Pelvic Nerve Injury Causes a Rapid Decrease in Expression of Choline Acetyltransferase and Upregulation of c-Jun and ATF-3 in a Distinct Population of Sacral Preganglionic Neurons.

Autonomic regulation of the urogenital organs is impaired by injuries sustained during pelvic surgery or compression of lumbosacral spinal nerves (e.g., cauda equina syndrome). To understand the impact of injury on both sympathetic and parasympathetic components of this nerve supply, we performed an experimental surgical and immunohistochemical study on adult male rats, where the structure of this complex part of the nervous system has been well defined. We performed unilateral transection of pelvic or hypogastric nerves and analyzed relevant regions of lumbar and sacral spinal cord, up to 4 w

C5 palsy after cervical laminoplasty: a multicentre study.

We have reviewed 1858 patients who had undergone a cervical laminoplasty and identified 43 (2.3%) who had developed a C5 palsy with a MMT (MRC) grade of 0 to 2 in the deltoid, with or without involvement of the biceps, but with no loss of muscular strength in any other muscles. The clinical features and radiological findings of patients with (group P; 43 patients) and without (group C; 100 patients) C5 palsy were compared. CT scanning of group P revealed a significant narrowing of the intervertebral foramen of C5 (p < 0.005) and a larger superior articular process (p < 0.05). On MRI, the poste

[Percutaneous electrical nerve stimulation of peripheral nerve for the intractable occipital neuralgia].

UNLABELLED: Occipital neuralgia is characterized by pain paroxysm occurring within distribution of the greater or lesser occipital nerves. The pain may radiates from the rear head toward the ipso-lateral frontal or retro-orbital regions of head. Though known causes include head injuries, direct occipital nerve trauma, neuroma formation or upper cervical root compression, most people have no demonstrable lesion. METHOD AND MATERIALS: A sample of 8 patients (5 females, 3 males) aging 63,5 years on the average with occipital neuralgia has been recruited. The occipital neuralgic pain had presente

[Somatosensory evoked potentials/fields--exploration of brain function].

We have summarized the history of electroencephalography(EEG) since 1875, when a paper by Richard Caton was published describing the first EEG recordings in animals. Somatosensory evoked potentials (SEPs) were recorded by George Dawson in 1951. Thereafter, SEPs were developed for clinical use with other evoked potentials such as auditory evoked potentials(VEPs). To understand evoked potentials, related mechanism of induction of far-fields-potentials(FFP) following stimulation of the median nerve has been discussed. SEPs consisted of P9, N9, N10, P11, N11, N13, P13, P14, N18, N20 and P20/P22. S

Topographic relations between the neural and ligamentous structures of the lumbosacral junction: in-vitro investigation.

Transient or persistent alterations in the L5 lumbar nerve root have been described as a complication after the reduction of spondylolistheses. In a retrospective analysis of our own patients, we observed a higher incidence of transient motor and sensomotor deficits after single-sitting anatomic correction of May-erding grade IV anterolistheses and spondyloptoses. These findings are consistent with those of other authors. The deficits pertained to muscles in the innervation range of the L5 nerve root, without there being any evidence of intradural root damage or nerve compression. In vitro stu

[Anatomical background of low back pain: variability and degeneration of the lumbar spinal canal and intervertebral disc].

The central and lateral lumbar canals constitute complex osteofibrous neurovascular tunnels, allowing movement and deformation of the spine without loss of their main configuration. Intervertebral discs play an important role in determining their configuration. Disc degeneration may alter or even threat the functional anatomical relationships between successive adjacent "juncturae" of the vertebral column. Shape and morphometric aspects of the bony neural canals reveal level dependency [39], inter-individual variation [11], and are particularly susceptible for changes with aging [49]. Articula

Surgical treatment of non-responsive cervicogenic headache.

OBJECTIVE: 102 patients suffering from long-lasting, very severe cervicogenic headache (CEH), non-responsive to physical or drug therapy, were surgically treated. METHODS: Different diagnostic procedures and their significance for determining the advisability of surgery are summarised. The particular importance of the patient history and local anaesthesia together with the clinical examination is outlined. Different surgical approaches are described: ganglionectomy, ventral and dorsal decompressive operation. RESULTS: CEH can be triggered by vascular or scar tissue compression of the C2 root

Cervical root stimulation in a case of classic neurogenic thoracic outlet syndrome.

We performed C8 nerve root stimulation in addition to other electromyographic (EMG) studies in a surgically proven case of classic thoracic outlet syndrome (TOS). The patient was a 19-year-old woman with a 2-year history of right hand cramps and progressive weakness and atrophy of hand muscles, especially the thenar eminence. Routine EMG studies showed evidence for an axon-loss lower trunk brachial plexopathy. Stimulation studies of the C8 nerve roots demonstrated proximal conduction block on the affected side only. The diagnosis was further supported by cervical spine radiographs, which demon

The double-crush phenomenon--an unusual presentation and literature review.

The double-crush syndrome was initially described by Upton and McComas in 1973. They postulated that nonsymptomatic impairment of axoplasmic flow at more than one site along a nerve might summate to cause a symptomatic neuropathy. This was suggested by their clinical observation that the majority of their patients had a median or ulnar neuropathy associated with evidence of cervicothoracic root lesions. They also hypothesized that one of the constraints on axoplasmic flow could be a metabolic neuropathy, and this is supported by the high association of diabetes and carpal tunnel syndrome. Othe

Morphometric evaluation of the sacral dorsal root ganglia. A cadaveric study.

An anatomic study of the sacral dorsal root ganglia (DRG) was performed to determine the location and dimensions of the S1-4 DRGs and to correlate this information to sacral nerve root ganglion lesions. S1 DRGs were located in the intraforaminal region in 55-60% and in the intracanalar region in 40-45%. S2 DRGs were in the intraforaminal region in 15-50% and in the intracanalar region in 50-85%. All the S3 and S4 DRGs were located in the intracanalar region. None of the sacral DRGs was located in the extraforaminal region. The intraforaminal position of the S1 and S2 ganglia renders them vulne

[Differential sciatica pain diagnosis from the neurologic viewpoint].

Low back pain and sciatica with or without neurological deficits represent frequent disorders in clinical practice. Both, history and neurological examination are essential for localizing underlying lesions and initiating appropriate investigations. Pain location, sensory symptoms, motor signs and reflexes are extremely reliable in indicating which motor root(s) or peripheral nerve(s) are affected. Sometimes, pain and/or paraesthesia are the only presenting symptoms. Therefore differential diagnosis of low back pain and sciatica is various. This article summarizes the differential diagnosis of

A variation of the median nerve at the level of brachial plexus.

We report a variation of the median nerve at the level of brachial plexus in a 38 years old male cadaver. The median nerve was formed by the fusion of three branches; two of them coming from the lateral cord, and one from the medial cord. The abnormal root coming from the lateral cord had a very close oblique course over the axillary artery. These kinds of variations are more prone to injury in radical neck dissections and in other surgical operations of the axilla. Secondly, the very close course of the unusual branch with the axillary artery may lessen the blood supply of the upper extremity

[Acute radicular paralysis of arm and hand].

Patients with cervical radicular lesions present themselves with neck pain and stiffness on the one side and with neurological deficits of radicular distribution on the other side. The most frequent cause is a root compression due to a herniated disk or spondylotic changes of the spine. The management of the patients and the differential diagnosis are discussed.

Evoked potentials in the investigation of traumatic lesions of the peripheral nerve and the brachial plexus.

Nerve conduction studies using nerve action potential (NAP), sensory nerve action potential (SNAP), evoked muscle action potential (M-response), retrograde conduction in the motor axon (F-response), and cortical and subcortical somatosensory evoked potential (SEP) are useful tools for evaluation of the peripheral nervous system. SEP recording has the advantages of being applicable to severely damaged nerves because of its amplification mechanism and of disclosing proximal root lesions that would not be disclosed by other methods. When SEP recording is used in an operating theater, the amplific
