Cholera toxin as a model AB5 toxin: functional significance of B-subunit pentamerization in GM1 recognition, pathogenicity, and delivery efficiency.

AB5 toxins constitute a major family of bacterial exotoxins. Their pathogenicity depends on the coordinated actions of two components: an enzymatically active A subunit and a pentameric B subunit. The B subunit mediates host cell recognition and intracellular trafficking. Among them, cholera toxin represents a representative model for investigating structure-function relationships governing toxin entry and delivery. While the catalytic activity of the A subunit has been extensively characterized, growing evidence indicates that pathogenic outcomes are critically shaped by trafficking strategie

Mechanism of periocular acupuncture in alleviating dry eye neuropathic pain via regulation of the "periocular acupoint-trigeminal ganglion-ventral posteromedial thalamic nucleus" pathway.

OBJECTIVE: Dry eye disease (DED) is a common ocular surface disorder characterized by tear film instability and trigeminal nerve-mediated corneal hyperesthesia. Periocular acupuncture has been demonstrated to relieve DED symptoms; however, the underlying mechanisms remain unclear. Thus, in this study, we aimed to investigate the therapeutic effect of periocular acupuncture on DED and its peripheral-central regulatory mechanisms. MATERIALS AND METHODS: A guinea pig model of dry eye neuropathic pain was established through subcutaneous injection of scopolamine hydrobromide combined with topical

Isothermal amplification and colorimetric detection of Vibrio cholerae in environmental matrices.

Vibrio cholerae (V. cholerae), causing deadly cholera, which is responsible for significant public health problems in endemic regions, should be monitored in contaminated environments. Here, we report an integrated protocol incorporating alkaline lysis DNA extraction and loop-mediated isothermal amplification (LAMP) targeting the V. cholerae rpoB gene and producing a colorimetric reading within 1 h. This protocol yielded 100% specificity for V. cholerae after testing three non-cholerae Vibrio species and four Enterobacteriaceae as negative controls. A positive signal was detected across a rang

Uganda's cholera elimination journey in a cholera endemic region of Africa.

Uganda has been endemic for cholera since 1971. To reduce the threat of cholera, Uganda reviewed its approach to cholera, emphasizing surveillance, continued using single dose doxycycline for cholera patients and their households, and conducted 16 oral cholera vaccine campaigns in high-risk districts between 2018 and 2021.To understand recent changes in cholera patternsand the impact of the OCV campaigns, we classified districts as endemic, eliminated, or relapsed using the method as defined by the "cholera elimination scorecard" for the years 2018-2024 using cholera surveillance data from the

Assessment of water quality and microbial contamination in institutional water resources: a necessity to understand health risks.

Lack of regular monitoring of water sources may lead to undetected contamination, posing serious health risks and necessitating regular water quality assessments. Sampling for physicochemical, microbial analyses, and online surveys across three higher education institutions was done to evaluate water quality. Spatiotemporal variations among physicochemical parameters showed that the pH, EC, and TDS decreased during the wet season, reflecting the dilution effect of rain. However, DO increased from 0.67 to 4.83 ppm, indicating better aeration. PCA showed seasonal variability, whereas the correla

Genomic evolution and climate related drivers of cholera surges in Dhaka Bangladesh between 1996 and 2024.

BACKGROUND: Cholera, a climate-associated diarrhea occurs endemically in Dhaka city, Bangladesh, often turning into large epidemics. In 2022, a massive outbreak struck Dhaka city, resulting in a very high number of cholera cases. Although extensive studies have focused on climate and cholera, including genomic evolution of the causative agent, Vibrio cholerae, their association with the case surges remains poorly understood. METHODS: We analyzed cholera data between 1996 and 2024 from the Diarrheal Disease Surveillance System (DDSS) of icddr,b, and also conducted a follow up genomic analysis

Molecular Methods to Detect Vibrio cholerae and Associated Bacteriophages among Diarrheal Patients in Bangladesh.

Molecular diagnostics to detect Vibrio cholerae (Vc) may be negatively impacted by pathogen-specific lytic bacteriophage (phage) predation. To address this problem, phage detection as a proxy for pathogen detection has been proposed. However, efforts to modernize cholera diagnostics with molecular tools require addressing knowledge gaps on best practices to detect Vc and associated bacteriophages. We conducted polymerase chain reaction (PCR), quantitative PCR (qPCR), and nano-liter (nl) qPCR targeting Vc and known phages (ICP1/2/3) on stool samples collected from patients admitted at hospitals

Immunoinformatic design of a multi-epitope vaccine candidate targeting multidrug-resistant Pseudomonas aeruginosa.

BACKGROUND: Pseudomonas aeruginosa, a World Health Organization (WHO) priority pathogen, causes severe multidrug-resistant (MDR) infections in immunocompromised and hospitalized patients, highlighting the critical need for effective vaccines. To this end, we designed a novel multi-epitope vaccine by integrating immunodominant domains from three key virulence proteins (PcrV, OprE, and CupC2) with the cholera toxin B subunit (CTB) as a mucosal adjuvant. METHODS: Immunoinformatics was used to predict epitopes and construct a vaccine with EAAAKEAAAK linkers. In silico assessments included antigen

Factors associated with in-hospital mortality during the 2023 cholera outbreak in Hammanskraal, South Africa: a cross-sectional study.

BACKGROUND: The 2023 cholera outbreak centred on Hammanskraal, a peri-urban township north of Pretoria, South Africa, resulted in high in-hospital mortality. Hammanskraal had experienced chronic failures in water and sanitation infrastructure for nearly two decades before the outbreak. We aimed to describe the clinical profile of patients managed at the principal case-management facility and to identify factors associated with in-hospital death. METHODS: We conducted a retrospective cross-sectional study based on review of medical records and the hospital cholera admission register at a distr

Human Metapneumovirus G Protein Immunogenicity and Safety Explored via Carrier Protein Fusion.

Human metapneumovirus (HPMV) is a significant pathogen that causes lower respiratory tract infections. Given the weak immunogenicity thereof, and the few relevant studies, the utility of the viral membrane protein G as a vaccine remains controversial. In this study, the G extracellular domain (RMG) of HMPV was expressed either alone or fused with the cholera toxin B subunit (CTB) and "cross-reacting material 197" (CRM197) carrier proteins (giving G-CTB/G and CRM197), to enhance immunogenicity. The non-glycosylated G protein (REG) expressed in Escherichia coli served as a control. SDS-PAGE and

Current methods and emerging strategies for the detection of foodborne bacterial protein toxins.

Foodborne bacterial pathogens are a major source of disease and mortality worldwide and have a significant impact on the global economy. Many of these pathogens produce lethal toxins that may persist in various food matrices even in the absence of viable microorganisms, making direct toxin detection essential for food safety surveillance. This review focuses on five major foodborne bacterial exotoxins selected based on acute clinical severity, structural and mechanistic diversity across toxin classes, a large volume of published data on detection platforms relative to other foodborne protein t

Estimating time-varying cholera transmission and oral cholera vaccine effectiveness in Haiti and Cameroon, 2021-2023.

BACKGROUND: In 2023, cholera caused over 95 000 deaths globally, predominantly in low-income and conflict settings, and contributed to the wasting, stunting and malnutrition of millions in countries where the disease is endemic. Moreover, the frequency and magnitude of cholera outbreaks are rising. As a result, the demand for outbreak control interventions is quickly outpacing existing resources. Oral cholera vaccination (OCV) is one intervention among many used to limit further transmission. Since 2022, one-dose OCV (OCV1) has replaced the standard two-dose OCV due to limited global supply. Mucosal Vaccine Development: From Adjuvant Design to Next-Generation Delivery Strategies.

Most infectious pathogens enter the host through mucosal surfaces, yet conventional injectable vaccines primarily induce systemic immunity without eliciting robust secretory immunoglobulin A (SIgA) responses at mucosal sites. The COVID-19 pandemic highlighted this limitation, as intramuscular mRNA vaccines failed to establish durable mucosal immunity in the upper respiratory tract. This review covers recent progress in mucosal vaccine development. We first discuss the organization of the mucosal immune system, focusing on SIgA induction, tissue-resident memory T (TRM) cells, and resident memor

Rotavirus Prevalence, Genetic Diversity, and Co-Infections During the 2023-2024 Cholera Outbreak in Zambia: Insights from Multi-Pathogen Diagnostics.

During Zambia's 2023-2024 cholera outbreak, reliance on single-pathogen diagnostics risked overlooking co-circulating enteric pathogens. This study estimated the prevalence of rotavirus and described co-detected enteropathogens and rotavirus genotypes among patients admitted with suspected cholera. A sub-analysis was conducted on diarrhoeal stool specimens collected from patients who met the syndromic suspected cholera case definition. Samples were tested using the Bosphore® Gastroenteritis Panel v2, a multiplex PCR enteric panel, to detect rotavirus and other gastrointestinal pathogens. Rotav

Somalia's Drought-Disease Spiral: Climate Shocks as an Emerging Public Health Security Threat.

This narrative commentary examines Somalia's drought-disease spiral using selected published literature, humanitarian reports, surveillance updates, and climate-health evidence. It focuses on how recurrent droughts, floods, food insecurity, displacement, malnutrition, and fragile health-system capacity interact to increase the risk of climate-sensitive infectious diseases. Somalia is among the countries most vulnerable to climate variability, and repeated environmental shocks have intensified humanitarian needs while placing additional pressure on an already fragile health system. Recent clima

Integration of infectious diseases in climate governance in the Horn of Africa: A document review of climate strategies, policies, and action plans across four countries.

INTRODUCTION: A range of climate-sensitive diseases are endemic to the Horn of Africa, which is increasingly vulnerable to climate extremes. Global health agencies recognise the need to abandon siloed approaches to climate change and health by integrating health in climate policies, strategies, and guidelines. This structured document review aimed to examine the extent to which health, and specifically infectious diseases, are integrated into climate governance documents in this region, with a focus on Ethiopia, Kenya, Somalia, and Uganda. METHODS: A desk-based search strategy identified clim

Cholera in Africa: Current trends, challenges, and control towards elimination strategies.

Cholera remains a substantial public health challenge in Africa, despite the availability of effective vaccines, treatment, and preventive measures. Systemic issues such as inadequate water, sanitation, and hygiene (WASH) infrastructure, weak disease surveillance, socio-economic disparities, and ongoing conflict and instability drive its persistence. This article provides the current situation of cholera in Africa, highlighting systemic barriers to control, including data deficiencies, limited diagnostic capacity, and infrastructural deficits. Crucially, we underline the importance of integrat

Project ODIN: advancing environmental genomic surveillance for public health across sub-Saharan Africa.

Persistent SARS-CoV-2 transmission, ongoing mpox outbreaks, and the continued spread of endemic diseases such as typhoid fever and cholera underscore the urgent need for global, multiomics surveillance. In this Personal View, we present Project ODIN, a consortium of European and African partners launched in 2023 that aims to meet this challenge by deploying innovative systems for near real-time pathogen detection and actionable public health insights. The project is a collaboration between high-income and low-income countries in northern Europe and sub-Saharan Africa. Focusing on low-income an

Cerebrospinal fluid-contacting nucleus mediates the balance of depth during general anesthesia in mice.

AIMS: Research on the mechanisms of underlying general anesthesia has traditionally focused on specific neural circuits. In this study, we investigated the role of the cerebrospinal fluid-contacting nucleus (CSF-CN), which directly interfaces with the CSF, in modulating general anesthesia in mice. METHODS: CSF-CN was labeled by administering the retrograde tracer cholera toxin subunit B-594 into the lateral ventricle, and it was selectively ablated using a conjugate of saporin and cholera toxin subunit B. Anesthesia was induced in mice via propofol injection through the tail vein or isofluran

Genomic analysis of Vibrio cholerae in Western Kenya (2015-2023) reveals persistence of pandemic ST69 and presence of the European-associated ST8 lineage.

BACKGROUND: Cholera remains a significant public health issue in Kenya, primarily driven by recurrent outbreaks associated with Vibrio cholerae O1 sequence type 69 (ST 69). Recently, non-O1/non-O139 strains have emerged and are associated with sporadic cases; however limited genomic data hinder our understanding of strain diversity, virulence, and antimicrobial resistance (AMR). This study characterized the genetic diversity, virulence, and AMR profiles of V. cholerae isolated in Western Kenya between 2015 and 2023 through an enteric pathogen surveillance study. METHODS: Twenty-four V. choler
