypopnea Index (AHI) scores in patients with obstructive sleep apnea (OSA); however, its effect on hypoxic burden (HB), a physiologic marker closely associated with OSA-related risks, remains unclear. OBJECTIVE: To investigate whether hypoglossal nerve stimulation therapy is associated with reduced HB in patients with OSA, and whether any HB reduction improves daytime sleepiness. DESIGN, SETTING, AND PARTICIPANTS: This was a secondary analysis of the Stimulation Therapy for Apnea Reduction (STAR) trial, a multicenter, n

Quantitative Analysis of Spontaneous Tongue Muscle Reinnervation after Long Nerve Resection for Hypoglossal Nerve Injury in Rats.

OBJECTIVES: This study aimed to investigate spontaneous tongue muscle reinnervation after long nerve resection for severe hypoglossal (XII) nerve injury in adult rats. METHODS: The right XII nerve was exposed, and a 9-mm section was resected. The tongue was removed and cut coronally at 4, 8, and 12 weeks after the XII nerve resection. The sections were processed for immunohistochemical staining for vesicular acetylcholine transporter (VAChT) to detect the regenerated right XII and the uninjured left XII nerve terminals. The numbers of VAChT-positive nerve terminals were counted on the injured

Circadian clock protein Bmal1 regulates respiratory motor plasticity in male rats.

Acute intermittent hypoxia (AIH) elicits respiratory motor plasticity in the phrenic, intercostal, and hypoglossal motor pools and has emerged as a promising therapeutic strategy to improve respiratory function in people with neuromuscular disorders that compromise breathing. Although we recently reported that time-of-day regulates moderate ([Formula: see text] ∼40-50 mmHg) AIH-induced respiratory motor plasticity, it is unknown whether diurnal effects on AIH-induced phrenic (pLTF) or ventilatory (vLTF) long-term facilitation are mediated via the endogenous circadian clock versus other factors

Facial Neuropathy in Patients with Recurrent Painful Ophthalmoplegic Neuropathy.

INTRODUCTION: Involvement of non-ocular motor cranial nerves, such as the trigeminal, facial or hypoglossal nerves, has rarely been reported in recurrent painful ophthalmoplegic neuropathy (RPON). This study aimed to determine the incidence and clinical significance of multi-cranial nerve involvement in RPON. METHODS: Eight patients (females = 5; mean age = 53.3 ± 15.6 years) who met International Classification of Headache Disorders-3 diagnostic criteria for RPON were enrolled at a single tertiary hospital. Repeated evaluations were performed for differential diagnosis, including cranial ner

Bilateral Hypoglossal Nerve Palsy After Thyroidectomy: A Case Report.

Surgical resection remains the primary treatment modality for well-differentiated thyroid carcinoma. However, hypoglossal nerve injury following thyroidectomy represents a rare complication. We report a case of bilateral hypoglossal nerve palsy in a 57-year-old female patient who underwent bilateral total thyroidectomy with neck dissection and thyroglossal duct cyst excision under general anesthesia. Comprehensive imaging and electrophysiological studies confirmed bilateral hypoglossal nerve dysfunction, although surgical re-exploration revealed anatomically intact nerves. Despite empirical st

Sleep-Disordered Breathing in Heart Failure: Phenotypes, Mechanisms, and Precision Approach.

Sleep-disordered breathing (SDB), comprising obstructive sleep apnea (OSA) and central sleep apnea (CSA), is highly prevalent in heart failure (HF) and is associated with adverse remodeling, arrhythmias, and worse clinical outcomes. However, despite improvements in physiological markers, treatment of SDB has not consistently translated into reductions in mortality or HF hospitalization. In OSA, continuous positive airway pressure (CPAP) improves symptoms, oxygenation, and selected physiological parameters, but evidence in HF is derived largely from small, short-term trials and reductions in ha

Polysomnography in otorhinolaryngology (1995-2025): Bibliometric analysis and database comparison.

BACKGROUND: Polysomnography (PSG) is essential for diagnosing sleep-related disorders in otorhinolaryngology. Despite its clinical importance, to our knowledge, no comprehensive bibliometric study has examined PSG research in this area. This study aimed to provide a 30-year bibliometric analysis (1995-2025) of articles related to PSG in otorhinolaryngology journals, assessing publication trends, author, journal, and institution contributions, keyword dynamics, and cross-database citation comparability, which is important as citation metrics vary across databases. METHODS: Original articles we

Hypoglossal Nerve Stimulation and the Incidence of Cardiovascular Disease.

IMPORTANCE: As hypoglossal nerve stimulation (HGNS) grows as an obstructive sleep apnea (OSA) treatment option, health risk reduction data are needed. OBJECTIVE: To determine whether HGNS is associated with lower incidence of cardiovascular disease (CVD). DESIGN, SETTING, AND PARTICIPANTS: This retrospective cohort study was conducted using the Merative MarketScan Commercial Database. Adults undergoing HGNS between 2015 and 2024 were matched to controls meeting HGNS candidacy criteria (ie, diagnosed with OSA, nonadherent to positive airway pressure therapy) who did not undergo implantation.

Sleep-Related Breathing Disorders: A Comprehensive Review of Surgical Innovations and Evolving Technologies.

Sleep-related breathing disorders (SRBDs) encompasses a spectrum of conditions that disrupt ventilation during sleep, leading to fragmented sleep and impaired gas exchange. Their high prevalence and substantial neurocognitive and mental health outcomes make SRBD clinically significant across multiple medical disciplines. Traditional management includes lifestyle modifications and positive airway pressure (PAP). When non-surgical measures fail or anatomical factors predominate, a range of surgical approaches may be employed, such as uvulopalatopharyngoplasty (UPPP) or maxillomandibular advancem

Current and Emerging Therapies for Pediatric Sleep Disorders: A Narrative Review.

Sleep disorders in children represent a significant public health concern with important implications for neurodevelopment, behavior, and overall health. Among these, pediatric obstructive sleep apnea (OSA), insomnia, narcolepsy, and sleep-related movement disorders are the most clinically relevant conditions requiring targeted therapeutic strategies. Pediatric OSA, affecting approximately 2-5% of children, is characterized by recurrent upper airway obstruction during sleep and is associated with cognitive, behavioral, and cardiovascular consequences. Management is multimodal, with adenotonsil

Hypoglossal nerve palsy secondary to cervical spondylosis mimicking angio-oedema.

A man in his 50s, with a history of cervical spondylosis, presented with acute right-sided tongue swelling, dysarthria and dysphagia. Although tongue deviation was present, unilateral tongue oedema led to initial suspicion of angio-oedema and malignancy; however, blood tests and tongue MRI did not lead to a definitive diagnosis. Tranexamic acid was prescribed for presumed angio-oedema; however, at follow-up, rightward tongue deviation persisted without any other neurological deficits. Localised tenderness was noted over the right occipital area, corresponding to the atlanto-occipital joint. Im

Obstructive Sleep Apnea.

OBJECTIVE: Obstructive sleep apnea (OSA) is a common disorder that can have a significant negative impact on quality of life and health, including many neurologic disorders. This article discusses the updates in the diagnosis, implications, and treatment of OSA. LATEST DEVELOPMENTS: New recommended criteria for diagnosing OSA increase the recognition of the disorder. Although the apnea-hypopnea index has been the metric used to diagnose, stratify severity, and monitor treatment, it has limitations. A growing number of other metrics, including hypoxic burden, sleep quality, arousal index, resp

Isolated Hypoglossal Nerve Palsy Associated with Internal Carotid Artery Dissection: A Systematic Review.

Background: Isolated hypoglossal nerve palsy is a rare neurological condition with a broad differential diagnosis. Although neoplastic disorders are the most common cause, internal carotid artery dissection (ICAD) is an uncommon but clinically important and potentially reversible vascular etiology that is frequently underrecognized. This systematic review synthesized the available evidence regarding the clinical presentation, imaging findings, pathophysiological mechanisms, treatment, and outcomes of isolated hypoglossal nerve palsy associated with ICAD through a standardized patient-level des

Therapeutic options for patients with obstructive sleep apnoea: a retrospective review of difficult-to-treat cases in a multidisciplinary approach.

INTRODUCTION: Continuous positive airway pressure (CPAP) is the first-line treatment for obstructive sleep apnoea (OSA) but long-term adherence is limited. This study evaluates the implementation and outcomes of a multidisciplinary team approach to managing CPAP-intolerant patients using alternative therapies. METHODS: A retrospective review was conducted on 'difficult-to-treat' OSA cases discussed at multidisciplinary team meetings at the Guy's and St Thomas' NHS Foundation Trust Sleep Disorders Centre (project number 17 160). Patient demographics, comorbidities, OSA severity (Apnoea-Hypopno

Age-related disruptions in ubiquitination and mitochondrial proteostasis are associated with neuronal compartment-specific mitochondrial changes.

Declining aerodigestive neuromotor function is a major aspect of human aging, with impaired airway defense and swallow manoeuvres implicated in pneumonia and dysphagia. Hypoglossal motor neurons (MNs) innervate tongue muscles, essential for these behaviours. Their degeneration contributes to age-related aerodigestive dysfunctions. In neurodegenerative diseases, the ubiquitin-proteasome system (UPS) is altered, disturbing mitochondrial proteostasis. We have previously shown reduced mitochondrial abundance, dysfunction and mitochondrial fragmentation in aging hypoglossal MN somas and dendrites.

[Age-Related Changes in Occlusion and the Clinical Management and Prevention Strategies].

Occlusion refers to the contact relationship between the upper and lower teeth. Its biomechanical properties influence motor function, most often in the orofacial and cervical regions, and the patient's psychological well-being as well. In this review, we summarized age-related changes in occlusion and the corresponding clinical management and preventive strategies from three perspectives-the functional characteristics of normal occlusion and their age-related changes, the effects of abnormal occlusion on health and the potential neurophysiological mechanisms involved, and age-related changes

A Word to the WISeR: Why Otolaryngologists Must Lead to Preserve Access to Hypoglossal Nerve Stimulation.

The Centers for Medicare & Medicaid Services' Wasteful and Inappropriate Service Reduction (WISeR) model introduced prior authorization review for hypoglossal nerve stimulation (HGNS) and took effect on January 1, 2026, in six states. HGNS was the only otolaryngology procedure included in the model, reflecting the rapid growth in utilization and potential payor concern about continued access to the procedure. This policy shift presents an opportunity for otolaryngologists to define and demonstrate high-quality, evidence-based stewardship of HGNS.
