Shear wave elastography (SWE) is an increasingly investigated ultrasound-based technique in musculoskeletal imaging that provides quantitative information on tissue stiffness and biomechanical properties. This narrative review aims to summarize the basic principles, technical considerations, current clinical applications, limitations, and future perspectives of SWE in musculoskeletal imaging. Unlike conventional grayscale and Doppler ultrasonography, which mainly assess morphology and vascularity, SWE may provide additional functional information in major musculoskeletal tissues, including ten

Lipomatosis of nerve is a rare, nonhereditary congenital overgrowth disorder involving fibroadipose tissue proliferation along nerve territories, most commonly the median and plantar nerves. We report a case of a 35-year-old man presenting with slowly progressive enlargement of the thumb and index finger since childhood, associated with increasing discomfort. Imaging demonstrated nonencapsulated fatty overgrowth with digital nerve involvement and associated bony hypertrophy. Surgical management included right thumb debulking and distal phalanx amputation of the index finger. Histopathologic ex

BACKGROUND: Cystic adventitial disease (CAD) and intraneural ganglion cysts (IGCs) are rare mucinous lesions typically linked to an articular origin via macroscopic conduits. While secondary propagation between vascular and neural systems may occur through shared fascial sheaths, a direct microvascular route has not been previously described.
OBSERVATIONS: A 60-year-old man presented with progressive left tibial neuropathy. High-resolution 7T MRI and surgery revealed a large popliteal CAD lesion connecting to the knee joint via the middle genicular artery, alongside a secondary tibial IGC lac

BACKGROUND: Many patients experience chronic pain, especially after trauma or surgery, despite a negative workup. We hypothesize that underdiagnosed peripheral neuropathy may be contributing to their intractable symptoms.
METHODS: This is a retrospective cohort of patients who underwent surgical neurolysis for chronic pain that was neuropathic in nature, in the distribution of a peripheral nerve, with a positive scratch collapse test. Before surgery, patients were administered a corticosteroid injection at the lesion. The principal outcome was a self-reported visual analog scale pain score. M

OBJECTIVE: Schwannomas in the tibial, plantar, or medial calcaneal nerves around the ankle can mimic symptoms of tarsal tunnel syndrome. Outcomes after resection of schwannomas from these nerves have mainly been reported in case reports. The objectives of this study were to investigate the presentation and results for resection of schwannomas around the tarsal tunnel in a large case series and systematically review the currently available literature.
METHODS: Data from 40 patients (27 sporadic and 13 schwannomatosis cases) treated at 2 centers were retrospectively analyzed for preoperative ty

BACKGROUND: Knee osteoarthritis is a leading cause of chronic pain and disability, and many patients have persistent pain despite conservative treatment or are not candidates for arthroplasty. Contemporary anatomical studies show that the classical three-nerve genicular radiofrequency ablation (RFA) model does not adequately reflect the multibranch innervation of the anterior knee capsule, which may limit efficacy. Structured preprocedural planning may help standardize cannula placement, however, its clinical and anatomical validity requires further investigation.

Neurologic complications after spinal anesthesia are uncommon but remain a major concern because of their potential severity. Distinguishing anesthesia-related neurologic injury from preexisting spinal pathology is often challenging. A 70-year-old man with hypertension, hyperlipidemia, and known lumbar degenerative disease underwent elective bilateral inguinal hernia repair under spinal anesthesia. Spinal anesthesia was performed via a paramedian approach at the L3-4 interspace using a 25-gauge Quincke needle and 2.6 mL (13 mg) of 0.5% hyperbaric bupivacaine without paresthesia or technical di

Severe plantar lesions are common during multistage desert ultramarathons and may require debridement and sequential dressing in austere field clinics. These procedures can be intensely painful because blistering, maceration, epidermal loss and sand contamination expose highly sensitive plantar tissue. We describe 3 runners treated during the Marathon des Sables who required advanced plantar wound care and received ultrasound-guided tibial nerve block for procedural analgesia. In each case, an isolated tibial nerve block was performed at the medial ankle using 8 mL of 1% lidocaine without epin

Upper limb disorders, including carpal tunnel syndrome (CTS), are a common cause of pain, functional limitations, and reduced quality of life. Increasing attention has been directed toward the role of inflammatory and angiogenic mediators in the pathophysiology of CTS and their potential relationship with patient-reported functional outcomes. The aim of this study was to evaluate selected cytokines as potential markers of upper limb disability assessed using the Disabilities of the Arm, Shoulder and Hand (DASH) questionnaire. This cross-sectional study included 55 patients with idiopathic CTS

AIMS: To evaluate the association between diabetes and four peripheral nerve entrapment syndromes: carpal tunnel syndrome (CTS), ulnar nerve entrapment (UNE), radial nerve entrapment (RNE), and tarsal tunnel syndrome (TTS).
METHODS: This retrospective cohort study used the Merative™ MarketScan® Commercial Database (2007-2023). Adults ≥18 years were classified as having diabetes or serving as controls. Incident-only cohorts excluded individuals with prior diagnoses during a washout period. Propensity score matching (1:1, sex-stratified) reduced confounding, and Cox proportional hazards regress

OBJECTIVE: Endometriosis is associated with neuropathic pain in approximately 40% of cases. Involvement of the sciatic nerve or sacral plexus represents one of the most frequently reported neuropathic pain presentations in endometriosis, for which no specific management guidelines currently exist. This study evaluate the efficacy and safety of laparoscopic decompression of the sciatic nerve or the sacral plexus for neuropathic sciatic pain caused by endometriosis.
METHOD: A systematic search was conducted in accordance with the Preferred Reporting Items for Systematic Reviews and Meta-Analyse

Neuromyelitis optica spectrum disorder (NMOSD) is a rare autoimmune astrocytopathy affecting the central nervous system, particularly the optic nerves and spinal cord. Post-infectious triggers, including SARS-CoV-2 (COVID-19), have been increasingly recognized as potential catalysts for this immune-mediated condition. We report the case of a nine-year-old boy who developed AQP4-IgG-positive NMOSD following confirmed SARS-CoV-2 infection. The patient's clinical onset began 1.5 months prior to admission with a seven-day episode of intractable vomiting, indicative of area postrema syndrome (APS).

OBJECTIVE: To explore the pathological changes of peripheral neuropathy in Sjogren's syndrome and the role of macrophages in it.
METHODS AND RESULTS: Methods: Sural nerve biopsy was performed in 12 patients diagnosed with primary Sjogren's syndrome associated peripheral nervous system involvement (pSS-PN) and 3 traumatic amputees. First, we collected clinical data and electromyography (EMG) findings from 12 pSS-PN patients. Histological assessment of sural nerve specimens was subsequently performed using hematoxylin-eosin (HE) and neurofilament protein (NF) staining under light microscopy. Th

BACKGROUND: Neurolymphomatosis (NL) is an uncommon manifestation of lymphoma caused by direct infiltration of the peripheral nervous system by lymphoma cells. Early recognition is particularly difficult when deep nerves such as the sciatic nerve are involved. Combining contrast-enhanced MRI and PET/CT to assess anatomic extent and metabolic activity, together with nerve ultrasound (US) to identify characteristic signs and guide biopsy, may improve diagnostic accuracy.

UNLABELLED: Adamantinomas and osteofibrous dysplasia-like (OFD-like) adamantinomas are rare tibial bone tumors with variable presentations, making diagnosis and management challenging. While en bloc resection with wide margins is generally preferred, the optimal method for reconstruction remains debated. This study assesses clinical and functional outcomes following tibial reconstruction using an intercalary free vascularized fibular graft harvested from the contralateral limb. A retrospective single-center review included patients treated between 1998 and 2020 for tibial adamantinoma or OFD-l

The high physiologic demands of sports create dynamic stress on joints, soft tissues, and nerves which may lead to injuries in the athlete. Electrodiagnostic (EDx) assessment is essential to identify the correct diagnosis, localization, and prognosis, to guide management of sports-related neuropathies. A comprehensive review was performed to provide the EDx medical consultant with a practical approach to the common peripheral nerve disorders seen in athletes. Sports-related neuropathies reviewed include transient traumatic irritation of the brachial plexus and/or cervical nerve roots ("Burners

INTRODUCTION: Schwannoma is a type of slow growing benign nerve sheath tumor arising from Schwann cells. Peripherally, they are found in association with the ulnar, sciatic and posterior tibial nerves. Peripheral schwannomas, neurofibromas and malignant PNSTs are collectively grouped as peripheral nerve sheath tumors.
PRESENTATION OF CASE: We report a case of a 35-year-old male, without clinical features of NF2, presenting with 2 painful lumps around the ankle, and was incidentally found to have multiple lumps in both lower limbs. MR imaging, surgical excision and histology revealed multiple

Recurrent nerve entrapment syndrome is a well-known complication in peripheral nerve surgery that often leads to multiple reoperations and increases the risk of unfavorable outcomes due to scarring. In our outpatient clinic, we found two patients with recurrent nerve entrapment syndrome with significant symptoms such as complete sensory loss and chronic pain who were willing to undergo re-exploration of their entrapped nerves and cover them with a human acellular dermal matrix (ADM) as a protective shield against recurrence. Both patients had complete recovery of the nerve entrapment syndrome
