From evidence to action: identification and strategic application of cholera priority areas for multisectoral interventions (PAMIs) in Sudan (2016-2025).

BACKGROUND: Sudan has a long history of struggling with recurrent cholera outbreaks. The interplay of infrastructure damage, massive displacement, and limited access to safe water and sanitation has increased the risk of cholera transmission. The study aims to identify priority areas for multisectoral interventions (PAMIs) to inform public health policy and ensure evidence-based strategic control of cholera in Sudan. METHODS: In this retrospective study, national cholera data from 2016 to 2024 (excluding 2020-2022) were used. The Global Task Force on Cholera Control (GTFCC) tool for identifyi

Between bullets and outbreaks: epidemic control in Sudan's forgotten war.

Two years into Sudan's prolonged war, the collapse of health systems has fueled multiple concurrent epidemics, triggering a complex humanitarian emergency with regional spillover risks. Over 30 million people need aid, more than 11 million displaced internally, and cross-border movement has exacerbated the spread of vaccine-preventable diseases, including Cholera, Measles, and circulating vaccine-derived Poliovirus type 2 (cVDPV2). This review analyzes emerging epidemiological patterns and operational barriers to epidemic control in the context of armed conflict, displacement, and infrastructu

Connectivity of the Superior Temporal Polysensory Area With the Presubiculum, Parasubiculum, Entorhinal Cortex, and Claustrum-Endopiriform Complex in the Common Marmoset (Callithrix jacchus).

The rodent hippocampal-presubicular-entorhinal circuit is thought to be a key part of the circuitry involved in memory formation. In primates, additional connections are believed to enable more complex and higher-order memory information processing. We investigated whether such additional connections exist in the cortical regions adjacent to the hippocampal formation using a tracer injection method. We discovered that the marmoset parasubiculum (ParS), presubiculum (PreS) (distal portion), and entorhinal cortex (EC) (proximal or medial portion) have strong reciprocal connections with the super

Elevated Zonulin-1 is Associated With an Increased Risk of Non-AIDS-Defining Cancers in People With HIV With Suboptimal Immune Recovery: A Case-Cohort Study.

People with HIV (PWH) on antiretroviral therapy (ART) exhibit a heightened risk of non-AIDS-defining cancers (NADCs), linked to chronic inflammation driven by microbial translocation. We investigated whether plasma biomarkers of gut barrier dysfunction are associated with NADC risk, hypothesizing this association is modified by immune status. We conducted a case-cohort study nested within the Spanish CoRIS cohort. Plasma levels of zonulin-1, lipopolysaccharide, LPS-binding protein, and flagellin were measured in 71 incident NADC cases and a 261-individuals subcohort. Multivariable Cox proporti

Total chemical synthesis of pentameric cholera toxin subunit B.

Vibrio cholerae annually infects millions worldwide, causing intense and life-threatening diarrheal disease. The pentameric cholera toxin subunit B (CtxB) binds to the ganglioside GM1 on epithelial cells, initiating endocytosis and downstream toxicity. We present the total chemical protein synthesis of L-CtxB and mirror-image D-CtxB using a three-segment N-to-C native chemical ligation approach. Oxidative folding produced the desired 62 kDa pentameric proteins, with L-CtxB capable of binding native GM1 ligand. Synthetic D-CtxB is now ready to serve as a target in mirror-image phage display, re

Human Organoids and Organ-on-Chip for Biotoxin Assessment: Applications, Best Practices, and a Translational Roadmap.

Human organoids and organ-on-chip/microphysiological systems (OoC/MPS) are increasingly used as new-approach methodologies for biotoxin assessment. They retain human-relevant tissue organization and enable interpretable analysis of exposure geometry, barrier transport, perfusion, and (when needed) multi-organ coupling. In this review, we synthesize primary evidence across major toxin classes, including bacterial enterotoxins (e.g., cholera toxin, heat-stable enterotoxins, Shiga toxins), mycotoxins (e.g., aflatoxin B1, ochratoxin A, deoxynivalenol), and algal/cyanobacterial toxins (e.g., saxito

Leeches and hand surgery: history, physiology, and a scoping review.

INTRODUCTION: Leech therapy or hirudotherapy has been part of the medical therapies and treatment for centuries. Their use decreased in the 19th century with the launch of asepsis and the great cholera epidemic in 1832. Later on, in 1980s, they were reintroduced in the management of post-operative venous congestion. Well known for free/pedicled flaps surgery, their use in hand surgery is more inconsistent. The purpose of this study was to review history, physiology of the leeches, and their use specifically in hand surgery. MATERIALS AND METHODS: The bibliography was compiled using the PubMed

A Nation of Disasters: Lessons on Systemic Risks and Cascading Impacts From Malawi's COVID-19 Experience.

The COVID-19 pandemic exposed the cascading and systemic nature of risks, in low-income, multi-hazard contexts such as Malawi, where pre-existing vulnerabilities amplified impacts across health, economic, and social systems. This study draws on qualitative data from stakeholder workshops and key informant interviews, triangulated with national statistics, to analyze how overburdened health systems, disrupted livelihoods, and inequitable resource distribution heightened socio-economic fragility. It also examines how concurrent hazards, including flooding and cholera outbreaks, interacted with t

Ethylene participates in strigolactone-triggered stomatal closure via Gα protein-activited hydrogen peroxide and hydrogen sulfide synthesis in Arabidopsis.

The signaling cascade of strigolactone (SL)-regulated stomatal closure in Arabidopsis thaliana was investigated using pharmacological assays, fluorescence microscopy, spectrophotometry, gas chromatography, RT-PCR, and qRT-PCR. In wild-type plants, GR24 (a synthetic strigolactone analog)-induced stomatal closure was significantly inhibited by ethylene biosynthesis/perception inhibitors or Gα inhibitor pertussis toxin (PTX). GR24 obviously promoted closure in eto1-1, cGα1 and wGα1 mutants, but failed to do so in mutants etr1-1, etr1-3, gpa1-1 and gpa1-2. GR24 also upregulated 1-aminocyclopropane

Liposomal simvastatin potentiates intranasal H1N1 influenza subunit vaccine via increasing transcytosis of antigen and submucosal dendritic cell recruitment.

Intranasal vaccines specifically eliciting mucosal immunity in the upper respiratory tract have shown advantages in protecting against respiratory virus invasion. Yet, no clinically licensed intranasal adjuvant remains a major hurdle for the development of intranasal vaccines with low immunogenic antigens like subunit vaccines. Here, we show that liposomes loading simvastatin (Lipo-SV) serve as potent mucosal adjuvants for the intranasal liposomal subunit vaccine encapsulating the hemagglutinin 1 (HA1) glycoprotein of A/PR/8/34 (PR8) H1N1 influenza (Lipo-HA1), providing robust protection again

Predator-prey dynamics of Vibrio cholerae on chitin suggest an alternative mode of biofilm formation in marine snow conditions.

Vibrio cholerae is a ubiquitous marine bacterium that solubilizes and consumes chitin in the marine water column. In both the marine environment and the intestinal tract, V. cholerae forms biofilms: how do the diverse surfaces that V. cholerae encounters influence its biofilm formation and, in turn, shape its ecological interactions with other microbes? Here, we use the interaction between the predator Bdellovibrio bacteriovorus and V. cholerae as a model to explore how the environmental chitin substrate alters V. cholerae biofilm formation and predator-prey dynamics. We find that glass-bound

Hospital wastewater-based surveillance as an early-warning system for toxigenic Vibrio cholerae: first detection, isolation, characterization, and traceback investigation in Beijing, China.

Toxigenic Vibrio cholerae (V. cholerae) was first sensitively and rapidly detected in hospital wastewater in Beijing, China, on 12 June 2025. Viable V. cholerae was successfully isolated from the wastewater sample and identified as the O1 Ogawa serotype. Whole-genome sequencing and comparative genomic analysis revealed that the isolate was closely related to locally circulating strains but clearly distinct from the seventh pandemic lineage, suggesting its link to indigenous transmission chains. In addition, we immediately initiated a comprehensive and multi-source traceback investigation upon

Cholera in Kenya: A scoping review of current research, evidence gaps and future directions.

Cholera remains a major public health challenge in Kenya, driven by environmental pollution, poor water, sanitation, and hygiene (WASH) facilities, weak monitoring systems, and climate effects. While there have been improvements in studying the disease, testing, monitoring, and environmental checks, the spread continues, especially in informal settlements, rural areas, and refugee camps. This review looked at 106 peer-reviewed studies published between 1979 and 2024. It aimed to describe the scope, progress, and gaps in cholera research in Kenya. Five databases, including Google Scholar, Web o

Projecting excess mortality due to infectious diseases during a crisis: methods and application to the Gaza Strip, 2023-2024.

Excess mortality is a key consequence of crises due to armed conflicts and natural disasters. Israel's campaign in the Gaza Strip has exposed people to multiple risk factors for increased infectious disease burden. We developed a novel method for forward-projecting mortality due to both endemic and epidemic-prone infections in Gaza between 7 October 2023 and 6 August 2024. After defining alternative crisis scenarios, we used expert elicitation to quantify the probability of specific epidemics and increases in transmissibility and case fatality. We analysed past vaccination coverage and natural

Genomic profiling of Pasteurella multocida strains isolated from ISA Brown (Gallus gallus domesticus) in Bangladesh.

Poultry farming faces fowl cholera caused by Pasteurella multocida, yet genomic data are limited. Isolates from ISA Browns were identified using matrix-assisted laser desorption/ionization time-of-flight (MALDI-TOF) and sequenced (8.6, 1.4, and 1.3 M reads). De novo assemblies revealed genome sizes of ~2.4-2.5 Mb and guanine or cytosine (GC) content of ~40%.

Evolution of pandemic cholera at its global source.

The seventh pandemic of cholera, caused by the seventh pandemic El Tor lineage of Vibrio cholerae, was previously shown to have emanated in three global waves from the Bay of Bengal, bordering Bangladesh and India1. However, the respective roles of the Ganges Delta and Basin regions in seeding these global pandemic waves were not known. Here we show that, although transmission events occur between Bangladesh and India, V. cholerae in the two countries has largely evolved separately over the past 20 years, apparently constrained by national borders rather than by hydrological features, such as

Identification of priority areas for cholera control, Cameroon.

OBJECTIVE: To identify priority areas for multisectoral interventions for cholera control in Cameroon. METHODS: We collected data on cholera cases from January 2016 to September 2023 in all 10 regions of Cameroon sourced from the DHIS-2 software, national cholera line lists, situation reports and databases of the Centre Pasteur du Cameroun and the National Public Health Laboratory. We entered these data into the Global Task Force on Cholera Control tool to determine a priority index for districts based on four cholera indicators: incidence, mortality, persistence and test positivity. We calcu

A decade of antimicrobial resistance in Vibrio spp.: genomic and functional insights.

UNLABELLED: Environmental pressures and repeated exposure to antimicrobials drive microbial evolution, often through genomic changes or acquisition of mobile genetic elements that enhance fitness and resistance. This has critically influenced the control of cholera, particularly with the emergence of multidrug-resistant Vibrio cholerae strains, such as those recently reported in Yemen. To explore the long-term dynamics of antimicrobial resistance (AMR), we analyzed 149 clinical Vibrio isolates collected in India over 16 years (2008-2023), including five cases of Vibrio paracholerae. Phenotypic

Evaluation of a Novel Climate-Driven SIR Model for Cholera Prediction.

The causative agent of cholera, Vibrio cholerae, is a bacterium native to the aquatic environment and commensal to zooplankton, namely copepods. V. cholerae thrives in warm, moderately saline water and its incidence is strongly influenced by environmental factors, which have proven critical for predictive awareness of cholera by identifying outbreak locations and timing. Susceptible-Infected-Recovered (SIR) models provide useful information for understanding transmission dynamics and epidemic curves of disease outbreaks. Previous such models lacked predictive ability due to limited data in reg

Strengthening healthcare worker knowledge for outbreak preparedness and response via mobile technology in Nigeria and Uganda.

Healthcare worker (HCW) training on epidemic preparedness is vital for effective outbreak detection, reporting and response, especially in resource-limited settings. However, traditional training methods often fail to address the specific needs of frontline HCWs, causing significant delays in disease surveillance and response. This paper explores the Clinical Integrated Disease Surveillance and Response (cIDSR) training model, designed to supplement existing IDSR efforts and address key barriers in epidemic detection and notification. Piloted in Nigeria and Uganda, cIDSR provides HCWs with free
