Socioeconomic disparities in vestibular schwannoma diagnosis and management: A systematic review.

Disparities in neurosurgical care are increasingly recognized, yet how they affect the diagnosis and management of vestibular schwannoma (VS) remains unclear. Differences in tumor size at diagnosis, treatment selection, and outcomes may be influenced by race, insurance status, income, and geography. To date, no comprehensive synthesis has addressed these inequities in patients with sporadic VS. To systematically evaluate the literature for evidence of social determinants of health (SDoH) in the diagnosis, treatment, and outcomes of patients with sporadic vestibular schwannoma. We conducted a P

Nystagmus phenotypes in posterior circulation stroke.

BACKGROUND: Acute vestibular syndrome (AVS) is characterized by sudden, persistent vertigo or dizziness with nausea, vomiting, postural instability, and spontaneous nystagmus. Differentiating peripheral vestibular disorders from posterior circulation stroke remains a major diagnostic challenge in emergency neurology, as vestibulo-cerebellar infarcts may present as "central pseudo-neuritis," mimicking peripheral vestibular disease. Although the HINTS examination is a valuable bedside tool, its reliability depends on the presence and characterization of nystagmus and is reduced in patients witho

Acute Inflammatory Demyelinating Polyradiculoneuropathy Presenting With Multiple Cranial Nerve Palsies and Total Ophthalmoplegia: The Importance of Accurate Diagnosis and Timely Immunotherapy.

Guillain-Barré syndrome (GBS) is an immune-mediated disorder of the peripheral nervous system characterized by progressive weakness and often triggered by infections. GBS has an estimated mortality rate of approximately 5%, with a worldwide incidence of 0.81-1.91 cases per 1,00,000 person-years. This case reported a 23-year-old man presented with progressive weakness in all four extremities. An initial neurological examination revealed bilateral total ophthalmoplegia, ptosis on both eyes, bilateral facial palsy lower motor neuron (LMN) type, dysarthria, dysphagia, flaccid tetra-paresis, gloves

Cancer predisposition syndromes: an imaging review.

BACKGROUND: Cancer predisposition syndromes (CPSs) are inherited disorders that increase the risk of developing cancer from childhood through adulthood. They account for up to 10% of pediatric tumors, making early recognition important for reducing morbidity and mortality. Because these syndromes show variable penetrance and a wide range of clinical presentations even within the same family, identifying affected children can be challenging. MAIN BODY: Imaging is an essential tool for diagnosis, surveillance and follow-up of children with cancer predisposition syndromes. In this review we summa

Pediatric Cranial Nerve Palsies.

Cranial nerve palsies in children offer unique challenges distinct from those in adults, and typically arise from congenital, traumatic, neoplastic, or postinfectious inflammatory disease. With rare reports of diplopia, diagnosis depends on indirect signs such as abnormal head posture, strabismus, or abnormal gaze. The oculomotor (III), trochlear (IV), and abducens (VI) nerves follow sometimes long, intricate courses from the brainstem to the target muscle(s) within the orbit. Accurate diagnosis requires integrating anatomic understanding with subtle clinical presentations and imaging findings

Spatiotemporal Heterogeneity in Isolated Neurosarcoidosis: Diagnostic Insights from Multimodal Imaging.

Isolated neurosarcoidosis (INS) is a rare inflammatory disorder of the central nervous system that lacks systemic manifestations and mimics neoplastic or infectious conditions, making diagnosis challenging. We report a case of a previously healthy 30-year-old man who presented with recurrent transient loss of consciousness and progressive right leg weakness. Brain magnetic resonance imaging (MRI) demonstrated multiple uniformly contrast-enhancing nodular lesions involving the pachymeninges and cranial nerves, while spinal MRI revealed conus medullaris enhancement with associated lumbosacral ne

Transcriptomic meta-analysis identifies dysregulated pathways and potential therapeutic targets in Vestibular Schwannoma.

Vestibular schwannoma (VS) is a benign Schwann cell-derived tumor that frequently causes progressive hearing loss and vestibulocochlear dysfunction, substantially impacting quality of life. The molecular mechanisms underlying VS pathobiology remain poorly defined, and reliable biomarkers or targeted therapies are lacking. This study aimed to delineate the molecular landscape of VS through a transcriptome-wide meta-analysis. We performed a genome-wide random-effects meta-analysis of four independent Affymetrix microarray datasets from the Gene Expression Omnibus (GEO) database. Differential exp

IgG4-related disease involving facial, infraorbital, auriculotemporal and optic nerves: multinervous cranial involvement in an adult patient.

IgG4-related disease (IgG4-RD) is a rare fibroinflammatory condition that can mimic infectious or neoplastic diseases, leading to diagnostic challenges. We report a case of a man in his mid-40s presenting with rapidly progressive orbital symptoms initially diagnosed as orbital cellulitis. Despite appropriate surgical and medical management, the patient's condition worsened with multiple cranial nerve involvement.MRI revealed an infiltrative process with perineural extension involving the optic, infraorbital, facial and auriculotemporal nerves. Histopathological examination confirmed IgG4-RD, d

Prospective pilot study of a smartphone app as a screening tool for unilateral vocal fold paresis/paralysis.

PURPOSE: Low-cost tools are needed to differentiate normal from abnormal vocal fold mobility in resource-limited settings or in absence of laryngoscopic examination tools. METHOD: This is a prospective observational pilot study. Acoustic samples were recorded on the OperaVOX app to detect unilateral vocal fold paralysis/paresis (UVFP). RESULTS: Seventy participants were enrolled in two parallel cohorts (37 cases, 33 controls), 43 (61.4%) were females. Comparing cases to controls, there were significant differences in F2, jitter, shimmer, shimmer (dB), and harmonic to noise ratio (HNR), (P < 0.

HINTS+ to diagnose AICA stroke: systematic review of the diagnostic impact of acute unilateral hearing loss in the acute vestibular syndrome.

PURPOSE: Head-Impulse, Nystagmus, Test-of-Skew (HINTS) testing is recommended to identify stroke in the acute vestibular syndrome (AVS). Diagnostic accuracy depends on the vascular territory involved, with lower sensitivity for anterior-inferior cerebellar artery (AICA)- than posterior-inferior cerebellar artery (PICA)-territory strokes. Adding new-onset, acute unilateral hearing loss (AUHL) as a fourth central sign has been proposed ("HINTS+") to improve sensitivity for AICA-stroke detection. METHODS: We performed a systematic search (MEDLINE, Embase) to identify studies reporting diagnostic

Isolated oculomotor dysfunction with downbeat nystagmus as a rare presentation of anti-GAD autoimmunity.

BACKGROUND: Glutamic acid decarboxylase (GAD) antibodies are associated with a range of autoimmune neurological syndromes, including cerebellar ataxia (CA). Ocular motor disturbances such as nystagmus and diplopia are frequently observed in CA but rarely are seen as isolated manifestation. CASE PRESENTATION: We describe the case of a 60-year-old woman who presented with oculomotor dysfunction consisting of persistent diplopia and vertical nystagmus, in the absence of any other neurological manifestation. Brain MRI and PET-CT scans were unremarkable. Videonystagmography documented downbeat nys

[4-Repeat Tauopathies: Progressive Supranuclear Palsy and Corticobasal Degeneration].

Progressive supranuclear palsy (PSP) and corticobasal degeneration (CBD) are primary tauopathies that are neuropathologically characterized by deposits of 4-repeat tau proteins. Both diseases lead to a progressive impairment of motor skills, balance, speech, cognition, and everyday functioning. While PSP is often associated with axially emphasized symptoms and characteristic midbrain atrophy, CBD usually presents with asymmetric frontoparietal atrophy with cortical and subcortical deficits. Due to overlapping clinical phenotypes, it is often difficult to distinguish the two diseases from other

Substrates of 8.5-year clinical outcomes in aquaporin-4 IgG-positive neuromyelitis optica spectrum disorders.

BACKGROUND: Only a few longitudinal studies, with follow-up duration up to 3.6 years, have assessed the substrates of relapses and disability in aquaporin-4 (AQP4) IgG-positive neuromyelitis optica spectrum disorders (NMOSD), primarily focusing on clinical and conventional MRI variables. OBJECTIVE: We evaluated the association of clinical and MRI measures, including a comprehensive multimodal advanced MRI protocol, with key long-term clinical outcomes in AQP4 IgG-positive NMOSD patients over a median follow-up of 8.5 years (interquartile range [IQR] = 3.5; 13.3). METHODS: Forty-six NMOSD pat

Current evidence and knowledge gaps in family planning and pregnancy in myasthenia gravis, NMOSD, and MOGAD.

Myasthenia gravis, neuromyelitis optica spectrum disorder (NMOSD), and myelin oligodendrocyte glycoprotein antibody-associated disease (MOGAD) are antibody-mediated neuroimmune disorders that frequently affect women in their reproductive years and require careful treatment planning around pregnancy. Disease exacerbations (for myasthenia gravis) and attacks (for NMOSD and MOGAD) can occur during pregnancy, are common postpartum, and can cause preventable, long-term maternal disability. Many drug labels are conservative or recommend unnecessary prolonged washouts or avoidance of breastfeeding, c

Orbital Imaging Modalities and Recent Updates.

Orbit is a complex and intricate structure composed of conical bony walls, globe, lacrimal apparatus, soft tissues, optic nerve and cranial nerves III to VI. High-resolution MRI delineates these delicate anatomical structures in orbit, and emerging radiologic techniques including cone beam CT and photon counting CT have shown potentials for added benefit in orbital imaging. Multiple different imaging modalities including US, CT, MRI, and PET play complementary roles; US excels in superficial structures; CT shines in bone assessment; MR offers better tissue characterization; PET can add biochem

Causal Exposures of Immune Cells in Neuromyelitis Optica Spectrum Disorders: A Mendelian Randomization Study and Flow Cytometry Analysis.

BACKGROUND: Neuromyelitis optica spectrum disorders (NMOSD) are an immune-mediated inflammatory disease of the central nervous system (CNS) primarily characterized by anti-aquaporin-4 immunoglobulin G (AQP4-IgG)-mediated astrocyte injury, neuroinflammation, and demyelination. However, the relationship between the disease and the immune trait from a genetic perspective still requires further confirmation. METHODS: This study used two-sample Mendelian randomization (MR) to assess causal relationship of immune cells with NMOSD. A total of 731 immune phenotyping traits were considered as exposure

Cerebrovascular risk factors impact on brain atrophy and DTI-ALPS decrease in neuromyelitis optica spectrum disorder.

BACKGROUND: Neuromyelitis optica spectrum disorder (NMOSD) is an autoimmune astrocytopathy where disability is largely relapse-driven. Cerebrovascular risk factors (cVRFs) may exacerbate astrocytic and small-vessel injury. Therefore, we investigated their impact on MRI damage and disability in NMOSD. METHODS: Twenty-eight aquaporin 4 immunoglobulin G-positive NMOSD patients and 56 age- and sex-matched healthy controls (HC) underwent 3T brain and cervical MRI, cVRF and neurological assessment, including the Expanded Disability Status Scale (EDSS). Fundus albipunctatus-like retinopathy in hypovitaminosis A-related optic neuropathy with skull hyperostosis.

A young man presented with progressive night blindness, optic neuropathy and bilateral retinal flecks, initially suggestive of an inherited retinal dystrophy. Genetic testing identified a heterozygous pathogenic variant in USH2A alongside multiple variants of uncertain significance, none of which fully explained the phenotype. Further investigation revealed severe vitamin A deficiency and cranial hyperostosis with narrowing of the optic canals. Retinal findings improved following vitamin A supplementation. This case illustrates the diagnostic complexity of retinal dystrophies, emphasises the n
