C5-Predominant Segmental Zoster Paresis Presenting With Right Shoulder Abduction Weakness Before Cutaneous Eruption: A Case Report.

Segmental zoster paresis (SZP) is an uncommon motor complication of herpes zoster (HZ) characterized by focal, asymmetric weakness in a myotomal distribution. Motor weakness usually follows the cutaneous eruption, and pre-eruptive weakness can mimic cervical radiculopathy, brachial plexopathy, neuralgic amyotrophy, or other causes of acute focal limb weakness. A 57-year-old woman developed right shoulder pain and, five days later, became unable to abduct her right arm. A vesicular eruption appeared on the lateral aspect of the right upper arm three days after the onset of weakness, and HZ was

A Large Palmar Atypical Lipomatous Tumor Extending Into the Carpal Tunnel and Guyon's Canal: Challenges in Nerve-Sparing Surgical Excision.

We report a complex case of a large atypical lipomatous tumor (ALT) extending into the carpal tunnel and Guyon's canal, highlighting the diagnostic considerations, anatomical challenges, and principles of safe surgical excision. A 67-year-old female presented with a progressively enlarging swelling involving the thenar, mid-palmar, distal palmar, and wrist regions. Examination revealed a large palmar mass without neurovascular deficit or muscle wasting. MRI demonstrated a 63 × 27 × 79 mm lipomatous lesion arising from the volar wrist and extending into the carpal tunnel and Guyon's canal, with

Tumor-Related Trigeminal Neuralgia: Clinical Features, Pathophysiology, and the Contemporary Role of Radiosurgery in Tumor Control and Pain Relief.

Trigeminal neuralgia arising from an intracranial tumor is uncommon but therapeutically distinctive because the responsible lesion is simultaneously the generator of facial pain and an independent oncological problem. When anatomically feasible, microsurgical treatment remains the preferred approach for achieving rapid, deep, and durable pain relief, particularly when decompression of the trigeminal nerve can be achieved. However, a clinically important subset of patients may be unsuitable for open surgery because of age, fitness, comorbidity, tumor anatomy, or operative risk, while others may

Outcomes of median nerve transfers in high ulnar nerve injuries in adults - Scoping review.

INTRODUCTION: High ulnar nerve (UN) injuries have unfavorable functional prognoses due to the limited distance for axonal regeneration. Median nerve (MN) transfer to the UN has emerged as an alternative treatment for this injury. METHODS: Analysis of studies following the PRISMA-Sc guidelines was conducted. Studies evaluating MN transfer in the forearm or hand for high UN injuries between 2005 and 2025 were identified and then included. These studies included descriptions of intrinsic muscle function, grip strength (pincer or fist), and reported outcome measures. A descriptive literature revi

Stereotactic radiosurgery for NF2-related schwannomatosis and schwannomatosis-associated spinal tumors: a single-institution cohort.

BACKGROUND: Neurofibromatosis type 2 (NF2)-related schwannomatosis and other schwannomatosis subtypes frequently involve multiple spinal nerve-sheath tumors. While stereotactic radiosurgery (SRS) is well established for intracranial disease, long-term spinal outcomes in genetically predisposed populations remain limited. Existing spinal SRS studies often include heterogeneous tumor types, limited follow-up and/or mixed sporadic and syndromic cohorts. This study reports a 21-year single-institution experience, including rare long-term follow-up extending to 19 years. METHODS: We retrospectivel

Therapeutic effects of selumetinib on diffuse neurofibroma and optic pathway glioma in neurofibromatosis type 1.

PURPOSE: Selumetinib is approved for the treatment of inoperable plexiform neurofibromas (PN) in patients with neurofibromatosis type 1 (NF1). However, its efficacy in treating NF1-associated diffuse neurofibromas (NF1-DN) or optic pathway gliomas (NF1-OPG) remains unclear. We evaluated the efficacy and safety of selumetinib in these subgroups. METHODS: This was a sub-analysis of a Korean phase II open-label trial focusing on non-target treatment effects on NF1-DN and NF1-OPG. A total of 88 pediatric and adult patients with NF1-PN (59 children and 29 adults) in this trial had been treated for

Upper-Limb Cryoneurolysis for Painful Post-Stroke Spasticity in Severely Impaired Upper Limbs: A Feasibility Case Series.

BACKGROUND: Post-stroke upper-limb spasticity can cause pain, hinder passive care, and lead to secondary musculoskeletal complications. Current minimally invasive treatments have important limitations. Cryoneurolysis, which creates a controlled cold lesion of peripheral nerves, may offer a partially reversible focal denervation alternative. METHODS: We conducted a feasibility case series in the outpatient department of a rehabilitation centre. Three adults with chronic post-stroke hemiparesis and a non-functional spastic upper limb underwent ultrasound- and nerve stimulation-guided cryoneurol

Comprehensive Clinicopathologic and Molecular Analysis of Mesenchymal Tumors With BRAF Fusions.

BRAF fusions are rare but recurrent driver events in a spectrum of mostly pediatric and young adult mesenchymal soft tissue tumors, including infantile fibrosarcoma, lipofibromatosis-like neural tumors, and other kinase fusion spindle cell neoplasms. Their clinicopathologic spectrum, co-occurring secondary genomic events, and frequency among various sarcoma histotypes remain incompletely characterized. The purpose of this study is to comprehensively investigate the incidence and structural variants of BRAF fusions among a large clinicopathologic and molecular sarcoma cohort. The additional goa

Selumetinib in the treatment of plexiform neurofibromas in neurofibromatosis type 1: a 6-year prospective case series with review of the literature.

BACKGROUND: Neurofibromatosis type 1 (NF1) is an autosomal dominant genetic disorder characterized by pigmented lesions, cutaneous neurofibromas, and brain and peripheral nerve tumors. Plexiform neurofibromas (PNs) are congenital tumors whose growth tends to accelerate in the pediatric age. Selumetinib, a MEK1/2 inhibitor, is the first pharmacological treatment that has shown efficacy in reducing the size of PNs by inhibiting their key pathway, with few and moderate adverse events. The purpose of this study is to describe the effect of selumetinib in a large cohort of patients with NF1. METHO

Early supraclavicular nerve grafting or distal nerve transfer for restoration of elbow flexion in incomplete adult traumatic brachial plexus injuries.

INTRODUCTION: Restoration of elbow flexion is the primary treatment goal in adults with supra-clavicular traumatic brachial plexus injuries (ATBPI). To date, many nerve surgeons will initially await spontaneous recovery when dealing with an incomplete brachial plexus injury to allow sufficient recovery in the median and ulnar nerves to enable distal fascicular nerve transfers (DFNT). An alternative strategy is reconstruction by supraclavicular nerve grafting, which should be performed as early as possible after trauma to minimize denervation time. RESEARCH QUESTION: Does early supraclavicular

Upper brachial plexus lesions in adults: long term results of direct infraspinatus muscle reinnervation with a radial nerve branch transfer.

INTRODUCTION: Although shoulder external rotation is crucial for upper limb functionality, attempts to restore it are not well represented in traumatic brachial plexus palsy literature. METHODS: A series of seven patients underwent a modified technique for direct reinnervation of the infraspinatus muscle using a branch of the radial nerve. They were evaluated after long-term follow-up. RESULTS: All seven patients recovered some degree of shoulder external rotation, and in five cases (71.42%) the recovery was equal to or greater than 45°, which was considered a good result. The median follow-

Microendoscopic Posterior Decompression for L5-S1 Foraminal and Extraforaminal Lesions: A Comparative Study of Herniation Versus Stenosis.

Introduction Foraminal and extraforaminal nerve root compression at the lumbosacral junction (L5/S1) is an underrecognized cause of lumbar radiculopathy. The L5 nerve root traverses a narrow space between the transverse process and the sacral ala and may be compressed by migrated disc fragments or fibro-osseous structures. Although lumbar disc herniation (LDH) and foraminal or extraforaminal stenosis (FES) at L5/S1 are often evaluated together, their mechanisms differ. This study aimed to compare postoperative clinical outcomes between patients with L5-S1 foraminal and extraforaminal disc hern

Pediatric orbital masses from a tertiary pediatric oncology referral center: clinical characteristics and survival outcomes from a 20-year single-center experience.

BACKGROUND: Pediatric orbital masses are uncommon and most reports originate from ophthalmology services, where benign lesions predominate; oncology-referral populations may differ and remain underreported. METHODS: We reviewed children evaluated for proptosis and orbital mass at a tertiary pediatric oncology unit (2000–2020). Demographic features, diagnostic spectrum, tumor origin, age distribution, diagnostic approach, and survival outcomes were analyzed. Overall survival (OS) was estimated by Kaplan–Meier analysis and compared by log-rank testing. Prespecified exploratory comparisons includ

Lipofibromatous Hamartoma of the Median Nerve in a Patient With Recurrent Disease and Preserved Neurological Function: A Case Report.

Lipofibromatous hamartoma (LFH) is a rare benign tumor typically characterized by fibrofatty infiltration and proliferation within nerve fascicles, often having a predilection for the median and ulnar nerves. In this case, a 53-year-old woman presented with a painful mass of the left distal forearm that had been present for several years. She had intermittent tingling of the thumb, index, and middle fingers, and a limited range of motion (ROM) in wrist flexion and extension. The percussion test was positive, and magnetic resonance imaging (MRI) revealed a 3 cm lobulated lesion along the ulnar
