Identification of structural disconnection subnetworks in acute and subacute post-stroke depression.

BACKGROUND: Post-stroke depression (PSD) is common after acute ischemic stroke, yet its neural substrates remain unclear. This study identified structural disconnection subnetworks associated with PSD and evaluated their association with depression severity and topology. METHODS: We retrospectively analyzed 615 first-ever acute ischemic stroke patients. Depressive symptoms were assessed with HAMD-17 at baseline and 3 months. Individual 135 × 135 structural disconnection matrices were constructed from acute DWI lesions (MRI median 3 days post-stroke [1-4]) and HCP-842 normative connectome. Net

Attempted Restoration of Elbow Flexion in Suspected Parsonage-Turner Syndrome Using the Oberlin Procedure: A Case Report With Pending Outcomes.

Parsonage-Turner syndrome (PTS) can present with upper extremity weakness that overlaps clinically with traumatic upper-trunk brachial plexus injury, cervical radiculopathy, and shoulder pathology. Although many patients improve with conservative management, persistent paralysis of elbow flexion after a defined period of observation creates a difficult management decision. In selected patients with preserved donor nerve function and no evidence of meaningful reinnervation, distal nerve transfer may be considered to attempt restoration of elbow flexion. A 58-year-old right-hand-dominant man dev

Surgical Neurolysis for Atypical Neuropathy Diagnosed via Physical Examination Significantly Improves Chronic Pain Despite Negative Workup.

BACKGROUND: Many patients experience chronic pain, especially after trauma or surgery, despite a negative workup. We hypothesize that underdiagnosed peripheral neuropathy may be contributing to their intractable symptoms. METHODS: This is a retrospective cohort of patients who underwent surgical neurolysis for chronic pain that was neuropathic in nature, in the distribution of a peripheral nerve, with a positive scratch collapse test. Before surgery, patients were administered a corticosteroid injection at the lesion. The principal outcome was a self-reported visual analog scale pain score. M

Leukocytoclastic Vasculitis Presenting With Peripheral Neuropathy and Foot Drop in an Adolescent: A Case Report.

Leukocytoclastic vasculitis (LCV) is a small-vessel vasculitis that typically presents with palpable purpura on the lower extremities and is often confined to the skin. Peripheral nerve involvement is uncommon. We describe the case of a 17-year-old female who developed progressive burning paresthesias in all four limbs, asymmetric sensory loss, autonomic symptoms, and left foot drop preceding the appearance of palpable purpura and necrotic lesions on both lower limbs. Nerve conduction studies demonstrated mixed demyelinating and axonal motor neuropathy in both lower limbs, more pronounced on t

Treatment of optic neuritis with erythropoietin: impact on white matter and optic nerve MRI characteristics.

We previously conducted the TONE study (Treatment of optic neuritis with erythropoietin, NCT01962571), which analysed 103 patients with acute optic neuritis and no prior history of multiple sclerosis after randomization to receive high-dose erythropoietin or placebo as an adjunct to methylprednisolone pulse therapy. All visual system-related outcomes yielded negative results. The goal of the current investigation is to assess whether erythropoietin administration influenced inflammatory optic nerve and cerebral lesions or optic nerve atrophy on MRI. We determined the sensitivity of MRI in dete

Low-dose add-on methadone for cancer pain management: a retrospective analysis of 102 Japanese patients.

BACKGROUND: Methadone was introduced in 2013 for the treatment of intractable cancer pain in Japan and is indicated for patients receiving opioid doses ≧60 mg/day as an oral morphine equivalent. Low-dose (≦10 mg/day) add-on methadone to prior opioids has been reported from European countries to successfully relieve various types of intractable cancer pain; however, there are few reports of such use in Japan. The aim of this study was to analyze more than a hundred cases with low-dose add-on methadone to treat intractable pain in Japanese cancer patients. METHODS: All cases in which 5 or 10 mg

Cell-type-specific networks during hippocampal seizures at the micro- and macroscale.

Epilepsy is a network disorder, involving neural circuits at both the micro- and macroscales. While local excitatory-inhibitory imbalances are recognized as hallmarks at the microscale, the dynamic roles of distinct neuron types during seizures remain poorly understood. At the macroscale, interactions between key nodes within the epileptic network, such as the central median thalamic nucleus (CMT), are critical to the hippocampal epileptic process. However, the precise mechanisms underlying these interactions remain unclear. In this study, we investigated the microcircuit dynamics within the s

Genicular Nerve Radiofrequency Ablation for Chronic Knee Joint Pain Using a V-Shaped Active Tip Needle: A Single-Center Retrospective Observational Study.

BACKGROUND: Chronic knee pain from osteoarthritis (OA) affects a significant proportion of adults over 40. Total knee arthroplasty (TKA) remains the standard for advanced OA, yet up to 20% of patients experience chronic postsurgical pain (CPSP). Genicular nerve radiofrequency ablation (GNRFA) can alleviate pain in those unresponsive to conservative treatments or TKA. However, anatomical variability of the genicular nerves may limit treatment durability. This retrospective, single-center observational study investigated whether using a novel V-shaped active tip needle-which creates larger lesio

Efficacy and Safety of Donut-Shaped Circumferential Spine CyberKnife Stereotactic Body Radiotherapy for Metastatic Spine Disease.

BACKGROUND AND OBJECTIVES: Spinal metastases (SM) with epidural spinal cord compression (ESCC) present a significant challenge because of the high risk of radiation-induced injury to critical structures such as the spinal cord and nerve roots. Traditional treatment approaches often avoid circumferential stereotactic body radiotherapy (SBRT) to reduce these risks. The efficacy and safety of donut-shaped circumferential SBRT, designed to target the spinal column while sparing the spinal cord, remains underexplored. The aim of this study was to evaluate the safety and efficacy of donut-shaped cir

Primary Neurolymphomatosis: A Literature Review.

BACKGROUND: Primary neurolymphomatosis (PNL) is a rare clinical entity resulting from direct lymphomatous infiltration into the peripheral nervous system. Its diagnosis is challenging as the hematological condition is unknown at the onset of neurological symptoms. METHODS: We report two of our own cases and the first extensive review of published cases of PNL to delineate its clinical features, paraclinical investigation results, progression, and treatment response more precisely. We extracted demographic data, clinical presentation, results of the investigations performed, type and number of

Management of Solitary Osteochondroma Arising from Mammillary Process of D12 Vertebra in an 11-Year-Old Female: A Rare Case Report and Literature Review.

INTRODUCTION: The spinal canal is narrower in the upper lumbar levels than in the lower lumbar levels. Due to these reasons, the selection of a surgical approach is challenging. Fenestration discectomy at this level is more prone to neurological injury in most cases. Wide laminectomy or transforaminal approach is commonly used in upper lumbar disc surgeries. Surgical procedures using wide laminectomy and facetectomy give wide decompression but cause instability and need surgical fusion in most cases. We discuss an alternative approach for high lumbar disc disease using spinous process osteotom

Decision-making in Petrous Bone Cholesteatoma (PBC): Surgical options according to PBC classification.

INTRODUCTION: Petrous bone cholesteatoma (PBC) is a rare, potentially life-threatening condition requiring surgical management to prevent severe complications. While classification systems, such as Sanna's, are widely used to guide surgical planning, actual decision-making is often influenced by additional clinical factors beyond lesion type. MATERIAL AND METHODS: A retrospective analysis of 94 PBCs in 92 patients operated between 1995 and 2024 was conducted. Data included demographics, clinical presentation, PBC classification, surgical approach, use of endoscopic assistance, recurrence, fol

Outcomes and Predictors of Cure After Reoperation for Persistent and Recurrent Hyperparathyroidism: A Cohort Study in Saudi Arabia.

BACKGROUND: Primary hyperparathyroidism is the leading cause of hypercalcemia in outpatients, and parathyroidectomy is the only curative treatment. Although initial surgery cures the majority of patients when performed by experienced surgeons, a subset of patients experience persistent or recurrent hyperparathyroidism, necessitating reoperation. Reoperation is technically demanding and associated with a higher complication risk, but it remains the only chance for a durable cure. Data on outcomes in the Middle East are scarce. METHODS: A retrospective cohort study of adults who underwent reope
