Prospective validation of mean metacarpophalangeal joint extension as a measure of diabetes-related fibrotic hand manifestations.

INTRODUCTION: Hand conditions in diabetes, namely, limited joint mobility (LJM), flexor tenosynovitis (FT), carpal tunnel syndrome (CTS), and Dupuytren disease (DD), share a common pathophysiological process involving pro-fibrotic inflammation in flexor structures. A unified, quantitative measure of disease severity across these conditions is lacking, limiting correlational research. We evaluated mean metacarpophalangeal (MCP) joint extension as a potential measure of severity. METHODS: We assessed 2,405 adults, including individuals with type 1 diabetes (n=291), type 2 diabetes (n=877), pred

Prognostic value of clinical and electrodiagnostic factors after corticosteroid injection in carpal tunnel syndrome.

BACKGROUND: Electrodiagnostic (EDX) studies are used to confirm the diagnosis of carpal tunnel syndrome (CTS), quantitatively assess disease severity, and distinguish CTS from other conditions. The purpose of this study was to evaluate whether the EDX study findings can predict the success or failure of corticosteroid injection (CI) in patients with CTS. METHODS: A retrospective review was conducted of 79 patients who received first-time CI for CTS without image-guidance between April 2019 and November 2022 and were followed for more than 1 year. Demographic information, disease characteristi

Minimally Invasive Endoscopic Palmaris Longus Abductorplasty for Severe Carpal Tunnel Syndrome and Thenar Muscle Paralysis Reconstruction.

Functional deficits in severe carpal tunnel syndrome arise from a combination of persistent diminished sensation and thenar muscle paralysis, leading to the disruption of thumb kinematics. Decompression alone does not address thenar atrophy. To restore thumb function, tendon transfers from donors, such as the palmaris longus, have been described. However, palmaris longus abductorplasty has disadvantages, including extensive dissection and large wounds, which increase the risk of higher morbidity, scarring, increased pain, and delayed rehabilitation. We introduce a minimally invasive endoscopic

Readability, Accuracy, and Lexical Diversity of New ChatGPT Models for Common Carpal Tunnel Syndrome Questions.

BACKGROUND: Carpal tunnel syndrome (CTS) is a prevalent neuropathy in hand surgery that significantly affects people's quality of life. Frequently, patients conduct research online before seeking medical care. Large language models (LLMs) like ChatGPT are increasingly used for health information, yet concerns remain regarding the accuracy, readability, and complexity of their responses. Previous studies have assessed older ChatGPT models but have not comprehensively compared newer versions. The purpose of this study is to compare ChatGPT-4-generated, ChatGPT-4o-generated, and ChatGPT-o1-genera

Analysis of the CTS-6 Questionnaire and Development of a Carpal Tunnel Syndrome Decision Tree.

PURPOSE: Carpal tunnel syndrome (CTS) is a common compressive neuropathy with no gold standard for diagnosis. The CTS-6 questionnaire is a previously validated tool used to determine the likelihood that a patient has CTS; however, numbness and tingling in the median nerve distribution is not a required symptom for a positive test result. The purpose of this study was to re-evaluate the components of the CTS-6 questionnaire to determine if a more succinct carpal tunnel screening tool could be developed. METHODS: A database of 295 upper extremities who had previously undergone CTS-6 testing wer

Preliminary Assessment of Pisotriquetral Joint Kinematics Following Transverse Carpal Ligament Release: A Cadaveric Pilot Study.

Introduction Transverse carpal ligament (TCL) release is the standard treatment for carpal tunnel syndrome; however, its potential to alter carpal biomechanics is unclear. Specifically, it has been hypothesized that TCL transection may destabilize the pisotriquetral (PT) joint or unmask latent arthritis. This pilot study investigated the immediate kinematic effects of TCL transection on the PT joint in an embalmed cadaveric model. Methods Six embalmed cadaveric wrists were mounted on a custom stabilization apparatus. Colored markers were affixed to the ulna, pisiform, and fourth metacarpal bon

Nerve Hydrodissection as Treatment for Entrapment Neuropathies: A Literature Review.

Entrapment neuropathies are disorders in which peripheral nerves are affected at anatomically restricted sites or at locations where they are vulnerable to external compression. They are prevalent in the general population and may lead to pain, sensory disturbances, and functional impairment. In recent years, nerve hydrodissection has gained attention as a minimally invasive, ultrasound-guided treatment option for entrapment neuropathies. This technique involves the injection of different substances into the perineural space to mechanically separate the nerve from adjacent tissues and potentia

Functional Outcomes and Early Complications Following Volar Locking Plate Fixation of Unstable Distal Radius Fractures: A Minimum Two-Year Follow-Up Study.

Background Volar locking plate fixation has become the preferred operative treatment for unstable distal radius fractures because it allows stable fixation and early mobilization. However, long-term clinical outcomes and early complications in Asian populations treated in routine practice remain less well documented. Methods This retrospective cohort study included 40 consecutive adult patients with unstable distal radius fractures treated with volar locking plates by a single hand and wrist surgeon at a university hospital between January 2012 and December 2021. Patients met predefined radiog

Determining the value of hand surgery through mapping algorithms.

INTRODUCTION: Health state utility values describe the desirability of health states and are needed to calculate quality-adjusted life years. Traditionally, they are derived using generic measures such as the EuroQol 5-Dimension scale. No hand-specific measures can be used to derive them. This study aimed to develop and evaluate mapping algorithms to estimate the EuroQol 5-Dimension utility index from the Patient Evaluation Measure. METHODS: Matched Patient Evaluation Measures and EuroQol 5-Dimension 5-Level data from 2043 patients were used to develop mapping algorithms. Transfer-to-utility

What Do Patients Want? Outcome Measures in a Diverse World.

PURPOSE: Quantifying outcomes of medical interventions has evolved but challenges remain in hand surgery. Initially, hand surgeons used concrete assessments like range of motion and/or pinch strength. With a shift toward patient-centered care, numerous pathology-specific patient-reported outcome measures (PROMs) were developed. These measures typically used standardized closed-ended outcomes and mostly originated from western countries. However, it is unclear how well measures with specific items capture diverse patients' values. Therefore, we used an open-ended PROM, the Patient-Specific Func

Persistent Median Arteries: A Cadaveric Study of Prevalence, Variants, and Clinical Relevance.

The median artery, an embryonic vessel, may persist into adulthood as the persistent median artery (PMA), a variant of clinical and surgical relevance. In a cadaveric study of 23 upper limbs from 12 donated adult cadavers, PMA was identified in eight limbs (34.8%). Half of these (4/8 limbs, 17.4%) pierced the median nerve in the proximal forearm, an uncommon finding. Two palmar-type PMAs were identified, both of which exhibited atypical terminal branching in the hand (2/23 limbs, 8.7%). These anatomical variations may contribute to median nerve compression syndromes, such as carpal tunnel synd
