Regional nerve injury after intra-arterial chemotherapy.

Eleven patients at M.D. Anderson Hospital were referred for neurologic evaluation after having their internal or external iliac arteries catheterized for the treatment of localized pelvic or lower extremity tumors. Nine patients developed lumbosacral plexopathies and two patients, mononeuropathies. All symptoms occurred within 48 hours of the intra-arterial infusion. All patients received cis-dichlorodiammine-platinum (cisplatin; CDDP) intra-arterially, alone or in combination with other agents. Follow-up examinations revealed that only one patient had made partial recovery from the neurologic

The pyriformis syndrome: review and case presentation.

The pyriformis syndrome is a rare entrapment neuropathy in which the sciatic nerve is compromised by the pyriformis muscle or other local structures. It usually presents with sciatic pain and hypoaesthesia and a limp caused by weakness of the gluteus maximus muscle. Peroneal nerve innervated muscles and hamstrings are also usually affected. The diagnosis relies on the clinical presentation and the EMG findings. Other causes of symptoms should be excluded by careful examination, detailed x-ray studies of the lumbosacral spine (including myelogram), sacro-iliac and hip joints. A case of this syn

Orthodromic sensory nerve conduction of the medial and lateral plantar nerves. A standardization.

A standardized method of obtaining orthodromic distal sensory latencies of the medial and lateral plantar nerves is presented. Forty-one normal volunteer subjects were tested. Stimulation is carried out on the plantar aspect of the base of each toe using ring electrodes. Surface recording electrodes are placed posterior to the medial malleolus over the tarsal tunnel at points measuring 14, 16, 18 and 20 centimeters from the dorsum of the base of the great toe. Sixty-four evoked responses were averaged utilizing an electronic averaging technique and temperature was maintained at 31 degrees cent

Charles Lasègue and his 'Considerations on Sciatica'.

The eponym Lasègue sign has been applied to the increase in sciatic pain caused by flexing the extended lower extremity on the abdomen. The sign was never put into writing by Lasègue but by his pupils. He did not describe the test in the usual reference, "Considerations on Sciatica," in 1864. That article has to do with his analysis of then-current theories of sciatica and his own clinical observations. Sciatica was divided into a benign and a serious form, and two examples of each were described. Emphasis was laid on the constant, fixed sciatic pain, as contrasted with the irregular, largely

Peripheral polyneuropathy and monoclonal gammopathy of undetermined significance.

The prevalence of peripheral polyneuropathy in patients with monoclonal gammopathy is known to be higher than in the general population. A prevalence as high as that in the series of Osby et al, who found clinical and/or electrophysiological evidence compatible with peripheral polyneuropathy in 15 of 21 patients has not been reported before. These results could not be confirmed in a study in which 19 patients with benign monoclonal gammopathy were investigated. In contrast there were only two patients with questionable evidence of peripheral neuropathy: one had lower limb symptoms and signs on

Muscular hypertrophy after chronic radiculopathy.

In four patients, calf muscular hypertrophy developed after the onset of sciatica. Hypertrophic muscles were weak and showed electromyographic signs of denervation. In all cases, calf muscle biopsy showed striking hypertrophy of type 1 and, especially, type 2 muscle fibers. This hypertrophy was associated with other signs indicating a neurogenic lesion. Muscle hypertrophy is a rare finding in neurogenic lesions. Stretch and exercise of muscle are probably the causative factors.

Tibial nerve branching in the tarsal tunnel.

To provide an anatomical basis for diagnosis and treatment of the tarsal tunnel syndrome, the relationship of the tibial nerve to the tarsal tunnel was investigated in 31 feet of 20 cadavers. The bifurcation into medial and lateral plantar nerves occurred within 1 cm of the malleolar-calcaneal axis in 90% of the feet. Seven of 11 bilateral specimens were bilaterally symmetrical in the bifurcation location; three varied within 1 cm between sides; and in the fourth cadaver, one side bifurcated at 3 cm and the other at 5 cm proximal to the axis. The calcaneal nerve showed great variability; in se

[An unusual cause of sciatica: soft tissue desmoid tumor. Apropos of 2 cases].

The authors report 2 cases of sciatica resulting from nervous compression by a desmoid tumour in women aged 33 and 27 years. In each case, careful exeresis did not prevent recurrence as shown by tomodensitometry. Eight other well-documented cases were found in the literature. The desmoid tumour penetrates the buttock and thigh muscles and presses the sciatic nerve. The patients often suffer from previous and persistent sciatica which can combine with hip restriction. The responsible tumour made up of abundant bundles of collagenous fibers, a small number of fusiform cells without mitosis or at

Electromyographic evidence of inferior gluteal nerve compromise: an early representation of recurrent colorectal carcinoma.

Lumbosacral or primary buttock pain and cutaneous anesthesia in the distribution of the posterior femoral cutaneous nerve were the initial symptoms in 5 patients later proven to have extensions of previously resected colorectal malignancy. The initial electromyographic impression of an inferior gluteal nerve mononeuropathy associated with recurrent neoplasia was confirmed by a positive colon biopsy in 1 case and widespread pelvic metastasis demonstrated at laparotomy in the 4 remaining cases. Four of the 5 patients also had increased levels of carcinoembryonic antigen. Roentgenographs and othe

The tarsal tunnel syndrome in children.

There are few reports on the tarsal tunnel syndrome in children. This paper concerns 10 such children. In adults the syndrome is equally distributed among the sexes but all these children were girls. Trauma preceded the symptoms in only two cases. The symptoms differed in some aspects from those usually seen in adults. Six of the children walked with the affected foot in supination. Three of the six, and one other, used crutches at intervals. All were operated on and at follow-up nine were symptom-free and the tenth had improved.

Postradiation ureteral obstruction: a reappraisal.

Thirty-four patients who received radiation therapy for carcinoma of the cervix developed hydronephrosis. Twelve of 34 (35%) had no evidence of pelvic malignancy, and the obstruction was caused by periureteral fibrosis. The incidence of obstructive uropathy due to periureteral fibrosis not associated with recurrent tumor increased when the obstructing lesion was unilateral, the clinical staging of the cervical carcinoma prior to therapy was Stage IB or Stage II rather than Stage III or Stage IV, and the obstructing lesion appeared 2 or more years after the completion of radiation therapy. The

Crossed leg palsy; with report of a recurrent case.

A form of peroneal palsy may be caused by crossing the legs. Two physical factors-pressure and tension - are the basic causes, although other factors may be contributory. Direct pressure is applied by the bones of the two legs, compressing the peroneal nerve between them at its superficial part near the head and neck of the fibula. The palsy may be overlooked as an integral part of a widespread disorder so that careful evaluation and observation of the patient's habits are required. Detection becomes especially difficult when the palsy is bilateral, for then the lesion by virtue of its symmetr
