Multiscale alterations of intrinsic neural timescales in anterior ischemic optic neuropathy.

BACKGROUND: Anterior ischemic optic neuropathy (AION) is primarily an optic nerve disorder, but loss of visual afferent input may also be associated with cortical reorganization. Intrinsic neural timescale (INT), derived from resting-state fMRI, indexes the temporal persistence of BOLD activity. We examined whether AION is associated with altered cortical INT and whether the spatial pattern of these alterations aligns with normative molecular and neurochemical architecture. METHODS: Resting-state fMRI data were analyzed from 30 patients with AION and 25 healthy controls. Voxel-wise INT was es

Villaret's Syndrome Secondary to Internal Carotid Artery Dissection: A Case Report.

Villaret's syndrome is a rare neurological condition characterized by ipsilateral palsies of the cranial nerves (CNs) IX, X, XI, and XII in association with Horner's syndrome. We report a 52-year-old man presenting with headache, hoarseness of voice, and dysphagia following strenuous exercise. Physical examination revealed left-sided lower CN palsies and partial Horner's syndrome, implying pathology in the retroparotid space, the only anatomical site where these nerves are colocated. Initial stroke computed tomography head and angiogram were reported as unremarkable, but a subsequent magnetic

The Application of the Balloon Pre-Inflating Technique in Percutaneous Balloon Compression for Primary Trigeminal Neuralgia: A Retrospective Study.

BACKGROUND: Percutaneous balloon compression (PBC) is a safe and effective surgical technique for treating trigeminal neuralgia. The filling balloon can compress the trigeminal ganglion in Meckel's cave to achieve the interruption of pain signals. The core of the operation is the successful placement of a balloon catheter into Meckel's cave so that effective compression of the trigeminal ganglion can be achieved after the balloon is inflated. OBJECTIVE: In this article, we try to determine the application value of the balloon pre-inflating technique (using contrast agents as guides during pos

Posterior Communicating Artery Aneurysm Microsurgery: PComA-CORE, an Anatomy-Informed Explainable AI Framework for Complexity, Neurovascular Risk, Oculomotor Recovery and Functional Outcome.

The posterior communicating artery (PComA) aneurysm is a challenging microsurgical problem due to multiple factors. These include the technical complexity of the surgery itself, potential injury to blood vessels during surgery, recovery of cranial nerve function, and overall neurological outcome after the operation. These are all related to the area where the PComA aneurysm is located, but they have fundamentally different biological determinants. Prior methods of describing aneurysms do not adequately describe how the relationships of the internal carotid artery (ICA) and PComA/P1 configurati

Purine Metabolism-Related Pathogenic Genes in Trigeminal Neuralgia: A Multiomics Mendelian Randomization Study.

PURPOSE: To investigate the association between purine metabolism-related genes and trigeminal neuralgia (TN) using multiomics approaches. METHODS: A multiomics summary-data-based Mendelian randomization (SMR) approach was applied to investigate associations between purine metabolism-related genes and TN. Genome-wide association study (GWAS) data from FinnGen R10 were integrated with blood-derived DNA methylation quantitative trait loci (mQTL), expression quantitative trait loci (eQTL), and protein quantitative trait loci (pQTL) datasets. Associations were evaluated by colocalization and repl

Motor-related white matter tracts are prominently affected in primary trigeminal neuralgia.

BACKGROUND: Primary trigeminal neuralgia (PTN) is a severe chronic pain disorder, yet whole-brain white matter microstructural alterations beyond the trigeminal nerve remain poorly understood, partly due to limitations of conventional diffusion tensor imaging in crossing fiber regions. METHODS: We prospectively enrolled 57 patients with classical PTN and 29 age‑ and sex-matched healthy controls. Seventy-two whole‑brain white matter tracts were reconstructed using constrained spherical deconvolution and fixel-based analysis. Fractional anisotropy (FA) was compared between groups using permutat

Evaluation of corneal subbasal nerve plexus as a predictor of radiosurgery outcome in trigeminal neuralgia.

Gamma Knife radiosurgery (GKRS) is an established treatment for trigeminal neuralgia (TGN); however, no validated biomarkers exist to predict outcomes. In vivo confocal microscopy (IVCM) allows noninvasive quantification of corneal subbasal nerve plexus (SBP) and dendritic cell (DC) density, potentially reflecting trigeminal nerve integrity. This study investigated whether IVCM-derived corneal parameters correlate with and predict pain relief after GKRS in medically refractory TGN. 27 patients with medically refractory idiopathic TGN and 40 healthy controls underwent ophthalmologic examination

Nasal-Predominant Ganglion Cell-Inner Plexiform Layer Thinning Distinguishes Early-Stage Leber Hereditary Optic Neuropathy From Acute Optic Neuritis.

BACKGROUND: To compare acute-phase macular ganglion cell-inner plexiform layer (GCIPL) thickness on optical coherence tomography (OCT) between Leber hereditary optic neuropathy (LHON) and optic neuritis (ON), and to evaluate the diagnostic performance of nasal GCIPL thickness. METHODS: In this retrospective single-centre study, OCT (CIRRUS HD-OCT; Carl Zeiss Meditec, Dublin) acquired within 45 days of symptom onset was analysed. Sectoral GCIPL thickness was compared between LHON and ON-including double-seronegative (DN-ON), aquaporin-4 antibody-positive (AQP4-ON) and myelin oligodendrocyte gl

Comprehensive characterization of spinal ependymomas in NF2-Schwannomatosis.

BACKGROUND: Spinal ependymomas (SE) account for 10% of ependymomas and are the most frequent spinal glial tumors. NF2-related Schwannomatosis (NF2-SWN) patients are predisposed to multiple SE in addition to other tumors, schwannomas and meningiomas. Surgery remains the main treatment option, but mays lead to severe morbidity. In many cases, it is the cystic rather than the solid component of the tumor that drives neurological symptoms and ultimately necessitates surgical intervention. Understanding tumor and associated cyst formation is therefore important to develop new therapeutic approaches

Genetically predicted association between immune cells and optic neuritis: evidence from a Mendelian randomization study.

PURPOSE: Optic neuritis (ON) is a common inflammatory disease of the optic nerve that usually precedes serious neurological disorders. The purpose of this study was to explore the causal relationship between immune cells and ON. METHODS: We adopted a bidirectional Mendelian randomization (MR) approach, analyzing genetic variants associated with 731 immune cells from a Sardinian cohort and ON data from the FinnGen consortium. Instrumental variables (IVs) for each phenotype were stringently selected based on linkage disequilibrium and genome-wide significance levels. Inverse-variance weighted (

Diagnostic discovery of structural variants causing foveal hypoplasia using SVRare and long-read nanopore sequencing.

Advances in DNA sequencing technology are increasing the rate of molecular diagnosis for patients and families with inherited Mendelian diseases. However, some patients remain unsolved following standard-of-care testing (typically either exome or genome sequencing), with structural variants (SVs) likely to account for a significant proportion of these missed diagnoses. We re-analysed ten research cases with the visual disorder foveal hypoplasia (FH) that were previously unsolved through whole exome sequencing (WES). We used SVRare, a tool that integrates outputs from Canvas and Manta and combi

Evaluation of the 2022 international consensus criteria for optic neuritis: the role of clinical features and visual evoked potentials.

BACKGROUND AND AIM: Optic neuritis (ON) diagnosis is challenging due to variable clinical presentations and the limited prospective validation of current criteria. This study evaluates the 2022 international consensus criteria for optic neuritis (ICON) criteria, and the impact of adding visual evoked potentials (VEPs) and new clinical definitions to improve diagnostic accuracy. METHODS: We introduced modifications to the ICON 2022 criteria, including a less strict clinical category allowing ON diagnosis with incomplete clinical features and the incorporation of VEPs as an additional paraclini

Life-threatening multisystem sarcoidosis with neurological, renal, ocular and autoimmune haemolytic manifestations treated with intravenous immunoglobulin, plasma exchange, cyclophosphamide and infliximab.

Multisystem sarcoidosis is uncommon and presentation as a stroke mimic with severe renal impairment and concurrent haemolysis has not previously been described. Evidence for the use of plasma exchange (PLEX) in sarcoidosis is limited. This case describes a patient with multisystem sarcoidosis who presented with encephalopathy, acute tubulointerstitial nephritis, optic neuritis and autoimmune haemolytic anaemia. Extensive immunological investigations were non-diagnostic. The patient's neurological and haematological manifestations improved rapidly with intravenous methylprednisolone, intravenou

Diagnostic Value of Pupil Involvement and Yield of Vascular Imaging in Isolated Third Cranial Nerve Palsy: A 10-Year Retrospective Study.

BACKGROUND: Isolated third cranial nerve palsy may result from presumed microvascular ischemia or clinically significant structural lesions. The reliability of pupil involvement for distinguishing non-microvascular causes remains uncertain. OBJECTIVE: To describe the etiologic spectrum of isolated third cranial nerve palsy and evaluate the diagnostic performance of pupil involvement and the yield of neuroimaging. METHODS: This retrospective observational study reviewed adult patients with isolated third cranial nerve palsy at Mukdahan Hospital, Thailand, between January 2015 and August 2025.

COVID-19-Induced Bilateral Sensorineural Hearing Loss in Cogan's Syndrome: A Case of Successful Cochlear Implantation in a Pediatric Patient.

Cogan's syndrome (CS) is a rare autoimmune disorder that can cause profound sensorineural hearing loss by affecting the eyes and inner ears. A case report of a 12-year-old patient with CS who underwent successful cochlear implantation is presented. The patient developed bilateral hearing loss after a COVID-19 infection and showed no improvement with multiple medical therapies. Radiological evaluation revealed round window ossification and decreased fluid intensity in the cochlea and vestibule, consistent with inner ear inflammation and early-stage ossification. Following bilateral simultaneous

Ciltacabtagene autoleucel in high-risk smoldering multiple myeloma: the CAR-PRISM phase 2 trial.

High-risk smoldering multiple myeloma (HR-SMM) carries an increased risk of progression to multiple myeloma, making it an ideal setting to test whether chimeric antigen receptor (CAR) T cell therapy can achieve curative outcomes. Here in this phase 2 study, patients with HR-SMM received ciltacabtagene autoleucel (cilta-cel) at 0.3-0.5 × 106 or >0.5 × 106 viable CAR+ T cells per kilogram without induction or bridging therapy. Patients with >40% marrow involvement were excluded. Primary endpoints were dose-limiting toxicities (DLTs) and treatment-emergent adverse events; secondary endpoints incl

[Analysis of vestibular function characteristics in vestibular schwannomas at different stages: two case reports and literature review].

Vestibular schwannoma(VS) grows slowly, with a striking contrast in clinical manifestations at different stages. During the growth phase of VS, it often presents as an acute vestibular syndrome similar to vestibular neuritis, with prominent vestibular symptoms such as vertigo and balance disorders. However, with the slow growth of the tumor, central vestibular compensation develops promptly, resulting in the absence of typical vertigo indicative of unilateral vestibular injury, which may be inconsistent with vestibular test findings. This paper reports two cases of VS at different disease stag

Vestibular Schwannoma in the Precision Medicine Era: An Updated Narrative Review of Molecular Insights, Imaging Biomarkers, and Evolving Management Strategies.

Vestibular schwannoma (VS) care is increasingly shaped by molecular and imaging advances that complement conventional size- and symptom-based decision-making. Recent multi-omic studies, including single-cell and spatial transcriptomics and methylation profiling, reveal biologically distinct Schwann cell states and microenvironmental programs that may help explain heterogeneous growth, hearing decline, and treatment response. In parallel, quantitative magnetic resonance imaging, radiomics, and early radiogenomic efforts are moving toward standardized, Image Biomarker Standardisation Initiative-
