[Cauda equina neurography. Results from normal persons and patients with lumbosacral root compression syndrome].

The results with the cauda-equina-neurography in more than 150 patients demonstrate a value for differentiation of lumbosacral disorders. The action potentials of normal adults are mostly biphasic, have the same configuration for both sides and their amplitude is between 0,3 and 20 uV. The conduction velocity decreases with age for both nerves. In cases of lumbosacral disorders with isolated nerve root affection by discogenic compressions the potentials are delayed, reduced, polyphasic or absent. Mostly the clinical, electromyographic, myelographic and surgical results correspond with neurogra

Selective lumbosacral radiculography and block.

Selective lumbosacral radiculography and block was performed on 106 patients, using 60% Conray or Dimer-X and 1% lidocaine mixed with corticosteroid. The technique of this method is reported, and the radiculographic findings and diagnostic value of this method are discussed. According to our experience, this method is technically simple and very useful in determining the limit of the lumbosacral nerve root lesion, and occasionally can be used to relieve radicular symptoms. We think it is an especially excellent diagnostic technique for disorders featuring nerve root entrapment in the lateral f

Feasibility of controlled micturition through electric stimulation.

Historically, man has been aware of bioelectric phenomena for some 4,000 years. Yet it has only been during the last 20 years that technology has advanced to the stage where controlled bladder emptying has become feasible. A great deal of interest followed the introduction of transistor and bladder stimulation via the principle of radio frequency induction. Spinal cord, sacral, and pelvic nerve and direct bladder stimulation have all been attempted. Only direct bladder stimulation in lower motor neuron situations has shown any promise. The many difficulties associated with bladder stimulation

Host defenses in herpes simplex infections of the nervous system: effect of antibody on disease and viral spread.

BALB/c mice passively immunized with antibody to herpes simplex virus type 1 and challenged in the footpad with 10(5.7) plaque-forming units of herpes simplex virus type 1 were shown to be protected from neurological disease and death compared with control mice treated with normal serum or antibody to Sindbis virus. One hundred percent of untreated mice had virus recoverable from dorsal root ganglia by 48 h after infection. Whereas amputation of the infected limb at 48 h had no effect, antibody administration (resulting in titers of 1:8 and 1:16) was found to prevent acute neurological disease

Lumbar spinal manipulation on trial. Part I--clinical assessment.

Thirty-two patients with chronic low back pain were treated three times at weekly intervals with rotational manipulation. Patients with femoral or sciatic root pain were included provided they did not exhibit root compression signs. Background therapy of codeine phosphate was administered throughout. There was a significant increase in spinal flexion measured clinically during the three-week period of manipulation followed by a significant decrease in the three-week period after manipulation. The first week of manipulative treatment was more painful than the corresponding week in the control g

Spinal cord and nerve root lesions in children presenting with urinary dysfunction.

Three cases of bladder dysfunction as the first indication of tumors involving the spinal cord and sacral nerve roots are presented. The correct diagnosis was delayed in each instance. The relationship of such tumors to bladder dysfunction is discussed and the need for careful history and physical examination is stressed. It is emphasized that neurologic disease may present with urinary dysfunction. Neurological symptoms, physical findings and radiographic abnormalities of the spine should be noted.

Unreliability of epidural venography in lumbar disc disease.

Lumbar epidural venography has been compared with myelography and discography in 69 patients suspected of having nerve root compression from herniated lumbar intervertebral discs. Only 78% agreement occurred when venograms were interpreted as unknowns by two radiologists. Twenty-three percent of the venographic examinations were suboptimal, most of these in patients operated upon previously. False-negative venography occurred in 13%. Midline disc herniations were missed more often than lateral herniations.

Abdominal pain caused by diabetic radiculopathy.

In diabetes mellitus, pain and weakness in a lower extremity can result from lumbosacral radiculopathy. However, abdominal pain caused by a similar type of thoracic nerve root disease is not generally recognized. Four diabetic patients with abdominal pain were studied. In none of the patients was the actual cause--radiculopathy--suspected until after extensive negative diagnostic investigation, which frequently included invasive procedures. Their clinical features were suggestive of malignant disease. Electromyographic examination revealed the correct diagnosis, and all four of the patients ha

Kyphosis secondary to infectious disease.

Infections of the spine usually involve the vertebral body and therefore by definition produce a kyphosis. Non-tuberculous infection usually staphylococcal and in the lumbar spine, is often diagnosed late and can involve the cord. Open exploration and stabilization with graft should therefore be considered. The destruction is usually less extensive and therefore the kyphosis less severe than in late neglected tuberculous infections. Tuberculous spinal infection accounts for 59% of all orthopedic tuberculosis. It invariably involves vertebral bodies and is progressive. Destruction of the bodies

Pseudo-radiculopathy in subacute trochanteric bursitis of the subgluteus maximus bursa.

Seventy patients, averaging 82 years of age, were referred for low back pain and/or a suspected herniated disk. Objective neurological deficits consistent with L5 or S1 root involvement were identified in 5 of the 70 patients. Trochanteric bursitis (TB), often mimicking radiculitis, was diagnosed in 31 patients. Trochanteric bursitis was associated with lumbosacral strain and lumbar osteoarthrosis in 21 of 31 patients and with an S1 disk in 1 of those 31 patients. Degenerative joint disease of the ipsilateral hip was present in 4 of 20 of these patients with TB. Six patients with low back pain

Herpes simplex virus infection in guinea pigs: an animal model for studying latent and recurrent herpes simplex virus infection.

Footpad infection of guinea pigs with herpes simplex virus led to an acute local inflammatory reaction, followed by a persistent latent infection of lumbosacral dorsal root ganglia. Spontaneous reactivation of the latent virus occurred, leading to recurrent lesions at the site of the initial infection. Clinical observations and virological studies during the acute latent and recurrent infection are reported.

Neuropathologic effects of intrathecal water.

In attempting to control the pain of disseminated carcinoma, a variety of agents has been injected intrathecally. This report deals with the neuropathologic changes in spinal roots and spinal cord of four patients into whose lumbar sacs 100 to 285 ml of distilled water was infused. Survival following the procedure ranged from 9 to 50 days. In most severely affected case (285 ml instilled 50 days prior to death) the abnormalities were: (1) severe breakdown of axons and myelin sheaths of the lower cauda equina and to a lesser extent of other roots at levels as high as the eighth cervical segment
