Tarsal tunnel syndrome is an entrapment neuropathy of the tibial nerve and its branches in the tarsal tunnel. The literature on surgical release of the tarsal tunnel shows variable outcomes with no studies reporting validated patient reported outcomes. We aim to determine clinical response after tarsal tunnel release using the Patient-Reported Outcomes Measurement Information System (PROMIS). CPT code 28035 was used to identify patients who underwent isolated tarsal tunnel release (TTR) between 1/1/2015 and 12/15/2022 at a single institution. Patient demographic data and PROMIS physical functi

Fecal incontinence is a common condition that can significantly impact patients' quality of life. Obstetric anal sphincter injury and anorectal surgeries are common etiologies. Endoanal ultrasound and anorectal manometry are important diagnostic tools for evaluating patients. There are various treatment options, including diet, lifestyle modifications, drugs, biofeedback therapy, tibial and sacral nerve neuromodulation therapy, and surgery. In this editorial, we will discuss current controversies and novel approaches to fecal incontinence. Screening for asymptomatic anal sphincter defects afte

Pure neuritic leprosy (PNL) is a rare clinical variant of leprosy, especially in children. This variant of leprosy only involves peripheral nerves without skin involvement or acid-fast bacilli (AFB) found on slit-skin smear examination. The challenging diagnosis and establishment of PNL due to the absence of skin lesions often lead to disability. A case of PNL with grade 2 disability of hands and feet in a 16-year-old boy was reported. The patient has been living in a leprosy-endemic area and has a history of contact with people who have symptoms of leprosy. On physical examination, there were

A 42-year-old female presented with acute onset of asymmetric lower extremity weakness, bilateral lower extremity areflexia and sensory loss, and urinary incontinence. Initial investigation with a spine MRI for a spinal cord process was negative, leading to further investigation with nerve conduction studies/electromyography (NCS/EMG). This revealed absent F waves bilaterally in the fibular nerves, and there was only one isolated F wave with decreased amplitude and increased latency in the left tibial nerve and had lack of heterogeneity in the right tibial nerve. Given the early presentation o

INTRODUCTION: Visualization and exposure of the medial meniscus in the posterior compartment during knee arthroscopy can be challenging. Using a posterolateral transseptal approach can help to have a better visualization to ensure a better meniscal repair in the posterior compartment.
HYPOTHESIS: The posterolateral transseptal approach allows a better visualization of the posterior segment of the medial meniscus when compared to a trans notch approach.
MATERIAL AND METHODS: A controlled laboratory study using 12 human cadaveric knees were included in this arthroscopic study. The first step w

While the pathophysiology of space-occupying lesions inside the tarsal tunnel causing tarsal tunnel syndrome (TTS) is obvious, the occurrence of a posterior tibial nerve (PTN) mass outside the tarsal tunnel but mimicking or in combination with TTS symptomatology is less clear. Therefore, we report three rare cases of patients presenting with TTS symptoms combined with a distal leg PTN schwannoma, all of whom were treated with tumor resection and tarsal tunnel decompression. All patients had a long-lasting history of leg and ankle pain radiating to the medial aspect of the foot and toes. Pain w

BACKGROUND: Friedreich's ataxia (FRDA) affects both the central and peripheral nervous system. Peripheral nerve involvement manifests predominantly as a progressive sensory neuropathy caused by dorsal root ganglionopathy. An additional direct involvement of peripheral nerves leading to abnormal myelination is increasingly discussed. Here, we characterize lower extremity peripheral nerve involvement in FRDA by quantitative magnetic resonance neurography (MRN).
METHODS: Sixteen genetically confirmed FRDA patients and 16 age-/sex-matched controls were prospectively enrolled. Patients underwent n

The present study provides in vivo trials of electrospun poly(l-lactide-co-ε-caprolactone), PLCL, copolymer 67:33 mol %, and electrospun PLCL blend with a low loading of collagen (0.5% w/v), PLCL-Col, as a connecting porous biodegradable nerve conduit to repair 7 mm long segmentary tibial nerve lesions in rats compared with the standard autograft technique. The electrospun PLCL scaffolds reveal a matrix of fibers with a mean diameter of 476 ± 60 nm and an average pore size of 253 ± 5 nm. Blending collagen with the PLCL results in a comparatively denser matrix of fibers with a mean diameter of

Knee osteoarthritis (OA) and obesity are closely linked, with obesity being the primary risk factor for OA. This study used the medial meniscus of sows (250 kg, 4-6 years old) as a model for spontaneous OA, comparing them to lighter pigs (110 kg, 7-8 months old). The joints were collected according to the 3Rs principles (Replacement, Reduction and Refinement) and compared using macroscopic and microscopic analyses, as well as advanced imaging techniques. The results revealed more severe pathological changes in the sows. Micro-computed tomography analyses showed thickening of radial tie fibers

Common peroneal nerve palsy is often caused by compression at the fibular head, with ganglion cysts being the most common etiology. Although rare, meniscal cysts can also compress the nerve. We report a case of common peroneal nerve palsy due to a lateral meniscal cyst, successfully treated with surgery. A 38-year-old man presented with progressive pain and numbness in the right lateral knee and foot, along with difficulty dorsiflexing the ankle without any apparent history of trauma. Examination revealed a palpable mass on the lateral knee and sensory deficits in the superficial peroneal nerv

Peripheral nerve invasion can occur during cancer progress, which seriously affects the patient's quality of life and brings pain. There is currently no effective treatment method. Therefore, in this study, we established a model of breast cancer bone metastasis and transplanted olfactory sheath cells (OECs) into rats to explore a new cell therapy strategy for treating cancer pain. The results showed that compared with the cancer pain group and the vehicle group (DMEM/F12 medium solution), OEC transplantation improved tibial bone destruction and osteolytic lesions, increased the mechanical wit

Rheumatoid vasculitis (RV) is an extra-articular manifestation (EAM) of rheumatoid arthritis (RA), which is typically observed in long-standing, severe disease. The skin and peripheral nerves are most commonly affected, although the clinical presentation can be highly variable. We report an unusual case of a 58-year-old male who presented with peripheral neuropathy, constitutional symptoms, and Raynaud's phenomenon and was subsequently diagnosed with RV despite having no history of RA. Rheumatoid factor (RF) and anti-cyclic citrullinated peptide (anti-CCP) antibodies were strongly positive, an

Low back pain is common in women, especially during pregnancy and puerperium. Septic sacroiliitis, a rare cause of back pain in the postpartum period can mimic other common causes of low back pain like muscle strain, urinary tract infection, pelvic inflammatory disease, endometritis and intervertebral disc prolapse. The proximity of the sacroiliac joint to the sacral nerve plexus results in septic sacroiliitis frequently presenting with symptoms mimicking intervertebral disc prolapse. Diagnosis of septic arthritis of the sacroiliac joint during the postpartum period requires a high index of su

BACKGROUND: Neurovascular complications after tibial plateau fractures are rarely reported, despite their clinical relevance. While Schatzker type IV fractures have traditionally been considered the most at risk, supporting evidence is limited. This is the first large-scale study systematically evaluating neurovascular injuries in tibial plateau fractures. The aim was to determine the incidence of arterial and neurological injuries and to identify fracture patterns most frequently associated with these complications.
METHODS: We conducted a retrospective review of patients who underwent open
