The recognition of mechanically induced pelvic pain and organic dysfunction in the low back pain patient.

Mechanical disorders of the lumbar spine have been identified as a cause of pelvic pain and organic dysfunction (PPOD). Categorically, the clinical features indicative of mechanically induced PPOD fall into three areas: the history of the development or onset of pelvic symptomatology attributable to lower sacral nerve root compression (LSNRC), identification of related symptomatology on presentation, and the recognition of clinical findings indicative of mechanically induced PPOD on examination. Characteristic features of each category are presented. The clinical signs that most reliably indic

Magnetic stimulation in the diagnosis of lumbosacral radiculopathy.

Five patients presenting with sensory-motor disturbances consistent with a clinical diagnosis of L5 or S1 radiculopathy were studied. All had conventional nerve conduction tests and electromyography. The lumbosacral roots were stimulated in the lumbosacral region by using the Cadwell MES-10 Magneto-electric stimulator. The compound muscle action potentials were recorded bilaterally by surface electrodes applied to the soleus and tibialis anterior muscles. The latencies to the affected muscles were significantly prolonged. The appropriate root dysfunction was confirmed at operation or by the im

Intervertebral foramen venous obstruction. A cause of periradicular fibrosis?

Disc herniation into the intervertebral foramen (IVF) or osteophytic outgrowths from the margins of the apophyseal joints that project into the IVF may compress the neural structures, but in this cadaveric study of 160 lumbar foramens (age range, 35-91 years), the authors have found that they were much more commonly associated with compression and distortion of the large venous plexus within the IVF. In the absence of direct nerve compression (seen in only eight specimens), the most severe neural changes were associated with compression, congestion, and resultant dilatation of foraminal veins.

[Traumatic lesions of the brachial plexus. Evaluation of 144 cases studied using direct cervical myelography and non-ionic contrast media].

The authors report their experience in 144 patients with traumatic brachial plexus injury who underwent Direct Cervical Myelography (DCM). Sometimes the diagnostic investigation was completed by CT. Various myelographic patterns are described: pseudomeningocele, missing sheet of the root, scarring lesions. In 9 cases only, myelography was not sufficient to provide a complete diagnosis. The examination showed all plexus roots lacerated in 14 patients, a monoradicular lesion in 75 cases, and no lesion in 26 cases. Twenty-one out of the 26 negative cases were confirmed during surgery, while in 2

AAEE minimonograph #32: the electrophysiologic examination in patients with radiculopathies.

A brief history of the evolution of radiculopathy as a clinical entity, and the use of electrodiagnostic studies to diagnose it, are provided. Root anatomy and the concept of myotomes and dermatomes are reviewed, as is the pathophysiology of radiculopathy. The value and limitations of the various electrophysiologic procedures used in the diagnosis of radiculopathies are discussed, including motor and sensory nerve conduction studies, late responses, somatosensory evoked potentials, nerve root stimulation, and the needle electrode examination. The specific muscles are enumerated which most ofte

Metastatic malignant glioma with resultant extradural compression of a sacral nerve root.

A patient with a malignant cerebral mixed oligodendroglioma-astrocytoma presented with a severe, painful S-1 radiculopathy. Neuroradiological examinations revealed an extradural soft-tissue mass compressing the first sacral nerve root. Spinal fluid examination and myelography gave no evidence of intradural disease. Surgery confirmed the presence of metastatic glioma compressing the S-1 nerve root. The tumor was entirely epidural in location. Mechanisms of dissemination are discussed.

Lumbar synovial or ganglion cysts.

Most reports regarding synovial cysts of the spinal canal have been presentations identifying an unusual pathological entity that is to be included in the differential diagnosis of cauda equina compression syndromes. Most of the 26 cases reported represent isolated examples of this pathological process. We present five cases of lumbar synovial cysts encountered in our practice in the past 8 years. Patients with lumbar synovial cysts do not demonstrate any predictable clinical picture. They may present with a unilateral sciatica or neurogenic claudication. Lumbar extension is usually restricted

[Thermographic studies with fluid crystals in peripheral nerve damage].

Common peripheral neurogenic lesion in various locations and of different origins were examined thermographically and the results were compared with those obtained by clinical and electromyographic methods. Lesions in the regions of the N. ulnaris and N. medianus can, except in the case of carpal tunnel syndrome, be identified without difficulty by thermography. The results obtained in the case of paresis of the radialis are non-specific, and thermographic examination of radicular lesions are also sometimes ambiguous. Damage in the plexus-brachialis and root region yield impressive results whi

C7 radiculopathy: importance of scapular winging in clinical diagnosis.

Lesions of the seventh cervical (C7) root are common and cause a readily recognised neurological syndrome. Recognition of this pattern is essential in differentiating C7 root lesions from lesions of the brachial plexus or peripheral nerves. Serratus anterior weakness is not generally included in this syndrome. We report six verified cases of C7 radiculopathy in which weakness of the serratus anterior was present in addition to the usual findings. This was manifest as winging of the scapula, when pushing forward against a wall, either with the hands at shoulder level or, in some cases, only whe

Combined trigeminal and glossopharyngeal neuralgia.

A case of combined trigeminal and glossopharyngeal neuralgia is described. The superior cerebellar artery and normal choroid plexus compressed and indented the root entry zones of the trigeminal and glossopharyngeal nerves, respectively. Complete relief was obtained after microvascular decompression and resection of the choroid plexus.

Electrodiagnostic localization of traumatic upper trunk brachial plexopathy.

Eighteen patients having traumatic upper trunk brachial plexopathy ("the stinger"), a common football injury, were investigated electrodiagnostically. Proximal nerve conduction was determined by stimulating the supraclavicular fossa and at the C5 root and recording from muscles supplied by the long thoracic, suprascapular, musculocutaneous, axillary, lateral pectoral and thoracodorsal nerves. The accessory nerve was stimulated in the lateral posterior triangle and the evoked potential recorded from the upper trapezius muscle. Median and ulnar nerves were also tested, sensory and motor fibers b

Evoked potentials in the investigation of traumatic lesions of the peripheral nerve and the brachial plexus.

Nerve conduction studies using nerve action potential (NAP), sensory nerve action potential (SNAP), evoked muscle action potential (M-response), retrograde conduction in the motor axon (F-response), and cortical and subcortical somatosensory evoked potential (SEP) are useful tools for evaluation of the peripheral nervous system. SEP recording has the advantages of being applicable to severely damaged nerves because of its amplification mechanism and of disclosing proximal root lesions that would not be disclosed by other methods. When SEP recording is used in an operating theater, the amplific

[Traumatic elongation-traction lesions of the roots of the sciatic nerve, secondary to fractures of the pelvis].

Three cases of complex pelvic fractures associated with lumbo sacral roots lesions are reported. The authors stress this possible association should be suspected through clinical and conventional radiological findings. The contrast sacco-radiculography brings up confirmation. However the so called pseudo-meningocele is by far of less topographical value than the same myelographic findings at cervical level (for brachial plexus avulsion injuries). Direct and concise evaluation is performed by surgical approach of roots during a homolateral hemi-laminectomy. This procedure allows to appreciate t

Neurologic painful conditions affecting the shoulder.

Radiating pain accompanied by motor or sensory change, particularly below the elbow, strongly suggests a neurologic etiology of shoulder pain. The patient's description of the pain often is misleading because the shoulder joint has a movable fulcrum, which may be the site of a secondary irritant rather than a primary site. The pain may originate medially in the spinal cord or as far distal as the axillary border of the scapula. Precise identification usually requires myelography or discography in addition to a routine careful investigation. Included in this group are spinal cord tumors, extrud

Digital nerve sensory potentials in lesions of cervical roots and brachial plexus.

Normal values for the amplitudes of digital sensory nerve action potentials were obtained from 190 control subjects, using an orthodromic surface-recording technique. Lower limits were calculated, taking into account laterality, age and sex; the ratios of amplitudes between different digits were also estimated. The results in control subjects were compared with those of 20 cases of trauma involving root avulsion or extraspinal injury, 38 patients with intraspinal disease, 6 patients with extraspinal disorder and 9 cases of neuralgic amyotrophy. The variability of the dermatomes is discussed an

[Electromyography and electroneurography in peripheral nerve lesions. Part II].

Electromyographical and electroneurographical examinations are indicated in the forefield of the surgical treatment for radicular syndromes, traumatic plexus and nerve lesions as well as compression or bottle-neck syndromes. The electrophysiological findings point out the neurogenic nature of the disturbance. They furthermore furnish statements with respect to the localisation, the extent, the kind and the prognosis of the lesions. The electromyographical and electroneurographical examination programme is determined by the anatomical innervation conditions and the questions that are of clinica

[Diagnostic value of somatosensory evoked potentials (SEP) in lesions of the brachial plexus (author's transl)].

Simultaneous recording of SEP from the contralateral scalp, the lower neck (C 7) and Erb's point following median or ulnar nerve stimulation may be helpful in the localisation of lesions of the brachial plexus and associated spinal roots. In lesions proximal to the dorsal root ganglia, the potential recorded from Erb's point (EP), as well as the first component of the neck-SEP ("NSEP 1", "N 9") are normal, whereas the following components of the cervical response ("NSEP 2 + 3", "N 11 + N 13") are reduced or absent, depending on the severity of the functional impairment (Fig. 1). In lesions dis

Micturition and the male genitourinary response to sacral root stimulation.

The sacral roots, because of their anatomic and functional organization, are well suited to chronic stimulation. Data from our previous experiments on female dogs suggested that effective voiding responses to stimulation could be obtained by using the ventral root of S2, the sacral nerve in canines with the greatest detrusor representation. Elimination of dorsal root afferents from the stimulus field was essential to the minimization of spinal reflex activity. A selective somatic neurotomy was also necessary to attenuate maximally sphincter activity. In the male, the presence of the prostate g

Spinal nerve block. A diagnostic test in sciatica.

The cause of sciatica has been studied by blocking spinal nerves of the lumbosacral plexus in intervertebral foramina. The nerve, which is singled out with the aid of an image intensifier, is injected with 1 ml of 1 per cent Xylocain. If the Xylocain injection eliminates the sciatic pain, the surgeon may be confident that a true nerve-root compression is involved, and he can then explore as far as he finds necessary. The correlation between the site of compression and the level indicated by the test was confirmed by operation in 19 patients.
