[Intramuscular myxoma. Apropos of two cases].

The authors report two cases of rare localization of intra muscular myxoma: one in the buttock, and the other in the lumbar area. The first case, a 58 years old woman had a four month history of a mass in the buttock. Computed tomography revealed the presence of a solid tumor. Diagnosis was based on histological examination. After surgical treatment no recurrence was observed after fourteen months. The second case was a 54 years old woman who presented a sciatica with back pain and neurological signs. The symptoms didn't response to medical treatment. Computed tomography showed an enlarged and

[Clinico-pathogenetic variants of the piriform muscle syndrome].

The method of diagnosis of pathogenetic variations or musculus piriformis syndrome (MPS) by means of computer tomography is described. 24 patients with early signs of lumbar osteochondrosis and MPS were examined. Two variations of MPS with specific clinical and tomographic features were revealed, namely: an early spastic (functional) type and degenerative-dystrophic one. The peculiarities of muscles (diameter, location, density) and of adjacent pelvic structures were investigated in 15 healthy controls. Differentiated treatment of MPS permitted to control the pain and to promote other clinical

The piriformis syndrome.

Piriformis syndrome is an often misdiagnosed cause of sciatica, leg, or buttock pain, and disability. The sciatic nerve may be compressed within the buttock by the piriformis muscle, with pain increased by muscular contraction, palpation, or prolonged sitting. A thorough medical history and physical examination are essential to proper diagnosis. Diagnostic testing may be used to differentiate piriformis syndrome from other causes of sciatica, lower extremity weakness, and pain. This article reviews the pathophysiology and management of piriformis syndrome.

Gastrocnemius muscle herniation at the knee causing peroneal nerve compression resembling sciatica.

STUDY DESIGN: This was a case report. OBJECTIVES: This is a case report of common peroneal nerve compression resembling sciatica caused by muscle herniation through the gastrocnemius muscle fascia in the popliteal area. SUMMARY OF BACKGROUND DATA: A review of the literature revealed various causes of nerve compression of the lower extremity that resembles symptoms of sciatica (pseudoradiculopathy). There is, however, no report of pseudoradiculopathy caused by gastrocnemius muscle herniation in the popliteal area. METHODS: The diagnosis of muscle herniation was confirmed during an explorator

Isolated femoral mononeuropathy to the vastus lateralis: EMG and MRI findings.

A 16-year-old female track athlete experienced sudden onset of right anterolateral thigh pain, initially thought to be cramping. After 2 months of continued postexercise pain she sought medical evaluation. A 3-cm thigh circumference discrepancy was noted on physical exam. She was referred for electromyography (EMG) and magnetic resonance imaging (MRI) studies. Femoral and screening nerve conduction studies were normal. Needle EMG revealed acute neuropathic changes confined to the right vastus lateralis only. These findings were confirmed by MRI, including short T1 inversion recovery (STIR) seq

Subject-based P1 latency inter-root comparison, a method to evaluate P1 latency in scalp recorded somatosensory evoked potentials obtained with sensory nerve stimulation in the lower extremities.

The purpose of the present study was to establish a method that allows the general use of subject-based criteria to evaluate P1 latency in scalp recorded somatosensory evoked potentials obtained with stimulation of the sural (S1), superficial peroneal (L5) and saphenous (L4) nerves bilaterally. The nerves were stimulated at the same distance from the registration electrode. Two groups of normal nerve roots were studied: (1) nerve roots on both sides in 20 asymptomatic volunteers, and (2) neuroradiologically normal nerve roots on the asymptomatic side in 22 patients with unilateral sciatica. Relations between upper limb soft tissue disorders and repetitive movements at work.

To make a preliminary assessment of whether upper limb soft tissue disorders might be associated with activities at work, we have conducted a case-control study of subjects attending orthopedic clinics in three cities. All subjects between the ages of 16 and 65 years, in whom defined soft tissue conditions of the upper limb were diagnosed by the participating orthopedic surgeons, were invited to take part. Controls were subjects attending the same clinics within the same age range whose clinical diagnosis did not include disease of the upper limb, cervical or thoracic spine. Information concer

Impaired sensory conduction in a mixed nerve induced by high frequency stimulation of the muscle fibres. An experimental study in cats.

To see if symptoms such as sensory disturbance or numbness worsen during exercise as is thought to occur in entrapment neuropathy, we studied the interaction between sensory and muscle nerve fibres in a mixed nerve in cats. Stimulation electrodes were placed on both the deep (muscle) and superficial (sensory) branch of the radial nerve at the elbow. High frequency stimulation at 100 Hz was applied to one branch for five minutes and the nerve action potential of the other branch was recorded at the axilla where the radial nerve is mixed. The stimulation decreased the amplitude of the nerve acti

[Persistent pain following discal sciatica: reflex sympathetic dystrophy, an unusual complication to be examined. Apropos of 4 cases].

Four patients who developed unilateral reflex sympathetic dystrophy of a lower limb associated with a bout of sciatica due to lumbar disc herniation are reported herein. In two cases, reflex sympathetic dystrophy developed after resolution of the sciatica, whereas the two conditions were concomitant in the two other patients. In all four patients, the diagnosis of reflex sympathetic dystrophy was missed. Surgery to release the root was considered in two cases. The reflex sympathetic dystrophy resolved within 1 to 3 months in every case. In view of the incidences of these two conditions, their

Somatosensory evoked potentials of the medial and lateral plantar and calcaneal nerves.

The ideal electrodiagnostic procedure to assess possible plantar neuropathies continues to elude investigators. Motor studies are rarely abnormal, pure sensory studies may be difficult to obtain, needle electromyography can demonstrate membrane instability in normal feet. Mixed nerve plantar studies may be more diagnostically valuable than the other techniques but they also have shortcomings. In this report, a technique utilizing somatosensory evoked potentials to assess the medial and lateral plantar and calcaneal nerves is demonstrated. Normative data with respect to latencies, amplitudes, a

The frequency and incidence of low back pain/sciatica in an urban population.

The frequency, incidence and severity of low back pain was assessed by a random telephone survey of 314 urban New Zealanders. Relationships between the severity and frequency of low back pain and referred lower extremity pain and other variables such as occupation, recreation, age, sex and predominant working posture was analysed. Point incidence was 17.5%, weekly incidence 33.4%, yearly incidence 63.7% and total incidence 79%. Some 28.3% get frequent minor episodes and 6.4% get frequent severe episodes of low back pain. Nearly 50% suffer the initial episode before the age of 30 years. Of thos

Nerve conduction studies and their clinical applications.

Nerve conduction studies provide the most sensitive and accurate account of peripheral neuropathophysiology. Though nerve conduction studies should never be performed in lieu of a clinical examination, the electrodiagnostic evaluation provides an optimal adjuvant to the clinical examination. Indications for nerve conduction studies are many, including evaluation of the nature of the pathophysiology, quantification of the severity of involvement, detection of the level of a neurologic deficit, and determining prognosis. Among other indications, limitations of the study should also be acknowledg

Referral diagnosis versus electroneurophysiological finding. Two years electroneuromyographic consultation in a rehabilitation clinic.

This study describes the referral practice to an electrodiagnostic unit in a rehabilitation clinic and compares the provisional diagnoses of different referring sources with the outcome of the neurophysiological tests. The study comprises data of 495 patients which were examined over a two years period. General practitioners, the author, physiatrists and surgeons, followed by occupational health physicians and neurologists used the electrodiagnostic service. Two thirds of the cases were referred to rule out upper limb mononeuropathies or cervical root and plexus neuropathies. Carpal tunnel syn

[An anatomical study of tarsal tunnel].

In order to understand the pathogenesis and improve the treatment of tarsal tunnel syndrome, we investigated the tarsal region anatomically on 62 feet of 31 cadavers and five freshly amputated feet. The following results were thus obtained: 1. The bifurcation into the medial and lateral plantar nerves mostly occurred within the flexor retinaculum. 2. The medial calcaneal branch showed many anatomical variations. 3. The flexor retinaculum was not clearly demarcated at its superior and inferior borders, and it was not as thick as previously thought. 4. The neurovascular bundle was separated from

[Acute and subacute leg and foot paralysis of radicular origin: clinical aspects, examination technique and differential diagnosis of root syndromes].

We describe symptoms and physical findings consistent with acute or subacute radicular lesions in the lower limb: radicular pain syndromes in the leg and the motor as well as sensory deficits of lumbar and sacral roots L2 to S1. We emphasize the importance of anterior thigh pain which is not sufficiently known, and we point out our neurological findings which do not correspond to descriptions in standard textbooks. Aids to the examination of these radicular syndromes are given, stressing the technique of examining motor signs. We also discuss the differential diagnosis to other, nonradicular s

Multiple spine surgical failures. The value of adjunctive psychological assessment.

The results of a retrospective 44-patient study of vocationally disabled patients who had undergone unsuccessful traditional lumbar spine surgical procedures for low back pain and/or lower extremity sciatic pain are presented. It was found that errors in patient selection and patient psychosocial pathology may have been contributory to the failures. If identified preoperatively, these conditions might have contraindicated one or more of the surgical procedures. The 24-month study required the completion of the Minnesota Multiphasic Personality Inventory by each patient; this test aids identifi

Computed tomography, electrodiagnostic and clinical findings in chronic workers' compensation patients with back and leg pain.

One hundred patients with complaints of low-back pain and leg pain, consistent with a diagnosis of sciatica, were evaluated. All patients had complaints for longer than 6 months and had recently undergone electrodiagnostic testing and computed tomography (CT). Correlation was made between symptoms, straight leg raising, clinical neurological deficits, electrodiagnostic and CT findings. The radiation of pain above or below the knee and pain on straight leg raising did not show a high correlation with each other or with neurological deficits or CT findings. Electrodiagnostic studies often define
