Myeloid lineage C3 induces reactive gliosis and neuronal stress during CNS inflammation.

Complement component C3 mediates pathology in CNS neurodegenerative diseases. Here we use scRNAseq of sorted C3-reporter positive cells from mouse brain and optic nerve to characterize C3 producing glia in experimental autoimmune encephalomyelitis (EAE), a model in which peripheral immune cells infiltrate the CNS, causing reactive gliosis and neuro-axonal pathology. We find that C3 expression in the early inflammatory stage of EAE defines disease-associated glial subtypes characterized by increased expression of genes associated with mTOR activation and cell metabolism. This pro-inflammatory s

Dysfunction of pancreatic exocrine secretion after experimental spinal cord injury.

Pancreatic exocrine dysfunction is an underdiagnosed comorbidity in individuals living with spinal cord injury (SCI) who often present cholestasis, acute pancreatitis or high levels of serum pancreatic enzymes. Parasympathetic control of pancreatic exocrine secretion (PES) is mediated in the medullary dorsal vagal complex in part through cholecystokinin (CCK) release. Our previous reports indicate high thoracic (T3-) SCI reduces vagal afferent sensitivity to GI regulatory peptides, like CCK and thyrotropin releasing hormone (TRH). To date, the effects of experimental SCI on PES are unknown. He

Exploring the Potential Role of the Cholinergic Anti-Inflammatory Pathway from the Perspective of Sepsis Pathophysiology.

Sepsis is one of the most prevalent conditions in critical care medicine and is characterized by a high incidence, mortality, and poor prognosis, with no specific treatment currently available. The pathogenesis of sepsis is complex with a dysregulated inflammatory response at its core. If the initial inflammatory response is not promptly controlled, patients often develop multiple organ dysfunction syndrome or die, whereas survivors may experience post-sepsis syndrome. Regulation by the central and autonomic nervous systems is essential for maintaining inflammatory homeostasis. Among these, th

Successful Outcome of Two-Stage Strabismus Surgery in a Rare Case of Unilateral Möbius Syndrome: A Case Report.

Möbius syndrome (MBS) is a rare congenital disorder characterized by facial and abducens palsy which is usually bilateral and may involve multiple cranial nerves. Additional features, such as craniofacial anomalies, tongue malformations, and limb defects, may also be present. The etiology of this syndrome is not well established but the ischemic necrosis of the brainstem leading to hypoplasia of the cranial nerve nuclei is the supposed to be most likely one. MBS leads to a large angle esotropia along with bilateral limitation of abduction with delayed diagnosis, which is very challenging to co

Cavernous sinus syndrome in a patient with occult nasopharyngeal carcinoma: A diagnostic challenge.

Nasopharyngeal carcinoma(NPC) is an epithelial malignancy of the nasopharyngeal mucosa. It typically exhibits local infiltration with frequent skull base involvement and often presents with multiple cranial neuropathies. It can invade the cavernous sinus via the skull base and foramen ovals, thereby causing cavernous sinus syndrome (CSS). When NPC invades nerves, the mass in the nasopharyngeal cavity may not be obvious. We report a case of CSS presenting as painful ophthalmoplegia or nonspecific inflammation. While initial glucocorticoid therapy provided transient symptom relief, the patient d

Internal Superior Laryngeal Nerve Injections Through the Piriform Sinus for Laryngeal Neurosensory Disturbances: A Prospective Analysis.

OBJECTIVE: Internal superior laryngeal nerve (iSLN) injections demonstrate short term improvements in patients with laryngeal neurosensory disturbances (ie, chronic refractory cough and globus pharyngeus) that can require multiple injections. A more targeted iSLN injection approach could provide greater effectiveness. This study prospectively evaluated whether transnasal endoscopic iSLN injections that target the internal iSLN as it transverses the piriform sinuses was effective at treating laryngeal neurosensory disturbances. METHODS: This was a prospective cohort analysis performed at a sin

Repeat round of auricular percutaneous electrical nerve field stimulation for pediatric disorders of gut brain interaction.

OBJECTIVES: Percutaneous electrical nerve field stimulation (PENFS) is an FDA-approved nonpharmacological modality for treating disorders of gut-brain interaction (DGBI) in pediatric patients. PENFS presumably functions through stimulation of cranial nerves. There is limited data on patients that require more than a 4-week cycle of treatment with PENFS. We examined the effect of repeated rounds of PENFS on recurrent symptoms and predictors for multiple treatments. METHODS: We evaluated charts of patients who underwent PENFS placement for DGBI. Data included demographics, medical history, and

Neuroimmune Control of Inflammation in Acute Kidney Injury and Multiorgan Dysfunction.

The nervous and immune systems engage in critical bidirectional communication that influences both physiologic regulation and disease progression. This review discusses the neuroimmune axis and its role in controlling inflammation in AKI and multiorgan dysfunction. Central to this regulation is the inflammatory reflex pathway, which consists of sensory afferent and motor efferent arcs. The cholinergic anti-inflammatory pathway (CAP) suppresses inflammation through vagus nerve activation, norepinephrine release, and α 7 nicotinic acetylcholine receptor signaling in macrophages. Inflammatory med

Idiopathic Bilateral Optic Nerve Sheath Calcification.

A 61-year-old female patient with a previous diagnosis of Graves disease and mild orbitopathy presented with bilateral optic nerve calcifications and multiple areas of intracranial calcification. Neuro-ophthalmological evaluation did not detect any functional or anatomical compromise of the optic nerves. Her renal function and serum levels of calcium, phosphorus, and parathyroid hormone were normal. Two CT scans of the orbits, performed 7 years apart, showed stable intracranial and bilateral optic nerve calcifications.

Autonomous robotic system-assisted trans-inferior alveolar nerve implantation for patient diagnosed with pemphigoid: a case report.

BACKGROUND: Inadequate bone height and the proximity of the inferior alveolar nerve following vertical resorption of the mandibular alveolar ridge pose great challenges for dental implant surgery. Pemphigoid is a chronic autoimmune disease affecting both the skin and mucous membranes. The presence of autoantibodies and the ongoing inflammatory response significantly impair the skin's ability to heal in pemphigoid patients, thus necessitating a minimally invasive surgical approach. Therefore, finding accurate and minimally invasive implant solutions to address insufficient bone height in the ma

Intraocular Pressure Matters.

To date, evidence from multiple randomized controlled trials has shown that effective intraocular pressure (IOP)-lowering therapy significantly reduces the risk of glaucomatous progression across all stages of the disease. Changes in IOP have a substantial impact on the load-bearing connective tissues of the optic nerve head (ONH), as well as the overlying neurovascular tissues of the ONH and retina. An initial treatment goal of reducing IOP by 25% to 35%-and possibly more in advanced cases-can help prevent progression. Additionally, maintaining stable IOP by minimizing both diurnal and long-t

Electrophysiological findings in immune checkpoint inhibitor-related neuromuscular events.

INTRODUCTION: Immune checkpoint inhibitors (ICIs) have become key therapies for various malignancies by restoring antitumor immunity. However, they may trigger immune-related adverse events (irAEs), including neurological irAEs (n-irAEs), which, though rare, can be severe. Among these, neuromuscular disorders affecting peripheral nerves, the neuromuscular junction (NMJ), and muscle are the most frequent and carry the highest mortality risk, warranting prompt recognition. OBJECTIVE: To describe the clinical and electrodiagnostic features of ICI-related neuromuscular disorders in 38 patients.

Primary central nervous system lymphomas at a tertiary academic hospital in Johannesburg.

BACKGROUND: Primary central nervous system lymphomas (PCNSL) are diffuse large B-cell lymphomas (DLBCL), which affect multiple regions of the neuroaxis, including the brain parenchyma, the orbits, cranial nerves and leptomeninges, without systemic disease. The aim of this study was to review the clinical characteristics as well as the outcome of patients referred to a tertiary academic hospital in Johannesburg (South Africa) with a presumed PCNSL diagnosis based on histology, cerebrospinal fluid (CSF) analysis and neuroimaging. METHODS: This retrospective record review at a tertiary academic

Relations Between Pulsatility in the Optic Nerve Head or Peripapillary Retinal Vessels and the Rate of Progression in Glaucoma.

PURPOSE: Glaucomatous pathophysiology may impair retinal vasodilation, leading to increased upstream pulsatility. We sought to quantify pulsatility at multiple locations, concurrently, and identify changes that correlate with the rate of disease progression. METHODS: One hundred eighty-one eyes of 139 participants were tested twice annually for six or more visits. Vessels within the optic nerve head (ONH) and peripapillary region were identified and classified as arteries versus veins using deep learning models applied to optical coherence tomography angiography scans. Vessels were then coloc

Gut-brain communication: types of sensory nerves and mechanisms of activation.

Understanding the locations of extrinsic sensory nerve endings in the gastrointestinal tract and their mechanisms of activation is essential to advancing our understanding of how communication along the gut-brain axis affects health and disease. The gastrointestinal tract detects diverse stimuli (chemical, mechanical and thermal signals) via two major types of primary afferent (sensory) nerves: vagal and spinal afferents. Viscerofugal neurons represent a third pathway that has been indirectly implicated in gut-brain signalling. These spinal and vagal afferents transmit sensory signals to the b

Basophils activate oncostatin M receptor-expressing vagal sensory neurons.

BACKGROUND: Vagal sensory neurons (VSNs) relay signals from internal organs to the brainstem via the vagus nerve. Published transcriptomic data sets suggest that VSNs express Mas-related G protein-coupled receptor family member D and oncostatin M (OSM) receptor, markers also found in dorsal root ganglia-resident pruriceptor neurons. However, the cellular source of OSM and its potential interactions with VSNs remain unknown. OBJECTIVE: We sought to validate the expression of pruritogen receptors on VSNs, identify basophils as a source of OSM, and determine how OSM modulates VSN activity. METH

Trigeminal nerve stimulation modulates dopaminergic circuits and neuroinflammation to alleviate depression.

BACKGROUND: Trigeminal nerve stimulation (TNS) is a promising noninvasive neuromodulation therapy for depression; however, its mechanisms remain unclear. OBJECTIVE: This study investigated antidepressant-like effects of TNS and its underlying mechanisms in post-traumatic brain injury depression (PTD). METHODS: The antidepressant efficacy of TNS was evaluated in multiple depression models using standardized behavioral assays. Peripheral mechanisms were probed by chemogenetic activation or inhibition of trigeminal ganglion (TG) neuronal subpopulations in Cre-line mice. Centrally, pathway-selec
