Extradigital Glomus Tumor of the Ankle Associated With Tibial Neuropathy: A Case Report.

We report the case of a 26-year-old man with a 14-year history of severe, highly localized pain and allodynia in the right medial retromalleolar region. He had undergone multiple previous medical evaluations and had repeatedly been treated for presumed tendinopathy without meaningful improvement. Magnetic resonance imaging (MRI) of the right ankle demonstrated multiple soft-tissue lesions involving superficial and deep tissue planes around the ankle and hindfoot. Electroneuromyography revealed severe focal axonal neuropathy of the right tibial nerve and its plantar branches. Surgical explorati

The Role of Electrophysiological Workup in Lumbar Spinal Canal Stenosis (LSCS): A Systematic Review.

STUDY DESIGN: Systematic literature review. OBJECTIVE: Update on diagnostic utility of electrophysiology in lumbar spinal canal stenosis (LSCS). SUMMARY OF BACKGROUND: LSCS is a highly prevalent degenerative spine condition characterized by neurogenic claudication, radicular pain, and muscle weakness. While lumbar spine MRI is the imaging modality for detecting spinal canal narrowing, it correlates poorly with clinical symptoms. Electrophysiological methods, including electromyography (EMG), nerve conduction studies (NCS), and evoked potentials (MEP and SEP), may provide complementary inform

Peripheral Neuropathy Expands the Neurological Phenotype in Glutaric Aciduria Type 1.

Glutaric aciduria type 1 (GA1) is a neurometabolic disorder characterized by striatal injury in infancy and extrastriatal central nervous system abnormalities, the latter depending on the biochemical subtype. Whether the peripheral nervous system (PNS) is also affected has not been systematically studied. Therefore, we conducted a cross-sectional study of 21 GA1 patients (15 high excretor [HE], 6 low excretor [LE]), identified either by newborn screening (NBS, n = 11) or targeted metabolic diagnostics (TMD, n = 10). All underwent clinical evaluation, cerebral MRI, neurophysiology, and MR-neuro

Ultrasound Depiction of the Optimal Window for Needle Placement for Electromyography of the Short Head of the Biceps Femoris.

INTRODUCTION/AIMS: Accurate needle electromyography (EMG) of the short head of the biceps femoris (SHBF) is often important to localize nerve damage when proximal to the fibular head. Techniques have been proposed for accurate SHBF EMG; however, needle placement accuracy is low and with the risk of inadvertent needle placement into the long head (LHBF) potentially resulting in false localization and inappropriate operative management. This study aimed to define the optimal window for accurate SHBF needle EMG placement through ultrasound (US) investigation of the anatomical relationships of the

MR neurography of tibial and common peroneal nerves in patients with Guillain-Barre syndrome and electrophysiological correlation.

INTRODUCTION: The early and accurate assessment of peripheral nerve injury in patients with Guillain–Barré syndrome (GBS) is crucial for clinical diagnosis and treatment. Recently, there has been growing interest in using MR neurography especially diffusion-weighted imaging with background suppression (DWIBS) to evaluate peripheral nerves and associated lesions. However, this technique has not yet been applied to GBS patients. The aim of this study was to investigate the potential value of MR neurography in assessing the tibial nerve (TN) and common peroneal nerve (CPN) in individuals with GBS

MR Neurography in Children and Adolescents: Multiparametric Assessment of Peripheral Nerve Involvement in Long-chain Fatty Acid Oxidation Disorders.

OBJECTIVES: MR neurography (MRN) is a modern technique for visualizing peripheral nerves and quantifying microstructural pathology, yet its use in pediatric populations remains largely unexplored. This study evaluates the applicability and diagnostic performance of MRN in children and adolescents with genetically confirmed long-chain 3-hydroxyacyl-CoA dehydrogenase deficiency (LCHADD) and mitochondrial trifunctional protein deficiency (MTPD), in which peripheral neuropathy is a known long-term complication. MATERIALS AND METHODS: In a prospective cross-sectional study, 15 patients (LCHADD n =

Histological and Immunohistochemical Characterization of the Tibial ACL Remnant: Implications for Ligament Healing.

Background and Objectives: The pathways mediating the beneficial effect of tibial stump preservation for anterior cruciate ligament (ACL) reconstruction remain insufficiently clarified. This study investigated key vascular, neural, and stromal aspects of cellular remodeling processes occurring across lesion stages in tibial remnant pre-reconstruction. Materials and Methods: Biopsies were obtained from 25 patients undergoing arthroscopic ACL reconstruction (paired free-end and tibial insertion sampling) and 10 from quasi-normal, macroscopically intact ligaments (controls). We evaluated intergro

Pearls and Pitfalls for the Direct Posterior Approach to the Proximal Tibia and Knee Joint: A Clinical Experience From a Tertiary Center in Central India.

Background The posterior approach to the proximal tibia provides direct access to posterior knee and tibial pathologies, allowing controlled identification and protection of neurovascular structures within the popliteal fossa. Despite these advantages, concerns regarding prone positioning, potential neurovascular injury, wound complications, and limited surgeon familiarity have restricted its widespread use. This study aimed to evaluate indications, perioperative complications, and functional outcomes of the posterior approach to the proximal tibia, highlighting practical pearls and pitfalls.

Sports-Related Posterior Ankle Pain: Diagnosis and Management of Lateral Calcaneal Branch Neuropathy of the Sural Nerve (A Case Series).

BACKGROUND: Posterior ankle pain has varied etiologies, with sural nerve (SN) entrapment contributing to posterolateral ankle discomfort. The SN is a pure sensory nerve that innervates the lateral ankle and foot up to the fifth metatarsal. Although SN pathologies are known, specific clinical features and management of neuropathy affecting the lateral calcaneal branch of the sural nerve (LCBSN), which supplies the lateral heel, are less defined. This condition is often exacerbated by repetitive ankle dorsiflexion in sports or external compression from tight footwear. We hypothesized that LCBSN

Long-term outcomes of selective tibial neurotomy for spastic foot deformity: Initial clinical experience in Vietnam.

INTRODUCTION: Spastic foot deformity after central nervous system lesions is clinically heterogeneous and often combines equinus, varus, claw toes, pain, and gait limitation. Selective tibial neurotomy (STN) is a peripheral neurosurgical option for focal lower-limb spasticity, but data from low- and middle-income settings remain limited. RESEARCH QUESTION: What are the long-term clinical and functional outcomes of STN for spastic foot deformity, and are these outcomes influenced by the extent of motor fascicle resection? MATERIAL AND METHODS: We conducted a retrospective study of 31 patients

Autologous Osteochondral Grafting From an Ipsilateral Ankle Osteophyte for Chronic and Large Osteochondral Lesion of the Talus.

Large osteochondral lesions of the talus (OLTs) in the chronic phase remain challenging to manage due to degeneration of the osteochondral fragment and underlying subchondral bone defects, which often make conventional fixation techniques difficult to perform. Although various surgical approaches-including autologous osteochondral transplantation, chondrocyte implantation, and structural allografting-have been employed, each carries limitations such as the requirement for an additional donor-site harvest, the need for staged procedures, or limited graft availability. We report the case of a 19

A novel microvascular arterio-neural pathway for intraneural ganglion cyst formation secondary to cystic adventitial disease: illustrative case.

BACKGROUND: Cystic adventitial disease (CAD) and intraneural ganglion cysts (IGCs) are rare mucinous lesions typically linked to an articular origin via macroscopic conduits. While secondary propagation between vascular and neural systems may occur through shared fascial sheaths, a direct microvascular route has not been previously described. OBSERVATIONS: A 60-year-old man presented with progressive left tibial neuropathy. High-resolution 7T MRI and surgery revealed a large popliteal CAD lesion connecting to the knee joint via the middle genicular artery, alongside a secondary tibial IGC lac

Surgical Neurolysis for Atypical Neuropathy Diagnosed via Physical Examination Significantly Improves Chronic Pain Despite Negative Workup.

BACKGROUND: Many patients experience chronic pain, especially after trauma or surgery, despite a negative workup. We hypothesize that underdiagnosed peripheral neuropathy may be contributing to their intractable symptoms. METHODS: This is a retrospective cohort of patients who underwent surgical neurolysis for chronic pain that was neuropathic in nature, in the distribution of a peripheral nerve, with a positive scratch collapse test. Before surgery, patients were administered a corticosteroid injection at the lesion. The principal outcome was a self-reported visual analog scale pain score. M

Surgical treatment of schwannomas around the tarsal tunnel: a case series of 40 patients and systematic review of the literature.

OBJECTIVE: Schwannomas in the tibial, plantar, or medial calcaneal nerves around the ankle can mimic symptoms of tarsal tunnel syndrome. Outcomes after resection of schwannomas from these nerves have mainly been reported in case reports. The objectives of this study were to investigate the presentation and results for resection of schwannomas around the tarsal tunnel in a large case series and systematically review the currently available literature. METHODS: Data from 40 patients (27 sporadic and 13 schwannomatosis cases) treated at 2 centers were retrospectively analyzed for preoperative ty

Refining Knee Radiofrequency Ablation with a Palisade- Guided Technique: A Case Report.

BACKGROUND: Knee osteoarthritis is a leading cause of chronic pain and disability, and many patients have persistent pain despite conservative treatment or are not candidates for arthroplasty. Contemporary anatomical studies show that the classical three-nerve genicular radiofrequency ablation (RFA) model does not adequately reflect the multibranch innervation of the anterior knee capsule, which may limit efficacy. Structured preprocedural planning may help standardize cannula placement, however, its clinical and anatomical validity requires further investigation. CASE PRESENTATION: We report

Postoperative Sciatic Neuropathy Following Spinal Anesthesia in a Patient With Severe Lumbar Degenerative Disease: A Case Report.

Neurologic complications after spinal anesthesia are uncommon but remain a major concern because of their potential severity. Distinguishing anesthesia-related neurologic injury from preexisting spinal pathology is often challenging. A 70-year-old man with hypertension, hyperlipidemia, and known lumbar degenerative disease underwent elective bilateral inguinal hernia repair under spinal anesthesia. Spinal anesthesia was performed via a paramedian approach at the L3-4 interspace using a 25-gauge Quincke needle and 2.6 mL (13 mg) of 0.5% hyperbaric bupivacaine without paresthesia or technical di

Beyond the Operating Room: Ultrasound-Guided Tibial Nerve Block for Plantar Wound Care in a Desert Ultramarathon.

Severe plantar lesions are common during multistage desert ultramarathons and may require debridement and sequential dressing in austere field clinics. These procedures can be intensely painful because blistering, maceration, epidermal loss and sand contamination expose highly sensitive plantar tissue. We describe 3 runners treated during the Marathon des Sables who required advanced plantar wound care and received ultrasound-guided tibial nerve block for procedural analgesia. In each case, an isolated tibial nerve block was performed at the medial ankle using 8 mL of 1% lidocaine without epin
