C5-Predominant Segmental Zoster Paresis Presenting With Right Shoulder Abduction Weakness Before Cutaneous Eruption: A Case Report.

Segmental zoster paresis (SZP) is an uncommon motor complication of herpes zoster (HZ) characterized by focal, asymmetric weakness in a myotomal distribution. Motor weakness usually follows the cutaneous eruption, and pre-eruptive weakness can mimic cervical radiculopathy, brachial plexopathy, neuralgic amyotrophy, or other causes of acute focal limb weakness. A 57-year-old woman developed right shoulder pain and, five days later, became unable to abduct her right arm. A vesicular eruption appeared on the lateral aspect of the right upper arm three days after the onset of weakness, and HZ was

Endoscopic versus Open Carpal Tunnel Release: A Systematic Review and Meta-analysis.

Carpal tunnel syndrome accounts for the most prevalent type of peripheral neuropathy encountered by surgeons in clinics worldwide. Despite several studies investigating endoscopic carpal tunnel release (ECTR) in comparison to open carpal tunnel release (OCTR), the best option is yet to be identified. In this systematic review and meta-analysis, we sought to evaluate and compare the surgery outcomes and complications associated with ECTR and OCTR by analyzing evidence from randomized controlled trials (RCTs) to determine if one technique offers superior results compared with the other. In this

[Carpal Tunnel Syndrome and Lumbar Spinal Stenosis: When to Suspect Amyloidosis and How to Proceed with the Diagnostic Work-up].

Transthyretin (TTR) amyloidosis, particularly the wild-type form (ATTRwt), is an age-related systemic disease characterized by the deposition of misfolded TTR in tendons and ligaments. Patients therefore frequently present initially to orthopaedic or hand surgeons with carpal tunnel syndrome (CTS) or lumbar spinal stenosis (LSS) rather than to cardiologists or neurologists. These musculoskeletal manifestations may precede ATTR cardiomyopathy (ATTR-CM) by several years to more than a decade, serving as "red flags" that provide an opportunity for early diagnosis, particularly now that disease-mo

Readability, CPT Billing Accuracy and Diagnostic Coding Performance of ChatGPT Models in Hand Surgery.

Background: Large language models (LLMs) like ChatGPT show their potential use in healthcare. However, limited research has been conducted on how effective these models are in hand surgery. Therefore, the purpose of this study is to compare ChatGPT-4, ChatGPT-4o and ChatGPT-o1 across patient education, billing and diagnostic coding in hand surgery. Methods: LLMs were tested using standardised prompts for five common hand conditions: distal radius fractures, trigger finger, carpal tunnel syndrome, cubital tunnel syndrome and ganglion cysts. Three tasks were evaluated: (1) patient education (15

Short-latency afferent inhibition in unilateral carpal tunnel syndrome.

Carpal tunnel syndrome (CTS) alters sensory afferent input and may affect cortical sensorimotor processing. We investigated short-latency afferent inhibition (SAI) and its relationship with fine motor performance in 25 patients with unilateral mild-to-moderate CTS. SAI was assessed using individualized interstimulus intervals based on N20 latency. SAI did not differ between the affected and unaffected sides (p = .539). However, higher SAI values, indicating less inhibition, were associated with poorer fine motor performance, with longer Nine-Hole Peg Test completion times on the affected side

A Large Palmar Atypical Lipomatous Tumor Extending Into the Carpal Tunnel and Guyon's Canal: Challenges in Nerve-Sparing Surgical Excision.

We report a complex case of a large atypical lipomatous tumor (ALT) extending into the carpal tunnel and Guyon's canal, highlighting the diagnostic considerations, anatomical challenges, and principles of safe surgical excision. A 67-year-old female presented with a progressively enlarging swelling involving the thenar, mid-palmar, distal palmar, and wrist regions. Examination revealed a large palmar mass without neurovascular deficit or muscle wasting. MRI demonstrated a 63 × 27 × 79 mm lipomatous lesion arising from the volar wrist and extending into the carpal tunnel and Guyon's canal, with

Tumor-Related Trigeminal Neuralgia: Clinical Features, Pathophysiology, and the Contemporary Role of Radiosurgery in Tumor Control and Pain Relief.

Trigeminal neuralgia arising from an intracranial tumor is uncommon but therapeutically distinctive because the responsible lesion is simultaneously the generator of facial pain and an independent oncological problem. When anatomically feasible, microsurgical treatment remains the preferred approach for achieving rapid, deep, and durable pain relief, particularly when decompression of the trigeminal nerve can be achieved. However, a clinically important subset of patients may be unsuitable for open surgery because of age, fitness, comorbidity, tumor anatomy, or operative risk, while others may

Change in Carpal Tunnel Pressure With Dynamic Wrist Flexion/Extension and Radioulnar Deviation: A Cadaveric Study.

Carpal tunnel pressure (CTP) is affected by flexion/extension and radioulnar deviation of the wrist. The purpose of this study was to investigate the elevations in CTP as a function of dynamic wrist motion in cadaveric hands. A goniometer was attached on hand dorsal side of cadaveric hands (n = 5). A micro-tip catheter pressure transducer was placed inside the carpal tunnel. With a baseline pressure set around 30 mmHg, the wrist was dynamically and randomly moved in its flexion/extension and radioulnar workspace while position and CTP were simultaneously recorded. CTP as a function of flexion/

Does Body Mass Index Influence Time to Surgery After Diagnosis of Carpal Tunnel Syndrome?

BACKGROUND: Symptom severity, failure of conservative treatment, and health care access may influence surgical timing of carpal tunnel release (CTR) after diagnosis of carpal tunnel syndrome (CTS). However, the relationship between body mass index (BMI) and the time to surgery (TTS) after diagnosis of CTS remains unclear. The purpose of this study was to assess whether BMI is associated with TTS after CTS diagnosis. METHODS: We conducted a retrospective cohort study using a multicenter registry to identify patients with a diagnosis of CTS. Patients were stratified into 6 BMI categories (20.0-

Efficacy of US-guided Hydrodissection for Carpal Tunnel Syndrome: A Prospective Randomized Controlled Trial.

Background US-guided hydrodissection is a minimally invasive treatment for nerve entrapment syndromes. However, its independent therapeutic efficacy remains unproven because most studies combine hydrodissection with corticosteroids or dextrose. Purpose To evaluate and compare the clinical and sonographic outcomes and complication rates of three US-guided hydrodissection techniques for the treatment of carpal tunnel syndrome (CTS) and to assess the relative efficacy of hydrodissection. Materials and Methods Adults with refractory CTS were included in this prospective randomized controlled trial

Comparative Analysis of Patient Perspectives on Surgical Centers: A Study of Carpal Tunnel Releases Performed At An Outpatient Surgery Center Versus Clinical Procedural Suite.

BACKGROUND: This study compares outcomes of carpal tunnel release (CTR) performed in a clinic procedural suite (PS) versus an ambulatory surgery center (ASC). CTR in the PS is performed using the wide-awake local anesthesia no tourniquet (WALANT) technique and does not require NPO compliance. In contrast, CTR in an ASC is performed in an operating room under either WALANT or monitored anesthesia care (MAC), the latter requiring preoperative NPO restrictions. Prior studies demonstrate that WALANT CTR performed in a PS is associated with significantly reduced in-house time, lower healthcare cost

Double Crush Syndrome: A Review of the Literature.

Double crush syndrome (DCS) is a condition characterized by multiple compression sites along a single peripheral nerve. The purpose of this review is to provide a focused review of the incidence, the most likely pathology, the best diagnostic tools and parameters, and treatment recommendations for DCS that have evolved in recent years. Currently reported incidence of DCS ranges broadly from 6.7% to 73%, which may be due to lack of established anatomic electrodiagnostic criteria to define DCS. Treatment outcomes were diverse, emphasizing the lack of consensus on optimal management strategies. T
