Selective posterior lumbar rhizotomy has recently become an alternative method for relieving spasticity in patients with spastic cerebral palsy. This procedure involves the selective sectioning of the lumbosacral posterior nerve rootlets. Because the whole nerve root is not cut, tactile and proprioceptive sensation remain intact. The present study measured the changes that occurred in 29 patients with spastic cerebral palsy 2 days before surgery and from 4 to 14 months after surgery. Each patient was used as his or her own control. In addition to the clinical evaluation of function, gait was a

In three patients pain in the groin and thigh which was initially attributed to either a malfunctioning hip prosthesis or osteoarthritic hip, was shown to arise in the spine. All patients had positive myelograms and one had electromyographic evidence of nerve root compression. Referred pain from the spine must always be considered in the differential diagnosis of pain thought to be arising from a hip replacement.

Inconsistent data exist regarding the anatomy of the spinal nerve roots lateral to the thecal sac. A newly developed in situ technique was used to precisely define anatomic parameters on 20 fresh human cadavers. The take-off angle of the nerve roots from the thecal sac decreases from a mean of approximately 40 degrees from L1-L5 to 22 degrees at S1. The motor bundles are directly ventral to the sensory fibers within individual roots extrathecally. Dorsal root ganglia size varies with vertebral level. The majority of ganglia lie directly beneath the vertebral pedicles and one third overlie a po

There has been experimental evidence of the tethered cord syndrome being a gray matter disease resulting from excessive tensile force in the lumbosacral cord. Clinical findings also support this concept. This study was designed to elucidate the possibility of the involvement of the long tract in tethered cord. Long tract continuity was evaluated utilizing the retrograde axonal transport of horseradish peroxidase in fasciculus gracilis. There were no significant differences between the normal and experimental tethered cord in the number of labeled dorsal root ganglion cells of the lumbosacral c

A small-animal model of genital herpes simplex virus (HSV) infection has not been available for investigating the pathogenesis of disease in males. Male Hartley guinea pigs were inoculated on the medial thigh with 10(5)-10(6) plaque-forming units of HSV type 2. Animals developed vesicular lesions in the genital area, including the penis. Transient urinary retention, hindlimb paralysis, and acute urethral shedding of HSV also occurred. During acute infection, virus was detected in neural and genital tissue homogenates, including the peripheral nerve, lumbosacral dorsal root ganglia (DRG), spina

Extruded lumbar intervertebral disks traditionally have been classified as posterior or central in location. A retrospective review of 250 MR imaging examinations of the lumbar spine that used mid- and high-field imagers revealed 145 positive studies, which included a significant number of extrusions extending anteriorly. With the lateral margin of the neural foramen/pedicle as the boundary, 29.2% of peripheral disk extrusions were anterior and 56.4% were posterior. In addition, a prevalence of 14.4% was found for central disk extrusions, in which there was a rupture of disk material into or t

A variety of surgical techniques is available for treatment of metastatic disease of the spine. Prior emphasis on the use of these procedures has been on their benefit as a palliative measure to relieve signs and symptoms of spinal cord and nerve root compression not aided by radiation therapy and corticosteroid administration. More recently, development of surgical techniques that combine neural decompression with restoration of spinal stability has brought about consideration of additional indications for surgery in the treatment of metastatic disease of the spine. The present scope of such

Analysis of 25 patients with normal computed tomographic appearances at the lumbosacral junction revealed wide variation in the anatomical level at which the first sacral nerve root sheaths were seen emerging from the theca. In nine patients (36%), the S1 nerve root sheaths were first recognized at the level of the lumbosacral disc. In 14 patients (56%), the sheaths emerged cranial to the disc; it is possible that these patients may be more prone to neurological complications related to disc or facet joint disease, especially if the sheath is laterally sited within the lateral recess. In this report some preliminary data after magnetic stimulation of the peripheral nerves in 8 normal individuals are presented. Compound muscle action potentials were recorded by surface electrodes after magnetic and conventional electrical stimulation of the median, ulnar, peroneal, tibial, femoral and sciatic nerves, and the lumbosacral roots. The data clearly show the ability of the magnetic coil to stimulate the roots in sites relatively inaccessible to electrical stimulation and obtain consistent muscle response suggesting important clinical application of magnetic stimulation in lumbosac

Bertolotti's syndrome refers to the association of back pain with lumbosacral transitional vertebrae. Such vertebrae were observed in 140 of 2,000 adults with back pain over a 4-year period of study. Each patient had radiographic evaluation of the lumbar spine by plain films as well as a sectional imaging modality (magnetic resonance [MR] or computed tomography [CT]). The overall incidence of structural pathology (eg, spinal stenosis and disc protrusion) detected by CT or MR was not apparently higher in patients with transitional vertebrae, but the distribution of these lesions was significant

In 492 patients with a lumbosacral radicular syndrome caused by a lumbar intervertebral disc herniation or by a stenosis of the lumbar spinal canal, the situation 1 year after a root decompressing operation was compared with the situation before surgery. About 75% of the patients reported satisfactory improvement, while about 10% were not satisfied. The judgement of the physicians is somewhat more favourable: 80% and 5%, respectively. The best results are obtained in patients in whom the preoperative diagnosis of lumbar disc herniation is highly probable; the worst results are obtained in pati

Mechanical disorders of the lumbar spine have been identified as a cause of pelvic pain and organic dysfunction (PPOD). Categorically, the clinical features indicative of mechanically induced PPOD fall into three areas: the history of the development or onset of pelvic symptomatology attributable to lower sacral nerve root compression (LSNRC), identification of related symptomatology on presentation, and the recognition of clinical findings indicative of mechanically induced PPOD on examination. Characteristic features of each category are presented. The clinical signs that most reliably indic

Neurological complications in patients with Acquired Immuno Deficiency Syndrome (AIDS) are frequent and in addition to central nervous system syndromes, involvement of the peripheral nervous system is increasingly seen. We evaluated the indications and results of myelographic examination in six AIDS-patients with signs of peripheral nervous system disease, out of 200 AIDS-patients with neurological complications. Five of these patients had a polyradiculopathy, with proven cytomegalovirus (CMV) infection in four cases. There were two abnormal myelographic examinations with findings of cauda equ

The effects of standing, and lying with the back extended, on F wave minimal latency and F chronodispersion were studied in patients with lumbar canal stenosis, lumbosacral root compression syndromes, and low back pain. Standing for 3 min produced increased F chronodispersion in lumbar canal stenosis and root compression syndromes but had less effect on the F minimal latency. In patients with low back pain these variables were unchanged by these postural manoeuvres. These observations illustrate the value of adapting electrophysiological investigations to those clinical features that induce sy

We report our experience in performing 52 CT-guided peripheral nerve root blocks and 27 CT-guided nerve root ablations. The nerve root block procedures were performed as diagnostic tests to determine the clinical significance of equivocal herniated disk or foraminal stenosis (n = 33) and before nerve root ablations (n = 19). To evaluate the clinical significance of these nerve root blocks, we performed 52 procedures at the vertebral level where a herniated disk or foraminal stenosis had been identified on CT or MR images. In 51 of 52 cases, the procedures were successful in anesthetizing the i

A man of 75 years of age had had lightning pains in the legs for 8 years. Clinical examination demonstrated mild pyramidal signs and involvement of the posterior columns. A morphine pump was placed in the epidural space in the lumbar region to treat the pain. He died two weeks later from a massive pulmonary embolism. The nervous system was examined using the classical techniques along with the use of poly- and monoclonal antibodies against nervous system specific proteins. The following features were demonstrated: neuroaxonal dystrophy of the posterior and anterior horns, and the posterior col

Many patients with low back pain demonstrate pelvic symptomatology attributable to lower sacral nerve root compression. Lower sacral nerve root compression has been identified as a cause of pelvic pain and pelvic organ dysfunction. Pelvic symptomatology secondary to lower sacral nerve root compression is given. Lower sacral nerve root compression is most commonly the result of lumbosacral disc lesion. A case of low back pain accompanied with pelvic symptomatology is presented along with its response to distractive manipulation. chiropractic treatment may be an effective means of treating pelvi

To study effects of herpesvirus reactivation on the nervous system, mice with latent genital herpes simplex virus type 2 (HSV-2) infections were immunosuppressed. Reactivated infection was detected by virus isolation or by antigen screening in histological sections of spines that contained cords, roots, and dorsal root ganglia. In ganglia, viral antigen was restricted to 2 distinct groups at T9-L2, and L6-S2 levels. In some ganglia, antigen was found in up to 4% of non-contiguous neurons, in their axons, and in endoneurial and satellite cells. Nerve roots distal to ganglia contained a few anti

A patient with a malignant cerebral mixed oligodendroglioma-astrocytoma presented with a severe, painful S-1 radiculopathy. Neuroradiological examinations revealed an extradural soft-tissue mass compressing the first sacral nerve root. Spinal fluid examination and myelography gave no evidence of intradural disease. Surgery confirmed the presence of metastatic glioma compressing the S-1 nerve root. The tumor was entirely epidural in location. Mechanisms of dissemination are discussed.

A case of nasopharyngeal carcinoma metastatic to the cauda equina is reported. The patient, treated 3 years earlier for his primary disease, presented with bilateral weakness of his legs. Myelographic studies showed only patchy thickening of certain lumbosacral nerve roots. A biopsy from a root in the cauda equina was made through a T-12 to L-2 laminectomy revealing carcinomatous involvement. Careful examination of the specimen was necessary to distinguish the tumor from a lymphoma. Review of the literature has revealed no other cases of this type of neoplastic metastasis.
