[Recurrent amyotrophic neuralgia with an unusual clinical expression. Clinical and electrophysiological study].

Neuralgic amyotrophy (NA) is an entity with great variability in its neurological expression. The patient reported here developed symptoms of NA one year before admission, with anterior interosseous nerve palsy. At the time of examination she reports pain in the left upper limb followed by sensory loss over lateral antebrachial cutaneous nerve. Electrophysiologic examination showed signs of chronic denervation in the right anterior interosseous nerve muscles, and reduced amplitude of the sensory nerve action potential of the left lateral antebrachial cutaneous nerve. This case confirms the var

Neuralgic amyotrophy in association with radiation therapy for Hodgkin's disease.

We describe 4 patients with Hodgkin's disease who developed neuralgic amyotrophy in the setting of radiation therapy. In contrast to tumor progression or radiation plexopathy, the symptom onset was abrupt and occurred within days to weeks of receiving radiation treatments. There is an association between Hodgkin's disease, radiation therapy, and neuralgic amyotrophy.

Blood lymphocytes are sensitized to branchial plexus nerves in patients with neuralgic amyotrophy.

The percentage of lymphocytic subsets in the blood of cases with neuralgic amyotrophy (NA), and the proliferative response of blood lymphocytes cultured with different nerve extracts, obtained from normal subjects at postmortem, were examined in 6 patients with NA and in 18 age-matched controls with shoulder pain not related to NA. Most (5/6) NA patients had decreased CD3 values and increased CD4/CD8 ratios due to a decreased of the CD8 subset. Lymphocytes of NA patients increased their blastogenic activity in cultures with nerve extracts from different brachial plexus nerves and its branches,

[A case of "neuralgic amyotrophy" with elevated serum antibody titer against Borrelia burgdorferi].

A 39-year-old man experienced an abrupt onset of right back pain. The pain improved spontaneously, but weakness of the right upper extremity developed. The weakness deteriorated during the next month, and he was admitted to our hospital. Neurological examination disclosed impairment of superficial sensation in his right upper extremity. Blood examination showed no abnormal data. The cerebrospinal fluid was normal. Neuroradiological findings were also negative. Electrophysiological examinations were normal except for needle electromyographic findings of the right upper extremity, which showed n

Neuralgic amyotrophy: a long term follow-up of four cases.

Neuralgic amyotrophy or brachial plexus neuralgia is a condition of uncertain etiology. It needs to be differentiated from the usual brachial plexus injuries. Nerves outside the plexus especially the spinal accessory nerve are involved. Neuralgia is also a feature. Four cases were followed-up in detail over a period of two years. The condition usually resolves almost completely on its own.

Isolated complete denervation of the flexor pollicis longus.

Isolated complete denervation of the flexor pollicis documented by electromyography has not been reported. We report a healthy 26-year-old man who sustained a gunshot wound at the base of his left index finger. He reported ipsilateral flexor pollicis longus (FPL) paralysis one week later, after undergoing surgical exploration of the radial digital artery and nerve supplying the left index finger under axillary block anesthesia. Forearm pronation and distal interphalangeal flexion of the second through fifth digits were normal. Electromyographic findings revealed complete isolated denervation o

Neuralgic amyotrophy with involvement of cranial nerves IX, X, XI and XII.

We report a patient who presented with involvement of multiple cranial nerves associated with otherwise typical neuralgic amyotrophy. This syndrome of unknown etiology is not limited to the brachial plexus. Simultaneous involvement of cranial nerves IX, X, XI, and XII is a unique presentation. The electrophysiological data indicate the presence of a multifocal neuropathy.

[Neuralgic amyotrophy. A clinical and neurophysiologic longitudinal study of 3 cases].

The Authors describe three cases of neuralgic amyotrophy examined clinically and neurophysiologically at the onset of symptoms and after six months. At that time patients showed an objective improvement in strength and functions, though a few SEP and EMG alterations persisted. These findings are considered of out discrepancy between clinical and neurophysiological picture.

Recurrent familial brachial plexus palsies as the only clinical expression of 'tomaculous' neuropathy.

Two familial cases of recurrent brachial plexus are described and similar episodes were noticed in other members of the family. Electrophysiological investigations found impaired motor and sensory nerve conduction velocity in affected and nonaffected members. Tomaculous neuropathy was found at biopsy of peripheral nerve in more than 40% of dissected fibers. In addition, two affected members showed a reduced interpupillary distance, i.e. the most common dysmorphic feature found in the hereditary neuralgic amyotrophy (HNA). A literature review showed only two other instances of recurrent familia

[Bilateral diaphragmatic paralysis: sequelae of neuralgic amyotrophy].

Diaphragmatic paralysis is rare in neuralgic amyotrophy. Total quick recovery is usual. We report a case with a diaphragmatic paralysis still present more than 30 years after the onset of a bilateral and recurrent neuralgic amyotrophy. Neuralgic amyotrophy should be considered as a possible cause of chronic diaphragmatic paralysis.

Neuralgic amyotrophy of the lumbar area. Case report.

Two cases of a rare peripheral neurological disease (neuralgic amyotrophy of the lower extremity) are presented, with clinical symptoms (severe sciatic pain with motor paresis) that pose differential diagnostic difficulties against a hernia of a lumbar disk. Because of a tendency to heal by itself, the disease has a good prognosis. The diagnosis is based on a process of exclusion of herniated disks, lesions of the lumbar plexus in tumors or after radiation therapy, hematoma of the psoas after coagulation disturbances, birth, or trauma, and nutritive disruptions of the nervous system as in diab

Postoperative isolated dysfunction of the long thoracic nerve: a rare entity of uncertain etiology.

A "winged" scapula is a rare, poorly understood, and potentially disabling curiosity following anesthesia and surgery. It is produced by dysfunction of the long thoracic nerve and consequent paralysis of the serratus anterior muscle. A survey of senior anesthesiologists indicated a consistent lack of familiarity with the entity. This article presents six cases of postoperative long thoracic nerve palsy. In a literature review of 111 instances of long thoracic nerve palsy, 51 were trauma-related, 47 were either idiopathic or of debatable origin, and 13 appeared following a surgical or obstetric

The neurological features of early and 'latent' human immunodeficiency virus infection.

Neurological manifestations of unknown cause occurring in patients who become or are HIV antibody positive with presumed normal immune function have been described recently. This report adds a further six cases, all of whom had normal CD4+ cell counts either throughout the period of observation or after the episode of seroconversion. Three had an acute presentation, two in the context of documented seroconversion consisting of one of the following: an encephalitis, an ataxia, and confusion with neuralgic amyotrophy. Three had a subacute disorder occurring at a later phase of HIV infection but

Neuralgic amyotrophy.

Neuralgic amyotrophy is an infrequent neuromuscular anomaly involving the shoulder girdle and upper extremity. Its course is highlighted by the sudden onset of severe pain followed by sensory deficits, muscle weakness, and severe atrophy. The diagnosis is based on the history and physical findings and is corroborated with electromyography. The prognosis is excellent. Treatment is supportive, using analgesics and physical therapy.

AAEE case report #14: neuralgic amyotrophy (acute brachial neuropathy).

Neuralgic amyotrophy is characterized by acute onset of arm pain, followed after a variable interval by flaccid paralysis in the distribution of the brachial plexus on one or sometimes both sides. Electromyography reveals findings suggestive of an axonal degeneration process affecting various portions of the brachial plexus or individual nerves emanating from it, sometimes difficult to localize to discrete cords, trunks, or nerves. Diagnosis is established by excluding other causes, including compression and vasculitides. Prognosis for recovery is excellent.

Recurrent forms of sporadic brachial plexus neuropathy. A report of two cases.

Two patients presenting a relapsing form of sporadic brachial plexus neuropathy, the so-called Parsonage Turner syndrome, are reported. The diagnosis is based on clinical and electromyographic features. Recurrent attacks, although infrequently encountered, have been well described in the past. Sporadic cases of this syndrome must be differentiated from the familial varieties of neuralgic amyotrophy in which, two main subgroups of patients are distinguished: those showing facial dysmorphic features and those with findings of a tomaculous neuropathy predisposing them to pressure palsies. Neurological syndromes of brucellosis.

Eleven patients with brucellosis presented with neurological features closely simulating transient ischaemic attacks, cerebral infarction, acute confusional state, motor neuron disease, progressive multisystem degeneration, polyradiculoneuropathy, neuralgic amyotrophy, sciatica and cauda equina syndrome. Most patients improved quickly after adequate antibiotic treatment but chronic cases responded poorly. These protean neurological manifestations of brucellosis indicate that the underlying pathological mechanisms are diverse.
