Obesity is a prevailing health concern, with bariatric surgery being a common treatment option despite the potential nutritional risks and poorly understood postoperative physiological changes, particularly in the gastrointestinal tract. Neurological complications following bariatric surgery, often secondary to micronutrient deficiencies like vitamin B12, thiamine, and copper deficiencies, have been increasingly reported. This report presents a case of a 20-year-old female patient who developed neurological deficits, specifically bilateral lower limb weakness, following laparoscopic sleeve gas

The patients were a 59-year-old man and a 53-year-old woman, both of whom had preceding fever, followed by impaired consciousness, urinary retention, and unsteadiness while standing. Neurological examination revealed truncal ataxia. Cerebrospinal fluid testing was positive for anti-glial fibrillary acidic protein (GFAP) α antibodies, leading to a diagnosis of GFAP astrocytopathy. Both patients showed improvement following steroid therapy. Somatosensory evoked potential (SEP) in the tibial nerve stimulation demonstrated prolonged N21-P38 conduction times in both cases, suggesting involvement of

RATIONALE: Piriformis syndrome (PS) is a neuromuscular condition in which irritation or dysfunction of the piriformis muscle leads to compression of the sciatic nerve, resulting in radiating pain in the buttock and lower limb. Accurate diagnosis is often complicated by the clinical overlap between PS and other neurologic conditions such as lumbar radiculopathy or sciatica, as these disorders may present with similar symptomatology.
PATIENT CONCERNS: This case report describes the diagnosis and treatment of PS in a 37-year-old male military pilot. The patient had a history of lumbar disc herni

Intraneural ganglion cysts (IGCs) are benign, mucinous lesions that are often found within the epineurium of peripheral nerves, causing mononeuropathy due to nerve compression. These cysts arise from synovial fluid tracking from an adjacent synovial joint along an articular nerve branch to the parent nerve, as proposed by the unifying articular theory. This theory has transformed the understanding of IGCs from their being considered as isolated nerve lesions but as joint-related pathologies. The emphasis of identifying the cyst's origin and its connection to the neighboring joint has shifted t

Foot drop has various etiologies, such as neurologic (peripheral nerve injury, neuropathy, lumbar radiculopathy, and cerebral lesions) or muscular (extensor muscle injury and compartment syndrome) causes. Tendon transfers can aid in the restoration of dorsiflexion in the treatment of foot drop. The tibialis posterior tendon is the primary tendon used in these surgeries for transfer. In this present study, the flexor hallucis longus tendon is transferred to the dorsum of the foot because of following knee dislocation that caused a nonfunctioning posterior tibial tendon. At the sixth month posto

BACKGROUND: Plantar warts, caused by the human papillomavirus (HPV), are a common skin condition characterized by painful lesions on the soles of the feet. These lesions can significantly impact skin appearance, quality of life, and, in severe cases, mobility. Traditional treatment methods, such as chemical cauterization or pharmaceutical therapies, are often painful and require multiple visits to achieve complete wart removal and skin regeneration. This study aims to assess the clinical effectiveness of the needling technique as an alternative treatment. This method involves repeatedly punctu

Managing soft tissue defects in the heel presents a significant challenge for surgeons due to the heel's crucial role in weight-bearing. A free instep flap with end-to-side nerve coaptation offers an effective solution for reconstructing large heel defects. Four patients were referred to our hospital with soft tissue lesions in the weight-bearing heel and instep area. Their heels were reconstructed using a free instep flap harvested from the opposite foot. In three cases, an end-to-side arterial anastomosis was performed. In the fourth case, due to a midfoot amputation, an end-to-end arterial

INTRODUCTION AND IMPORTANCE: Morton's Neuroma is a neuropathy of the forefoot, specifically located on the interdigital nerve, and is associated with the thickening of the intermetatarsal ligament and nerve fibrotic. The management of Morton's Neuroma is challenging. Various treatments have been introduced for treating this lesion, starting from conservative treatment using the metatarsal pad, physical therapy, and corticosteroid injection to surgical treatment. This report highlights a novel approach to Morton's Neuroma using a pulsed radiofrequency (PRF).

INTRODUCTION: Cubital tunnel syndrome (CuTS) is the second most common compressive neuropathy of the upper extremity. Several surgical approaches are available for a definitive treatment. This systematic review and meta-analysis aimed to comprehensively assess and compare the efficacy and safety of endoscopic in situ decompression (EISD) and open in situ decompression for CuTS.
METHODS: A systematic search was conducted using online databases (PubMed/MEDLINE, Embase, and Cochrane) on the 14th of April 2024. All human RCTs and observational studies in English were included in this review.

BACKGROUND: Extramedullary plasmacytomas (EMPs) are rare plasma cell tumors forming outside the bone marrow. EMPs spread to soft tissues with a predilection for the head and neck region. Therefore, EMPs are rarely included in the differential for peripheral nerve sheath tumors (PNSTs). The authors discuss a case of tibial nerve sheath invasion by an EMP in the context of multiple myeloma (MM).
OBSERVATIONS: A 70-year-old female with MM presented with right leg pain and a suspected PNST of the tibial nerve. Imaging revealed a T1-hypointense lesion in the popliteal fossa. Resection was attempte

Tarsal tunnel syndrome (TTS) is elicited by compression of the tibial nerve in the tarsal tunnel. TTS in the absence of a lesion tends to be idiopathic, and most TTS is idiopathic. Patients complain of several plantar symptoms, and TTS affects their quality of life. The symptoms tend to worsen with walking, and ankle joint movement and arterial distortion may also be involved. Because TTS symptoms are similar to those of diabetic neuropathy and lumbar disease, clinical symptoms are diagnostically important. While magnetic resonance imaging reveals nerve compression, it is difficult to identify

BACKGROUND/OBJECTIVES: Among the various types of masses that can cause compression, hematomas are a relatively common but often overlooked cause. Rheumatoid arthritis (RA) is associated with bleeding problems due to vascular inflammation, platelet dysfunction, impaired production of clotting factors, and medication use.
CASE PRESENTATION: We report a case of a 76-year-old woman with RA who developed vascular and neurological symptoms in her right lower leg due to compression of the popliteal artery and tibial nerve by an organized hematoma in the popliteal fossa. She experienced swelling, pa

BACKGROUND: Frankincense, an ancient aromatic substance known for promoting blood circulation, is widely used in traditional Chinese medicine to treat skin lesions.
METHODS: This study aimed to evaluate the efficacy and underlying mechanisms of frankincense oil extract (FOE) in addressing high-risk diabetic foot by improving microvascular circulatory disorders. The vascular repair effects of FOE were assessed in a streptozotocin-induced high-risk diabetic foot animal model. Various evaluations were performed, including infrared thermometry, motor nerve conduction velocity (MNCV) testing, lase

Popliteal schwannomas are rare benign tumors arising from the peripheral nerve sheaths, often misdiagnosed due to their nonspecific symptoms. We report a case of a 36-year-old female with a 10-year history of refractory left leg pain localized to the S1 dermatome, characterized by persistent burning and aching sensations along the plantar foot, heel, and posterior calf. Despite multiple consultations with various specialists over the years, her symptoms were repeatedly misattributed to lumbar spine pathology, resulting in delayed diagnosis. A detailed clinical evaluation eventually revealed a

INTRODUCTION: Double crush syndrome (DCS) refers to compressive neuropathy at multiple sites along a peripheral nerve (PN), yet its relevance in the lower extremity remains poorly defined. This study aimed to (1) determine the prevalence of PN lesions in patients undergoing surgery for lumbosacral radiculopathy (LR), (2) identify commonly affected nerves, (3) assess associated risk factors, and (4) evaluate the DCS hypothesis by comparing the incidence of PN lesions in patients undergoing surgery for LR versus matched controls.
METHODS: A retrospective cohort study was conducted using the Pea

BACKGROUND: Cubital tunnel syndrome, the second most common compression neuropathy of the upper extremity, has several surgical treatment options including simple (in situ) decompression, decompression with medial epicondylectomy (ME), or anterior transposition to a subcutaneous, submuscular, or subfascial position. The existing literature does not clearly establish the superiority of one procedure over the other. This study aims to compare two techniques, ME combined with an adipofascial flap versus anterior subcutaneous transposition, in terms of outcomes including resolution of neurological

OBJECTIVE: Resection of common peroneal nerve (CPN) schwannomas is generally believed to be associated with a high risk of postoperative deficit, especially the chance for development of a foot drop. The goal of this study was to investigate the surgical results for resection of schwannomas from the CPN around the knee and specifically the chance of developing a postoperative motor deficit.
METHODS: Data from 36 patients with sporadic schwannomas and 8 patients with schwannomatosis (12 schwannomas total) treated at two centers were retrospectively analyzed. For sporadic cases, different locat

Entrapment neuropathies of the lower extremity are often underdiagnosed due to limitations in clinical examination and electrophysiological testing. Advanced imaging techniques, particularly MR neurography and high-resolution ultrasonography (US), have significantly improved the evaluation and diagnosis of these conditions by enabling precise visualization of nerves and their surrounding anatomical structures. This review focuses on the imaging features of compressive neuropathies affecting the lumbosacral plexus and its branches, including the femoral, obturator, sciatic, common peroneal, and
