Extradigital Glomus Tumor of the Ankle Associated With Tibial Neuropathy: A Case Report.

We report the case of a 26-year-old man with a 14-year history of severe, highly localized pain and allodynia in the right medial retromalleolar region. He had undergone multiple previous medical evaluations and had repeatedly been treated for presumed tendinopathy without meaningful improvement. Magnetic resonance imaging (MRI) of the right ankle demonstrated multiple soft-tissue lesions involving superficial and deep tissue planes around the ankle and hindfoot. Electroneuromyography revealed severe focal axonal neuropathy of the right tibial nerve and its plantar branches. Surgical explorati

Comparative study of photobiomodulation therapy and low-intensity pulsed ultrasound for nerve regeneration and pain alleviation in an acute spinal cord injury model.

Spinal cord injury (SCI) remains a significant medical challenge due to the complexity of its pathology. This study evaluated the effects of photobiomodulation therapy (PBMT) and low-intensity pulsed ultrasound (LIPUS) as non-invasive treatments for SCI. Thirty-six male Wistar rats were randomly assigned to 6 groups: control, SCI, PBMT, and LIPUS at 0.1, 0.3, and 0.5 W/cm2 intensities. SCI was induced using an aneurysm clip, and treatments began 30 minutes post-injury, continuing for 4 weeks. During the seventh week, no treatment was administered. Behavioral assessments, including the acetone

A Rare and Interesting Case of Sciatic Nerve Compression Due to Proximal Femoral Osteochondroma in a Young Adult - A Case Report.

INTRODUCTION: Osteochondromas (OCs) are benign hamartomatous lesions composed of cartilage and bone, arising from aberrant growth-plate cartilage. They characteristically project from the bone surface through the cortex and grow by endochondral ossification beneath the periosteum. Lesion growth usually parallels skeletal growth and typically ceases after physeal closure, although minimal growth of the cartilage cap may persist and generally stops by the third decade of life. This developmental mechanism explains the predilection for involvement of the distal femur, proximal tibia, and proximal

Differential awakening of the sciatic nerve: illustrative case.

BACKGROUND: Differential awakening is a well-known, yet poorly understood phenomenon seen in patients with intracranial pathologies. Because acute postoperative neurological decline has many potentially reversible causes, distinguishing differential awakening from other etiologies is critical. To the authors' knowledge, differential awakening has not been reported following peripheral nerve procedures. OBSERVATIONS: A 40-year-old man presented with a 1.5-year history of painful bilateral cervical and right lumbosacral radiculoplexus neuropathy recently associated with bilateral facial and ves

Shear Wave Elastography in Musculoskeletal Imaging: A Narrative Review.

Shear wave elastography (SWE) is an increasingly investigated ultrasound-based technique in musculoskeletal imaging that provides quantitative information on tissue stiffness and biomechanical properties. This narrative review aims to summarize the basic principles, technical considerations, current clinical applications, limitations, and future perspectives of SWE in musculoskeletal imaging. Unlike conventional grayscale and Doppler ultrasonography, which mainly assess morphology and vascularity, SWE may provide additional functional information in major musculoskeletal tissues, including ten

Lipomatosis of the Median Nerve: A Case Report.

Lipomatosis of nerve is a rare, nonhereditary congenital overgrowth disorder involving fibroadipose tissue proliferation along nerve territories, most commonly the median and plantar nerves. We report a case of a 35-year-old man presenting with slowly progressive enlargement of the thumb and index finger since childhood, associated with increasing discomfort. Imaging demonstrated nonencapsulated fatty overgrowth with digital nerve involvement and associated bony hypertrophy. Surgical management included right thumb debulking and distal phalanx amputation of the index finger. Histopathologic ex

Surgical treatment of schwannomas around the tarsal tunnel: a case series of 40 patients and systematic review of the literature.

OBJECTIVE: Schwannomas in the tibial, plantar, or medial calcaneal nerves around the ankle can mimic symptoms of tarsal tunnel syndrome. Outcomes after resection of schwannomas from these nerves have mainly been reported in case reports. The objectives of this study were to investigate the presentation and results for resection of schwannomas around the tarsal tunnel in a large case series and systematically review the currently available literature. METHODS: Data from 40 patients (27 sporadic and 13 schwannomatosis cases) treated at 2 centers were retrospectively analyzed for preoperative ty

Beyond the Operating Room: Ultrasound-Guided Tibial Nerve Block for Plantar Wound Care in a Desert Ultramarathon.

Severe plantar lesions are common during multistage desert ultramarathons and may require debridement and sequential dressing in austere field clinics. These procedures can be intensely painful because blistering, maceration, epidermal loss and sand contamination expose highly sensitive plantar tissue. We describe 3 runners treated during the Marathon des Sables who required advanced plantar wound care and received ultrasound-guided tibial nerve block for procedural analgesia. In each case, an isolated tibial nerve block was performed at the medial ankle using 8 mL of 1% lidocaine without epin

Pediatric Post-COVID-19 Neuromyelitis Optica Spectrum Disorder: A Case Report.

Neuromyelitis optica spectrum disorder (NMOSD) is a rare autoimmune astrocytopathy affecting the central nervous system, particularly the optic nerves and spinal cord. Post-infectious triggers, including SARS-CoV-2 (COVID-19), have been increasingly recognized as potential catalysts for this immune-mediated condition. We report the case of a nine-year-old boy who developed AQP4-IgG-positive NMOSD following confirmed SARS-CoV-2 infection. The patient's clinical onset began 1.5 months prior to admission with a seven-day episode of intractable vomiting, indicative of area postrema syndrome (APS).

Clinical Resolution of Plantar Warts Using the Needling Technique.

BACKGROUND: Plantar warts, caused by the human papillomavirus (HPV), are a common skin condition characterized by painful lesions on the soles of the feet. These lesions can significantly impact skin appearance, quality of life, and, in severe cases, mobility. Traditional treatment methods, such as chemical cauterization or pharmaceutical therapies, are often painful and require multiple visits to achieve complete wart removal and skin regeneration. This study aims to assess the clinical effectiveness of the needling technique as an alternative treatment. This method involves repeatedly punctu

Heel reconstruction using a combination of free sensate medial plantar flap and skin graft: A case series.

Managing soft tissue defects in the heel presents a significant challenge for surgeons due to the heel's crucial role in weight-bearing. A free instep flap with end-to-side nerve coaptation offers an effective solution for reconstructing large heel defects. Four patients were referred to our hospital with soft tissue lesions in the weight-bearing heel and instep area. Their heels were reconstructed using a free instep flap harvested from the opposite foot. In three cases, an end-to-side arterial anastomosis was performed. In the fourth case, due to a midfoot amputation, an end-to-end arterial

The role of radiofrequency therapy in the treatment of Morton's neuroma: A case report.

INTRODUCTION AND IMPORTANCE: Morton's Neuroma is a neuropathy of the forefoot, specifically located on the interdigital nerve, and is associated with the thickening of the intermetatarsal ligament and nerve fibrotic. The management of Morton's Neuroma is challenging. Various treatments have been introduced for treating this lesion, starting from conservative treatment using the metatarsal pad, physical therapy, and corticosteroid injection to surgical treatment. This report highlights a novel approach to Morton's Neuroma using a pulsed radiofrequency (PRF). CASE PRESENTATION: A 53-year-old fe

Tarsal Tunnel Syndrome: A Clinical Review.

Tarsal tunnel syndrome (TTS) is elicited by compression of the tibial nerve in the tarsal tunnel. TTS in the absence of a lesion tends to be idiopathic, and most TTS is idiopathic. Patients complain of several plantar symptoms, and TTS affects their quality of life. The symptoms tend to worsen with walking, and ankle joint movement and arterial distortion may also be involved. Because TTS symptoms are similar to those of diabetic neuropathy and lumbar disease, clinical symptoms are diagnostically important. While magnetic resonance imaging reveals nerve compression, it is difficult to identify

Topical frankincense treatment on relieving high-risk diabetic foot in rats by reducing inflammation and improving microcirculation.

BACKGROUND: Frankincense, an ancient aromatic substance known for promoting blood circulation, is widely used in traditional Chinese medicine to treat skin lesions. METHODS: This study aimed to evaluate the efficacy and underlying mechanisms of frankincense oil extract (FOE) in addressing high-risk diabetic foot by improving microvascular circulatory disorders. The vascular repair effects of FOE were assessed in a streptozotocin-induced high-risk diabetic foot animal model. Various evaluations were performed, including infrared thermometry, motor nerve conduction velocity (MNCV) testing, lase

Missed for Years, Cured in Hours: Surgical Excision of a Popliteal Schwannoma With Intraoperative Neuromonitoring.

Popliteal schwannomas are rare benign tumors arising from the peripheral nerve sheaths, often misdiagnosed due to their nonspecific symptoms. We report a case of a 36-year-old female with a 10-year history of refractory left leg pain localized to the S1 dermatome, characterized by persistent burning and aching sensations along the plantar foot, heel, and posterior calf. Despite multiple consultations with various specialists over the years, her symptoms were repeatedly misattributed to lumbar spine pathology, resulting in delayed diagnosis. A detailed clinical evaluation eventually revealed a

Double Crush Syndrome in Surgically-Treated Lumbosacral Radiculopathy: Prevalence, Risk Factors, and Clinical Implications.

INTRODUCTION: Double crush syndrome (DCS) refers to compressive neuropathy at multiple sites along a peripheral nerve (PN), yet its relevance in the lower extremity remains poorly defined. This study aimed to (1) determine the prevalence of PN lesions in patients undergoing surgery for lumbosacral radiculopathy (LR), (2) identify commonly affected nerves, (3) assess associated risk factors, and (4) evaluate the DCS hypothesis by comparing the incidence of PN lesions in patients undergoing surgery for LR versus matched controls. METHODS: A retrospective cohort study was conducted using the Pea

Lessons in clinical reasoning - pitfalls, myths, and pearls: a case of tarsal tunnel syndrome caused by an intraneural ganglion cyst.

OBJECTIVES: Intraneural ganglionic cysts are non-neoplastic cysts that can cause signs and symptoms of peripheral neuropathy. However, the scarcity of such cases can lead to cognitive biases. Early surgical exploration of space occupying lesions plays an important role in identification and improving the outcomes for intraneural ganglionic cysts. CASE PRESENTATION: This patient presented with loss of sensation on the right sole with tingling numbness for six months. A diagnosis of tarsal tunnel syndrome was made. Nerve conduction study revealed that the mixed nerve action potential (NAP) was

A Case of Isolated Central Nervous System Rosai-Dorfman Disease.

Rosai-Dorfman disease (RDD) is a benign histiocytosis with unknown etiology. It generally occurs in cervical lymph nodes. Isolated central nervous system (CNS) RDD is very rare in the literature. We reported a case of no systemic involvement Rosai-Dorfmann which is rarely seen and shows CNS involvement by mimicking meningioma. A 32-year-old man presented with diplopia and a headache he has been experiencing for the past two years. His neurological examination showed left facial paresthesia, consistent with trigeminal nerve trace. Tendon reflexes were increased at the right side and the right p

Discovery of a Novel Yadav-Kunal Reflex in Context of Upper Motor Neuron Lesions: A Report of Three Cases.

INTRODUCTION: Numerous reflexive responses have been documented as alterations to the Babinski sign within upper motor neuron lesions. However, scant attention has been given to reflexes beyond these, which exhibit independence from the extensor plantar response. These reflexes predominantly form polysynaptic arcs, with nociceptive stimuli acting as afferents. CASE REPORT: The reflex was serendipitously discovered in an 18-year-old female patient who presented with spastic paraplegia with bowel and bladder involvement, as a consequence of an aneurysmal bone cyst of the D3 (dorsal) vertebrae,
