Cancer predisposition syndromes: an imaging review.

BACKGROUND: Cancer predisposition syndromes (CPSs) are inherited disorders that increase the risk of developing cancer from childhood through adulthood. They account for up to 10% of pediatric tumors, making early recognition important for reducing morbidity and mortality. Because these syndromes show variable penetrance and a wide range of clinical presentations even within the same family, identifying affected children can be challenging. MAIN BODY: Imaging is an essential tool for diagnosis, surveillance and follow-up of children with cancer predisposition syndromes. In this review we summa

Pediatric Cranial Nerve Palsies.

Cranial nerve palsies in children offer unique challenges distinct from those in adults, and typically arise from congenital, traumatic, neoplastic, or postinfectious inflammatory disease. With rare reports of diplopia, diagnosis depends on indirect signs such as abnormal head posture, strabismus, or abnormal gaze. The oculomotor (III), trochlear (IV), and abducens (VI) nerves follow sometimes long, intricate courses from the brainstem to the target muscle(s) within the orbit. Accurate diagnosis requires integrating anatomic understanding with subtle clinical presentations and imaging findings

Spatiotemporal Heterogeneity in Isolated Neurosarcoidosis: Diagnostic Insights from Multimodal Imaging.

Isolated neurosarcoidosis (INS) is a rare inflammatory disorder of the central nervous system that lacks systemic manifestations and mimics neoplastic or infectious conditions, making diagnosis challenging. We report a case of a previously healthy 30-year-old man who presented with recurrent transient loss of consciousness and progressive right leg weakness. Brain magnetic resonance imaging (MRI) demonstrated multiple uniformly contrast-enhancing nodular lesions involving the pachymeninges and cranial nerves, while spinal MRI revealed conus medullaris enhancement with associated lumbosacral ne

Transcriptomic meta-analysis identifies dysregulated pathways and potential therapeutic targets in Vestibular Schwannoma.

Vestibular schwannoma (VS) is a benign Schwann cell-derived tumor that frequently causes progressive hearing loss and vestibulocochlear dysfunction, substantially impacting quality of life. The molecular mechanisms underlying VS pathobiology remain poorly defined, and reliable biomarkers or targeted therapies are lacking. This study aimed to delineate the molecular landscape of VS through a transcriptome-wide meta-analysis. We performed a genome-wide random-effects meta-analysis of four independent Affymetrix microarray datasets from the Gene Expression Omnibus (GEO) database. Differential exp

IgG4-related disease involving facial, infraorbital, auriculotemporal and optic nerves: multinervous cranial involvement in an adult patient.

IgG4-related disease (IgG4-RD) is a rare fibroinflammatory condition that can mimic infectious or neoplastic diseases, leading to diagnostic challenges. We report a case of a man in his mid-40s presenting with rapidly progressive orbital symptoms initially diagnosed as orbital cellulitis. Despite appropriate surgical and medical management, the patient's condition worsened with multiple cranial nerve involvement.MRI revealed an infiltrative process with perineural extension involving the optic, infraorbital, facial and auriculotemporal nerves. Histopathological examination confirmed IgG4-RD, d

Chemotherapy-associated Optic Neuropathy in a Patient with Chronic Myeloid Leukemia and Hepatitis B: A Diagnostic Challenge.

Optic neuritis is a sight-threatening condition marked by inflammation or dysfunction of the optic nerve, typically presenting with sudden, often painful vision loss in one eye. While idiopathic and demyelinating forms are most common, optic nerve damage related to systemic chemotherapy remains an underreported but clinically significant entity. Patients undergoing intensive cancer treatment - particularly those with compromised immunity - are vulnerable to optic neuropathy from direct neurotoxic effects, opportunistic infections, or malignancy-related infiltration. We describe the case of a 3

Spatial mapping of immune cell environments in NF2-related schwannomatosis vestibular schwannoma.

NF2-related Schwannomatosis (NF2 SWN) is a rare disease characterised by the growth of multiple nervous system neoplasms, including bilateral vestibular schwannoma (VS). VS tumours are characterised by extensive leucocyte infiltration. However, the immunological landscape in VS and the spatial determinants within the tumour microenvironment that shape the trajectory of disease are presently unknown. In this study, to elucidate the complex immunological networks across VS, we performed imaging mass cytometry (IMC) on clinically annotated VS samples from NF2 SWN patients. We reveal the heterogen

Uncommon Non-MS Demyelinating Disorders of the Central Nervous System.

PURPOSE OF REVIEW: Definitive diagnosis of multiple sclerosis (MS) requires exclusion of other central nervous system (CNS) disorders sharing similar clinical, pathological and radiological features. In this review we discuss some relatively uncommon disorders with special emphasis on their differentiation from MS clinically and radiologically. While most conditions have a demyelinating pathology, a few very important mimics may have a non-demyelinating pathology to merit some discussion. RECENT FINDINGS: Two major areas of diagnostic advances have been made in recent times, the recognition o

Optic neuropathy in imaging.

The aim of this work is to provide a diagnostic approach to the potential causes of optic neuropathy, focusing on the radiological findings associated with this pathology. Various etiologies have been identified, including inflammatory and demyelinating optic neuritis, developmental and hereditary diseases, neurodegenerative disorders, infectious conditions, post-traumatic causes, ischemic optic neuropathy (with anterior ischemic optic neuropathy being the most common form), and neoplastic etiologies. Optical coherence tomography and magnetic resonance imaging play a fundamental role in the di

Development of a Panel of Biomarkers for Differential Diagnosis of Multiple Sclerosis.

Demyelinating diseases are a group of heterogeneous pathologies that affect the nervous system and reduce the quality of life. One of such diseases is multiple sclerosis (MS), an inflammatory autoimmune neurodegenerative disease of the central nervous system (CNS). At the initial stages, MS can mimic some infectious, neoplastic, genetic, metabolic, vascular, and other pathologies. Accurate differential diagnosis of this disease is important to improve the quality of life of patients and reduce possible irreversible damage to the central nervous system. In this work, we confirmed the possibilit

The sellar region as presenting theater for hematologic malignancies-A 17-year single-center experience.

Hematological neoplastic mass lesions of the sellar region are rare. We identified five cases of hematological malignancy with first presentation in the sellar region from our departmental database of 1,405 patients (0.36%) with sellar lesions diagnosed over the 17-year period (2005-2021). All patients were females (mean age 55.2 ± 3.4 years). One patient had multiple myeloma (MM), one patient had acute myeloid leukemia (AML), while three other patients had lymphoma (intravascular lymphoma (IVL, n = 1) or non-Hodgkin's lymphoma (NHL, n = 2). Most patients presented with ophthalmoplegia, and on

Incidence and morphology of secondary TDP-43 proteinopathies: Part 1.

Transactive response DNA binding protein of 43 kDa (TDP-43) is considered to play an essential role in the pathogenesis of frontotemporal lobar degeneration and amyotrophic lateral sclerosis. Growing body of evidence indicate that pathological TDP-43 inclusions frequently occur in the context of other distinctive hallmark pathologies, referred to as secondary TDP-43 proteinopathies. Comorbid TDP-43 pathology is well-documented in several neurodegenerative disorders, including Alzheimer's disease, Parkinson's disease, multiple system atrophy, or progressive supranuclear palsy. It may also appea

Could hyperlipidemia be a risk factor for corticobasal syndrome? - a pilot study.

INTRODUCTION: Corticobasal syndrome (CBS) is a specific clinical manifestation shared by multiple pathologies. The exact mechanism of this phenomenon remains unclear. Differential diagnosis of CBS in everyday clinical practice is challenging, as this syndrome can overlap with other entities, especially progressive supranuclear palsy Richardson-Steele phenotype (PSP-RS). Several papers have suggested a possible role of vascular pathology as a linking factor in the pathogenesis of CBS based on different neuropathologies. This paper analyses differences in the occurrence of the most common vascul

Giant Cell Arteritis Presenting With Multiple Cranial Neuropathies - Case Report.

Background: Vision loss accounts for most ophthalmic presentations of giant cell arteritis (GCA), but an important minority of patients present with diplopia and other cranial neuropathies. Case study: Here we present the case of an 84-year-old woman with a prior history of multiple cancers who was admitted to our hospital after developing double vision. She was found to have mydriasis, ptosis, and ophthalmoplegia in the right eye (OD) consistent with a combined R CNIII/CNVI neuropathy, as well as highly elevated inflammatory markers. Given her cancer history, the patient was initially worked

Opsoclonus Following Downbeat Nystagmus in Absence of Visual Fixation in Multiple System Atrophy: Modulation and Mechanisms.

We report atypical opsoclonus in a patient with multiple system atrophy and propose a mechanism based on the patterns of modulation by visual, vestibular, and saccadic and vergence stimulation. Firstly, the 6-Hz opsoclonus mostly in the vertical plane occurred only after the development of downbeat nystagmus in darkness without visual fixation. Even after a substantial build-up, visual suppression of the opsoclonus was immediate and complete. Furthermore, the latency for re-emergence of opsoclonus in darkness was greater when the duration of preceding visual fixation was longer. Secondly, the

Developmental delay with hypotrophy associated with homozygous functionally relevant REV3L variant.

REV3L encodes a catalytic subunit of DNA polymerase zeta (Pol zeta) which is essential for the tolerance of DNA damage by inducing translesion synthesis (TLS). So far, the only Mendelian disease associated with REV3L was Moebius syndrome (3 patients with dominant REV3L mutations causing monoallelic loss-of-function were reported). We describe a homozygous ultra-rare REV3L variant (T2753R) identified with whole exome sequencing in a child without Moebius syndrome but with developmental delay, hypotrophy, and dysmorphic features who was born to healthy parents (heterozygous carriers of the varia

A Rare Case of Epidural Myeloma Presenting as Recurrent Subdural Bleeding.

Multiple myeloma (MM) is an incurable clonal B-cell malignancy that usually presents with neoplastic monoclonal plasma cells in either bone or soft tissues. Central nervous system involvement of the myeloma (CNS-MM), such as dural myeloma or intraparenchymal infiltration, or diffuse leptomeningeal involvement, is uncommon. Dural involvement of myeloma without parenchymal or leptomeningeal disease is even rarer, with only seven cases reported previously. We present a case of epidural myeloma in a 50-year-old man with known kappa light chain MM, presenting with multiple episodes of subdural hemo
