Anterior horizontal rectus anatomy in children with concomitant strabismus.

BACKGROUND: Anterior ciliary vessel (ACV) counts, tendon width (TW), and limbus-to-insertion distance (LID) describe vascular, structural, and positional features of the anterior horizontal rectus. Data integrating these parameters in children with concomitant strabismus remain limited. We aimed to characterize ACV counts, TW, and LID in pediatric horizontal concomitant strabismus and examine their associations with diagnosis and clinical variables. METHODS: In this hospital-based cross-sectional study, 227 children undergoing primary horizontal rectus surgery were included. ACV bundles were

Effect of Electromyography Findings of Traction-Related Signal Loss on Prognosis During Neuromonitored Thyroidectomy.

BACKGROUND: Intraoperative neuromonitoring (IONM) is a reliable tool to detect electromyographic loss of signal (LOS). Here, we investigated the effect of electromyography features of traction-related LOS on prognosis. METHODS: Thyroidectomy patients with a traction-related LOS were included. The cases were divided into four groups as follows: Type 1 LOS with no amplitude (0 μV) (Group 1), Type 1 LOS with low-amplitude (1-99 μV) (Group 2), Type 2 LOS with no amplitude (Group 3), and Type 2 LOS with low-amplitude (Group 4). RESULTS: A total of 788 patients (1429 nerves) were analyzed. Tractio

Surgical management of parapharyngeal vagal schwannomas: a systematic review and practical considerations for approach selection.

OBJECTIVE: Vagal schwannomas of the parapharyngeal space are rare benign tumours and the optimal surgical approach remains debated. This study aimed to identify clinical and tumour-related factors associated with different surgical strategies. METHODS: A systematic review was conducted in accordance with Preferred Reporting Items for Systematic Review and Meta-Analyses ('PRISMA') 2020 guidelines. Studies published between January 1997 and March 2026 were identified, and analyses were restricted to surgically treated patients. RESULTS: Twenty-five studies including 87 patients were analysed.

Non-Arteritic Anterior Ischaemic Optic Neuropathy Associated with Semaglutide use.

PRESENTATION: Non-arteritic ischaemic optic neuropathy (NAION) is a common cause of acute, painless monocular visual disturbance in older adults. This case discusses a woman in her 70s with vascular risk factors who presented with a three-day history of progressive left sided visual disturbance. Examination demonstrated a left relative afferent pupillary defect and inferior quadrantopia. DIAGNOSIS: Haematological investigations and neuroimaging were unremarkable. Formal ophthalmological assessment demonstrated superior optic disc swelling with an inferior visual field defect, consistent with

Anatomical variation in the cervical branch of the facial nerve: a systematic review.

BACKGROUND: There is a paucity of literature on the anatomical variations of the cervical branch of the facial nerve (CBFN). Poor awareness of its anatomy and variations risks intraoperative damage, misdiagnosis of facial nerve injury and undesirable functional and aesthetic outcomes for patients. PURPOSE: To review anatomical studies on the CBFN and synthesise data on its course, innervations and common variations. METHODS: A PRISMA-compliant systematic review of anatomical studies of the cervical branch of the facial nerve was conducted. PubMed, Embase, Scopus, Cochrane Library, Web of Sci

Neuroimaging signatures of facial onset sensory and motor neuronopathy (FOSMN).

Facial onset sensory and motor neuronopathy (FOSMN) is an exceptionally rare condition characterized by progressive facial paresthesia and the subsequent appearance of bulbar symptoms with rostro-caudal motor and sensory deficits sometimes extending to the hand muscles. The pathophysiological understanding of this disorder is very limited. In order to identify patterns of neuroanatomical involvement within the brain, we applied advanced magnetic resonance imaging-based techniques to a group of six patients with the diagnosis of FOSMN, using unbiased analysis techniques with atlas-based volumet

[Parinaud syndrome : a diplopia unlike any other].

Sudden-onset diplopia warrants an emergency management in ophthalmology. A binocular, vertical and persistent diplopia, should raise suspicion of a central lesion, particularly within the brainstem. We reporte the case of a 53-year-old man, who presented with acute vertical binocular diplopia. Ophthalmological examination revealed a bilateral elevation limitation, characteristic of Parinaud's syndrome. Brain magnetic resonance imaging identified a right-sided ischemic midbrain infarction, and further cardiovascular work-up demonstrated a patent foramen ovale causing a right-to-left shunt.

Force feedback and vocal cord dysfunction after robotic esophagectomy: a comparison of the da Vinci 5 and Xi systems.

The da Vinci 5 (DV5) is the first robotic platform to integrate force feedback (FFB), which may limit excessive traction and tissue injury. Its clinical impact in robotic transthoracic esophagectomy remains unclear. We compared perioperative outcomes between the da Vinci Xi and DV5 systems. This retrospective study included consecutive patients who underwent robotic transthoracic esophagectomy for esophageal squamous cell carcinoma by a single high-volume surgeon between December 2024 and July 2026. Patients treated with the Xi system without FFB (n = 126) and the DV5 system with FFB (n = 37)

Macroprolactinoma with an atypical course.

Prolactinomas are among the most common pituitary adenomas. The typical symptoms of hyperprolactinemia associated with a prolactinomaprimarily include symptoms of hypogonadism resulting from the suppression of the hypothalamic-pituitary-gonadal axis.In men and postmenopausal women, symptoms related to the mass effect of the tumor usually predominate. In some cases, however, prolactinomas may initially present with atypical symptoms, which can make it difficult to establish the correct diagnosis. An extensive literature search covering the past 30 years was conducted using the PubMed database. T

Case Report: Pituitary apoplexy temporally associated with acute alcohol intoxication, presenting as complete oculomotor nerve palsy in an SF-1 lineage gonadotroph PitNET.

Pituitary apoplexy (PA) is an acute, potentially life-threatening neuroendocrine emergency caused by hemorrhage or infarction within the pituitary gland, most often arising in a previously unrecognized pituitary neuroendocrine tumor (PitNET). Reported precipitants include surgery, anticoagulation, dynamic pituitary testing, hypertension, and head trauma; alcohol is rarely implicated. We report a 61-year-old man with long-standing hypertension, type 2 diabetes mellitus and coronary artery disease (on aspirin after coronary stenting) who developed sudden thunderclap headache and complete right o

Determining the Impact of Race and Ethnicity on Diagnosis and Management of Vestibular Schwannoma.

OBJECTIVE: While prior literature has demonstrated that patients of lower socioeconomic groups may have larger tumors on presentation and poorer outcomes, there is limited literature on the impact of race and ethnicity on management of vestibular schwannoma (VS). The objective of this study was to determine whether race and ethnicity impacted treatment choice and postoperative management of VS. METHODS: Retrospective review of patients diagnosed with VS (ICD9 225.1, ICD10 D33.3, and ICD 10 H93.3) who presented to a tertiary care center between January 1, 2018 and October 31, 2023 was complete

Clinical management of vestibular schwannoma - what's the evidence?

BACKGROUND: Clinical management of vestibular schwannomas (VS) encompasses three main strategies: observation ("wait-and-scan"), stereotactic radiosurgery, and microsurgery. Despite extensive literature, a consensus on the optimal modality is lacking due to the scarcity of high-level evidence. METHODS: This narrative review explores the current understanding of the most significant clinical and radiographic outcomes following wait-and-scan, radiosurgery, and microsurgery. This is not a systematic review of the literature. RESULTS: Observational studies indicate that approximately one-third o

Probable vestibular paroxysmia mimicking benign paroxysmal positional vertigo of the superior semicircular canal in the presence of a neurovascular conflict: a case report and review of the literature.

BACKGROUND: Positional vertigo is a frequent complaint in otology and vestibular physiotherapy, arising from functional or structural causes affecting the peripheral or central vestibular systems. Although benign paroxysmal positional vertigo (BPPV) is the most common form, other etiologies such as vestibular paroxysmia (VP), central positional vertigo, or vertebrobasilar insufficiency should be considered. Recognizing the specific features and differential diagnosis of these conditions is crucial for accurate assessment. CASE PRESENTATION: We describe the case of a 52-year-old French woman p

Association between intraoperative balloon morphology and postoperative sensory dysfunction after percutaneous balloon compression for trigeminal neuralgia: a propensity score-matched cohort study.

BACKGROUND: Percutaneous balloon compression (PBC) is widely used for trigeminal neuralgia, but the association between intraoperative balloon morphology and postoperative sensory outcomes remains unclear. Defining this relationship may improve technique and risk stratification. Prior studies suggest selective injury to large myelinated fibers rather than generalized sensory fiber or ganglion injury, and balloon morphology should be interpreted as a procedural marker rather than a direct measure of neural injury. METHODS: We retrospectively identified patients who underwent PBC for trigeminal

Nerve Injury During Thyroid Radiofrequency Ablation: Mechanisms, Prevention, and Practical Management.

Voice change is an uncommon but clinically important complication of thyroid radiofrequency ablation (RFA), because it may indicate injury to the recurrent laryngeal nerve or, less commonly, the vagus nerve. Early recognition and appropriate intervention are important, because neural dysfunction may remain reversible before the occurrence of irreversible structural nerve injury. Thermal nerve injury during thyroid RFA is not always a single instantaneous event. Rather, it may involve direct thermal exposure of the nerves, leading to acute endoneurial or perineural edema, microvascular compromi

Trigeminal sensory neuropathy developing as a late complication after the treatment of surgically induced necrotizing scleritis following pterygium surgery: a case report and literature review.

BACKGROUND: Surgically induced necrotizing scleritis (SINS) is an immune-mediated disease that occurs infrequently after ocular procedures, with latency ranging from days to years. There are various early and late complications that can negatively affect quality of life after SINS. This case report aims to present late trigeminal sensory neuropathy (TSN), corresponding in this patient to painful post-traumatic trigeminal neuropathy (ICHD-3 13.1.2.3), as a complication in a patient who developed SINS after pterygium surgery (PS). The effect of routine intraoperative neuromonitoring on thyroidectomy complications: A propensity score-matched retrospective cohort study.

Intraoperative neuromonitoring (IONM) is commonly used during thyroid surgery to facilitate recurrent laryngeal nerve (RLN) identification and potentially reduce related postoperative complications. However, the efficacy of routine IONM in decreasing complication rates remains debated. In this study, we investigated the impact of IONM on postoperative complications after thyroidectomy. This retrospective single-center study included 214 thyroidectomy operations performed between 2021 and 2024. Patients were divided into 2 groups: Group 1 (n = 107) underwent surgery with routine IONM, and Group
