A scoping review of the impact of transverse carpal ligament sectioning on the thumb carpometacarpal joint: Does it increase the risk of ostearthritis?

PURPOSE: We aimed to review the effects of carpal tunnel ligament sectioning on the thumb carpometacarpal (CMC) joint with the hypothesis that carpal tunnel release (CTR) may result in biomechanical changes at the thumb CMC joint predisposing an individual to pain and or accelerated articular wear. METHODS: This review was performed using the Pubmed and Medline databases from January 1, 1970 to January 7, 2023. Articles that examine the thumb CMC joint after CTR were categorized as cadaveric, clinical, or computational studies. The anatomy, pathophysiology, and treatment of both CMC arthritis

Finger Joint Releases in the Setting of Complex Regional Pain Syndrome: Worthwhile or Risky?

PURPOSE: The literature remains pessimistic about surgery for finger stiffness in patients with complex regional pain syndrome (CRPS), citing the risks of exacerbation and limited motion gains. This study reports the outcomes of joint-release procedures and details the surgical technique. METHODS: All patients with CRPS and contracted finger joints who presented with unyielding stiffness and consented to surgery were included. Patients underwent a minimally invasive release of the metacarpophalangeal and/or proximal interphalangeal (PIP) joints. By protocol, all patients also underwent carpal

Treatment Monitoring of Carpal Tunnel Syndrome Using Shear Wave Elastography: A Pilot In Vivo Study.

PURPOSE: Shear wave elastography (SWE) is a non-invasive technique for assessing median nerve (MN) stiffness in carpal tunnel syndrome (CTS). This study evaluated whether SWE measurements of the MN improve after corticosteroid injection (CSI) and correlate with symptom improvement in CTS. METHODS: Ten patients (18 wrists) with CTS underwent SWE, grip strength testing, and completed questionnaires before and six weeks after ultrasound-guided CSI. SWE measurements included pressure and velocity in longitudinal/transverse planes at two locations: proximal to and at the carpal tunnel. Progression

ECTR With Tourniquet and Local Anesthesia Only: Efficiency and Patient Satisfaction.

BACKGROUND: Carpal tunnel release (CTR) is commonly performed using wide-awake local anesthesia no tourniquet (WALANT) in a clinic or ambulatory surgery center and is shown to have similar satisfaction rates and postoperative complication profile compared to operating room (OR) procedures. Endoscopic carpal tunnel release (ECTR) is commonly performed; however, it is usually done in an OR setting with the use of anesthesia (OR-ECTR). We present a single institution's experience performing ECTR utilizing a tourniquet under local only anesthesia (LO-ECTR). We compared procedural efficiency and en

Rising Use of Ultrasound-Guided Carpal Tunnel Injections: National Trends, Drivers of Utilization, and Outcomes.

PURPOSE: Carpal tunnel syndrome may be considered for carpal tunnel injection (CTI). Ultrasound (US) guidance has been proposed to improve CTI accuracy, but the trend across the years, factors associated with utilization, and outcomes are not well characterized. METHODS: Patients undergoing CTI with or without US guidance were identified from the PearlDiver M170 database (2010-2022). The trend of US-guided CTI was assessed by linear regression. Patient demographics and provider specialty were compared by multivariable analyses. Two-year survival to repeat injection and carpal tunnel release (

Incidence of Median and Ulnar Neuropathy Following Nonupper Extremity Surgery.

PURPOSE: Carpal tunnel syndrome (CTS) and cubital syndrome (CuTS) are common neuropathies typically associated with upper-extremity surgeries or systemic risk factors. This study investigates the incidence of CTS and CuTS following anterior cervical discectomy and fusion (ACDF), total hip arthroplasty (THA), and total knee arthroplasty (TKA) to evaluate whether postoperative upper-extremity neuropathy is linked to perioperative systemic responses, patient-specific risk factors, or a combination of the above. METHODS: Patients who underwent ACDF, primary THA, or primary TKA at a single institu

Proof-of-concept for the application of Raman spectroscopy in differentiating the subtypes of cardiac amyloidosis.

Cardiac amyloidosis is a clinically severe condition classified into three major subtypes based on the precursor protein: immunoglobulin light-chain (κ) (ALκ) amyloidosis, immunoglobulin light-chain (λ) (ALλ) amyloidosis, and transthyretin (ATTR) amyloidosis. To improve patient outcomes, subtype-specific treatment must be initiated promptly. Therefore, rapid differentiation is required. In this study, we aimed to differentiate cardiac amyloidosis subtypes using Raman spectroscopy, which enables rapid, non-destructive, label-free and preparation-free sample analysis. Raman spectra were acquired

Postoperative Functional Recovery After Carpal Tunnel Release: A Narrative Review on Exercise-Based Rehabilitation.

Carpal tunnel release is a commonly performed surgical procedure that effectively alleviates compressive symptoms in patients with carpal tunnel syndrome. However, postoperative recovery of hand function, including grip strength, pinch strength, and coordinated hand use, varies considerably among individuals. This variability indicates that functional recovery is influenced by factors beyond the surgical intervention itself. This narrative review synthesizes the existing literature on postoperative functional recovery following carpal tunnel release, with emphasis on functional outcomes, rehab

Transcultural Adaptation of the Spanish Version of the ABILHAND Scale for Hand Surgery in a Colombian Population of Patients with Hand Pathologies.

PURPOSE: Because manual ability is a latent and complex variable that is closely linked to an individual's well-being and daily functioning, it is important to assess it through patient-reported outcome measures that capture the patient's own perception of dexterity in everyday life. This study aimed to develop a Spanish version of the ABILHAND-Hs scale that is equivalent to the original but adapted to evaluate the most prevalent hand pathologies in Colombia. METHODS: A group of bilingual experts translated the ABILHAND scale from English into Spanish using a forward-backward translation meth

Radial Artery Occlusion Impairs Median Nerve Perfusion-A Study Using Microvascular Imaging in Healthy Volunteers.

Background/Objectives: The transradial approach is widely used for vascular access in many disciplines. Radial artery occlusion (RAO) is a frequent sequel, and hand/arm pain affects 7.8% of patients. We aimed to elucidate whether RAO or ulnar artery occlusion (UAO) causes impaired neural blood flow and, thus, if symptoms may be attributable to claudication or nerve damage. Methods: Forty healthy volunteers (73% female), with a mean age of 38 years and without clinical or sonographic signs of carpal tunnel syndrome, were included. All underwent a standardized ultrasound examination (Aplio i800

Carpal tunnel syndrome caused by tophaceous gout: a case series and review of the literature.

INTRODUCTION AND IMPORTANCE: Carpal tunnel syndrome (CTS) secondary to gout is a rare but potentially overlooked condition in clinical practice. Early diagnosis and timely intervention are essential for restoring patient function and preventing recurrence. CASE PRESENTATION: We present five cases, with two patients having bilateral tophaceous CTS and two with unilateral CTS, all attributed to gout. All patients underwent open carpal tunnel release surgery. No adverse events or recurrences were noted during the follow-up period. CLINICAL DISCUSSION: CTS due to tophaceous gout is rare but impo
