Persistent Post-traumatic Upper Limb Pain and Paraesthesia Following a Fall on an Outstretched Hand: A Case of Inconclusive Localisation.

Persistent upper limb pain following trauma can present a diagnostic challenge, particularly when clinical findings and investigations are discordant. We report the case of a 31-year-old female patient with persistent right upper limb and anterior chest wall pain associated with sensory disturbance in a multi-regional distribution, following a fall on an outstretched hand (FOOSH). Five years after the initial injury, the patient's symptoms, pain severity, functional limitations and neurological findings remained unchanged despite extensive conservative and interventional management, including

[State-of-the-art in peripheral nerve injury diagnosis and therapy].

The undulating characteristics of nerve fibers and the endoneurium are important factors in resisting traction force. The diagnostic accuracies of magnetic resonance neurography and ultrasonography for nerve injuries are 85.4% and 70.6%, respectively. Epineurial repair is the most commonly used nerve repair technique. Nerve grafts are generally required when the defect exceeds 2 cm. Nerve transfers are primarily indicated for brachial plexus root avulsions or intraforaminal lesions. Painful neuromas can be treated with target muscle reinnervation. Surgery yields reliable results for carpal-tun

Glymphatic dysfunction in trigeminal neuralgia: A multimodal MRI study.

BACKGROUND: Trigeminal neuralgia (TN) has traditionally been attributed to peripheral neurovascular compression at the trigeminal nerve root entry zone. Recent evidence, however, suggests central mechanisms may contribute to disease pathogenesis. We investigated whether glymphatic system dysfunction occurs in TN and contributes to its pathophysiology. METHODS: This cross-sectional study enrolled 71 patients with classical TN and 52 age-matched healthy controls. All participants underwent comprehensive multimodal MRI assessment, including: (1) diffusion tensor imaging along perivascular spaces

Pre-Plexal Extension of Parsonage-Turner Syndrome With Nerve Root Involvement on Needle Electromyography.

Parsonage-Turner syndrome (PTS) is an inflammatory, immune-mediated neurogenic condition characterized by acute-onset, severe unilateral shoulder/upper arm pain followed by patchy weakness and atrophy. Often described as an idiopathic brachial plexitis, most cases of PTS involve the brachial plexus or its terminal branches, and root-level or paraspinal muscle involvement is rare. Among the differential diagnoses for proximal neuropathic root involvement, cervical radiculopathy by mechanical compression of the exiting nerve roots and diabetic cervical radiculoplexus neuropathy by way of immune-

Diagnosis and treatment of abdominal pain of spinal origin: a case report.

BACKGROUND: Clinically, abdominal pain can also be caused by lesions of the vertebral bodies, intervertebral discs, and other structures. It is usually due to degenerative changes, injuries, or herniations of the intervertebral discs, as well as stimulation or compression of the sympathetic nerve plexus associated with the vertebral bodies, leading to nonspecific abdominal pain, medically referred to as "abdominal pain of spinal origin". CASE PRESENTATION: A 70-year-old Asian female presented with a 20-year history of unexplained right lower abdominal pain, initially managed as a gastrointest

From pitch to palsy: A rare case of acute brachial plexopathy following medial clavicle fracture in women's rugby.

BACKGROUND: Medial third clavicle fractures represent less than 5% of all clavicle fractures and are rarely associated with acute brachial plexus injury. Early recognition is critical, as timely surgical intervention may influence neurological recovery. CASE: A 17-year-old elite rugby player sustained a closed, displaced medial third clavicle fracture following a high-energy tackle, confirmed on radiographs. Within 48 h, she developed profound C5-C6 sensorimotor deficits. CT angiography excluded vascular injury. MRI demonstrated focal T2 hyperintensity and enlargement of the upper trunk consi

Case Report on Contralateral Neural Complications following OLIF.

Oblique Lumbar Interbody Fusion (OLIF) is widely used minimally invasive techniques for lumbar degenerative diseases. While neurological complications are recognized, they predominantly affect the ipsilateral lumbar plexus. Contralateral L4 nerve root compression is an exceptionally rare and often overlooked complication, with limited understanding of its biomechanical mechanisms and sparse radiological evidence linking symptoms to specific causes. A 61-year-old female underwent L3/4 and L4/5 OLIF via a left retroperitoneal approach for degenerative spondylolisthesis and spinal stenosis. Posto

Correlation between lumbar disc herniation nerve root compression and clinical symptoms: a study using lumbosacral plexus magnetic resonance neurography combined with T2 mapping.

OBJECTIVE: Lumbar disc herniation (LDH) is common, often leading to lumbosacral nerve damage in affected patients. This study aims to assess the correlation between nerve root compression and clinical symptoms of LDH using magnetic resonance neurography (MRN) combined with T2 mapping. METHODS: This prospective study included patients with unilateral LDH-related nerve root compression. All participants underwent 3.0T MRI with 3D SHINKEI and T2 mapping sequences. Clinical symptoms were evaluated using ODI and VAS scores. Measure T2 values of the lumbosacral plexus (LSP) using T2 mapping and cal

An incidental finding of additional vascular compression of the brachial plexus during ultrasound-guided anterior scalene bupivacaine injection for neurogenic thoracic outlet syndrome: addition of hydrodissection-a case report.

BACKGROUND: Bony, muscular, and fibrous abnormalities in the thoracic outlet are implicated in the pathogenesis of neurogenic thoracic outlet syndrome (nTOS). Compression of the brachial plexus by the dorsal scapular artery (DSA) is rarely reported and is often diagnosed incidentally during cadaveric dissection or surgical decompression. CASE PRESENTATION: We present the case of a 51-year-old Middle Eastern female who presented with chronic left cervicobrachialgia associated with left-hand paresthesia and weakness. Physical examination supported the diagnosis of nTOS. Imaging revealed an elon

Ultrasound-guided brachial plexus hydrodissection combined with acupotomy release for the treatment of cervical spondylotic radiculopathy: a multicenter retrospective study.

INTRODUCTION: The incidence of cervical spondylotic radiculopathy (CSR) remains high. Conventional conservative treatments have limited efficacy in relieving mechanical nerve root compression, while surgery is highly invasive. Both acupotomy release and ultrasound-guided brachial plexus hydrodissection are minimally invasive interventions for CSR. Nevertheless, high-level multicenter evidence-based medical data regarding the safety and efficacy of their combined application are still lacking. METHODS: This multicenter retrospective study enrolled 264 patients with CSR treated at four traditio

Successful management of interdural spinal cysts presenting as radiculopathy caused by epidural venous enlargement: illustrative case.

BACKGROUND: The authors report the first case of thoracic interdural spinal cysts presenting as radiculopathy attributed to overdrainage-related cervical venous plexus enlargement. This case emphasizes the importance of considering interdural spinal cysts and cerebrospinal fluid overdrainage in the differential diagnosis of radiculopathy. OBSERVATIONS: A 37-year-old male patient with a history of orthostatic headache presented with bilateral deltoid muscle atrophy consistent with C5 radiculopathy. Postcontrast magnetic resonance imaging (MRI) revealed cervical epidural venous plexus enlargeme

Twelfth Rib Length as a Predictor of Anatomic Variations in the Lumbosacral Plexus Associated With Atypical Radiculopathy in Lumbar Disc Herniation.

STUDY DESIGN: A combined clinical and cadaveric observational study. OBJECTIVES: To investigate whether anatomic variations in the lumbosacral plexus (LSP) are associated with diagnostic discrepancies in lumbar disc herniation (LDH) and to corroborate clinical findings with anatomic evidence. SUMMARY OF BACKGROUND DATA: LDH is typically diagnosed based on clinical neurological symptoms and the level of the compressed spinal nerve root (the responsible lesion) identified by magnetic resonance imaging. However, in some patients, radiculopathy symptoms do not always align with the responsible l

Interventional Therapies to Treat Cancer Associated Pain.

OPINION STATEMENT: Cancer associated pain (CAP) is challenging to treat and may be complicated by type of cancer, location, presence of metastases, cancer therapies, medical co-morbidities, psychosocial factors, and barriers to treatment. Inadequate CAP management can lead to delays in receiving chemo/radiation therapy, and medications or interventions that provide analgesia. In this narrative review, we aim to provide an evidence-based overview of current and emerging interventional pain techniques used to manage CAP. Intractable visceral cancer pain refractory to conservative medical managem

Traumatic Spinal Epidural Hematoma Associated with Cervical Nerve Root Avulsion without Vertebral Fractures: Case Report.

Traumatic spinal epidural hematoma (TSEH) is a rare condition that may cause acute spinal cord compression and lead to irreversible neurological impairment. TSEH not only compresses the cord, but it can also worsen cervical nerve root avulsion. To our knowledge, only five cases of combined TSEH and cervical nerve root avulsion have been reported in the literature. We present the case of a 42-year-old woman who suffered a motorcycle accident. On admission, she presented with mild traumatic brain injury and cervical spine and right shoulder trauma. A physical examination revealed numbness and fl

Cadaveric Report of an Unusual Axillary Artery Branching Variation: A Potential Compression Site for the Lateral Root of the Median Nerve.

The median nerve is formed by the union of lateral and medial roots originating from the lateral and medial cords of the brachial plexus, respectively. The lateral root of the median nerve is responsible for motor innervation to some of the anterior forearm muscles as well as sensory innervation to the lateral palm and the palmar surfaces of the lateral three-and-a-half digits. While anatomical variations in the axillary artery's branching pattern are relatively common, vascular compression of the median nerve, particularly its individual roots, is exceedingly uncommon. In this report, we obse

Epidemiology of Stingers in the National Football League, 2015-2019.

BACKGROUND: Transient traumatic neuropraxia of either the brachial plexus or cervical nerve root(s) is commonly described as a "stinger" or "burner" by the athlete. Stingers in American Football commonly occur acutely as isolated injuries; however, concomitant injuries, including cervical spine pathologies, have also been reported. HYPOTHESIS: Among National Football League (NFL) athletes, the incidence rate of stingers is higher during the regular season than during the preseason and among positions with high velocity impacts such as running backs, linebackers, defensive backs, and receivers

A clinical review of hand manifestations of cervical myelopathy, cervical radiculopathy, radial, ulnar, and median nerve neuropathies.

Cervical spondylotic myelopathy (CSM) is defined as compression of the spinal cord in the neck, resulting in problems with fine motor skills, hand numbness, pain or stiffness of the neck, and difficulty walking due to loss of balance. Brachial plexus (BP) neuropathies arise due to compression to any distal branches arising from C5-T1, whereas cervical radiculopathy involves compression at the nerve root in the neck. Such conditions can present with variable degrees of musculoskeletal pain, weakness, sensory changes, and reflex changes. The pronounced convergence in symptomatic manifestation wi
