Anterior cervical discectomy without fusion: long-term results.

The long-term follow-up of patients operated by one surgeon by simple anterior discectomy without fusion have been presented. The results are much better in patients with soft extruded discs than with painful discs or degenerative discs with varying degrees of radiculopathy or myelopathy. The same statement can be made for the posterior approach. The anterior approach appears to be superior for soft central extruded discs compressing the spinal cord. There appears to be a greater incidence of another disc at an adjacent level with the anterior approach than when the operation is done posterior

The surgery of cervical disk disease: new perspectives.

Current trends in the surgical treatment of cervical disk disease, when compared with those of the 1960s, have been unimpressive. There is an ongoing refinement of anterior procedures, which seem to be effective for both myelopathy and radiculopathy, ranging from simple diskectomy and/or fusion to variously radical anterior decompressions. This refinement has demonstrated that however simple or radical the anterior procedure is, the results seem to be the same and that bone grafting is not necessarily critical to success. Posterior procedures, both for root exploration and spinal cord decompre

Ill effects of improper oral hygeine procedure.

Improper use of the tools that prevent tooth decay and gingival disease can also produce harmful effects. For example, dental floss can cause inflammation; toothbrushes could cause abscesses; and water-irrigation devices might drive foreign material into soft tissue. This report discusses the effects of incorrect oral hygiene, the signs the practioner should notice, and the proper corrective steps. Although effective oral hygiene is essential for the maintenance of healthy teeth and supporting tissues, analysis of the literature and clinical observation suggested that six problem areas may be

The hypermineralization of diseased root surfaces.

Root surfaces which have lost their attachment due to chronic inflammatory periodontal disease present significant increases in calcium, magnesium, phosphorus, and fluoride. The clinical significance of this effect is not known in regards to wound healing, but it may be helpful in control of cervical sensitivity.

Breast pain: a symptom of cervical radiculopathy.

Eighteen women, all of whom had extensive but noninformative breast evaluations, including 10 mammograms and 4 biopies, were successfully treated by cervical traction for chronic breast pain. Each patient had distinct clinical or electromyographic evidence of cervical root compromise. Fifteen had roentgenographic evidence of cervical spondylosis, primarily at levels C6 and C7. Cervical angina, as a symptom constellation produced by cervical radiculopathy and mimicking coronary ischemic disease, is a well-defined entity. Less well recognized is persistent breast pain as a primary presenting sym

The clinical spectrum of ankylosing spondylitis.

Ankylosing spondylitis is more common in young men than in young women and is insidious in onset. Typically, patients complain of pain and stiffness around the sacroiliac region. At the disease progresses, pain is often felt in the mid-lumbar, thoracic and cervical regions resulting in a significant reduction in the range of motion of the entire spine. About one fourth of the patients display involvement of proximal synovial joints. Acute anterior uveitis may precede AS or can occur when the disease is otherwise in apparent remission. Each patient with established disease should be carefully e

Cysts and sinuses of the face resulting from dental abscesses.

Cysts and sinuses of the face and neck should always bring to mind the possibility of causative underlying dental disease. This should be investigated by history, physical examination and appropriate roentgenograms of the teeth. Abscesses of the teeth are the most common dental causes for the suspicious facial mass or sinus tract. Appropriate management includes extraction or root canal therapy of the diseases tooth. Any residual facial scar can later be excised and reconstructed as appropriate.

Total myelography in the evaluation of lumbar discs. With the presentation of three cases of thoracic neoplasms simulating nerve root lesions.

It has been our practice to perform total myelography in all cases of lumbar disc disease including the various syndromes of spinal stenosis, because experience has indicated that myelographic abnormalities in the cervical and thoracic areas could have clinical importance despite the presence of gross changes in the lumbar region. Failure to establish a definite diagnosis by restricted lumbar myelography makes it mandatory to evaluate all levels of the spinal axis. Recently, 3 patients with thoracic spinal cord tumors presented with primary signs and symptoms of lumbar spine disorders. Evidenc

Quantitative studies of dorsal root ganglia and neuropathologic observations on spinal cords in familial dysautonomia.

Intrauterine development and postnatal maintenance of dorsal root ganglion neurons are abnormal in familial dysautonomia, an autosomal recessive disorder associated with autonomic, motor and sensory deficits. Normally, dorsal root ganglion weight increases with age. This does not occur in the cervical plexus ganglia of dysautonomic patients. Neurons in dorsal root ganglia are found to be markedly diminished in the youngest patients and slow degeneration causes further depletion with age. Quantitative studies on C8 dorsal root ganglia show the normal neuron content to be between 42,500 and 53,6

A long term study of root retention in the treatment of maxillary molars with furcation involvement.

A study was done to evaluate long-term results of treating 387 maxillary molars with furcation involvement in 100 patients with chronic destructive periodontal disease. Results showed a favorable long term functional survival rate of 341 teeth (88%) 5 to 24 years after treatment, despite the fact that many teeth had at least one root with 50% or less of bone support before treatment. Of the 46 teeth (12%) that were ultimately extracted, 25 were present for a significant length of time (6 to 18 years) before extraction. The radiographs of 292 teeth (75%) showed no significant change in bone sup

[The value of lateral views of the cervical spine in the diagnosis of chronic cervical myelopathy (author's transl)].

Factors in the production of chronic cervical myelopathy are considered to be developmental narrowing of the cervical spinal canal and the development of osteochondrotic changes. Lateral view of the cervical spine are a valuable survey method to be used before employing more invasive techniques. In four out of five patients with chronic cervical myelopathy, the quotient of lateral spinal to vertebral diameter was less than 0.8. In a control group this was found in only one out of four patients. Conversely, a quotient below 0.8 in men and 0.9 in women is associated with chronic cervical myelopa

The cervical myelopathy associated with rheumatoid arthritis: analysis of patients, with 2 postmortem cases.

The clinical findings in a group of 32 rheumatoid patients with symptomatic myelopathy are described. The myelopathy appeared in late middle age after many years of rheumatoid disease (average duration, 18 years). The most common radiological abnormality was anterior subluxation of C1. Other patterns occurred in about half the cases. The most common neurological findings were those of a mild to moderate spastic paraplegia, often with atrophy of muscles of the hands and sensory changes in the hands. Symptoms originating above the foramen magnum (nystagmus, diplopia, slurred speech) appeared to

Cervical myeloradiculopathy caused by arthrotic hypertrophy of the posterior facets and laminae.

Five patients with typical cervical myeloradiculopathy caused by focal cervical spinal stenosis are presented. Dorsal intrusions into the spinal canal by hypertrophied apophyseal joints and thickened laminae resulted in cord and nerve root compression. Minor spondylotic changes were present in the floor of the spinal canal. Laminar decompression with formainotomy and facetectomy relieved the patients of their symptoms. An anterior approach should not be considered in the management of this disorder. Our findings of severe apophyseal arthrosis with lesser degrees of associated spondylosis are s

Differential patterns of elastic fibers in scarring and non-scarring alopecias.

Elastic fiber stain (acid alcoholic orcein) reveals diagnostically significant differences between several types of alopecia of the scalp. A short outline of elastic fiber distribution on the normal hair follicle emphasizes the elastic coat of the follicular isthmus, the sparsity of elastic fibers on the cyclic lower portion of the hair root, and the presence in the neck of the dermal papilla of an elastin-like body which is formed anew with each hair cycle. This body provides a marker of the gradual shortening of successive anagen hairs in male pattern alopecia. Patterns of elastic fibers in

[The Sudeck-Leriche syndrome as a disturbance in distant regions of the body, clinical picture, and histology (author's transl)].

On the whole, every Sudeck-Leriche syndrome represents a serious complication. The causal noxae are various in nature. In a large case material during a period of observation extending over 26 years a Sudeck-Leriche syndrome was observed as a disturbance in distant regions of the body only in rare cases, for example after herpes zoster, apoplexy, and confusion of the cervical part of the medulla, with cervical and lumbal root irritations, etc. Histological findings in the case of Sudeck-Leriche syndrome are very rarely presented in literature. Histological investigations by the author carried

Cervical osteoid osteoma: a cause of chronic upper back pain.

A case of an osteoid osteoma occurring in the cervical spine and presenting as scapular and arm pain has been described. Correct diagnosis was delayed for 2 years. Plain radiographs may fail to demonstrate a lesion in the spine. Radiographic evaluation, when osteoid osteoma is a possibility, should include bone scanning. If the bone scan is abnormal, tomography of the area is indicated. If the diagnosis is still unclear, selective angiography may reveal the tumor blush typical of osteoid osteoma. In certain instances, a myelogram may be helpful to establish whether there is encroachment on the

[Evaluation of lymphography in neurologic complications of neoplastic diseases (author's transl)].

1. By lymphographic examination, first of all, the primary and secondary tumours of the lymphatic system may be revealed and localized. Further on, it makes possible to state the stage of the disease and thus to apply the appropriate treatment. 2. In case of symptoms indicating that nerve-roots of unknown origin have been damaged, the result of lymphographic examination may call the attention to pathomechanism related to some tumour-disease. If the symptoms of the nervous system are caused by compression or infiltration of a tumour, most frequently the presence of pathologic lymph nodes in the

Microsurgery of Arnold-Chiari malformation in adults with and without hydromyelia.

Microsurgical exploration of 15 adults with Arnold-Chiari malformation with and without hydromyelia using 3 to 20 X magnification has led to the following conclusions. Hydromyelia, associated with Arnold-Chiari malformation, is a progressive mechanical disorder that causes spinal cord deficits by pressure distention of the cord. Arnold-Chiari malformation causes slowly or suddenly progressive bulbar dysfunction by impaction of the malformation in the foramen magnum. Decompression of both can be achieved by a suboccipital carniectomy, upper cervical laminectomy, establishing an outlet from the

Sympathetic disorders in obstetrical paralysis of the newborn.

In addition to the classical description of involvement of the cervical sympathetic in low root lesions (Claude Bernard-Horner syndrome), other sympathetic syndromes have been described accompanying the motor and sensory changes in obstetrical paralysis of the upper limb. Loss of sweat secretion can be deomonstrated by electrical skin resistance tests, and the authors describe their apparatus and method of investigating this in the newborn. Although sweat secretion is often scanty in the newborn, it can be enhanced with appropriate techniques, particularly in the areas richer in sweat glands.
