Postoperative Reduction and Persistent Enlargement of Median Nerve Cross-Sectional Area in Carpal Tunnel Syndrome: Influence of Metabolic Syndrome.

PURPOSE: Carpal tunnel syndrome (CTS) causes enlargement of the median nerve cross-sectional area (CSA), which often decreases after carpal tunnel release. However, the clinical relevance of postoperative median nerve CSA changes remains unclear. This study aimed to assess the association between postoperative changes in median nerve CSA and clinical outcomes and to identify factors linked to a persistent or increased CSA. METHODS: We enrolled 117 patients (136 wrists) with CTS who underwent endoscopic carpal tunnel release. Preoperative and 12-month postoperative assessments included ultraso

An AI-Driven Pipeline for Localization, Segmentation, and Classification of Carpal Tunnel Syndrome Using Ultrasound Images of the Median Nerve.

BACKGROUND: Carpal tunnel syndrome (CTS), the most common peripheral neuropathy, is currently diagnosed by clinical suspicion supported by tools such as the CTS-6 questionnaire and ultrasound. While these tools are widely used and clinically valuable, diagnostic accuracy may vary based on examiner experience and technique. This study introduces a pilot artificial intelligence (AI)-driven ultrasound pipeline using YOLOv11 for localization, U-Net for segmentation, and ConvNeXt for classification to support CTS diagnosis. METHODS: In this observational cohort study, patients ≥18 years were scree

The Relationship Between Duration of Dialysis and Carpal Tunnel Release Outcomes: A Systematic Review.

PURPOSE: Carpal tunnel syndrome (CTS) is a common compressive neuropathy that can become more severe in patients undergoing long-term hemodialysis (HD). Although carpal tunnel release (CTR) is the standard surgical treatment for CTS, outcomes may worsen among patients undergoing long-term HD. The aim of this systematic review is to evaluate the relationship between CTR and HD. METHODS: A systematic review was conducted by screening 569 studies. Variables included duration of HD prior to CTS diagnosis, duration of symptomatic CTS, and total duration of HD therapy prior to surgical intervention

Flexor Tendon Sheath Ganglion Causing Concurrent Trigger Wrist and Carpal Tunnel Syndrome.

INTRODUCTION: Trigger wrist is a rare condition often misdiagnosed as trigger finger or carpal tunnel syndrome (CTS). It is characterized by painful clicking, snapping, or locking of the wrist during movement and can result from space-occupying lesions, such as ganglion cysts. This case highlights an unusual presentation of trigger wrist associated with CTS caused by a ganglion cyst in the flexor tendon sheath. CASE PRESENTATION: A 44-year-old male presented with paresthesia, severe pain, and triggering of the right wrist. Imaging and histopathology confirmed a ganglion cyst between the flexo

Wild-type transthyretin amyloid cardiomyopathy diagnosed by 99mTc-HMDP scintigraphy and an endomyocardial biopsy despite inconclusive 99mTc-PYP findings.

UNLABELLED: Transthyretin amyloid cardiomyopathy (ATTR-CM) is a progressive but under-diagnosed disease. With the emergence of novel therapies, including transthyretin (TTR) stabilizers and RNA silencers, an accurate diagnosis and timely therapeutic intervention have become increasingly important. 99mTechnetium (99mTc)-labeled bone radiotracer scintigraphy, using pyrophosphate (PYP) or hydroxymethylene diphosphonate (HMDP), is a key noninvasive diagnostic tool. We herein report a case of wild-type ATTR-CM in an 87-year-old woman who presented with heart failure with a preserved ejection fracti

Diagnostic accuracy of the Hoffmann-Tinel sign for carpal tunnel syndrome: A systematic review and meta-analysis with comparison of techniques.

BACKGROUND: The Hoffmann-Tinel sign is used to diagnose and assess Carpal Tunnel Syndrome (CTS) but reported diagnostic accuracy varies widely, with multiple methods used to perform the test. This systematic review and meta-analysis compares the diagnostic accuracy of the Hoffmann-Tinel test when performed using fingertip percussion versus tendon hammer percussion, to establish the superior method which could standardise clinical practice. METHODS: A diagnostic test accuracy systematic review using PubMed, Scopus and Embase (November 2025) identified studies that reported sensitivity/specific

Explainable Ensemble Learning for Robust Severity Stratification of Carpal Tunnel Syndrome from Clinical Data.

Background/Objectives: This paper aims to design an explainable and accurate ML framework to support the automatic classification of Carpal Tunnel Syndrome (CTS) severity from structured patient data. Methods: For the experiment, an open-source dataset of 1037 samples was used. Following stratified partitioning, 305 samples were held out as the test set; the remaining training set (n = 732) was augmented to 1216 balanced samples via ADASYN, yielding an 80/20 train/test ratio relative to the final dataset (n = 1521). In order to solve the problem of imbalance associated with CTS cases of modera

Age-Stratified Sequelae After Nonoperative Distal Radius Fractures: A Propensity-Matched Cohort Study.

BACKGROUND: Distal radius fractures (DRFs) in older adults are frequently managed nonoperatively. However, treating "older adults" as a single group may obscure meaningful age-related differences in outcomes. We compared early and late sequelae after initial nonoperative management of DRFs in patients aged 50 to 69 versus 70 to 89 years. METHODS: We performed a retrospective propensity-matched cohort study using the TriNetX electronic health record network. Patients with DRFs initially treated with closed management were stratified by age (50-69 vs 70-89 years) and matched 1:1 on demographics

Clinical and injury-related factors associated with specific postoperative complications after surgical treatment of distal radius fractures: a retrospective observational study.

BACKGROUND: With an ageing population and increased physical activity, distal radius fractures (DRF) are relatively common. Surgical treatment is becoming increasingly prevalent, yet postoperative complications including complex regional pain syndrome (CRPS), tendon irritation/rupture, carpal tunnel syndrome, internal fixation‑related discomfort, and non‑union/malunion significantly impact functional recovery and quality of life. Identifying associated risk factors is crucial for optimising treatment strategies. OBJECTIVE: To investigate the clinical and injury-related risk factors for specif

Early Electrophysiological Recovery After Carpal Tunnel Release in Severe Carpal Tunnel Syndrome With Absent Preoperative Nerve Potentials.

BACKGROUND: In severe cases of carpal tunnel syndrome (CTS), compound muscle action potential (CMAP) and sensory nerve action potential (SNAP) may be absent on preoperative nerve conduction studies (NCS), reflecting advanced nerve damage. Although clinical improvement after carpal tunnel release is common, early electrophysiological recovery in this subgroup and its predictive factors remain poorly defined. This study investigated reappearance of CMAP and SNAP within 3 months after surgery and explored predictors of recovery. METHODS: This study included 53 patients with CTS who demonstrated

Wrist Flexor Spasticity and Hemiplegic-Contralateral Median Nerve Latency Asymmetry After Stroke: A Bilateral Nerve Conduction and Ultrasound Study.

Background/Objectives: The paretic wrist after stroke may exhibit median nerve conduction abnormalities, but factors underlying hemiplegic-contralateral asymmetry remain uncertain. We compared electrodiagnostic and ultrasonographic wrist measures between sides and assessed predictors of side-to-side differences in distal motor latency (ΔDML) and distal sensory latency (ΔDSL). Methods: We retrospectively analyzed 85 patients with stroke. Distal motor latency (DML), distal sensory latency (DSL), wrist-to-forearm ratio (WFR), and median nerve inlet cross-sectional area (CSA) were measured bilater

Updates in ultrasound-guided musculoskeletal interventions.

Musculoskeletal ultrasound (MSK-US) has evolved from a diagnostic tool into a key modality for guiding minimally invasive interventions across the musculoskeletal system. Its real-time imaging, portability, and absence of ionizing radiation make it a cost-effective and safe alternative to fluoroscopy and CT for numerous procedures, including biopsies, aspirations, injections, and percutaneous releases. Recent technological advances in ultrasound, including high-frequency probes, microvascular imaging, needle-tracking systems, and robotic probe steering, have improved precision and expanded its

A rare case of trifid median nerve with persistent median artery and vein in carpal tunnel syndrome.

The trifid median nerve is an anatomical variation that is rarely observed in the general population. With variations, persistent median artery and persistent median vein may also be encountered. These variations are detected incidentally during diagnostic evaluations. In our case, a trifid median nerve was found in a 64-year-old female who was diagnosed with bilateral carpal tunnel syndrome as a result of electroneuromyography results and clinical evaluation. Identification of variations of the median nerve prior to injection and surgical intervention is important to reduce the risk of compli
