Fusion of the lumbar and sacral spine by internal fixation with screw plates.

From 1972 to 1982, 455 patients had arthrodesis and other operations with screw-plate internal fixation of the lumbosacral spine. Of these 455 patients, 440-413 of whom had had spinal fusion and 27 of whom had undergone pars interarticularis reconstruction--were evaluated for a mean follow-up period of 31.6 months. The principal indications for single-stage posterior fusion, which was performed in 266 cases, were severe degenerative joint disease and painful spondylolysis, while the primary indications for combined anterior and posterior repair, which was undertaken in 145 cases, were spondylo

Intraspinal seeding from intracranial tumors in children.

Cerebrospinal fluid-borne metastases were demonstrated by metrizamide myelography in 20 children with primary intracranial tumors, posterior fossa medulloblastomas being the most common. The lumbosacral region was affected most often, with nerve root thickening, nodularity, and irregularity of the thecal sac. Cord cloaking, nodularity, and meningeal disease were common in the cervical and thoracic areas. Intraspinal spread from primary intracranial tumors signals a poor prognosis. However, the early detection of intraspinal spread afforded by metrizamide myelography can alter therapy and there

Scoliosis caused by section of dorsal spinal nerve roots.

Experimental evidence has accumulated in recent years to suggest that scoliosis can be caused by asymmetrical spinal muscle weakness due to sensorineural loss, though this suggestion has not achieved universal acceptance. The evidence is supported by histopathological observations on cases of clinical idiopathic scoliosis. A study is presented in which cynomolgus monkeys had one, two or three dorsal spinal nerve roots cut. Scoliosis developed, convex to the damaged side; its severity was dependent on the number of nerve roots cut. Section of the first lumbar dorsal spinal nerve root had a mark

[Primary lymphoma of the peripheral and central nervous system].

Initial neurological disorders in a patient with primary lymphoma of the nervous system were due to multiple lumbosacral root and meningeal lesions. Recovery after intrathecal methotrexate was followed ten years later by a relapse when lesions were localized in a cerebral hemisphere.

[Thermography in the diagnosis of spontaneous lumbosacral radiculitis].

Employing the Soviet thermal scanner "Rubin", 59 patients aged 20 to 65 years suffering from lumbosacral radiculitis were examined. Clinical examinations were supplemented with spondylography , epidurography, pneumomyelography and myelography. Operation performed on 29 patients showed a slipped disk in 26 and hypertrophy of the yellow ligament in three patients. The level and the side of compression of one or another neural root by the slipped disk detected at operation correlated with the thermographic data in 72% of all cases. An analysis of the thermograms in 59 patients with lumbosacral ra

Neuroanatomy of penile erection: its relevance to iatrogenic impotence.

The neuroanatomy of erection in men is not well defined. Recently, we isolated successfully the cavernous nerves for acute and chronic neurostimulation to induce penile erection in dogs and monkeys. We then investigated the anatomy of these nerves in humans by cadaveric dissection and serial histologic sectioning. Our experience in tracing the spinal nuclei responsible for vesical and urethral function by transportation of horseradish peroxidase enabled us to explore the location and organization of the spinal center for erection. Thus, systemic knowledge of the neuroanatomy of erection was ac

Latency of late responses in lesions of lumbosacral nerve roots.

Latencies of the F response and the H reflex discharge have been measured in 16 patients with clinical signs of lesions affecting the lumbosacral nerve roots. Responses were recorded from relevant muscles with stimulation of the posterior tibial or lateral popliteal nerves. In seven of the 12 patients with electrophysiological abnormalities, one or more late responses were absent. Radiographic examination in the same group confirmed disc lesions at L4-L5 or L5-S1 levels in nine patients and at T12-L1 in one and showed diffuse degenerative spine disease in two. Three of the four patients with n

Segmental motor paralysis in herpes zoster.

Herpes zoster infection may be complicated by motor involvement, the most common example of this being facial palsy in the Ramsay Hunt syndrome. Segmental lower motor neurone weakness is an uncommon disorder which does occur in the limbs, such paralysis usually occurring in the same segments as the dermatomes involved, or in those immediately contiguous. Nine cases of limb weakness are described, the weakness often being severe and accompanied by wasting and segmental reflex impairment. Long tract signs were absent. The cervical region was affected more commonly than the lumbosacral area. The

The nervous system in genital herpes simplex virus type 2 infections in mice. Lethal panmyelitis or nonlethal demyelinative myelitis or meningitis.

Female mice were inoculated vaginally with the MS strain of herpes simplex virus type 2, and serially positive vaginal cultures were used to confirm infection. The proportion of mice infected and the mortality rate in infected mice decreased with increasing age. In mice 12 weeks old, clinical, neuropathologic, and virologic criteria defined four patterns of disease. Moribund mice had severe genital lesions, hindleg paralysis, and urinary and fecal retention, and most died during the second week of infection. These mice had a panmyelitis with a decreasing gradient of both viral antigen and lesi

Syndrome of the incidental herniated lumbar disc.

Lumbar myelographic defects consistent with herniated disc were found in 108 asymptomatic patients undergoing myelography for other reasons. Within 3 years, 64% of these patients developed symptoms of lumbosacral radiculopathy. The clinical features of these patients comprise a syndrome significantly different from that typically associated with classical lumbar disc herniation: the syndrome described here carries a much higher incidence of silent root compression with minimal pain. Incidental lumbar myelographic defects are not necessarily benign findings, and patients in whom they are encoun

Surgical treatment of 63 cases of conjoined nerve roots.

The operative results of 63 cases of lumbar disc disease with surgically confirmed conjoined nerve roots are reviewed. The first 55 patients were treated by standard hemilaminectomy and discectomy, with only 30% reporting a good result. Of the last eight patients treated by hemilaminectomy, pediculectomy, and discectomy, seven patients returned to work. Te rationale for and the technique of pediculectomy are discussed in detail. Clinical, radiological, and surgical clues indicating the presence of the conjoined nerve root anomaly are reviewed.

Isolated hypogastric artery aneurysms.

Isolated hypogastric artery aneurysms are unusual variants of aortoiliac aneurysmal disease. These vascular lesions can pose a diagnostic dilemma and are difficult to manage operatively. Five patients with six isolated hypogastric artery aneurysms were treated at UCLA Hospital, Los Angeles, in addition to 59 patients with 76 isolated hypogastric artery aneurysms previously described in the literature, to form a collected series. Although the aneurysms were asymptomatic in 43% of cases, symptoms were often due to rupture or expansion of the rupture of expansion of the aneurysms, with pressure o

Genital herpes in guinea pigs: pathogenesis of the primary infection and description of recurrent disease.

Guinea pigs inoculated intravaginally with herpes simplex virus type 2 (HSV-2) developed a self-limiting infection characterized by vesiculo-ulcerative lesions on the external genital skin, urinary retention, and hindlimb paralysis. Infection rarely resulted in death. Virologic, histologic, and immunoperoxidase data suggested the following scheme for viral pathogenesis: initial replication in the introitus, vagina, and bladder; spread via sensory nerves to the lumbosacral dorsal root ganglia and spinal cord, and transmission via peripheral nerves to the external genital skin to produce the cha

Lumbar spondylolisthesis. Clinical syndrome and operative experience with Cloward's technique.

The paper gives a survey, based on literature reports and our own experiences in 59 cases. According to Kilian (1853) spondylolisthesis is defined as ventral slipping of a vertebral body together with the pedicles. In pseudospondylolisthesis (Junghanns) the whole vertebra slips ventrally. Elongation of the isthmus of the pedicle or a cleft in the interarticular portion are prerequisites for spondylolisthesis. A dysplastic origin during childhood and adolescence is assumed. The relation of males to females is 2 : 1. Evolution of spondylolisthesis occurs during childhood and adolescence. The sli

[Selective posterior radiculotomy in the treatment of spasticity: immediate and long-term results].

Immediate and late results of a series of 44 patients suffering from spastic syndromes, operated on from January 1973 to December 1976 by selective posterior rootlet section, have been analyzed and reported. The operation consisted of a selective section of the dorsomedial part of the rootlets constituting each posterior root and was performed usually from L1 to S1 bilaterally. Thirty patients improved markedly after the operation and there were no immediate or late complications. The procedure caused no sensory disorders. The indications and contraindications to the operation are discussed.

Usefulness of electrophysiological studies in the diagnosis of lumbosacral root disease.

Clinical, electrophysiological, and myelographic findings were correlated in 57 patients with the clinical diagnosis of lumbosacral root disease. Conventional motor and sensory (including sural nerve) conduction studies were normal in all patients. Electromyography, late response studies in different muscles of the lower extremity, the myelogram, or combinations of these tests were abnormal in 44 patients (77%). Of 36 patients (63%) with abnormal myelograms, 14 had normal electrophysiological studies. Twenty-nine (51%) had an abnormal electrophysiological or myelographic finding; although 8 pa

Conjoined lumbosacral nerve roots. Management of herniated discs and lateral recess stenosis in patients with this anomaly.

Anomalous L-5 and S-1 nerve roots occur infrequently. If not properly recognized, surgery for entrapment disorders may result in serious neural injury because of an improper surgical approach in exposure and in removing the underlying herniated discs. The diagnosis has been made preoperatively since the introduction of water-soluble myelography because of improved filling of the nerve roots. A herniated disc beneath the bifid root causes extreme pain and disability with marked signs of entrapment because of firm fixation of the conjoined root in the lateral recess between the two pedicles. Calcium pyrophosphate dihydrate deposition in lumbar disc fibrocartilage.

Calcium pyrophosphate dihydrate deposits were found in the lumbar disc fibrocartilage in 4 patients undergoing surgery for spinal cord or nerve root compression. All of the patients had prior surgery at the same lumbar area. None of the patients had the articular or roentgenographic manifestations of calcium pyrophosphate deposition disease (pseudogout). Andres and Trainer have recently reported 7 similar patients. Calcium pyrophosphate dihydrate deposition in axial skeleton fibrocartilage may be a common finding in patients undergoing repeat spinal surgery.
