We report the case of a 26-year-old man with a 14-year history of severe, highly localized pain and allodynia in the right medial retromalleolar region. He had undergone multiple previous medical evaluations and had repeatedly been treated for presumed tendinopathy without meaningful improvement. Magnetic resonance imaging (MRI) of the right ankle demonstrated multiple soft-tissue lesions involving superficial and deep tissue planes around the ankle and hindfoot. Electroneuromyography revealed severe focal axonal neuropathy of the right tibial nerve and its plantar branches. Surgical explorati

Peripheral nervous system involvement is a possible manifestation of antineutrophil cytoplasmic antibody (ANCA)-associated vasculitis (AAV). We present a case of a 48-year-old female patient with known AAV presenting with refractory left thigh pain. She underwent extensive workup for musculoskeletal, spinal, and vascular causes of her pain that were all unrevealing. Physical examination localized her pain to the femoral nerve, and magnetic resonance imaging (MRI) of the pelvis with neurography revealed findings consistent with femoral neuritis. She was promptly started on immunosuppressive tre

UNLABELLED: In human cells TORC1 mTOR (target of rapamycin) protein kinase complex renders heat shock transcription factor 1 (Hsf1) competent for stress activation. In such cells, as well as in yeast, the selective TORC1 inhibitor rapamycin blocks this activation in contrast to Hsp90 inhibitors which potently activate Hsf1. Potentially therefore rapamycin could prevent the Hsf1 activation that frequently compromises the efficiency of Hsp90 inhibitor cancer drugs. Little synergy was found between the effects of rapamycin and the Hsp90 inhibitor radicicol on yeast growth. However certain rapamyc

PURPOSE: Carpal tunnel syndrome is one of the most prevalent upper-extremity disorders. It may present unilaterally, bilaterally, or on the contralateral hand at a subsequent time to initial carpal tunnel release (CTR). We aimed to assess the incidence, timing, and factors associated with contralateral CTR at the time of an index unilateral surgery.
METHODS: We performed a retrospective chart review of 1,291 primary unilateral CTR surgeries performed from 2008 to 2013 with a minimum follow-up period of 10 years within our integrated health network. In total, 742 (58%) patients met our inclusi

BackgroundThis study aimed to evaluate outcomes after a minimum 5-year follow-up of feet with tarsal tunnel syndrome (TTS) after electroneuromyographic (ENMG) diagnosis and treatment. Possible predictive factors of outcome were sought.MethodsAll patients underwent, at the least, initial clinical evaluation, ENMG diagnosis, ultrasound and medical treatment for TTS. If treatment was unsuccessful after 6 months, tibial nerve release was proposed. Outcomes were classified as satisfactory (excellent and good) or unsatisfactory (fair and poor) based on Pfeiffer's classification.ResultsSeventy-six fe

Objectives: Compression neuropathies such as Anterior Tarsal Tunnel Syndrome are usually associated with focal thickening at the compression site. This study aimed to determine the frequency and location of thickenings of dorsal foot nerves in asymptomatic, healthy volunteers. We hypothesized that focal nerve thickening of dorsal foot nerves is a frequent finding in asymptomatic individuals and occurs at anatomically plausible locations, potentially limiting the specificity of ultrasound in the diagnosis of anterior tarsal tunnel syndrome. Materials and Methods: In this prospective study, the

Background/Objectives: Posterior tarsal tunnel syndrome is a compressive neuropathy of the tibial nerve at the level of the ankle within the tarsal tunnel. However, there is no established gold standard for the diagnosis of tarsal tunnel syndrome to date. High-resolution ultrasound could add important value in this setting. But up to date, to the best of our knowledge, only six clinical studies have investigated the use of ultrasound for the diagnosis of tarsal tunnel syndrome, with partially conflicting results. Most authors identify nerve swelling at the level of anatomical compression as th

PURPOSE: Tarsal tunnel neurovascular vulnerability lacks robust anatomical classification. We established an MRI-based retromalleolar canal classification to assess its clinical utility for surgical approach guidance and pathology diagnosis.
METHODS: This retrospective study analyzed normal ankle 3 T MRIs from 250 consecutive patients (2018-2023). Three musculoskeletal radiologists independently classified retromalleolar canal morphology using axial T2-weighted turbo spin-echo sequences (Siemens Skyra 3 T) acquired at a standardized plane 4 mm superior to the talar dome.
RESULTS: Three radio

BACKGROUND: The lack of a gold standard in tarsal tunnel syndrome (TTS) diagnosis leads to diagnostic inconsistencies and variation in patient selection for treatment. Therefore, the aim of this review is to summarize the diagnostic criteria used in current studies on TTS based upon this best-evidence synthesis.
METHODS: Three databases were searched to identify all studies on TTS. Studies were included when they included (1) diagnosis or treatment of TTS as the primary focus, (2) a description of the diagnosis of TTS, (3) an original data set of TTS cases, and (4) a minimum of 10 adult patie

Tarsal tunnel syndrome is an entrapment neuropathy caused by the compression of the tibial nerve and its terminal branches in the tarsal tunnel. Electrophysiological examinations are often used to diagnose tarsal tunnel syndrome. Surgical decompression of the tibial nerve is performed in patients who are resistant to conservative treatment. However, the preoperative electrophysiological findings that predict surgical outcomes remain unknown. This study aimed to clarify the preoperative electrophysiological findings that predict the surgical outcomes of tarsal tunnel syndrome. We reviewed 28 fe

OBJECTIVES: To evaluate the functional outcomes of surgical decompression for tarsal tunnel syndrome (TTS) and to explore the clinical relevance of diagnostic tests such as electromyography (EMG), magnetic resonance imaging (MRI), and ultrasound.
METHODS: We performed a retrospective single-center study of 15 patients with clinically diagnosed TTS who underwent open decompression (2015-2022). All had failed conservative management and completed ≥ 12 months of follow-up. Preoperative evaluation combined clinical exams with at least one confirmatory test (EMG, MRI, or ultrasound). Outcomes incl

Osteochondroma is the most common benign bone tumor; however, its presentation in the small bones of the appendicular skeleton, particularly the talus and subtalar joint, is quite rare. Although most cases of osteochondroma are asymptomatic, symptomatic presentations can lead to joint deformity, pain, swelling, tarsal tunnel syndrome, gait alterations, and a limited range of motion. Surgical resection has proven to be an effective treatment for symptomatic osteochondromas. This case report describes the management of a rare presentation of an isolated lateral osteochondroma of the talus succes

PURPOSE: The aetiology of tarsal tunnel syndrome is not always clear. Treatment commonly involves a single release of the flexor retinaculum, but post-operative outcomes are not always successful. This may be due to the lack of understanding the tissues that may compress the neurovasculature. The aim of this study was to define the tissues that separate the contents of the tarsal tunnel.
METHODS: Feet from embalmed body donors (n = 15; mean age = 83.9 ± 10.2 years; female = 9, male = 6) were examined. One underwent magnetic resonance imaging. All were dissected and modelled in virtual three-d

Foot and ankle injections are commonly performed by family physicians for the management of musculoskeletal conditions such as plantar fasciitis, osteoarthritis, interdigital neuromas, tarsal tunnel syndrome, and pathology of the first metatarsophalangeal joint. If symptoms persist despite conservative measures, injections may improve function and reduce pain. Before an injection, the clinician should discuss procedural risks, including pain, swelling, redness, stiffness, infection, and damage to surrounding tissues, and obtain informed consent. Corticosteroid injections can provide short-term

PURPOSE: The posterior tibial artery (PTA) branching within the tarsal tunnel is subject to significant anatomical variation, which can complicate surgical procedures such as tarsal tunnel decompression, flap harvesting, and medial ankle fixation. This study aimed to quantify the morphometry of the PTA bifurcation relative to the Dellon-McKinnon malleolar-calcaneal line (DML) in order to define vascular "safe zones."
METHODS: Twenty-five formalin-fixed adult cadaveric limbs were dissected. The bifurcation of the PTA into the medial and lateral plantar arteries was identified. Measurements wer

PURPOSE: This study aimed to characterize the anatomical features of the ankle flexor retinaculum and to provide a coordinate-based anatomical data on tibial nerve and posterior tibial artery branching points in Vietnamese cadaveric specimens.
METHODS: A descriptive cadaveric study was conducted on 60 lower leg and foot specimens from Vietnamese adult cadavers preserved in formalin. Anatomical measurements were obtained using calibrated electronic calipers. The branching points of the tibial nerve and posterior tibial artery were mapped using an O-xy Cartesian coordinate system, with the medi

BACKGROUND: Tarsal tunnel syndrome (TTS) is a relatively uncommon compressive neuropathy of the posterior tibial nerve that is often underdiagnosed due to variable presentation and overlap with other conditions. As patients increasingly turn to YouTube for health information, concerns have arisen about the reliability and comprehensibility of such content.
METHODS: On December 23, 2024, YouTube was queried using the terms "tarsal tunnel syndrome," "tarsal tunnel release," and "tarsal tunnel injection." After excluding duplicates, unrelated, non-English, and short (< 30 seconds) videos, 88 wer

Repetitive use injuries are commonly seen by musculoskeletal providers, usually in the form of tendinopathies. However, focal peripheral nerve entrapment can mimic or coexist with tendon pathology, particularly when weakness, sensory symptoms, or imaging findings are subtle or nonspecific. This review provides a practical, symptom-first framework to help clinicians distinguish tendon-dominant, nerve-dominant, and mixed presentations in clinical practice. Five clinically relevant tendon-nerve mimic pairs are emphasized: (1) rotator cuff tendinopathy versus suprascapular neuropathy; (2) lateral

STUDY DESIGN: Systematic literature review.
OBJECTIVE: Update on diagnostic utility of electrophysiology in lumbar spinal canal stenosis (LSCS).
SUMMARY OF BACKGROUND: LSCS is a highly prevalent degenerative spine condition characterized by neurogenic claudication, radicular pain, and muscle weakness. While lumbar spine MRI is the imaging modality for detecting spinal canal narrowing, it correlates poorly with clinical symptoms. Electrophysiological methods, including electromyography (EMG), nerve conduction studies (NCS), and evoked potentials (MEP and SEP), may provide complementary inform
