The impact of climate and environmental factors on the incidence of Bell's palsy: a nationwide population study.

BACKGROUND: Bell's palsy, a sudden paralysis of the facial nerve, lacks a clear etiology, with climatic and environmental factors suggested as possible triggers. Identifying these associations could offer insights into effective preventive and therapeutic strategies. OBJECTIVES: This study aimed to explore the potential relationships between the incidence of Bell's palsy in the Korean population and specific meteorological and environmental factors. MATERIALS AND METHODS: A retrospective analysis was conducted using data from the Korean National Health Insurance Service for 2010 to 2020. The

Case Report: Pituitary apoplexy temporally associated with acute alcohol intoxication, presenting as complete oculomotor nerve palsy in an SF-1 lineage gonadotroph PitNET.

Pituitary apoplexy (PA) is an acute, potentially life-threatening neuroendocrine emergency caused by hemorrhage or infarction within the pituitary gland, most often arising in a previously unrecognized pituitary neuroendocrine tumor (PitNET). Reported precipitants include surgery, anticoagulation, dynamic pituitary testing, hypertension, and head trauma; alcohol is rarely implicated. We report a 61-year-old man with long-standing hypertension, type 2 diabetes mellitus and coronary artery disease (on aspirin after coronary stenting) who developed sudden thunderclap headache and complete right o

Determining the Impact of Race and Ethnicity on Diagnosis and Management of Vestibular Schwannoma.

OBJECTIVE: While prior literature has demonstrated that patients of lower socioeconomic groups may have larger tumors on presentation and poorer outcomes, there is limited literature on the impact of race and ethnicity on management of vestibular schwannoma (VS). The objective of this study was to determine whether race and ethnicity impacted treatment choice and postoperative management of VS. METHODS: Retrospective review of patients diagnosed with VS (ICD9 225.1, ICD10 D33.3, and ICD 10 H93.3) who presented to a tertiary care center between January 1, 2018 and October 31, 2023 was complete

Clinical management of vestibular schwannoma - what's the evidence?

BACKGROUND: Clinical management of vestibular schwannomas (VS) encompasses three main strategies: observation ("wait-and-scan"), stereotactic radiosurgery, and microsurgery. Despite extensive literature, a consensus on the optimal modality is lacking due to the scarcity of high-level evidence. METHODS: This narrative review explores the current understanding of the most significant clinical and radiographic outcomes following wait-and-scan, radiosurgery, and microsurgery. This is not a systematic review of the literature. RESULTS: Observational studies indicate that approximately one-third o

Probable vestibular paroxysmia mimicking benign paroxysmal positional vertigo of the superior semicircular canal in the presence of a neurovascular conflict: a case report and review of the literature.

BACKGROUND: Positional vertigo is a frequent complaint in otology and vestibular physiotherapy, arising from functional or structural causes affecting the peripheral or central vestibular systems. Although benign paroxysmal positional vertigo (BPPV) is the most common form, other etiologies such as vestibular paroxysmia (VP), central positional vertigo, or vertebrobasilar insufficiency should be considered. Recognizing the specific features and differential diagnosis of these conditions is crucial for accurate assessment. CASE PRESENTATION: We describe the case of a 52-year-old French woman p

Association between intraoperative balloon morphology and postoperative sensory dysfunction after percutaneous balloon compression for trigeminal neuralgia: a propensity score-matched cohort study.

BACKGROUND: Percutaneous balloon compression (PBC) is widely used for trigeminal neuralgia, but the association between intraoperative balloon morphology and postoperative sensory outcomes remains unclear. Defining this relationship may improve technique and risk stratification. Prior studies suggest selective injury to large myelinated fibers rather than generalized sensory fiber or ganglion injury, and balloon morphology should be interpreted as a procedural marker rather than a direct measure of neural injury. METHODS: We retrospectively identified patients who underwent PBC for trigeminal

Nerve Injury During Thyroid Radiofrequency Ablation: Mechanisms, Prevention, and Practical Management.

Voice change is an uncommon but clinically important complication of thyroid radiofrequency ablation (RFA), because it may indicate injury to the recurrent laryngeal nerve or, less commonly, the vagus nerve. Early recognition and appropriate intervention are important, because neural dysfunction may remain reversible before the occurrence of irreversible structural nerve injury. Thermal nerve injury during thyroid RFA is not always a single instantaneous event. Rather, it may involve direct thermal exposure of the nerves, leading to acute endoneurial or perineural edema, microvascular compromi

Trigeminal sensory neuropathy developing as a late complication after the treatment of surgically induced necrotizing scleritis following pterygium surgery: a case report and literature review.

BACKGROUND: Surgically induced necrotizing scleritis (SINS) is an immune-mediated disease that occurs infrequently after ocular procedures, with latency ranging from days to years. There are various early and late complications that can negatively affect quality of life after SINS. This case report aims to present late trigeminal sensory neuropathy (TSN), corresponding in this patient to painful post-traumatic trigeminal neuropathy (ICHD-3 13.1.2.3), as a complication in a patient who developed SINS after pterygium surgery (PS). CASE PRESENTATION: A 31-year-old woman was admitted to our clini

The effect of routine intraoperative neuromonitoring on thyroidectomy complications: A propensity score-matched retrospective cohort study.

Intraoperative neuromonitoring (IONM) is commonly used during thyroid surgery to facilitate recurrent laryngeal nerve (RLN) identification and potentially reduce related postoperative complications. However, the efficacy of routine IONM in decreasing complication rates remains debated. In this study, we investigated the impact of IONM on postoperative complications after thyroidectomy. This retrospective single-center study included 214 thyroidectomy operations performed between 2021 and 2024. Patients were divided into 2 groups: Group 1 (n = 107) underwent surgery with routine IONM, and Group

Case Report: Unplanned pregnancy during treatment with inebilizumab in patients with AQP4-IgG-seropositive NMOSD: report of two cases.

Neuromyelitis optica spectrum disorder (NMOSD) predominantly affects women and has a median age at onset of 40 years, meaning that many patients are of childbearing age. However, management of pregnant women with NMOSD remains extremely challenging. We report two patients with NMOSD who had unplanned pregnancies during inebilizumab (INE) treatment and successful deliveries. These cases provide preliminary clinical information regarding maternal disease control and pregnancy outcomes following INE exposure.

Bilateral cranial polyneuropathy associated with an intracranial mass in a cat with concurrent tonsillar lymphoma.

A 14-year-old spayed female Siamese cat was presented with signs consistent with a rapidly progressive cranial polyneuropathy. Magnetic resonance imaging of the brain revealed an extra-axial mass at the calvarial floor, extending caudally to involve bilateral trigeminal nerves and rostrally to involve the pituitary gland and optic chiasm. Additional findings included contrast-enhancing content within the bilateral tympanic bullae, mild mandibular and medial retropharyngeal lymphadenopathy, and a left tonsillar mass. Cytologic and histopathologic findings confirmed tonsillar lymphoma. The concu

High-frequency vestibulo-ocular reflex (VOR) evaluation in patients with spontaneous downbeat nystagmus: a case-control study.

INTRODUCTION: Spontaneous downbeat nystagmus (DBN) is generally attributed to vestibulocerebellar dysfunction, although its exact pathophysiological mechanism remains unclear. The video head impulse test (vHIT) quantifies vestibulo-ocular reflex (VOR) gain in semicircular canals, allowing evaluation of VOR imbalance. OBJECTIVE: To elucidate the role of VOR in spontaneous DBN of central origin we measured gain across semicircular canals and gain asymmetry between coplanar canals, compared with matched healthy subjects. METHODS: Nested case-control study within a retrospective cohort. We inclu

Multi-sequence MRI signal intensity ratios as candidate imaging correlates for audiovestibular dysfunction in vestibular schwannoma.

INTRODUCTION: Vestibular schwannoma (VS) typically induces gradual sensorineural hearing impairment and vestibular dysfunction. Nevertheless, clinically applicable non-invasive candidate imaging markers capable of reflecting pathological alterations within the inner ear have not yet been well established. The present study sought to systematically investigate the correlative relationships between labyrinth signal intensity ratios acquired from multi-sequence inner ear magnetic resonance imaging (MRI) and audiovestibular functional deficits in patients diagnosed with VS. METHODS: A total of 22

Acute Imbalance Syndrome vs Acute Vestibular Syndrome: Clinical and Topographical Differentiation.

BACKGROUND AND OBJECTIVES: Stroke is a major differential diagnosis in patients with acute vertigo/dizziness or imbalance. Current diagnostic algorithms such as head impulse test, gaze-evoked nystagmus, and test of skew are optimized for presentations with spontaneous nystagmus (SPN), traditionally called acute vestibular syndrome (AVS). However, in recent studies, a relevant proportion of patients with stroke-related vertigo/dizziness had no SPN, but rather postural or gait imbalance, a phenotype lately termed acute imbalance syndrome (AIS). We aimed to outline the pathoanatomical differences

Astrocytic and Axonal Injury Biomarkers in AQP4 Antibody-Positive NMOSD: Insights From PREVENT and CHAMPION-NMOSD.

BACKGROUND AND OBJECTIVES: Glial fibrillary acidic protein (GFAP) and neurofilament light chain (NfL) are biomarkers of astrocytic and axonal injury, respectively, in aquaporin-4 antibody-positive neuromyelitis optica spectrum disorder (AQP4-Ab+ NMOSD). We evaluated serum GFAP and NfL levels during treatment with complement component 5 inhibitors eculizumab or ravulizumab and examined associations between pretreatment biomarker levels, clinical characteristics, and on-trial relapse risk using samples from the PREVENT (eculizumab) and CHAMPION-NMOSD (ravulizumab) phase 3 trials. METHODS: Serum

Interpreting the S1BF-ACC-LHb pathway in trigeminal neuralgia-associated affective behavior.

The study by Wang and colleagues proposes that three connected brain areas link facial nerve injury to pain and depression-like behavior in mice. The experiments are ambitious and potentially important. However, the nerve-injury model differs from the defining pattern of human trigeminal neuralgia, several behavioral and drug controls leave alternative explanations open, and measurements from many cells were not clearly separated from the animals that supplied them. Clarifying these issues, and distinguishing results from whole tissue from results specific to neurons, would make the causal and

Interocular symmetry of ocular rigidity and related ocular parameters in isometropia.

PURPOSE: Driven by scleral stiffness, ocular rigidity (OR) is emerging as a critical factor in refractive development. Although a common assumption is that OR is symmetric in a healthy individual's eyes, empirical data remains scarce. This study directly tests this assumption by quantifying intereye OR differences in healthy, isometropic subjects. METHODS: Isometropia was defined as intereye differences in spherical equivalent refraction (SER) of <0.50 diopters (D) and axial length (AL) of <0.15 mm. OR was measured using a validated method based on video-rate optical coherence tomography imag

Efficacy and Safety of Selective Extracranial Radiofrequency Thermocoagulation and Percutaneous Balloon Compression for V3 Trigeminal Neuralgia.

BACKGROUND: Trigeminal neuralgia (TN) is a neuropathic disorder causing paroxysmal severe facial pain. Radiofrequency thermocoagulation (RFTA) and percutaneous balloon compression (PBC) are standard surgical treatments. While RFTA is constrained by its lack of branch selectivity and the risks of repeated intracranial punctures, selective extracranial RFTA (SRFTA) has attracted attention due to its high targeting specificity and fewer neural complications. This study aims to compare the efficacy and safety of SRFTA and PBC in patients with idiopathic TN involving the V3 branch (TN V3). METHODS
