The pattern of motor axon regeneration following unilateral sciatic nerve lesions (freezing or transection) was studied in adult rats. Transected nerves were repaired with epineurial or fascicular sutures. Four months after the lesion, the motor neuron cell body localization in the spinal cord of plantar or common peroneal nerve axons were examined bilaterally with retrograde transport of horseradish peroxidase. Motor neuron cell body localization was similar bilaterally after freezing, indicating that regenerating axons had reached their original peripheral innervation territory. However, aft

Silk ligatures were tied round the sciatic nerve in guinea-pigs and left in place in order to produce persistent nerve constriction. Serial nerve conduction studies over the following 9 weeks showed a reduction in motor conduction velocity distal to the ligature. The presence of axonal atrophy in tibial nerve fibres in the leg was subsequently confirmed by histological studies. These changes were not seen in a second group of animals in which similar ligatures were tied but removed after 6 hours. When the ligatures were left in place, the animals developed local plantar nerve lesions in the so

Somatosensory evoked potentials (SEPs) following median nerve stimulation were abnormal in 7 patients with sensory impairment due to vitamin B12 deficiency. Extensor plantar reflexes indicated a central sensory pathway lesion in 4 cases and absent tendon jerks suggested peripheral neuropathy in 4, but median nerve SEPs indicated a predominantly central lesion without marked peripheral nerve involvement in 6 and an axonal neuropathy without CNS involvement in 1. The latter had evidence of central slowing of conduction in SEPs following posterior tibial nerve stimulation. Consequently, it is sug

In the glabrous skin of the rat's hindfoot the same triple set of low-threshold mechanoreceptors is present as has been found in other mammals: slowly adapting (SA), rapidly adapting (RA), and very rapidly adapting Pacinian corpuscle-like (PC) receptors. Their functional characteristics were examined in normal rats and compared with those of sensitive mechanoreceptors found in the glabrous skin of the foot 2-24 wk after crush of the plantar nerves, resulting in regeneration of the transected nerve fibers. After 2 wk of nerve regeneration, low-threshold RA and SA cutaneous mechanoreceptors reap

The anterograde transport of wheat germ agglutinin-horseradish peroxidase conjugate was used to study the normal distribution of sensory nerve axons in the plantar skin of the rat hindlimb and at various times after chronic sciatic nerve injury in adult and neonatal rats. In adults, thin saphenous nerve axons were found in a small area laterally to the normal saphenous nerve territory 2-24 weeks after sciatic nerve lesion. In neonatal rats, at 6 and 10 weeks after sciatic nerve injury thin saphenous nerve axons were found almost or all over the sole of the foot, respectively, and in all 5 toes

Defects of the skin of the heel need to be repaired by skin which is both thick and sensitive. A technique of isolation of an island flap from the non-weight-bearing part of the medial side of the sole is described. It is vascularised by the medial plantar artery and innervated by the medial plantar nerve. Lesions of the heel, the medial malleolus and the lower part of the tendo Achilles can be covered by a one-stage surgical procedure without microsurgical suture.

The present study deals with the effects of two lipidosis-inducing drugs (chlorphentermine and perhexiline) upon the ultrastructure of large nerve trunks (sciatic and plantar nerves) of adult rats. Subchronic oral administration of high doses of either drug led to comparatively mild lipidosis-like alterations in Schwann cells and in other cell types of both nerve trunks. In addition, plantar nerves, and more rarely sciatic nerves, showed some unspecific lesions such as myelin whorls and ovoids within the outer Schwann cell cytoplasm, intra-axonal accumulations of polymorphous material, and sin

The authors report the case of a 45-year-old man with perforant ulceration of the foot. The rapid evolution of the cutaneous and bone lesions (three years) necessitated a bilateral intermetacarpo-phalangeal amputation. After conventional therapy, a large necrotic zone remained on the sole of the left foot as well as hyperkeratosis of the right foot. Following Bourrel's protocol for the treatment of leprosy, the authors proposed a posterior tibial nerve neurography, which confirmed the compression of the nerve in the tarsal tunnel. In addition, histological examination of the nerve branches of

This study was performed to determine the electrophysiological status of the posterior tibial nerve and its muscular innervation in patients both with and without plantar ulcers without osteitis. In both cases, motor distal latencies remained normal, but the denervation of the plantar muscles was significantly more important in patients with ulcers. These results show that axonal degeneration is predominant in leprous neuritis affecting the tibial nerves. Neurolysis was performed in twelve patients in the retromalleolar part of the tibial nerve. All patients have recover from their ulcers, but

From January 1960 to March 1979 25 children with spinal cord injury were admitted to our hospital (10 newborns with birth injury to the spinal cord were excluded). Among 12 patients with complete thoracic lesions four remained permanently flaccid. These four cases who had sustained relatively minor trauma showed marked muscular atrophy of the lower limbs, areflexia, absence of anal and cremasteric reflexes, no response to plantar stimulation and no foot deformities. In contrast to children with spastic traumatic paraplegia, motor nerve conduction velocities and H-reflexes were not measurable i

H-reflex amplitudes were recorded after stimulation of the tibial nerve and different electrical stimuli in 18 normal persons and 26 patients showing pyramidal spasticity (8 spastic spinal paralysis, 6 spastic hemiparesis, 12 spinal lesions). A just subthreshold stimulus of the tibial nerve facilitated the H-reflex in spastic patients slightly after about 300 ms (up to 113%), following an early strong facilitation (10 ms) and a longer lasting depression (20-200ms). Similar postinhibitory facilitation was obtained in spastic patients after ipsilateral stimulation of the plantar surface and afte

Although nerve conduction slowing is a well-accepted abnormality in rats with acute experimental diabetes, reports of neuropathological changes in diabetic rat nerves have been inconsistent. To examine this further, we studied electrophysiological and morphological features of posterior tibial nerves and their distal branches from four-week streptozotocin-induced diabetic rats and matched controls. Diabetic rat posterior tibial motor conduction was slowed (mean +/- 1 SD, 34.8 +/- 3.1 m/sec; controls, 41.2 +/- 2.5 m/sec), and evoked muscle response amplitudes were only half of control values. U

Ligation of either the common and internal iliac or the internal and external iliac arteries produced ischaemic lesions of the sciatic nerve and its branches, as well as direct muscle damage, in 5 out of 6 rabbits. In one animal, ligation of the aorta and of the internal iliac artery on one side produced a similar mixture of nerve and muscle damage on the side of the double ligation. Ligation of the femoral artery alone in 3 animals failed to produce significant changes. In the 6 affected animals there was paralysis of the hind leg on the side of the iliac ligations, with loss of tendon reflex

A method for recording the medial plantar sensory nerve action potential at the ankle with surface electrodes is described. Normal values in 69 control subjects are given and compared with the sural sensory nerve action potential in the same limb in the same subjects. Clinical applications were studied in 33 patients. The procedure may be applied in the diagnosis of L4-5 nerve plexus or root lesions, lesions of the sciatic, posterior tibial, and medial plantar nerves, and is a more sensitive test than other sensory nerve action potentials in the diagnosis of peripheral neuropathy.

The central response of the medical group of muscles in the foot sole, as a reaction to a stimulation of the tibial nerve has an antidrome character in normals over 20-22 years of age. However, in children and adolescents, as well as in disturbance of the supra-segmental control in this sort of response is a prevalent reflectory (monosynaptical) component. A similar trait was found in patients with discogenic radiculitis in an isolated lesion of the L5 radicle. And on the contrary, a pathology of the S1 radicle not only did not change the antidrome character of the response, but prevented the

By the neurological, electromyographic, anatomopathological and bacterioscopic examinations of 100 pacients with leprosy the following conclusions have been drawn: There was a high incidence of reflex abnormalities. The proprioceptive reflexes were abnormal in 64.5% and the superficial reflexes in 70.8% of the cases. According to the topography of the segments examined, the deep reflexes showed the following abnormal values: 25.3% in the cephalic segment; 69.1% in the upper limbs and 74.5% in the lower limbs. Of the 2,289 reflexes investigated, abnormalities were found in 773 superficial refle

The dissected plantar nerves of 105 patients with clinical symptoms of Morton's disease were examined under light and electron microscopy. Of the 105 cases, 75 showed characteristic neuronal lesions; in the others different pathologic substrates responsible for the clinical symptoms could be demonstrated. The nervous lesions are characterized by: 1. Thickening of the walls of the endoneurial vessels produced by multiple layers of basement membranes. 2. Edema and sclerosis of the endoneurium. In the electron microscope the endoneurium was filled by deposits of fibrils with a tubular substructur

The authors have observed 15 cases of the tarsal canal syndrome: the 14 cases operated on showed compression of the nerve by bone fragments resulting from trauma by local varices, by a muscular anomaly, or as a result of enclosure by post-trauma fibrosis. Surgery resulted in 10 cures and considerable improvement in 2 other cases. In addition to these 15 "pure" cases the authors report their experience of neurolysis of the posterior tibial nerve and of the plantar nerves of the tarsal canal in the treatment of perforating plantar lesions in leprosy (88 cases) and diabetes (12 cases). In 50 patients with lumbar nucleus pulposus prolapse the force of femoral abductors was determined by the test of Lovett and by means of dynamometric measurements. In 90% of patients paresis of these muscles was found. A comparison of the incidence of other signs of nerve root damage such as paresis of plantar and dorsal flexion of the foot and toes, sensory impairment on the foot and lack or weakness of ankle jerk showed that paresis of fermoral abductors was most frequent among them. This fact is explained by a particular predisposition for development of uncleus pulposus prolapse at the lev
