Why repair fails in AQP4-IgG-positive neuromyelitis optica spectrum disease: an astrocyte-centered framework for remyelination and neuroaxonal preservation.

Aquaporin-4 immunoglobulin G (AQP4-IgG)-positive neuromyelitis optica spectrum disease (NMOSD) is an autoimmune astrocytopathy in which a single attack may cause permanent visual or neurological disability. Although targeted immunotherapies reduce relapse risk, they do not establish repair of previously injured tissue. Repair failure is framed as disruption of an astrocyte-organized environment required for glial communication, oligodendrocyte lineage progression, myelin formation, and axonal survival. Five linked barriers organize the review: loss of astrocytic homeostatic support, glial unco

Therapeutic Window of AAV-SIRT1 Gene Therapy in Mouse Models of Optic Nerve Degeneration.

PURPOSE: Optic neuropathies involve progressive loss of retinal ganglion cells (RGCs) and their axons, leading to irreversible vision deficits. While pretreatment with drugs or gene therapy that upregulate sirtuin 1 (SIRT1), an NAD⁺-dependent deacetylase, has shown neuroprotective effects, its effectiveness when administered after disease onset remains uncertain. We investigated whether adeno-associated virus (AAV)-mediated SIRT1 overexpression in RGCs can provide post-onset neuroprotection in preclinical models of optic neuropathy. METHODS: RGC degeneration was induced in C57BL/6 mice (8-10/

Hemi-see-saw nystagmus with superimposed pendular oculopalatal tremor: case report and systematic review.

BACKGROUND: Hemi-see-saw nystagmus (HSSN) is an uncommon subtype, characterized by elevation and intorsion of one eye with contralateral depression and extorsion, typically in a jerk pattern. It is associated with unilateral lesions of the interstitial nucleus of Cajal and the medial longitudinal fasciculus, both of which are involved in the ocular tilt reaction (OTR). Although its association with oculopalatal tremor (OPT) has been described, it is rare. METHODS: We performed a systematic review of seven databases on HSSN, extracting data on demographics, etiology, lesion location, oculomoto

Tissue-Engineered Fibrous Nerve Conduits Link Facial Nerve Repair with Central Network Reorganization.

Peripheral nerve injury leads to persistent motor and sensory dysfunction and is increasingly recognized to induce alterations in central sensorimotor networks. However, most current repair strategies primarily focus on peripheral regeneration, with limited understanding of its relationship to central neural changes. Here, we performed clinical resting-state functional magnetic resonance (rs-fMRI) in patients with facial nerve injury and identified the sensorimotor network alterations associated with the nerve injury. Guided by the results, we developed a tissue-engineered fibrous nerve guidan

Multimodal MRI fusion and three-dimensional reconstruction for preoperative assessment of neurovascular compression in trigeminal neuralgia: A retrospective study.

BACKGROUND: Microvascular decompression is the primary surgical intervention for trigeminal neuralgia aimed at resolving neurovascular compression. Given the potential surgical risks, accurate preoperative assessment of neurovascular compression is critical. Although 3D visualization has been applied to preoperative assessment of neurovascular relationships, quantitative evaluation of the trigeminal nerve-vessel relationship and its association with intraoperative NVC findings remains limited. MATERIALS AND METHODS: We retrospectively analyzed data from trigeminal neuralgia patients, hemifaci

Blood Glial Fibrillary Acidic Protein in MRI-Negative Attack-Like Episodes of Neuromyelitis Optica Spectrum Disorder.

OBJECTIVES: Magnetic resonance imaging (MRI) is widely used to support relapse diagnosis in neuromyelitis optica spectrum disorder (NMOSD), yet some patients present with attack-like symptoms without corresponding lesions. We assessed serum glial fibrillary acidic protein (sGFAP) levels in MRI-negative attack-like episodes. METHODS: We retrospectively analyzed aquaporin-4-IgG-positive NMOSD patients presenting with relapse-like symptoms but without corresponding new MRI lesions. sGFAP was measured from stored serum collected at presentation and converted to demographic-adjusted z-scores. A Novel Biomarker for Bell's Palsy: The Pan-Immune-Inflammation Value.

BACKGROUND: The pan-immune-inflammation value (PIV) is a recently described novel biomarker obtained from complete blood count parameters. Systemic biomarkers have been investigated in a number of studies aimed at confirming the role of the inflammatory process in the etiology of Bell's palsy (BP) and elucidating the association with prognosis. This study investigated the relationship between PIV and BP. METHODS: Sixty patients aged 18-75 years presenting with BP and 51 healthy controls with similar age and sex profiles were included in this retrospective study. Pan-immune-inflammation value

Anterior horizontal rectus anatomy in children with concomitant strabismus.

BACKGROUND: Anterior ciliary vessel (ACV) counts, tendon width (TW), and limbus-to-insertion distance (LID) describe vascular, structural, and positional features of the anterior horizontal rectus. Data integrating these parameters in children with concomitant strabismus remain limited. We aimed to characterize ACV counts, TW, and LID in pediatric horizontal concomitant strabismus and examine their associations with diagnosis and clinical variables. METHODS: In this hospital-based cross-sectional study, 227 children undergoing primary horizontal rectus surgery were included. ACV bundles were

Effect of Electromyography Findings of Traction-Related Signal Loss on Prognosis During Neuromonitored Thyroidectomy.

BACKGROUND: Intraoperative neuromonitoring (IONM) is a reliable tool to detect electromyographic loss of signal (LOS). Here, we investigated the effect of electromyography features of traction-related LOS on prognosis. METHODS: Thyroidectomy patients with a traction-related LOS were included. The cases were divided into four groups as follows: Type 1 LOS with no amplitude (0 μV) (Group 1), Type 1 LOS with low-amplitude (1-99 μV) (Group 2), Type 2 LOS with no amplitude (Group 3), and Type 2 LOS with low-amplitude (Group 4). RESULTS: A total of 788 patients (1429 nerves) were analyzed. Tractio

Neuroimaging signatures of facial onset sensory and motor neuronopathy (FOSMN).

Facial onset sensory and motor neuronopathy (FOSMN) is an exceptionally rare condition characterized by progressive facial paresthesia and the subsequent appearance of bulbar symptoms with rostro-caudal motor and sensory deficits sometimes extending to the hand muscles. The pathophysiological understanding of this disorder is very limited. In order to identify patterns of neuroanatomical involvement within the brain, we applied advanced magnetic resonance imaging-based techniques to a group of six patients with the diagnosis of FOSMN, using unbiased analysis techniques with atlas-based volumet

[Parinaud syndrome : a diplopia unlike any other].

Sudden-onset diplopia warrants an emergency management in ophthalmology. A binocular, vertical and persistent diplopia, should raise suspicion of a central lesion, particularly within the brainstem. We reporte the case of a 53-year-old man, who presented with acute vertical binocular diplopia. Ophthalmological examination revealed a bilateral elevation limitation, characteristic of Parinaud's syndrome. Brain magnetic resonance imaging identified a right-sided ischemic midbrain infarction, and further cardiovascular work-up demonstrated a patent foramen ovale causing a right-to-left shunt.

Force feedback and vocal cord dysfunction after robotic esophagectomy: a comparison of the da Vinci 5 and Xi systems.

The da Vinci 5 (DV5) is the first robotic platform to integrate force feedback (FFB), which may limit excessive traction and tissue injury. Its clinical impact in robotic transthoracic esophagectomy remains unclear. We compared perioperative outcomes between the da Vinci Xi and DV5 systems. This retrospective study included consecutive patients who underwent robotic transthoracic esophagectomy for esophageal squamous cell carcinoma by a single high-volume surgeon between December 2024 and July 2026. Patients treated with the Xi system without FFB (n = 126) and the DV5 system with FFB (n = 37)

Macroprolactinoma with an atypical course.

Prolactinomas are among the most common pituitary adenomas. The typical symptoms of hyperprolactinemia associated with a prolactinomaprimarily include symptoms of hypogonadism resulting from the suppression of the hypothalamic-pituitary-gonadal axis.In men and postmenopausal women, symptoms related to the mass effect of the tumor usually predominate. In some cases, however, prolactinomas may initially present with atypical symptoms, which can make it difficult to establish the correct diagnosis. An extensive literature search covering the past 30 years was conducted using the PubMed database. T
