Evaluation of the Safety, Genetic Stability, and Immunogenicity of an Attenuated Shigella Live Vector Expressing Enterotoxigenic Escherichia coli Antigens, Vaccine Strain CVD 1208S-122.

BACKGROUND: There are no licensed vaccines to protect against Shigella or enterotoxigenic Escherichia coli (ETEC), yet they are among the most common causes of bacterial diarrhea. We engineered an attenuated Shigella flexneri 2a expressing colonization factor antigen 1 (CFA/I) and heat-labile enterotoxin B subunit (LTB) from ETEC and conducted a first-in-human randomized-controlled trial of ascending doses of the vaccine (CVD 1208S-122). METHODS: Dose-escalating cohorts received a single oral dose of 108-1010 colony-forming units (CFU) of CVD 1208S-122 or placebo. A fourth cohort received 1 o

Food Allergen Dose Differentially Controls the Intensity of the Developing Food Allergy Response in an Oral Sensitization-Based Murine Model.

Although mouse models are invaluable tools for allergenicity assessment, the relationship between allergen dose and the intensity of induced food allergy (FA) response remains poorly understood in oral sensitization-based murine models. This study aimed to develop an oral sensitization BALB/c mouse model and identify the optimal antigen dose required to elicit a maximal response, thereby establishing a robust framework for assessing the allergenicity of food proteins. BALB/c mice were orally administered varying doses of ovalbumin with cholera toxin as an adjuvant. The optimal sensitization do

Tannin Phytocomplex Improves IBS-D Symptoms and Gut-Brain Markers: Prospective Pilot Study.

Diarrhea-predominant irritable bowel syndrome (IBS-D) is a multifactorial disorder involving gut-brain axis dysregulation, altered barrier integrity, microbial imbalance, and stress-related mechanisms, with limited nutritional strategies targeting these pathways. Tannin-rich extracts may exert barrier-protective, microbiota-modulating, and stress-related effects. In a prospective before-after pilot study (NCT05207618), 28 adults with IBS‑D completed a 60‑day tannin-based supplementation (480 mg/day). Outcomes included IBS Severity Scoring System (IBS-SSS), bowel habits, somatization, perceived

Role of communities in preparedness, response and resilience to cholera epidemics: a systematic review protocol.

INTRODUCTION: Cholera remains a major global health concern, particularly in fragile and resource-limited settings despite global efforts to reduce cholera morbidity and mortality. While community engagement is widely recognised as a cornerstone of sustainable cholera control, evidence on its effective implementation, particularly the concrete roles of communities in preparedness, response and resilience, is currently fragmented and poorly documented. This protocol describes a systematic review that aims to identify and synthesise global evidence on how communities contribute to cholera prepare

Evolving behavioral dynamics and environmental risk in cholera epidemics.

Cholera, an infectious disease transmitted by water, is a harmful and sometimes life-threatening public health issue in a significant number of poor and middle-income nations all over the world, where inadequate water and sanitation infrastructure, together with noticeable climate variability, contribute to regular and frequently severe outbreaks. Consequently, it is important to understand the interplay between environmental risks and human decision-making in exacerbating the disease burden in our society. This work develops and analyzes a seasonally driven cholera model that integrates with

Impact of tongue exercise on hypoglossal axis survival, structure, and output in a rodent model of hypoglossal motor neuron degeneration.

Motor neuron diseases (MNDs) are progressive neurological disorders in which motor neurons necessary for walking, swallowing, and breathing are destroyed. Obstructive sleep apnea (OSA) occurs more commonly in patients with MNDs and may contribute to disease progression. Intralingual injections of cholera toxin B conjugated to saporin (CTB-SAP) mimic hypoglossal (XII) motor neuron loss observed in MND models and result in airway deficits observed in MNDs and OSA, often leading to respiratory failure and death. Using this model, we investigated the effects of a high-repetition/low-resistance ton

The gap between knowledge and action in Zimbabwe: The limits of individual awareness in the face of structural violence in cholera endemicity.

BACKGROUND/OBJECTIVES: Zimbabwe faces a protracted cholera crisis exacerbated by El Niño-induced droughts, transitioning from episodic outbreaks to an endemic public health emergency. While recurrent outbreaks are often attributed to poor community awareness, this study moves beyond the conventional Knowledge, Attitudes, and Practices (KAP) paradigm. Grounded in the Integrated Behavioral Model for WASH and the theory of structural violence, we investigate the structural determinants of the knowledge-practice gap to understand how infrastructural disparities constrain cholera prevention. Serological responses to killed oral cholera vaccine (OCV) when given 4 years after initial receipt of OCV in Cameroon: A randomized controlled trial.

Oral cholera vaccination (OCV) is one of strategies to help control cholera with two doses recommended as initial vaccination and booster doses recommended 3-5 years later to prevent future outbreaks. There is no actual recommendation on the number of doses required during booster vaccination. An OCV campaign conducted in Mogode-Cameroon provided an opportunity to determine if revaccination four years after would induce a booster response. 350 people living in Mogode-Cameroon, who received 0, 1 or 2 doses of OCV four years earlier, were stratified into age groups, < 5, 5-14, and >14 and random

Silent spreaders: optimal control of asymptomatic cholera transmission through adaptive stochastic modeling.

Cholera transmission dynamics are complicated by asymptomatic carriers and environmental stochasticity often overlooked in standard models. We develop a novel Susceptible-Asymptomatic-Infected-Treated-Recovered-Bacterial (SAITR-B) model integrating dual adaptive controls—vaccination and symptom-targeted isolation—within a stochastic framework. Numerical simulations of 10,000 outbreak scenarios show environmental noise amplifies outbreak variability by 28–34% and shifts optimal intervention timing by 1.5–3 days compared to deterministic predictions. Combined control strategies reduce peak sympt

CTBPro: A Next-Generation Cholera Toxin Subunit B-Based Neuroanatomical Tracer With Superior Brightness, Stability, and Sensitivity for Enhanced Neural Circuit Mapping.

Mapping complex neural circuits demands bright and stable tracers, yet conventional Cholera Toxin Subunit B (CTB) conjugates exhibit suboptimal brightness, which limits high-fidelity morphological reconstruction. Here, we engineer CTBPro, a next-generation CTB-based tracer, by genetically fusing CTB to the ultra-stable fluorescent protein mBaojin. In vitro, CTBPro exhibits an estimated nine-fold enhancement in peak fluorescence output per mole of tracer compared to conventional CTB-Alexa488 conjugates. This enhanced brightness translates to superior in vivo tracing across four distinct adminis

Oral administration of Lactiplantibacillus plantarum displaying multiple ASFV antigen proteins on the surface induces systemic immune responses in mice.

African swine fever (ASF) is a highly fatal viral disease that poses a severe threat to the global swine industry. Developing a safe and effective vaccine remains an urgent and complex challenge. Lactobacillus plantarum (L. plantarum), recognized as a food-grade antigen delivery vehicle, has demonstrated great promise in vaccine development and application for various infectious diseases. To overcome the weak intestinal immunogenicity and short mucosal residence of conventional vaccines, we employed the antibiotic-free selection system of L. plantarum NC8. By fusing the nontoxic, intestinal-ep

Climate change, migration, and infectious disease vulnerability at Europe's southern border: Lampedusa as a sentinel interface.

BACKGROUND: The Mediterranean basin is warming faster than the global average, reshaping ecological niches for vector-borne, water-borne, and environmentally linked pathogens. Concurrent climate shocks across the Sahel, West Africa, and the Horn of Africa-combined with conflict, food and water insecurity, and fragile health systems-drive sustained mobility toward North Africa and Europe. Frontline entry points along the Central Mediterranean route often capture early clinical signals of these pressures. METHODS: We conducted a narrative, expert-informed evidence synthesis integrating peer-rev

Identification and characterization of a novel lysin-like endopeptidase from the Vibrio cholerae predator phage ICP1.

International Centre for Diarrheal Disease Research, Bangladesh cholera phage 1 (ICP1)-type vibriophages are specialized predators of the Gram-negative (G-) pathogen Vibrio cholerae. However, the muralytic lysins critical to the lytic mechanism of these vibriophages remain uncharacterized. In the present study, we identified a highly conserved peptidoglycan (PG) NlpC/P60 peptidase-encoding gene (osLys) through data mining of 85 ICP1 vibriophage genomes collected during seasonal cholera outbreaks. OsLys clustered within a monophyletic group of modular lysin-like endopeptidases from phages infec

Pathogen spillover in a seasonal cholera model.

The risk of pathogen spillover from a natural reservoir to a host population, or from an infectious host population to an environmental reservoir is a major concern in the emergence and re-emergence of infectious diseases. Understanding the source of the pathogen and the environmental conditions driving the emergence or re-emergence are vital to prevention and control. We investigate pathogen spillover in a well-known infectious disease, cholera, where the environment harboring the bacterial pathogen is the natural reservoir. Applying a stochastic seasonal cholera model and methods from contin

Computational design of a multi-epitope vaccine against Helicobacter pylori targeting CagA, FliD, urease, and OipA with cholera toxin B subunit (CTB) adjuvant.

BACKGROUND: Helicobacter pylori, a Gram-negative, microaerophilic bacterium with a characteristic spiral morphology, inhabits the stomachs of approximately half of the world’s population. This study aimed to design a multi-epitope vaccine against H. pylori using immunoinformatics approaches, targeting key virulence factors including FliD, CagA, OipA, and urease. METHODS: A multi-epitope vaccine was constructed by linking nine immunodominant epitopes from FliD, urease, CagA, and OipA and the cholera toxin B subunit (CTB) adjuvant. The resulting construct was rigorously evaluated through computa

Controlled immobilization of single-domain antibodies using cellulose-binding modules for AB5-type toxin detection in paper-based assays.

The rapid and sensitive detection of toxigenic Vibrio cholerae and enterotoxigenic Escherichia coli remains a critical challenge for effective disease screening and environmental monitoring, which is essential for guiding clinical management and outbreak control in regions where diarrheal diseases are endemic. To address this, we developed a single-domain antibody (VHH)-based sandwich-format lateral flow assay for the rapid and sensitive detection of cholera toxin from V. cholerae and heat-labile enterotoxin from enterotoxigenic Escherichia coli in human fecal and environmental water samples.

Efficacy of Purified Borrelial Lipoproteins (PBL) as an oral formulation in reducing transmission of Lyme spirochetes from reservoir hosts to Ixodes scapularis ticks.

Blocking transmission of Borrelia burgdorferi (Bb), the causative agent of Lyme disease (LD), from reservoir hosts to humans via Ixodes scapularis ticks represents an alternative strategy to reduce LD incidence. Here, we evaluated Purified Borrelial Lipoproteins (PBL) with a combination of adjuvants, for their ability to limit Bb transmission using C3H/HeN mice and Peromyscus leucopus reservoir models. Immunization with PBL as oral gavage, either alone or nanoparticle-encapsulated, elicited increased antibody responses and reduced pathogen burden in fed larvae and select host tissues. A formul
