Obstructive Sleep Apnea and Snoring: Diagnosis, Treatments, and Prognosis.

Obstructive sleep apnea (OSA) is a heterogeneous disorder with varying clinical presentations and pathophysiologic mechanisms. Multiple studies demonstrated increased cardiovascular morbidity and mortality, impaired daytime function and quality of life in moderate to severe OSA. New sleep study metrics are being developed to better predict OSA-related cardiovascular risk that may help to target therapy to patients most likely to benefit. Novel treatments, including hypoglossal nerve stimulation, as well pharmacologic approaches, provide options to personalize therapy.

[Regulation of SARM1 on SNPH expression and its participation in glaucomatous optic neuropathy].

Objective: To investigate the molecular mechanism of toll-like receptor adaptor protein SARM1 in glaucomatous optic neuropathy. Methods: The experimental study was conducted from February 2024 to October 2025. A chronic ocular hypertension glaucoma model was established by injecting micro-magnetic beads into the anterior chamber of 8- to 10-week-old male Wistar rats. At 3 days, 1 week, and 2 weeks post-modeling, retinal and optic nerve tissues from 6 eyes of 6 rats were collected as the chronic ocular hypertension glaucoma model group, and 6 eyes from 6 wild-type rats that received an equal vo

Perfluorooctanoic acid exposure induces visual impairment via microglia-mediated neuroinflammation and retinal ganglion cell apoptosis.

Perfluoroalkyl and polyfluoroalkyl substances are closely associated with visual impairment; however, the pathogenic mechanisms through which they cause optic nerve damage and their relationships with the onset of ocular diseases remain poorly defined. In this study, a mouse model of perfluorooctanoic acid (PFOA) exposure was established by oral administration of PFOA (1 mg/kg/day) for 60 consecutive days to investigate the effects of PFOA on retinal ganglion cells (RGCs), the primary constituents of the optic nerve, and retinal microglia, the immune sentinels of the optic nerve. Multiple attention based deep multimodal fusion network for glaucoma and neurodegenerative disease diagnosis.

Glaucoma and other neurodegenerative diseases involve progressive damage to the optic disc (OD) and can be identified using visual field (VF) tests, colour fundus photographs (CFP) and optical coherence tomography (OCT). However, most studies overlook neurodegenerative cohorts and primarily rely on unimodal approaches, thus failing to leverage advanced deep multimodal fusion techniques to capture modality-specific features and cross-modal interactions. To address the issue, this study aims to design a multi-attention-based deep multimodal network to fuse functional VF with structural CFP and O

Mesenchymal tumors of the central nervous system: illustrative case.

BACKGROUND: FET-CREB (EWSR1-ATF1) fusion-positive intracranial mesenchymal tumors (IMTs) are locally aggressive with a high recurrence rate, particularly for pediatric patients. Management is challenging because of their complex diagnostic criteria, tumor location, and limited evidence of effective adjuvant therapy. The authors present the case of a child with multiple recurrences of FET-CREB fusion-positive IMT. OBSERVATIONS: A 6-year-old male with cerebral palsy presented with 3 weeks of vomiting, gait decline, and headaches. Physical examination revealed right-sided weakness, facial asymme

Activation of TREK-1 and TREK-2 Two-Pore Domain Potassium Channels by the Kv4 Channel Modulator, NS5806.

NS5806 is a diaryl-urea small molecule developed for the treatment of pain and neurological disorders. A potent activator of Kv4.3 potassium channels, it is widely used to study Kv4 channel physiology in excitable cells, including trigeminal neurons. Surprisingly, we found that NS5806 produced a significant, robust hyperpolarization of the resting membrane potential of all trigeminal neurons tested. Given the structural similarity of NS5806 to other negatively charged activator compounds with broad potassium channel activity, we investigated whether NS5806 might also modulate the two-pore doma

A deep-learning framework reveals whole-body perturbations at cell level.

Many diseases, including obesity, have systemic effects that perturb multiple organ systems throughout the body1,2. However, tools for comprehensive, high-resolution analysis of disease-associated changes at the whole-body scale have been lacking. Here we developed MouseMapper, a suite of foundation-model-based deep-learning algorithms enabling multi-system analysis of disease across the entire mouse body. MouseMapper enables whole-body quantitative analysis of nerves and immune cells, resolving fine axonal branches and immune-cell clusters while automatically segmenting 31 organs and tissues.

Robotic thyroidectomy using Artificial Intelligence (AI) model in real-time recognizing Recurrent Laryngeal Nerves (RLN).

BACKGROUND: Robotic surgery is gaining momentum in thyroidectomy particularly among young female patients. Swallowing, voice disorders, throat discomfort and cervical scarring represent major sources of anxiety due to social and occupational concerns. This has driven demand for precise and scarless approaches to improve safe and cosmetic outcomes to reduce psychological burden [1]. While endoscopic thyroidectomy has gained popularity, its technical limitations including restricted endoscopic visualization, limitation of fixed magnification, and the 'chopstick' effect of rigid instruments compr

Irbesartan Promotes Retinal Ganglion Cell Survival and Modulates Mitochondrial and Inflammatory Pathways in Mouse Retinal Explant and in Optic Nerve Crush Models.

Glaucoma is a neurodegenerative eye disease characterized by progressive loss of retinal ganglion cells (RGCs), resulting in irreversible vision loss. Previous work demonstrated that irbesartan, an angiotensin II type I receptor blocker, promotes RGC survival in rat retinal explants. This study extends these findings to ex vivo mouse explants, in vivo optic nerve crush model, and transcriptomic analysis to identify associated pathways. Retinal explants from C57BL/6 mice were cultured for 4 days with irbesartan (10 μmol/L), vehicle, or angiotensin II (2 μmol/L). RGC survival was assessed by βII

Neurological Manifestations and Diagnostic Delay in Pediatric Isolated Sphenoid Sinus Disease: A Systematic Review.

Background: Isolated sphenoid sinus disease (ISSD) in children is an uncommon but clinically significant condition that may present primarily with neurological or neuro-ophthalmological symptoms rather than typical sinonasal complaints. Owing to the anatomical proximity of the sphenoid sinus to the optic nerves, cavernous sinus, and multiple cranial nerves, even localized sphenoid pathology may produce symptoms that mimic primary neurological, neuro-ophthalmological, or headache disorders, contributing to diagnostic delay. Objective: To characterize the spectrum and frequency of neurological a

Duplication of Right Superior Cerebellar Artery Leading to Trigeminal Neuralgia: A Rare Neurovascular Conflict.

Trigeminal neuralgia (TN) is a chronic neuropathic pain condition characterized by brief, intense, and electrical shock-like facial pain. The most common causes of this are due to neurovascular compression of the superior cerebellar artery (SCA). However, as seen in this case report, duplication of the SCA resulting in TN is extremely uncommon. A 63-year-old woman presented with a 1-day history of paroxysmal, shock-like pain on the right side of her face affecting both the ophthalmic (V1) and maxillary (V2) branches of the trigeminal nerve. Each episode of pain would last 1-2 min, occur multip

Peripheral trigeminal nerve branch lesions: CT and MRI findings in a pathologically confirmed case series.

PURPOSE: The peripheral branches of the trigeminal nerve (V1, V2, and V3) follow an intricate and tortuous course through various skull base foramina and soft tissue spaces. Due to the rarity of lesions specifically involving these distal segments, radiological diagnosis remains a significant challenge. This study aims to summarize the characteristic imaging patterns of diverse pathologically confirmed lesions affecting the peripheral trigeminal branches using CT and MRI. METHODS: We retrospectively reviewed 18 patients (2008-2022) with pathologically proven peripheral trigeminal lesions. The

Impaired brain-gut circuit for gut motility dysfunction in mouse models of Parkinson's disease.

Gut motility dysfunction is a common symptom of Parkinson's disease (PD), but its central mechanisms remain unclear. Here, we utilize multiple mouse models, which mimic both genetic and sporadic PD, to reproduce PD-related gut motility dysfunction. We find that α-synuclein pathology significantly impairs the excitability of cholinergic neurons in the dorsal motor nucleus of the vagus (DMV). Chemogenetic activation of these neurons effectively restores gut motility in PD models, revealing a causal link between DMV neuronal hypoexcitability and gut motility dysfunction. Notably, electroacupunctu

Bursting Parameters Alter Excitatory Postsynaptic Currents in Nucleus Tractus Solitarius Neurons: Implications for Clinical Vagus Nerve Stimulation.

Vagus nerve stimulation (VNS) is an approved therapy for multiple neurological and autonomic disorders; however, the mechanisms underlying its therapeutic effects and optimal stimulation parameters remain poorly understood. Vagal afferents activated by VNS terminate within the nucleus tractus solitarius (NTS), where sensory information is integrated and propagated throughout central autonomic networks. Although conventional VNS paradigms are used therapeutically, previous studies have demonstrated high rates of synaptic transmission failure within NTS circuits, suggesting that alternative stim

Vagal neuroimmune circuits in inflammation and immunity.

The peripheral nervous system serves as a critical interface between the brain and the immune system, orchestrating bidirectional communication that maintains homeostasis and coordinates host defense responses. Peripheral neuroimmune crosstalk occurs through multiple anatomical pathways, including autonomic efferents, sympathetic circuits, and sensory afferents that detect tissue perturbation. Among these pathways, the vagus nerve has emerged as a prominent conduit for neuroimmune communication, integrating homeostatic and inflammatory cues to shape both local and systemic immune responses. [Olfactory Nerve: The Vulnerability Inherent in its Unique System and Neurological Diseases].

The olfactory nerve possesses unique anatomical features, including direct central nervous system (CNS) projection and continuous regeneration. Scientific advances have elucidated mechanisms such as combinatorial receptor coding and signal amplification. This review summarizes these foundations and examines olfactory dysfunction in COVID-19 and Parkinson's disease (PD). In COVID-19, evidence suggests that SARS-CoV-2 targets sustentacular cells rather than olfactory neurons, causing gene downregulation and parosmia attributed to incomplete peripheral filtering, while direct CNS invasion remains

Pediatric Cranial Nerve Palsies.

Cranial nerve palsies in children offer unique challenges distinct from those in adults, and typically arise from congenital, traumatic, neoplastic, or postinfectious inflammatory disease. With rare reports of diplopia, diagnosis depends on indirect signs such as abnormal head posture, strabismus, or abnormal gaze. The oculomotor (III), trochlear (IV), and abducens (VI) nerves follow sometimes long, intricate courses from the brainstem to the target muscle(s) within the orbit. Accurate diagnosis requires integrating anatomic understanding with subtle clinical presentations and imaging findings

Spatiotemporal Heterogeneity in Isolated Neurosarcoidosis: Diagnostic Insights from Multimodal Imaging.

Isolated neurosarcoidosis (INS) is a rare inflammatory disorder of the central nervous system that lacks systemic manifestations and mimics neoplastic or infectious conditions, making diagnosis challenging. We report a case of a previously healthy 30-year-old man who presented with recurrent transient loss of consciousness and progressive right leg weakness. Brain magnetic resonance imaging (MRI) demonstrated multiple uniformly contrast-enhancing nodular lesions involving the pachymeninges and cranial nerves, while spinal MRI revealed conus medullaris enhancement with associated lumbosacral ne

IgG4-related disease involving facial, infraorbital, auriculotemporal and optic nerves: multinervous cranial involvement in an adult patient.

IgG4-related disease (IgG4-RD) is a rare fibroinflammatory condition that can mimic infectious or neoplastic diseases, leading to diagnostic challenges. We report a case of a man in his mid-40s presenting with rapidly progressive orbital symptoms initially diagnosed as orbital cellulitis. Despite appropriate surgical and medical management, the patient's condition worsened with multiple cranial nerve involvement.MRI revealed an infiltrative process with perineural extension involving the optic, infraorbital, facial and auriculotemporal nerves. Histopathological examination confirmed IgG4-RD, d

Differential Diagnosis of Orofacial and Jaw Pain: A Clinical Review for Physicians.

Orofacial and jaw pain represents a diagnostic challenge frequently encountered across multiple medical specialties. Although odontogenic disease is a common cause of pain in the facial region, the present review focuses on non-odontogenic causes that should be considered when no clear dental source is identified or symptoms remain unexplained or persist despite appropriate dental treatment. Such pain may originate from the temporomandibular joints, masticatory muscles, cranial nerves, facial skeleton, adjacent head-and-neck structures, systemic diseases, or distant referred sources.
