Proximally evoked soleus H reflexes in the evaluation of axonal neuropathy.

It is sometimes difficult to differentiate physiologically distal axonopathy from proximal root or nerve disease. Standard soleus H reflexes evoked by distal tibial nerve stimulation at the popliteal fossa are often absent in both syndromes. In this study we distinguish the two by the presence or absence of soleus H reflexes evoked by proximal sciatic nerve stimulation at the gluteal fold. In 12 normal subjects maximum H reflex amplitudes evoked by distal tibial and proximal sciatic stimulation were essentially equal. In 12 patients with suspected distal polyneuropathy, proximal but not distal

Nerve root hypertrophy as the cause of lumbar stenosis in chronic inflammatory demyelinating polyradiculoneuropathy.

Two patients with chronic inflammatory demyelinating polyradiculoneuropathy (CIDP) developed signs of lumbar stenosis. Radiologic, intraoperative, and pathologic findings demonstrated lumbar stenosis secondary to nerve root hypertrophy. This syndrome has been demonstrated in hereditary but not acquired demyelinating neuropathies. CIDP should be considered in the differential diagnosis of sciatica and nerve root hypertrophy.

Prospective evaluation of MRI lumbosacral nerve root enhancement in acute Guillain-Barré syndrome.

Nerve root enhancement of the cauda equina occurs in Guillain-Barré syndrome (GBS), but the frequency, diagnostic value, and meaning of this finding is unknown. We prospectively obtained gadolinium-enhanced lumbosacral spine MRIs in 24 consecutive patients with acute GBS and blindly rated nerve root enhancement as absent, mild, or prominent. The MRIs were obtained 13 days, mean, after onset of symptoms (range 2 to 42 days). Twenty of 24 patients had cauda equina nerve root enhancement, which was mild in 6 and prominent in 14. Eighteen of 19 with "typical" GBS had enhancement, compared with 2 o

The post-irradiation lower motor neuron syndrome neuronopathy or radiculopathy?

It is not known whether the post-irradiation lower motor neuron syndrome results from radiation damage to motor neuron cell bodies or from damage to the nerve roots of the cauda equina. We studied six cases who had presented with testicular neoplasms, subsequently undergoing irradiation that encompassed inter alia para-aortic nodes with co-irradiation of the distal spinal cord and cauda equina. A predominantly motor disorder affecting the legs ensued after variable and often prolonged latencies (3-25 years). However, all patients also developed mild sensory features either initially or on prol

Massive nerve root enlargement in chronic inflammatory demyelinating polyneuropathy.

OBJECTIVE: To report three patients with chronic inflammatory demyelinating polyneuropathy (CIDP) presenting with symptoms suggestive of cervical (one patient) and lumbar root disease. METHODS: Nerve conduction studies, EMG, and nerve biopsy were carried out, having found the nerve roots to be very enlarged on MRI, CT myelography, and at surgery. RESULTS: Clinically, peripheral nerve thickening was slight or absent. Subsequently one patient developed facial nerve hypertrophy. This was mistaken for an inner ear tumour and biopsied, with consequent facial palsy. Neurophysiological tests sugges

No relationship between epidural fibrosis and sciatica in the lumbar postdiscectomy syndrome. A study with contrast-enhanced magnetic resonance imaging in symptomatic and asymptomatic patients.

STUDY DESIGN: Symptomatic patients were retrospectively analyzed and compared with a control group from an ongoing prospective and consecutive study. OBJECTIVES: To determine the presence and extent of epidural fibrosis in patients with and without recurrent sciatic pain after previous lumbar discectomy, contrast-enhanced magnetic resonance images were evaluated and correlated with surgical findings in the symptomatic patients. Recurrent hernia and bony stenosis were ruled out as the probable causative agent, as well as any morphologic explanation other than fibrosis. SUMMARY OF BACKGROUND D

Charcot-Marie-Tooth disease.

OBJECTIVE: To discuss the similarities and differences in the clinical presentation of Charcot-Marie-Tooth Disease, an inherited peripheral neuropathy, and acquired lumbar spinal stenosis. CLINICAL FEATURES: Patients with lumbar spinal stenosis causing nerve root entrapment often have leg pain and weakness during such activities as walking or standing. Additional symptoms of poor balance, foot deformity and signs of cerebellar and sensory ataxia suggest a diagnosis of peripheral neuropathy rather than nerve root entrapment. INTERVENTION: Electrodiagnostic testing and nerve conduction studies

Multiple protracted cauda equina constrictions cause deep derangement in the lumbosacral spinal cord circuitry in the dog.

Neuropathological changes of the neuronal pools and spinal cord circuitry in the lumbosacral segments were studied in a canine model of multiple protracted cauda equina constrictions. Anterograde degeneration of all sacrococcygeal and L7 dorsal root fibers was detected in S1-S3 and lower lumbar segments. A narrow degenerated gracile fascicle was found in all thoracic and cervical segments terminating in the gracile nucleus. Transneuronal degeneration of middle-sized and large neurons, located in S1-S3 and sporadically in L7 segments, was noted. Identical transneuronal degeneration was seen in

The diagnostic utility of dermatomal evoked potentials.

A study was completed to evaluate the diagnostic utility of dermatomal evoked potentials (EPs) for identifying single and multiple sensory nerve root compromise. We examined the clinical records of 37 patients referred for neck and lower back pain with particular attention paid to presenting clinical symptoms, magnetic resonance (MR) imaging results, and cervical and/or lumbar dermatomal EP results. The primary object of this study was to determine whether or not dermatomal EPs provided useful diagnostic information about nerve root function that would supplement the structural information pro

Neurotrophin-4/5 postpones the death of injured spinal motoneurons in newborn rats.

We have compared the short- and long-term effects of Neurotrophin-4/5 (NT-4/5) on the survival of spinal motoneurons in neonatal rats. To this aim, the sciatic nerve was cut in newborn rats and NT-4/5 or buffer were applied to the proximal nerve stem in a gelfoam as well as by daily local injection. The number of motoneurons or ventral root fibers was determined 4, 11, and 14 days later. Four days after received NT-4/5 lost only 19% + 2.7% (540 + 81). No systemic effect on the contralateral motoneurons was observed. Surprisingly, 11 and 14 days following the operation, the loss of motoneurons

Neuromuscular disorders in systemic malignancy and its treatment.

Neuromuscular dysfunction in patients with known or suspected malignancy has three basic etiologies: (1) a direct effect of the neoplasm, either by compression or infiltration; (2) a "remote," or paraneoplastic, effect of cancer; or (3) a side effect of anticancer treatment, radiation or chemotherapy. A variety of clinical features or syndromes are due to damage either at the level of the neuron (anterior horn cell or dorsal root ganglion neuron), nerve root(s), brachial or lumbosacral plexus, peripheral nerve (motor, sensory, and/or autonomic), neuromuscular junction, or muscle. A complex cli

Spinal cord stimulation in low back and leg pain.

We have reviewed our experience with spinal cord stimulation (SCS) in patients with low back and leg pain. 33 patients complaining of leg and low back pain underwent percutaneous tests of SCS. 28 patients had failed back surgery syndromes, 1 patient had pain related to an L1 vertebral body fracture, another from Tarlow cysts and the remaining 3 patients had lumbosacral spondyloarthrosis and osteoporosis without radiological signs of root compression. 28 patients showed mono- or pluriradicular deficits. At the end of the test period (5-65 days), 21 patients (63.6%) reported more than 50% of pai

[Clinical value of the posterolateral approach, supplemented with pedicle excision of the arch, to the anterior wall of the spinal canal within the thoracolumbar (Th11-L1) and lumbar spine].

UNLABELLED: The author validates clinically his own lateral approach to the lumbosacral (Th11-L1) and lumbar spine. In this approach an enlargement of the intervertebral foramen is achieved by the partial excision of adjacent pedicles or excision of one or several pedicles. The access to the anterior canal wall is gained in this manner. This surgical approach has been used in 48 patients with neurological injury (spinal cord, conus medullaris, cauda equina or the roots). An active tuberculosis was the causative factor in 12 cases, the sequelae of tuberculous infection in 10 cases, infection in

Evaluation of the motor and sensory components of the pudendal nerve.

Extensive neurophysiological investigations consisting of different techniques to evaluate the efferents and afferents of the pudendal nerve were carried out in 27 healthy subjects. These investigations included motor evoked potential recordings from the external anal sphincter in response to magnetic stimulation of the cortex and lumbosacral roots, measurement of sacral reflex latency to magnetic and electrical stimulation, and cortical sensory evoked potential recording after stimulation of the dorso-genital nerve and anal canal. Motor latencies after transcranial magnetic stimulation to the

Nerve root hypertrophy in chronic inflammatory demyelinating polyneuropathy.

A patient with chronic inflammatory demyelinating polyneuropathy (CIDP) and central demyelinating disease is described in whom striking nodular filling defects on multiple lumbar-sacral nerve roots, mimicking neurofibromata, were observed at myelography and magnetic resonance imaging. We suggest that these lesions are secondary to recurrent segmental demyelination and remyelination and that the differential diagnosis of this radiological feature should include CIDP.

Transcutaneous electrical localization and reversible blockade of the sacral nerve in the diagnosis of the urge syndrome.

To date, conventional urodynamic evaluation is not able alone to distinguish motor urgency with uninhibited detrusor contraction from sensory urgency. Transcutaneous electrical localization and reversible monolateral blockade of the 3rd sacral root was performed in 3 women with interstitial cystitis and in 2 men with idiopathic detrusor hyperreflexia by intranerve injection of 6 ml 0.5% Bupivacaine solution. The paralysis of the detrusor was completely reserved in all patients within 6 hours of the blockade. Cystometry and cystography were performed at 10 min, 2 and 6 hour intervals after the

Substance P, neurofilament, peripherin and SSEA4 immunocytochemistry of human dorsal root ganglion neurons obtained from post-mortem tissue: a quantitative morphometric analysis.

Immunocytochemical studies on lumbar dorsal root ganglia obtained at routine postmortem 24-36 h after death were carried out, and neuronal cross-sectional areas measured. The subjects were elderly (76-81 years), of both sexes, had died from heart attack or haemorrhage, and had no clinical evidence of clinical neuropathy or of disease known to be associated with neuropathy. The data were consistent between ganglia from the three subjects. There were striking similarities with data from other species. Two populations of cell profiles with overlapping size distributions were distinguished with an
