Fibrous and muscular components of the tarsal tunnel roof; a quantitative 3D anatomical study using dissection and a digital microscribe.

PURPOSE: There is no gold standard technique to diagnose and treat tarsal tunnel syndromes. This is due to the lack of consensus on the structures that form the roof of the tarsal tunnel. The flexor retinaculum is consistently stated to form the roof, but there is no clear understanding of the anatomy (borders, spatial relations) of this fibrous tissue. This makes the surgical release inconsistent, and potentially incomplete, when treating tarsal tunnel syndrome. METHODS: The aim of this study was to accurately describe the roof of the tarsal tunnel. Feet of embalmed Body Donors (n = 15; mean

A Novel Approach: Ultrasound-Guided Transgluteal Sciatic Nerve Block Using D5W and Bupivacaine/Dexamethasone for Acute Sciatica in the Emergency Department

BACKGROUND: Acute sciatica can cause severe radicular pain that is often unresponsive to systemic medications in the emergency department (ED). Ultrasound-guided transgluteal sciatic nerve blocks offer a targeted, opioid-sparing method of pain. CASE REPORT: We present a case of a patient with intractable bilateral lower extremity pain secondary to sciatica, successfully treated with transgluteal nerve blocks under ultrasound guidance. The sciatic nerves were visualized in the gluteal region between the greater trochanter and ischial tuberosity. One side was anesthetized using dextrose 5% in w

An atypical presentation of microscopic polyangiitis without renal involvement in an older patient with falls, bilateral lower limb swelling and neuropathy.

Microscopic polyangiitis (MPA) is a primary systemic small-vessel vasculitis associated with antineutrophil cytoplasmic antibodies (ANCA). The condition is uncommon among the UK population, although it increases with age. MPA manifests heterogeneously and may affect any organ, although glomerulonephritis is almost always present. This case report describes a rare presentation of MPA with isolated mononeuritis multiplex, without glomerulonephritis or other hallmark features, and reflects the varied manifestations of MPA and challenges in recognising the condition. Awareness of MPA is essential

Symptom Improvement after Surgery for Tarsal Tunnel Syndrome.

BACKGROUND: Tarsal tunnel syndrome (TTS) is an entrapment neuropathy of the posterior tibial nerve in the tarsal tunnel. Patients with TTS exhibit plantar symptoms, including numbness, foreign body sensation, and cold/burning sensation. We investigated postoperative improvement of symptoms and the association of preoperative symptoms with surgical outcomes in patients with idiopathic TTS. METHODS: We enrolled 17 consecutive patients (21 sides) with idiopathic TTS (mean age 68.6 years). The presence and severity of symptoms were evaluated before and 6 months after surgery and recorded on a num

Management Dilemma: Myeloperoxidase (MPO)-Positive Vasculitis in a Patient With Metastatic Malignant Melanoma.

A man in his 80s with a history of eosinophilic granulomatosis with polyangiitis (EGPA) presented with progressive bilateral leg weakness, sensory loss, and pain, on a background of metastatic malignant melanoma awaiting pembrolizumab therapy. Investigations revealed a relapse of anti-neutrophil cytoplasmic antibody (ANCA)-associated vasculitis, manifesting as vasculitic mononeuropathy. This created a therapeutic dilemma: corticosteroids were required to control vasculitis but risked blunting the effect of imminent immune checkpoint inhibition, highlighting the need for multidisciplinary manag

Spine-related leg pain: Definitions, etiology, clinical presentation, and management.

Spine-related leg pain (SRLP) is a common and clinically important presentation, affecting a substantial proportion of individuals with lumbar spine disorders. Compared to those with isolated axial low back pain, these patients experience higher pain intensity, greater disability, worse long-term outcomes, and increased healthcare utilization. Clinical management is complicated by inconsistent terminology among musculoskeletal healthcare providers, with the widespread and imprecise use of the term sciatica contributing to diagnostic uncertainty and treatment variability across heterogeneous co

Deep Gluteal Space Anatomy, Pathology, Clinical Presentation, and Management.

The deep gluteal space is a complex anatomic region associated with posterior hip symptoms. The clinical presentation involves symptoms and signs attributable to the deep gluteal space itself, in addition to other periarticular structures. Imaging involves both 2 and 3 dimensional modalities. Collision-detection software may be highly valuable for dynamic assessment. Nonsurgical treatment involves education-driven activity modification and physical therapy, combined with medications and selective injections. Surgical treatment involves endoscopic or open management, performed in isolation or c

Peripheral Neuropathy Expands the Neurological Phenotype in Glutaric Aciduria Type 1.

Glutaric aciduria type 1 (GA1) is a neurometabolic disorder characterized by striatal injury in infancy and extrastriatal central nervous system abnormalities, the latter depending on the biochemical subtype. Whether the peripheral nervous system (PNS) is also affected has not been systematically studied. Therefore, we conducted a cross-sectional study of 21 GA1 patients (15 high excretor [HE], 6 low excretor [LE]), identified either by newborn screening (NBS, n = 11) or targeted metabolic diagnostics (TMD, n = 10). All underwent clinical evaluation, cerebral MRI, neurophysiology, and MR-neuro

Ultrasound Depiction of the Optimal Window for Needle Placement for Electromyography of the Short Head of the Biceps Femoris.

INTRODUCTION/AIMS: Accurate needle electromyography (EMG) of the short head of the biceps femoris (SHBF) is often important to localize nerve damage when proximal to the fibular head. Techniques have been proposed for accurate SHBF EMG; however, needle placement accuracy is low and with the risk of inadvertent needle placement into the long head (LHBF) potentially resulting in false localization and inappropriate operative management. This study aimed to define the optimal window for accurate SHBF needle EMG placement through ultrasound (US) investigation of the anatomical relationships of the

MR neurography of tibial and common peroneal nerves in patients with Guillain-Barre syndrome and electrophysiological correlation.

INTRODUCTION: The early and accurate assessment of peripheral nerve injury in patients with Guillain–Barré syndrome (GBS) is crucial for clinical diagnosis and treatment. Recently, there has been growing interest in using MR neurography especially diffusion-weighted imaging with background suppression (DWIBS) to evaluate peripheral nerves and associated lesions. However, this technique has not yet been applied to GBS patients. The aim of this study was to investigate the potential value of MR neurography in assessing the tibial nerve (TN) and common peroneal nerve (CPN) in individuals with GBS

MR Neurography in Children and Adolescents: Multiparametric Assessment of Peripheral Nerve Involvement in Long-chain Fatty Acid Oxidation Disorders.

OBJECTIVES: MR neurography (MRN) is a modern technique for visualizing peripheral nerves and quantifying microstructural pathology, yet its use in pediatric populations remains largely unexplored. This study evaluates the applicability and diagnostic performance of MRN in children and adolescents with genetically confirmed long-chain 3-hydroxyacyl-CoA dehydrogenase deficiency (LCHADD) and mitochondrial trifunctional protein deficiency (MTPD), in which peripheral neuropathy is a known long-term complication. MATERIALS AND METHODS: In a prospective cross-sectional study, 15 patients (LCHADD n =

Comparative study of photobiomodulation therapy and low-intensity pulsed ultrasound for nerve regeneration and pain alleviation in an acute spinal cord injury model.

Spinal cord injury (SCI) remains a significant medical challenge due to the complexity of its pathology. This study evaluated the effects of photobiomodulation therapy (PBMT) and low-intensity pulsed ultrasound (LIPUS) as non-invasive treatments for SCI. Thirty-six male Wistar rats were randomly assigned to 6 groups: control, SCI, PBMT, and LIPUS at 0.1, 0.3, and 0.5 W/cm2 intensities. SCI was induced using an aneurysm clip, and treatments began 30 minutes post-injury, continuing for 4 weeks. During the seventh week, no treatment was administered. Behavioral assessments, including the acetone

Pearls and Pitfalls for the Direct Posterior Approach to the Proximal Tibia and Knee Joint: A Clinical Experience From a Tertiary Center in Central India.

Background The posterior approach to the proximal tibia provides direct access to posterior knee and tibial pathologies, allowing controlled identification and protection of neurovascular structures within the popliteal fossa. Despite these advantages, concerns regarding prone positioning, potential neurovascular injury, wound complications, and limited surgeon familiarity have restricted its widespread use. This study aimed to evaluate indications, perioperative complications, and functional outcomes of the posterior approach to the proximal tibia, highlighting practical pearls and pitfalls.

Sports-Related Posterior Ankle Pain: Diagnosis and Management of Lateral Calcaneal Branch Neuropathy of the Sural Nerve (A Case Series).

BACKGROUND: Posterior ankle pain has varied etiologies, with sural nerve (SN) entrapment contributing to posterolateral ankle discomfort. The SN is a pure sensory nerve that innervates the lateral ankle and foot up to the fifth metatarsal. Although SN pathologies are known, specific clinical features and management of neuropathy affecting the lateral calcaneal branch of the sural nerve (LCBSN), which supplies the lateral heel, are less defined. This condition is often exacerbated by repetitive ankle dorsiflexion in sports or external compression from tight footwear. We hypothesized that LCBSN

Long-term outcomes of selective tibial neurotomy for spastic foot deformity: Initial clinical experience in Vietnam.

INTRODUCTION: Spastic foot deformity after central nervous system lesions is clinically heterogeneous and often combines equinus, varus, claw toes, pain, and gait limitation. Selective tibial neurotomy (STN) is a peripheral neurosurgical option for focal lower-limb spasticity, but data from low- and middle-income settings remain limited. RESEARCH QUESTION: What are the long-term clinical and functional outcomes of STN for spastic foot deformity, and are these outcomes influenced by the extent of motor fascicle resection? MATERIAL AND METHODS: We conducted a retrospective study of 31 patients

A Rare and Interesting Case of Sciatic Nerve Compression Due to Proximal Femoral Osteochondroma in a Young Adult - A Case Report.

INTRODUCTION: Osteochondromas (OCs) are benign hamartomatous lesions composed of cartilage and bone, arising from aberrant growth-plate cartilage. They characteristically project from the bone surface through the cortex and grow by endochondral ossification beneath the periosteum. Lesion growth usually parallels skeletal growth and typically ceases after physeal closure, although minimal growth of the cartilage cap may persist and generally stops by the third decade of life. This developmental mechanism explains the predilection for involvement of the distal femur, proximal tibia, and proximal

Differential awakening of the sciatic nerve: illustrative case.

BACKGROUND: Differential awakening is a well-known, yet poorly understood phenomenon seen in patients with intracranial pathologies. Because acute postoperative neurological decline has many potentially reversible causes, distinguishing differential awakening from other etiologies is critical. To the authors' knowledge, differential awakening has not been reported following peripheral nerve procedures. OBSERVATIONS: A 40-year-old man presented with a 1.5-year history of painful bilateral cervical and right lumbosacral radiculoplexus neuropathy recently associated with bilateral facial and ves

Autologous Osteochondral Grafting From an Ipsilateral Ankle Osteophyte for Chronic and Large Osteochondral Lesion of the Talus.

Large osteochondral lesions of the talus (OLTs) in the chronic phase remain challenging to manage due to degeneration of the osteochondral fragment and underlying subchondral bone defects, which often make conventional fixation techniques difficult to perform. Although various surgical approaches-including autologous osteochondral transplantation, chondrocyte implantation, and structural allografting-have been employed, each carries limitations such as the requirement for an additional donor-site harvest, the need for staged procedures, or limited graft availability. We report the case of a 19
