A modified David V: Technique and outcomes from a large single centre experience.

IntroductionReplacement of the aortic root with a composite valve-graft and coronary reimplantation is the most widely performed operation for patients requiring aortic root replacement; however valve sparing operations have been developed to preserve the aortic valve in certain populations. Since they were first introduced, the remodeling and reimplantation techniques have undergone many refinements and modifications. We describe the technical steps and outcomes of a modified David V procedure, adapted from the Stanford modification, performed at our institution.MethodsBetween November 2007 a

Assessing changes in aortic motion and hemodynamics after valve-sparing aortic root surgery in Marfan syndrome using four-dimensional balanced steady-state free precession and four-dimensional flow cardiovascular magnetic resonance.

BACKGROUND: Aortic root surgery in patients with Marfan syndrome (MFS) has significantly improved survival, allowing time for distal aortic complications such as type B aortic dissection (TBAD) to emerge. The implantation of a non-compliant synthetic graft may alter aortic biomechanics and hemodynamics, potentially contributing to these late complications. This proof-of-concept study aimed to assess changes in aortic motion and flow characteristics following aortic root surgery using advanced cardiovascular magnetic resonance (CMR) techniques. METHODS: Three MFS patients (aged 26-37 years), t

Unintended Impact of Left-Shifting Aortic Guidelines on Aortic Valve Interventions.

BACKGROUND: This study examines the impact of lowering size threshold for intervention of aortic root or ascending thoracic aortic aneurysm (ATAA) on the fate of the aortic valve. METHODS: The Treatment in Thoracic Aortic aNeurysm: Surgery vs Surveillance (TITAN:SvS) surgical registry is a multicenter prospective registry of patients who underwent surgery for ATAA between 5.0 and 5.4 cm at expert aortic centers. Echocardiographic, procedural, and outcome data were prospectively collected. Multivariable analyses examining predictors of aortic valve replacement (AVR) included demographic, preop

Rehabilitation management for a dysfunctional/non-painful (DN) joint causing low back disorder in an elite athlete: A case report.

Low back pain in athletes can arise from asymptomatic dysfunctional/non-painful (DN) joints, which cause compensatory stress on adjacent regions. This report highlights the clinical importance of identifying and correcting these underlying 'DN joints,' integrating concepts from the Selective Functional Movement Assessment and the joint-by-joint approach to prevent recurrence. A 25-year-old female professional volleyball player underwent surgery for L5 radiculopathy due to disc herniation. Postoperatively, a functional assessment revealed DN joints:restricted mobility in her right glenohumeral

Single Anastomosis Duodeno-Ileal Bypass with Sleeve Gastrectomy Prevents Atherosclerosis Development in LDL Receptor Deficient Mice.

INTRODUCTION: Bariatric surgeries improve cardiovascular health through modulation of plasma lipid profiles. Among these procedures, single-anastomosis duodeno-ileal bypass with sleeve gastrectomy (SADI-S) yields marked metabolic benefits; however, its impact on atherosclerosis has not yet been defined. This study aimed to assess the impact of SADI-S on atherosclerosis development in a mouse model of atherosclerotic disease. METHODS: Low-density lipoprotein receptor–deficient (Ldlr−/−) male mice were fed a high-fat diet for 12 weeks and subsequently underwent either SADI-S or sham laparotomy.

Accessory Dural Sleeve: Imaging Appearance on Lateral Decubitus CT Myelography and Relationship to CSF-Venous Fistulas.

BACKGROUND AND PURPOSE: Cerebrospinal fluid-venous fistulas (CVFs) are a major cause of spontaneous intracranial hypotension (SIH) and are best detected with lateral decubitus-CT myelography (LD-CTM) and lateral decubitus-digital subtraction myelography (LD-DSM). On LD-CTMs, we have often noticed an accessory dural sleeve (ADS) separate from the nerve root sleeve, a previously underrecognized imaging finding. The purpose of this study is to describe the imaging appearance of the ADS and its relationship to CVFs. MATERIALS AND METHODS: Retrospective single-center review of LD-CTM examinations

Lip piercing-related prosthetic aortic graft infection and mediastinitis in filaminopathy: A case report.

We report a rare case in which a lip piercing led to Staphylococcus aureus bacteremia and subsequent prosthetic aortic graft infection complicated by anterior mediastinitis in a patient with filaminopathy A. A 32-year-old woman, who had undergone valve-sparing aortic root and arch replacement with a hybrid thoracic stent graft 5 years earlier, presented 3 weeks after a lip piercing with fever, chest pain, and elevated inflammatory markers. Computed tomography revealed a periprosthetic fluid collection and mediastinal inflammation consistent with graft infection. Blood cultures grew methicillin

Surgical Approaches for Vascular-Related Aerodigestive Compression.

Aerodigestive compression syndromes of vascular origin can be broadly classified based on congenital and acquired causes. Congenital causes include various types of arch anomalies (vascular rings including double arches, right aortic arch with aberrant left subclavian artery, circumflex aorta, etc.), arch branching anomalies (left aortic arch with aberrant right subclavian artery, innominate artery-related compression), aneurysms of either the aorta or main pulmonary artery or branch pulmonary arteries, and any combinations of these. Acquired causes of aerodigestive compression are mostly post

Strategies for Aortic Root Measurement in Patients Undergoing Surveillance for Thoracic Aortic Disease.

Objectives: Several measurement techniques have been proposed to address the non-circular geometry of the aortic root. The Laplace diameter metric incorporates the cloverleaf anatomy of the aortic root and is derived via measurement of sinus-to-commissure lengths with subsequent doubling of the largest radius from the center. This study compares the conventional sinus-to-sinus with the novel Laplace method for sizing the aortic root and quantifying its implication on surgical decision-making. Methods: Patients undergoing surveillance at a high-volume aortic center were categorized by aortic ro

Development and validation of a prediction model for Haller index based on posteroanterior and lateral chest radiographs in pectus excavatum.

BACKGROUND: Computed tomography (CT) is the gold standard for assessing the severity of pectus excavatum; however, it involves high radiation exposure and substantial costs. This study aimed to develop a statistical model based on measurements from posteroanterior and lateral chest radiographs (2D) to derive a novel X-ray Index (XI) for predicting the CT-derived Haller Index, thereby reducing the need for CT examinations in a subset of patients. METHODS: A total of 305 patients with pectus excavatum scheduled for Nuss procedure were retrospectively enrolled. All patients underwent both poster

New Frontiers in the Management of Aortic Disease.

The aorta has multiple functions within the cardiovascular system and disease progression remains incompletely understood. Diseases of the aorta cause substantial morbidity and mortality. This review summarizes the management of thoracic aortic disease, current knowledge gaps, and future opportunities. There is an increasing effort to incorporate aortic function, beyond diameter, into risk stratification for aortic disease. Medical therapies focus on slowing growth of aortic aneurysm, minimizing perioperative risk, and reducing overall cardiovascular mortality, focusing on treating hypertensio

Targeted microbubble cavitation has no therapeutic effect on atherosclerotic plaques of the aorta in diabetic ApoE-/- mice.

AIMS: Diabetes mellitus (DM) increases plaque instability via inflammation. Ultrasound-activated microbubble cavitation has previously reduced intra-plaque neovascularisation in non-DM models. Here we studied how ultrasound-activated microbubbles targeted against intercellular adhesion molecule-1 and CD90 affect atherosclerotic plaques in a DM model. METHODS: DM was induced in C57BL/6 ApoE-/- mice via streptozotocin injection. Sixteen weeks later, either 5 × 107 targeted microbubbles or the vehicle was applied, followed by 1 min of 1 MHz pulsed thoracic ultrasound with 100 kPa peak negative a

Repair strategy for acute type A aortic dissection in a regional center.

BackgroundThe optimal extent of repair for DeBakey type I acute aortic dissection remains controversial, particularly in low- to medium-volume centers. We compared outcomes of ascending and arch replacement under a tear-oriented strategy in DeBakey type I dissection.MethodsWe retrospectively analyzed 114 consecutive patients with acute DeBakey type I dissection between April 2008 and July 2025 at a regional cardiovascular center. Ascending aortic replacement was performed in 55 patients, and arch replacement, including both partial and total arch replacement, in 59 patients. The primary endpoi

TAVI and coronary interventions: indications, technical considerations, and clinical scenarios.

Transcatheter aortic valve implantation (TAVI) has become the preferred treatment for severe aortic stenosis. Nonetheless, interplay between aortic valve disease, transcatheter valve implantation, and the coronary arteries is frequent and clinically relevant. Coronary intervention in the context of TAVI encompasses distinct but interrelated scenarios, including the management of concomitant coronary artery disease and the prevention or treatment of coronary artery obstruction. These aspects introduce additional complexity to procedural planning and long-term management. This review provides a

Personalized External Aortic Root Support Before Pregnancy in Heritable Thoracic Aortic Disease.

BACKGROUND: Heritable thoracic aortic disease (HTAD) increases the risk of aortic dissection, particularly during pregnancy and the puerperium. Standard management includes beta-blockers and, in selected cases, prophylactic aortic root surgery. Personalized external aortic root support (PEARS) offers an extravascular approach to aortic root support, but pregnancy data are scarce. This case series describes pregnancies in women with HTAD after PEARS. CASE SUMMARY: Three pregnancies in 2 women with Marfan syndrome and familial thoracic aortic aneurysm and dissection who underwent preconception

Single aortic sinus repair with double annular sutures in acute type A aortic dissection: a single center case series of seven patients.

BACKGROUND: We evaluated a modified surgical technique designed to preserve and restore the aortic root geometry in patients with acute type A aortic dissection (ATAAD) when the intimal tear is limited to a single coronary sinus. METHODS: Between January 2015 and December 2021, 160 patients with ATAAD underwent emergency open replacement of the ascending aorta. Most patients (137, 85%) underwent supracoronary ascending aorta replacement. Thirteen patients underwent aorta root replacement with a valved conduit and three underwent valve-sparing aortic root replacement. The remaining seven patie

Thoracic joint abnormalities and impaired thoracic motion in idiopathic pleuroparenchymal fibroelastosis: an exploratory study using inspiratory and expiratory CT.

BACKGROUND: Idiopathic pleuroparenchymal fibroelastosis (IPPFE) is a rare interstitial lung disease characterized by restricted ventilation and upper-lobe fibroelastosis. Historically, ankylosing spondylitis has been implicated in upper-lobe fibrosis and ventilatory impairment due to chest wall restriction, and impaired thoracic motion has recently been appreciated in IPPFE. We hypothesized that thoracic joint abnormalities (costovertebral, manubriosternal, and sternocostal joints) contribute to the impaired thoracic motion in patients with IPPFE. METHODS: Seventeen patients with IPPFE and 15

Incidentally diagnosed non-dissecting thoracic aortic aneurysm in a dog with suspected aortic valve abnormality.

A thoracic aortic aneurysm (TAA) is a relatively common cardiovascular disorder in humans. However, it is rarely reported in dogs. To date, several reported canine cases are associated with clinical signs and severe complications, such as aortic dissection and rupture. Herein, we report an incidentally diagnosed TAA in an asymptomatic elderly dog with suspected aortic valve abnormality. A 12-year-old spayed female Shiba Inu was referred for the evaluation of anorexia and vomiting. It was subsequently diagnosed with gallbladder mucocele. Thoracic radiography revealed cranial mediastinal widenin

Challenges in the Early Diagnosis, Screening and Management of Heart Failure in Patients with Chronic Obstructive Pulmonary Disease.

In patients with co-morbid CHF and COPD, the diagnosis of CHF can be delayed. It is also well known that left ventricular dysfunction can arise from progressive disease-related hyperinflation. Apart from the longitudinal risk of developing CHF in some patients, a short-term or subclinical risk of cardiac events has been reported after hospitalization for COPD exacerbation. Currently there are no data or strategies to support screening for the early diagnosis of CHF in patients with COPD. Similarly, pulmonary function testing results can also be confounding and inaccurate in establishing the se
