TAC-Tic syndromes: a case series on the complex overlap between trigeminal autonomic cephalalgias and trigeminal neuralgia.

INTRODUCTION: Trigeminal autonomic cephalalgias (TACs) and trigeminal neuralgia (TN) are classified as distinct disorders, although overlap syndromes are increasingly recognized. CASE REPORTS: We retrospectively describe five women with TAC-tic overlap: two with SUNCT-tic, two with cluster-tic, and one with paroxysmal hemicrania-tic syndrome. Each had distinguishable neuralgic and autonomic pain phenotypes, appearing either simultaneously or sequentially. In both SUNCT-tic cases, repeat imaging disclosed neurovascular compression after an initially negative study; microvascular decompression

Ear-Worn Inertial Measurement of Sit-to-Stand and Sit-to-Walk Movements: Algorithm Development and Validation.

BACKGROUND: Sit-to-stand and sit-to-walk are common functional movements that provide insight into an individual's functional mobility. Developing comprehensive measures of these movements using ear-worn inertial measurement units (IMUs), which are increasingly incorporated into widely used devices such as hearing aids and headphones, could provide a means of monitoring older adults and individuals with movement disorders with minimal additional burden. OBJECTIVE: This paper aimed to introduce comprehensive outcome measures for sit-to-stand and sit-to-walk movements based on data from ear-wor

[Decision-Making for Cervical Spine Lesions: An Orthopedic Surgeon's Perspective].

In patients presenting with numbness, sensory disturbances, or motor deficits in the upper extremities, it is important to differentiate between cervical spinal cord or nerve root disorders and peripheral neuropathies. This article discusses the key points for distinguishing C8 spinal cord segment and nerve root disorders from peripheral neuropathies, particularly ulnar neuropathy and posterior interosseous nerve palsy. A comprehensive evaluation of the patterns of muscle weakness, distribution of sensory disturbances, and electrophysiological findings is essential, with careful consideration

Lumbar Epidural Steroid Injection

Lumbar epidural steroid injections (LESIs) are minimally invasive, image-guided procedures used to treat lumbosacral radicular pain by depositing corticosteroid, with or without local anesthetic, into the epidural space. Radicular pain results from inflammation, irritation, or compression of a lumbar or sacral nerve root. This pain commonly presents in the low back and radiates into the buttock or lower extremity in a dermatomal distribution. Lumbar disc herniation is the most common cause; degenerative lateral recess or foraminal stenosis, spondylolisthesis, postoperative epidural fibrosis, a

[Expert consensus on combined periodontal and endodontic management of multiple idiopathic cervical root resorption].

Multiple idiopathic cervical root resorption (MICRR) is a rare, idiopathic condition characterized by multiple resorptive lesions in the cervical region of teeth. It manifests as progressive, painless hard tissue loss, often accompanied by periodontal tissue destruction, which may ultimately lead to tooth fracture, loosening, or even exfoliation. In early stages, MICRR is typically asymptomatic and is often incidentally detected through imaging as characteristic "concave" resorptive defects in the cervical area. Currently, no specific treatment is available. The rarity, insidious nature, invas

Zeylenone-mediated inhibition of TIMP-1-induced Bax/Bcl-2/Cle-caspase-3 expression and apoptosis via FAK-PI3K/AKT pathway regulation in thyroid cancer cells: in vitro and in silico study.

Thyroid cancer, a prevalent endocrine malignancy, has shown a significant global rise in incidence over the past 15 years and currently ranks as the sixth most common cancer worldwide, with higher prevalence among women. Dysregulation of the TIMP-1-induced FAK/PI3K/AKT signaling pathway plays a pivotal role in disease progression, highlighting its potential as a therapeutic target. This study investigated the anticancer effects of zeylenone in thyroid carcinogenesis, based on its reported inhibition of TIMP-1-mediated FAK/PI3K/AKT signaling in tumor cells. The ability of zeylenone to suppress

Pathogenic Characteristics of Berkeleyomyces basicola Causing Black Root Rot of Carrot.

The pathogenic soilborne and postharvest fungus Berkeleyomyces basicola, a newly reported pathogen in Serbia, caused significant disease symptoms on carrot roots and seedlings in inoculation assays. In October 2023, machine-washed and cold-stored carrot roots showed symptoms of black rot of patches and abundant sporulation. The influence of the postharvest treatment of machine washing was confirmed by additional sampling at the production site. The collected roots were machine or manually washed and stored under similar conditions as during shelf life (cold storage followed by incubation at ro

Transcriptome of the human C2 dorsal root ganglia in C1-2 arthrodesis surgery: insight for neck pain.

Neurons in the dorsal root ganglion (DRG) receive and transmit sensory information from the tissues they innervate and from the external environment. Upper cervical (C1-C2) DRGs are functionally unique as they receive input from the neck, head and occipital cranial dura, the latter two of which are also innervated by the trigeminal ganglion (TG). The C2 DRG also plays an important role in neck pain, a common and disabling disorder that is poorly understood. Advanced transcriptomic approaches have significantly improved our ability to characterize RNA expression patterns at single-cell resoluti

Stem cell therapy for osteoradionecrosis of the jaws: mechanisms, advances, and clinical potential.

Osteoradionecrosis of the jaws (ORNJ) is a severe aseptic complication of high-dose radiotherapy for head-and-neck cancers, characterized by chronic jawbone necrosis, functional impairment, and poor responses to traditional treatments (eg, hyperbaric oxygen and surgical resection) that fail to address its root pathophysiology (microcirculatory impairment, bone metabolism dysfunction). Its incidence is 1.2%-40% in patients receiving >60 Gy radiotherapy, especially with concurrent trauma. In recent years, stem cell therapy has garnered attention as a potential treatment for a variety of bone-rel

[Research progress on external cervical resorption].

External cervical resorption (ECR) is a progressive, osteoclast-mediated destructive disease of dental hard tissues that originates at the cervical region of the root. Characterized by its insidious onset and diagnostic challenges at early stages, ECR can lead to severe tooth structure loss and even tooth extraction. Although the precise patho-genesis remains unclear, it is thought to be associated with local stimuli, certain medications, and systemic conditions. Cone beam computed tomography (CBCT) is currently the key imaging modality for three-dimensional assessment and accurate staging of
