[Long-term outcome of monoradicular lumbosacral compression syndromes-a comparison of pain and neurological deficits with lumbar CT scan.].

A total of 97 patients with an acute monoradicular lumbosacral compression syndrome and a herniated disc at the same level were reinvestigated an average of 25 months after the acute event. Assessment was based on CT investigation and "root score", which was compiled from reports of subjective complaints and neurological deficits. All patients had initially undergone conservative treatment as inpatients. At the time of reinvestigation CT examination of the herniated discs revealed that they were unchanged in 56.7%, some-what smaller in 34%, no longer visible in 4.1% and larger in 5.2%. Neverth

Failed back syndrome: a study on 95 patients submitted to reintervention after lumbar nerve root decompression for the treatment of spondylotic lesions.

The persistence of lumbar and nerve root pain after nerve root decompression surgery may be attributed to one of five causes; 1) progression of the spondylotic disease in the presence of peridural fibrosis; 2) recurrence of disc herniation or new hernia; 3) stenosis of the spinal or nerve root canal; 4) arachnoiditis; 5) vertebral instability. In most patients with peridural fibrosis and worsening of spondylotic lesions regression of nerve root symptoms was obtained after several months of conservative treatment, which continues to constitute essential treatment for most patients with recurren

Spinal involvement in children and adolescents with Gaucher disease.

Nineteen patients with type I Gaucher disease with spinal involvement first diagnosed during childhood, were followed for 2 to 24 years (average 9 years), and their clinical and radiologic history reviewed. Patients presented with three types of pain: mild pain that was defined as nonspecific, severe pain due to bone crisis, and pain associated with vertebral collapse. Collapse of vertebra occurred gradually anywhere along the thoraco-lumbar spine, and usually more than one vertebra was involved. In three patients, rectangular collapse was noted. Six patients suffered from central vertebra col

Immunohistological characterisation of acute herpes simplex virus infection of lumbar dorsal root ganglia in the rat.

After inoculation of herpes simplex virus (HSV) into the dorsal skin of the hind paw of rats, HSV antigen was detected in dorsal root ganglion neurones by immunohistochemistry. Antigen was first detected 2 days after inoculation. Between 2 and 4 days after inoculation, numerous small-type unmyelinated neurones gave positive staining reactions for HSV antigens. Necrosis of neuronal cells was evident by 4 days. Antigen was not detected 6 or 9 days after inoculation. Immunohistological studies of co-cultivated ganglia removed from the infected monolayers, immediately after cytopathic effect first

[Clinical significance of lumbosacral transitional vertebrae (Bertolotti syndrome)].

500 lumbosacral plain x-ray injured patients and 1000 myelography of patients suffering from low back pain and nerve root sign were reviewed to evaluate the rate of transitional lumbosacral vertebra. This anomaly appeared in 4.6% in the normal population. The rate of the transitional vertebra in patients with nerve root syndrome was double (8.4%) whereas that with lumbar disc herniation four time as many as in the normal population. The data demonstrate that the asymmetrical anomalies have more clinical significance. The authors consider, that the role of the transitional vertebra is twofold:

MRI of benign lumbosacral nerve root enhancement.

Enhancement of nerve roots in the lumbosacral spine after intravenous administration of gadolinium contrast agents is seen in benign conditions such as disc herniation and spinal stenosis and may be observed in either the unoperated or postoperative spine. Correlating reasonably well with the clinical syndrome, nerve root enhancement represents actual pathology within nerve roots reflected by this breakdown in the blood-nerve barrier. Nerve root enhancement may be used in certain circumstances to show and confirm clinically relevant neurological disease.

Cytomegalovirus and the peripheral nervous system in AIDS.

Cytomegalovirus (CMV) has been associated with a spectrum of peripheral nerve syndromes in patients with AIDS. Lumbosacral polyradiculopathy, a rapidly progressive syndrome occurring in about 1% of AIDS patients, is characterized by low back pain, sphincter disturbance, progressive flaccid paraparesis, and neutrophil cerebrospinal fluid pleocytosis. CMV has been found consistently in the lumbosacral roots in association with inflammation, demyelination, and axonal loss. Anti-CMV therapy has been reported in open studies to prolong survival and may lead to improvement of the neurologic deficit.

Peripheral nervous system alterations in small cell lung cancer. Clinico-pathological study.

Small cell lung cancer (SCLC) is one of the most malignant tumors, especially often associated with nonmetastatic neurological disorders, corresponding to paraneoplastic neurological syndromes. The pathogenesis of which is unknown, however, mostly attributed to autoimmune processes. The aim of the study was to determine the pattern of the peripheral nervous system damage in SCLC. To provide further data contributing to the pathomechanism underlying these syndromes, immunocytochemical studies were initiated. Autopsy material was collected from 47 cases of SCLC. All these patients were examined

Intraoperative monitoring of the dorsal sacral roots: minimizing the risk of iatrogenic micturition disorders.

In 31 children (age, 2-17 years) and 1 adult, individual dorsal root action potentials (DRAPs) from the S1-S3 roots were recorded intraoperatively after electrical stimulation of the dorsal penile or clitoral nerves, in preparation for surgery within the cauda equina. In most patients, pudendal afferent activity was present in S2 and S3 bilaterally; in some, the afferent activity was confined to a single root bilaterally, and in one, to a single root on one side. Dorsal root action potentials of small amplitude were recorded from S1 in 15 patients, although in no patient was S1 the primary car

Anatomical and physiological study of interneurons in an oligosynaptic cutaneous reflex pathway in the cat hindlimb.

Morphological and physiological studies were undertaken in the cat lumbosacral spinal cord in order to examine individual last-order excitatory interneurons in the disynaptic pathway between low threshold superficial peroneal (SP) afferents and flexor digitorum longus (FDL) motoneurons. Last-order interneurons projecting to the FDL motor nucleus were visualized with activity-dependent trans-synaptic transport of lectin-conjugated horseradish peroxidase (WGA-HRP). The location of lumbar interneurons that responded at short latencies (less than 3 ms) to electrical stimulation of the SP nerve was

Dermatomal somatosensory evoked potentials in the diagnosis of lumbosacral spinal stenosis: comparison with imaging studies.

Dermatomal somatosensory evoked potentials (DSEPs) and computerized tomography/magnetic resonance (CT and/or MR) images were retrospectively analyzed to evaluate their relationship in the diagnosis of lumbosacral spinal stenosis (SS). Of 155 patients referred for DSEPs with a clinical suspicion of lumbosacral SS, 58 met the inclusion criteria. DSEP abnormality was defined as: (1) N1 latency absent or greater than 2.5 SD; (2) side-to-side latency difference greater than 2 SD; (3) amplitudes greater than 2 SD below the mean; or (4) amplitude ratio greater than 2 SD. Involvement of two or more DS

Translaminar facet joint screws to enhance segmental fusion of the lumbar spine.

From 1984 to 1988, 70 consecutive lumbar and lumbosacral spine fusions enhanced with translaminar facet joint screws were performed for segmental degenerative disease. Twenty patients had partial decompression of central stenosis, 15 concomitant discectomy, and 19 lateral nerve root decompression. The mean time to fusion was 4.5 months (range 2-7 months). At followup (average 45 months; range 24-74 months) 98.5% were judged to have solid fusion. Satisfactory results were observed in 84% of cases, 91% without previous surgery and 75% after previous discectomy. Supplementation of posterolateral

Transient expression of GABA immunoreactivity in the developing rat spinal cord.

The development of GABAergic neurons in the spinal cord of the rat has been investigated by immunocytochemical staining of frozen sections with anti-gamma-aminobutyric acid (GABA) antiserum. In the cervical cord, GABA-immunoreactive fibers first appeared at embryonic day (E) 13 in the presumptive white matter within the ventral commissure, ventral funiculus, and dorsal root entrance zone, and in the ventral roots. There were no GABA-immunoreactive cell bodies detected at this age. By E14, motoneurons, the earliest generated spinal cells, were the first cell population to become GABA-immunoreac

[The pathophysiological basis for the treatment of patients with a vertebrogenic pain syndrome localized in the lumbosacral region].

Experience of treatment of 84 patients with radicular syndrome of lumbo-sacral localization revealed the efficacy of multifactorial effect on different structures of the nervous system responsible for generation and realization of pain impulsation. Treatment was carried out with consideration of pathogenetic factors forming the pain syndrome, namely, compression of the root, functional blocks, spinal reactive aseptic epiduritis, arterial dyscirculation, disorders of venous outflow, psychogenic factors.

Contribution of electromyography to the diagnosis, treatment and prognosis of cervical disc disease: a study of 114 patients.

The aim of this study is to verify whether EMG has the same diagnostic and prognostic capacity in cervical root syndrome as it does in lumbo-sacral syndromes. Our population includes 114 patients affected by a cervico-brachialgia with EMG denervation provoked by cervical disc disease. 26 patients were submitted to a myelography confirming the discal origin of the syndrome: there were 3 cases of disc protrusion and 23 cases of cervical spondylosis. The agreement between EMG and myelographical data was significant in 20 cases (76.9%). There were no significant differences of evolution between op

[Tumor compression of the conus and cauda].

The author analyzed a group of 17 patients with spinal tumours with symptoms of compression of the conus or cauda. Early clinical symptoms were sought and the entire clinical course before surgery was evaluated. The objective clinical finding was compared with the surgical one. Causes of diagnostic errors were analyzed. Most frequently a discogenic or other vertebrogenic cause of complaints was suspected. In all patients a local or root symptoms were the first manifestation of the disease. From the investigation ensues that a tumour must be considered when pain radiates into both lower extremi

Disturbances of C-fibre-mediated sensibility in lumbosacral disc disease.

In nine patients with chronic lumbosacral disc disease and radicular symptoms clearly restricted to one leg, C-fibre-mediated sensibility was measured by determination of the thresholds for heat pain and warmth on the foot, ipsi- and contralaterally to the nerve root compression. The thresholds were compared with the values for 19 healthy subjects. In the patients the warmth threshold was increased in the ipsilateral dermatome and normal in the contralateral dermatome. In contrast, the heat pain threshold was near normal ipsilaterally but was clearly decreased contralaterally. These findings a

Magnetic stimulation of motor cortex and motor roots for painless evaluation of central and proximal peripheral motor pathways. Normal values and clinical application in disorders of the lumbar spine.

Magnetic stimulation of the motor cortex, motor roots, and proximal nerve trunks was performed in 46 healthy adults and in 73 consecutive patients with disorders of the lumbar spine. In combination with neurography and F-wave recordings, the fractionated stimulation of the motor pathways allowed calculation of conduction times of the pyramidal tract fibers, of the motor roots (ie, caudal fibers), and of the motor fibers of the lumbosacral plexus. Normal values for motor conduction times to the quadriceps, anterior tibial, and extensor digitorum brevis muscles were established. Patients had cli

Cauda equina syndrome after in situ arthrodesis for severe spondylolisthesis at the lumbosacral junction.

Relative stretching of the cauda equina over the posterosuperior border of the sacrum can be found in all patients who have Grade-III or IV spondylolisthesis at the lumbosacral junction. We identified twelve patients, all less than eighteen years old, who had cauda equina syndrome after in situ arthrodesis for Grade-III or IV lumbosacral spondylolisthesis. In all twelve patients, posterolateral arthrodesis had been done bilaterally through a midline or paraspinal muscle-splitting approach. Nothing in the operative reports suggested that the cauda equina had been directly injured during any of
