PURPOSE: Meralgia paraesthetica (MP) is a rare neuropathy of the lateral femoral cutaneous nerve (LFCN), characterized by thigh pain, paraesthesia, or sensory loss. When conservative treatments fail, surgical interventions such as neurolysis or neurectomy are considered. This study aims to evaluate the effectiveness of surgical decompression of the LFCN in alleviating pain, addressing sensory deficits, and improving quality of life.
METHODS: A prospective study was conducted on 26 patients who underwent LFCN decompression surgery between 2015 and 2023, with a minimum follow-up period of one y

BACKGROUND: Meralgia paresthetica (MP) is a compressive neuropathy of the lateral femoral cutaneous nerve (LFCN), characterized by pain, paresthesia, and numbness in the anterolateral thigh. Despite its well-documented etiology, diagnosing MP remains challenging, as complementary tests such as electromyography (EMG) and ultrasound may yield normal results due to anatomical variations and the dynamic nature of nerve compression.
OBJECTIVE: This study introduces the Hip Abduction Maneuver (HAM) as a novel diagnostic tool for LFCN compression and investigates its utility in clinical and intraope

BACKGROUND: Meralgia paraesthetica (MP) is caused by lateral femoral cutaneous nerve (LFCN) compression. Surgical intervention is considered for refractory cases, with LFCN decompression being the most commonly performed procedures. In this study, we present the clinical outcomes of LFCN neurectomy performed at a single neurosurgical department. Our findings are juxtaposed meta-analysis assessing the success rates of different surgical strategies.
MATERIAL AND METHOD: 14 LFCNs neurectomies were performed in 13 MP patients at our Department between 2015 and 2022. Follow-up evaluations were con

Meralgia paraesthetica (MP) is a mononeuropathy of the lateral femoral cutaneous nerve (LFCN), typically caused by compression at the level of the inguinal ligament. The syndrome presents with burning pain, paraesthesia, and hypaesthesia in the anterolateral thigh, without motor deficits. The LFCN, derived from the L2-L3 spinal nerves, is vulnerable to compression or traction due to its anatomical course under the inguinal ligament. MP may result from both traumatic (e.g., surgery, injury) and non-traumatic causes (e.g., obesity, pregnancy, tight clothing). In some cases, the etiology remains

BACKGROUND: Surgical treatment of entrapment neuropathies based on nerve decompression. A different approach applies in the case of meralgia paraesthetica affecting the purely sensory lateral femoral cutaneous nerve (LFCN). Established treatment options include both decompression and LFCN neurectomy. The latter not only provides symptomatic relief but also enables proper histopathological analysis, offering deeper insight into the etiopathogenesis of meralgia paraesthetica.
MATERIAL AND METHOD: 14 LFCNs neurectomies were performed 13 patients at our department between 2015 and 2022. Histopath

INTRODUCTION: Talocalcaneal coalition (TCC) is one of the most common tarsal coalitions in children. However, current classifications of TCC are limited and focus on histologic or morphologic features without clinical correlation or therapeutic guidance. Moreover, no classification system specifically designed for pediatric populations. We hypothesize that the novel classification system will be clinically relevant, reliable, and easy to use for pediatric TCC management.
METHODS: A total of 43 patients (58 feet) with pediatric TCC confirmed by 3-dimensional CT scans were included in this stud

BACKGROUND: Multinodular/plexiform schwannoma is an exceedingly rare benign peripheral nerve sheath tumor that often arises in the dermis or subcutaneous tissue, particularly of the trunk and head and neck region. The ankle is an uncommon location for this peculiar condition.
CASE REPORT: A 50-year-old man without neurofibromatosis presented with a 10-year history of a slowly growing, occasionally painful mass in the medial aspect of the right ankle. Physical examination revealed a 5-cm, elastic-soft, poorly mobile, non-tender mass. Magnetic resonance imaging (MRI) showed multiple nodular les

BACKGROUND: Local ultrasound (US)-guided injections of anesthetics with corticosteroids are commonly performed for the conservative treatment of tarsal tunnel syndrome (TTS).
OBJECTIVE: This retrospective study aimed to investigate the outcomes of TTS after US-guided injections.
METHODS: The study included patients who were diagnosed with TTS and received US-guided injections as part of their initial treatment. The pain levels were noted on a scale between zero and ten before and after each injection. The patients were divided into non-surgical and surgical groups. The nonsurgical group incl

Caused by age or trauma, collapsed connective tissue can cause nerve entrapment and damage within the tarsal tunnel. Tarsal tunnel syndrome is relatively underdiagnosed. This study presents an intervention targeting damaged tissues surrounding the nerves and replacing the structural cushioning with a Wharton's jelly tissue allograft. The eight patients in our study, selected from four clinical sites, had tarsal tunnel-related defects. Patient outcomes were tracked on a 90-day calendar utilizing the Numeric Pain Rating Scale (NPRS) and the Western Ontario and McMaster University Arthritis Index

BACKGROUND: Talus bipartitus is a rare skeletal anomaly that may be mistaken for an os trigonum, a relatively common accessory ossicle found posterior to the talus. Both are believed to be formed similarly with the failed fusion of the talus with a second ossification center. Os trigonum is often diagnosed incidentally or found on radiographs obtained in symptomatic patients with signs of os trigonum syndrome, which presents with posterolateral ankle pain. A bipartite talus may also present with similar symptoms but may be differentiated through its articular involvement.

PURPOSE: Tarsal tunnel syndrome is well documented following lateralizing calcaneal osteotomy to manage varus hindfoot deformity. Traditionally, calcaneal osteotomy is performed with an oscillating saw. No studies have investigated the effect of alternative surgical techniques on postoperative tarsal tunnel pressure. The purpose of this study was to investigate the difference in tarsal tunnel pressures following lateralizing calcaneal osteotomy performed using a high-torque, low-speed "minimally invasive surgery" (MIS) Shannon burr versus an oscillating saw.
METHODS: Lateralizing calcaneal os

BACKGROUND: Tarsal tunnel syndrome (TTS) is a common entrapment neuropathy that is sometimes elicited by ganglia in the tarsal tunnel.
METHODS: Between August 2020 and July 2022, we operated on 117 sides with TTS. This retrospective study examined data from 8 consecutive patients (8 sides: 5 men, 3 women; average age 67.8 years) with an extraneural ganglion in the tarsal tunnel. We investigated the clinical characteristics and surgical outcomes for these patients.
RESULTS: The mass was palpable through the skin in 1 patient, detected intraoperatively in 1 patient, and visualized on MRI scann

BACKGROUND: The sensitivity of diagnosing tarsal tunnel syndrome with an electrodiagnostic study is just over 50%. Given this low reliability, many surgeons prefer to make a diagnosis solely from a physical examination, despite reported electrodiagnostic findings. Thus, to understand the clinical ramifications between these 2 methods of diagnosis, this investigation compared the postoperative outcomes following a tarsal tunnel release between patients with positive and negative preoperative electrodiagnosis (EDx).
METHODS: This study retrospectively reviewed 53 consecutive patients who underw

INTRODUCTION: Posterior tibial nerve (PTN) cross-sectional area (CSA) reference values for the diagnosis of tarsal tunnel syndrome (TTS) using ultrasound imaging exist in several countries but not in South Africa (SA). Therefore, the objective was to measure the CSA reference values for PTN in SA.
METHODS: Ultrasound CSA measurements of PTN in both ankles on 112 participants were performed, the mean measurement was recorded, and the effect of race, age, gender, and body mass index (BMI) were recorded.
RESULTS: In this study, the primary variables age and BMI affect the CSA measurement of the

BACKGROUND: Children with Hunter syndrome have a high prevalence of nerve compression syndromes given the buildup of glycosaminoglycans in the tendon sheaths and soft tissue structures. These are often comorbid with orthopedic conditions given joint and tendon contractures due to the same pathology. While carpal tunnel syndrome and surgical treatment has been well-reported in this population, the literature on lower extremity nerve compression syndromes and their treatment in Hunter syndrome is sparse.
OBSERVATIONS: We report the case of a 13-year-old male with a history of Hunter syndrome wh

The neuropathic compression of the tibial nerve and/or its branches on the medial side of the ankle is called tarsal tunnel syndrome (TTS). Patients with TTS presents pain, paresthesia, hypoesthesia, hyperesthesia, muscle cramps or numbness which affects the sole of the foot, the heel, or both. The clinical diagnosis is challenging because of the fairly non-specific and several symptomatology. We demonstrate a case of TTS caused by medial dislocation of the talar bone on the calcaneus bone impacting the tibial nerve diagnosed only by ultrasound with the patient in the standing position.

BACKGROUND: Tarsal tunnel syndrome (TTS) is a condition in which the tibial nerve (TN) (or its terminal branches) is compressed by the flexor retinaculum (FR) and the deep fascia of the abductor hallucis muscle at the tarsal tunnel, causing symptoms that negatively impact the patient's quality of life, including numbness, a sensation of a foreign object, coldness, and pain. FR release via microtrauma using needle-knife has proven to be effective in China and is widely used by clinicians. The traditional acupotomy, however, is the "blind knife" treatment, which cannot guarantee patient safety d
