Tarsal tunnel syndrome is an entrapment neuropathy at the tarsal tunnel. The diagnosis and the prediction of the surgical outcome are difficult. We compared preoperative magnetic resonance imaging findings with the postoperative results. We examined preoperative magnetic resonance imaging findings in 38 consecutive patients with Tarsal tunnel syndrome (47 feet); their mean age was 73.8 years. We inspected the nerve width on the slice showing the most compressed nerve, and the hyperintensity of that nerve on preoperative T2* fat-suppressed axial magnetic resonance imaging images and examined th

BACKGROUND: The accessory soleus muscle (ASM) is a rare anatomical variant and the most common accessory muscle in the leg/ankle region. Though asymptomatic in most cases, ASM could exhibit clinical manifestations leading to medical consultation. Since no evidence-based synthesis has been conducted yet, this review aims to collate pooled quantitative data on the clinical presentation and management of ASM.
METHODS: The electronic databases of Medline and Embase were searched from inception, met the inclusion criteria. Only clinical cases of symptomatic ASM were included. Clinical and cadaveri

Background: Clinical procedures involving the tibial nerve (TN) are complex procedures due to its deep anatomical position and the variability of its course in the retromalleolar region. Few studies have described the ultrasound characteristics of the TN in vivo. This study aims to describe the ultrasound position of the TN and its relationship with the posterior tibial artery (PTA) at the retromalleolar level, evaluating the influence of sex, weight, height, and body mass index (BMI). Methods: A cross-sectional ultrasound study was performed on 100 volunteers. Anthropometric variables were re

Tarsal tunnel syndrome is an entrapment neuropathy of the tibial nerve and its branches in the tarsal tunnel. The literature on surgical release of the tarsal tunnel shows variable outcomes with no studies reporting validated patient reported outcomes. We aim to determine clinical response after tarsal tunnel release using the Patient-Reported Outcomes Measurement Information System (PROMIS). CPT code 28035 was used to identify patients who underwent isolated tarsal tunnel release (TTR) between 1/1/2015 and 12/15/2022 at a single institution. Patient demographic data and PROMIS physical functi

While the pathophysiology of space-occupying lesions inside the tarsal tunnel causing tarsal tunnel syndrome (TTS) is obvious, the occurrence of a posterior tibial nerve (PTN) mass outside the tarsal tunnel but mimicking or in combination with TTS symptomatology is less clear. Therefore, we report three rare cases of patients presenting with TTS symptoms combined with a distal leg PTN schwannoma, all of whom were treated with tumor resection and tarsal tunnel decompression. All patients had a long-lasting history of leg and ankle pain radiating to the medial aspect of the foot and toes. Pain w

Tarsal tunnel syndrome (TTS) is elicited by compression of the tibial nerve in the tarsal tunnel. TTS in the absence of a lesion tends to be idiopathic, and most TTS is idiopathic. Patients complain of several plantar symptoms, and TTS affects their quality of life. The symptoms tend to worsen with walking, and ankle joint movement and arterial distortion may also be involved. Because TTS symptoms are similar to those of diabetic neuropathy and lumbar disease, clinical symptoms are diagnostically important. While magnetic resonance imaging reveals nerve compression, it is difficult to identify

INTRODUCTION: Double crush syndrome (DCS) refers to compressive neuropathy at multiple sites along a peripheral nerve (PN), yet its relevance in the lower extremity remains poorly defined. This study aimed to (1) determine the prevalence of PN lesions in patients undergoing surgery for lumbosacral radiculopathy (LR), (2) identify commonly affected nerves, (3) assess associated risk factors, and (4) evaluate the DCS hypothesis by comparing the incidence of PN lesions in patients undergoing surgery for LR versus matched controls.
METHODS: A retrospective cohort study was conducted using the Pea

Entrapment neuropathies of the lower extremity are often underdiagnosed due to limitations in clinical examination and electrophysiological testing. Advanced imaging techniques, particularly MR neurography and high-resolution ultrasonography (US), have significantly improved the evaluation and diagnosis of these conditions by enabling precise visualization of nerves and their surrounding anatomical structures. This review focuses on the imaging features of compressive neuropathies affecting the lumbosacral plexus and its branches, including the femoral, obturator, sciatic, common peroneal, and

INTRODUCTION: Talocalcaneal coalition (TCC) is one of the most common tarsal coalitions in children. However, current classifications of TCC are limited and focus on histologic or morphologic features without clinical correlation or therapeutic guidance. Moreover, no classification system specifically designed for pediatric populations. We hypothesize that the novel classification system will be clinically relevant, reliable, and easy to use for pediatric TCC management.
METHODS: A total of 43 patients (58 feet) with pediatric TCC confirmed by 3-dimensional CT scans were included in this stud

BACKGROUND: Local ultrasound (US)-guided injections of anesthetics with corticosteroids are commonly performed for the conservative treatment of tarsal tunnel syndrome (TTS).
OBJECTIVE: This retrospective study aimed to investigate the outcomes of TTS after US-guided injections.
METHODS: The study included patients who were diagnosed with TTS and received US-guided injections as part of their initial treatment. The pain levels were noted on a scale between zero and ten before and after each injection. The patients were divided into non-surgical and surgical groups. The nonsurgical group incl

Caused by age or trauma, collapsed connective tissue can cause nerve entrapment and damage within the tarsal tunnel. Tarsal tunnel syndrome is relatively underdiagnosed. This study presents an intervention targeting damaged tissues surrounding the nerves and replacing the structural cushioning with a Wharton's jelly tissue allograft. The eight patients in our study, selected from four clinical sites, had tarsal tunnel-related defects. Patient outcomes were tracked on a 90-day calendar utilizing the Numeric Pain Rating Scale (NPRS) and the Western Ontario and McMaster University Arthritis Index

BACKGROUND: Talus bipartitus is a rare skeletal anomaly that may be mistaken for an os trigonum, a relatively common accessory ossicle found posterior to the talus. Both are believed to be formed similarly with the failed fusion of the talus with a second ossification center. Os trigonum is often diagnosed incidentally or found on radiographs obtained in symptomatic patients with signs of os trigonum syndrome, which presents with posterolateral ankle pain. A bipartite talus may also present with similar symptoms but may be differentiated through its articular involvement.

PURPOSE: Tarsal tunnel syndrome is well documented following lateralizing calcaneal osteotomy to manage varus hindfoot deformity. Traditionally, calcaneal osteotomy is performed with an oscillating saw. No studies have investigated the effect of alternative surgical techniques on postoperative tarsal tunnel pressure. The purpose of this study was to investigate the difference in tarsal tunnel pressures following lateralizing calcaneal osteotomy performed using a high-torque, low-speed "minimally invasive surgery" (MIS) Shannon burr versus an oscillating saw.
METHODS: Lateralizing calcaneal os

BACKGROUND: Tarsal tunnel syndrome (TTS) is a common entrapment neuropathy that is sometimes elicited by ganglia in the tarsal tunnel.
METHODS: Between August 2020 and July 2022, we operated on 117 sides with TTS. This retrospective study examined data from 8 consecutive patients (8 sides: 5 men, 3 women; average age 67.8 years) with an extraneural ganglion in the tarsal tunnel. We investigated the clinical characteristics and surgical outcomes for these patients.
RESULTS: The mass was palpable through the skin in 1 patient, detected intraoperatively in 1 patient, and visualized on MRI scann

BACKGROUND: The sensitivity of diagnosing tarsal tunnel syndrome with an electrodiagnostic study is just over 50%. Given this low reliability, many surgeons prefer to make a diagnosis solely from a physical examination, despite reported electrodiagnostic findings. Thus, to understand the clinical ramifications between these 2 methods of diagnosis, this investigation compared the postoperative outcomes following a tarsal tunnel release between patients with positive and negative preoperative electrodiagnosis (EDx).
METHODS: This study retrospectively reviewed 53 consecutive patients who underw

INTRODUCTION: Posterior tibial nerve (PTN) cross-sectional area (CSA) reference values for the diagnosis of tarsal tunnel syndrome (TTS) using ultrasound imaging exist in several countries but not in South Africa (SA). Therefore, the objective was to measure the CSA reference values for PTN in SA.
METHODS: Ultrasound CSA measurements of PTN in both ankles on 112 participants were performed, the mean measurement was recorded, and the effect of race, age, gender, and body mass index (BMI) were recorded.
RESULTS: In this study, the primary variables age and BMI affect the CSA measurement of the

BACKGROUND: Children with Hunter syndrome have a high prevalence of nerve compression syndromes given the buildup of glycosaminoglycans in the tendon sheaths and soft tissue structures. These are often comorbid with orthopedic conditions given joint and tendon contractures due to the same pathology. While carpal tunnel syndrome and surgical treatment has been well-reported in this population, the literature on lower extremity nerve compression syndromes and their treatment in Hunter syndrome is sparse.
OBSERVATIONS: We report the case of a 13-year-old male with a history of Hunter syndrome wh
