LM22B-10 promotes corneal nerve regeneration through in vitro 3D co-culture model and in vivo corneal injury model.

Corneal nerve wounding often causes abnormalities in the cornea and even blindness in severe cases. In this study, we construct a dorsal root ganglion-corneal stromal cell (DRG-CSC, DS) co-culture 3D model to explore the mechanism of corneal nerve regeneration. Firstly, this model consists of DRG collagen grafts sandwiched by orthogonally stacked and orderly arranged CSC-laden plastic compressed collagen. Nerve bundles extend into the entire corneal stroma within 14 days, and they also have orthogonal patterns. This nerve prevents CSCs from apoptosis in the serum withdrawal medium. The conditi

How Prone Position Affects the Anatomy of Lumbar Nerve Roots and Psoas Morphology for Prone Transpsoas Lumbar Interbody Fusion.

BACKGROUND: Lumbar nerve root injury is a serious complication of transpsoas lumbar interbody fusion. Researchers have reported lumbar nerve roots and psoas muscle anatomy in the supine position, but no previous studies have used prone position magnetic resonance imaging to evaluate these structures. OBJECTIVE: The purpose of this study was to show the changes in the lumbar nerve roots and psoas muscle related to the lumbar intervertebral disc using supine and prone magnetic resonance imaging. METHODS: Thirty volunteers without spinal or hip abnormalities were included in this observational

Anatomy of the posterolateral spinal epidural ligaments.

BACKGROUND: The epidural ligaments (ELs) (of Hofmann) were described as fibrous bands interconnecting the ventrolateral spinal dura and the posterior longitudinal ligament below L1. They are hardly ever discussed in the literature or considered in hypothesis-driven basic science experiments or spine biomechanical models. METHODS: Intraoperative photographs were obtained to illustrate a group of posterolateral spinal ELs. In addition, electronic database searches (PubMed, Ovid Embase, and SCOPUS) were utilized to summarize the anatomy, and relevant clinical and surgical factors impacting these

Utility of Preoperative Electrodiagnosis Together with Peripheral Nerve High-Resolution Ultrasound: A Complex Case Report of Neurofibromatosis Type I.

Our case report underscores the importance of electroneuromyography (ENMG) combined with peripheral nerve high-resolution ultrasound (HRUS) in the evaluation of neurofibromatosis type 1 (NF1). A 49-year-old woman affected by NF1 came to our attention because of new-onset left arm weakness and atrophy. Debulking of a cervicothoracic C7-T1 neurofibroma had been performed 8 years earlier. On current admission, magnetic resonance imaging disclosed increased lesion volume that was thought to cause the neurologic deficits by compressing the C8 root. Findings from intraoperative neurophysiologic moni

High-Resolution Ultrasonography in the Assessment of Lumbar Radiculoplexus Neuropathy in Diabetics: Preliminary Results of a Case-Control Study.

Background  The traditional diagnosis of lumbar radiculoplexus neuropathy (LRN) is based on a classical sequence of symptoms and targeted electrodiagnostic examination by means of electromyography. Ultrasonography reliably indicates the level of lumbar radiculopathy by assessing edema mesial to the site of compression. Materials and Methods  This case-control study was undertaken at a tertiary care hospital between July 2017 and June 2019 on 15 diabetic patients with symptoms of LRN. Fifteen healthy volunteers with no symptoms or clinical signs of LRN were included in the control group. The di

Endovascular Treatment of a Lumbar Spinal Epidural Arteriovenous Fistula with Radiculopathy: A Case Report.

Spinal epidural arteriovenous fistulas (SEDAVFs) are rare spinal vascular malformations that are difficult to diagnose and treat. SEDAVFs can be asymptomatic; however, symptoms can arise from the compression of adjacent nerve roots by dilated vein and perimedullary venous reflux, caused by shunting into the epidural venous plexus. A 31-year-old male presented to our institution with a 2-year history of progressively worsening low-back pain, radiating thigh pain, and sensory changes in his lower extremities. MRI and CT angiography demonstrated dilated epidural vascular lesion compressing the ne

Neurologic Causes of Thoracic Limb Lameness.

Although lameness of the thoracic limb typically is due to orthopedic disease, there are several important neurologic conditions that result in lameness. Neurologic diseases cause lameness due to disease of the nerves, nerve roots, spinal cord, or muscles. Common differentials include lateralized intervertebral disc extrusions, caudal cervical spondylomyelopathy (wobbler disease), brachial plexus avulsion, neuritis, and peripheral nerve sheath tumors. Many of these diseases compress or destroy the nerve roots of the cervical intumescence, resulting in non-weight-bearing lameness, or root signa

Microsurgical removal of a misplaced intraspinal venous stent in a patient with inferior vena cava atresia.

Inferior vena cava atresia (IVCA) is a rare vascular condition that may be treated by venous stenting. The authors report on the microsurgical removal of an intraspinally misplaced stent causing nerve root compression and neurological deficits.A 42-year-old patient with IVCA and painful cutaneous collaterals had been scheduled for treatment by stenting of the iliocaval confluence and associated venous collaterals. Initial stenting of the right iliac vein was successful; however, during recanalization of the left paravertebral plexus, the stent entered the spinal canal via extraspinal-to-intras

C5 palsy after C5/6/7 posterior foraminal decompression: A case report.

RATIONALE: Although C5 palsy is a common complication of cervical spine surgery, its cause has not been confirmed. There are various hypotheses for its mechanism, including spinal cord impairment and nerve involvement. Therefore, prophylactic foraminotomy is one of the methods recommended for preventing C5 palsy. However, we describe a patient who experienced C5 palsy after microendoscopic foraminotomy between the left C5/6 and C6/7 levels. PATIENT CONCERNS: A 43-year-old man presented with a 14-month history of progressive numbness in the left upper limb. We performed microendoscopic left fo

Finger sensory impairment after elbow flexion reconstruction using concomitant nerve transfer from the median and ulnar nerves.

PURPOSE: Single and double fascicular nerve transfer using the ulnar or median nerve is performed to restore elbow flexion following injuries to the brachial plexus or nerve root. However, little is known regarding the postoperative changes involved in the sensory alteration of the hand after a single and double fascicular nerve transfer. We evaluated the sensory alteration of the hand in patients who underwent single and double fascicular nerve transfer for two years. METHODS: A total of five patients that underwent single or double fascicular nerve transfer participated in this study. The i

Late Cervical Spinal Cord Herniation Resulting from Post-Traumatic Brachial Plexus Avulsion Injury.

BACKGROUND: Spinal cord herniation (SCH) is often described as occurring spontaneously in the thoracic spine, with few cases of cervical SCH reported as a late complication of traumatic brachial plexus avulsion. We present 2 cases of nerve root avulsion and pseudomeningocele formation, resulting in delayed cervical SCH and neurologic deterioration. CASE DESCRIPTION: Case 1: A 37-year old man presented with progressive leg weakness 2 years after experiencing traumatic C8 and T1 root avulsions. Magnetic resonance imaging (MRI) showed previously documented C8-T1 nerve avulsions with new SCH in a

Microanatomical considerations for safe uncinate removal during anterior cervical discectomy and fusion: 10-year experience.

Cervical radiculopathy from uncovertebral joint (UVJ) hypertrophy and nerve root compression often occurs anterior and lateral within the cervical intervertebral foramen, presenting a challenge for complete decompression through anterior cervical approaches owing to the intimate association with the vertebral artery and associated venous plexus. Complete uncinatectomy during anterior cervical discectomy and fusion (ACDF) is a controversial topic, many surgeons relying on indirect nerve root decompression from restoration of disc space height. However, in cases of severe UVJ hypertrophy, indire

Clinical Presentations of Lumbar Disc Degeneration and Lumbosacral Nerve Lesions.

Lumbar disc degeneration is defined as the wear and tear of lumbar intervertebral disc, and it is mainly occurring at L3-L4 and L4-S1 vertebrae. Lumbar disc degeneration may lead to disc bulging, osteophytes, loss of disc space, and compression and irritation of the adjacent nerve root. Clinical presentations associated with lumbar disc degeneration and lumbosacral nerve lesion are discogenic pain, radical pain, muscular weakness, and cutaneous. Discogenic pain is usually felt in the lumbar region, or sometimes, it may feel in the buttocks, down to the upper thighs, and it is typically present

Severe lumbar radiculopathy with epidural venous plexus engorgement in a morbidly obese pediatric patient: A case report.

INTRODUCTION: Engorgement of the epidural venous plexus (EVP) is a rare cause of nerve root impingement. Dilated epidural veins cause compression of the thecal sac and spinal nerve roots, leading to lumbar radiculopathy. PATIENT CONCERNS: Here we describe a case of severe lumbar radiculopathy in a 15-year-old morbidly obese boy. DIAGNOSIS: Enhanced lumbar magnetic resonance imaging revealed left sided L1-L2 disc protrusion and engorgement of the lumbar EVP, resulting in narrowing of the thecal sac in the entire lumbar spine. There was no evidence of an intra-abdominal mass, thrombosis of the

Outcome Of Dynamic Compression Plate With Dorsal Radial Sliding Graft Technique For Wrist Arthrodesis In Brachial Plexus Injury Patients.

We have meticulously read the article titled "Outcome of dynamic compression plate with dorsal radial sliding graft technique for wrist arthrodesis in brachial plexus injury patients". We would like to comment on some aspects of the study and set forth our experience in wrist arthrodesis performed to improve functional outcome in the scenarios cited above. This manuscript aims to highlight the following. First, to the best of our knowledge, there exists a paucity of comparative studies on the use of a reamer/iliac crest /radial sliding graft for total wrist arthrodesis, evaluating fusion times

Sciatica Caused by Venous Varix Compression of the Sciatic Nerve.

BACKGROUND: The authors report the case of a 76-year-old woman presenting with leg pain, numbness, and weakness mimicking a lumbosacral radiculopathy. CASE DESCRIPTION: Initial lumbar spine magnetic resonance imaging demonstrated mild root compression, but lumbar decompression afforded only transient symptomatic relief. Postoperative magnetic resonance imaging of the lumbosacral plexus and sciatic nerve revealed a gluteal venous varix compressing the sciatic nerve just distal to the piriformis muscle. Neurolysis and surgical resection of the offending varix resulted in resolution of her sympt
