Conditioning Electrical Stimulation for Patients with Moderate or Severe Carpal Tunnel Syndrome: Double Blinded Randomized Controlled Trial.

OBJECTIVE: Carpal tunnel syndrome (CTS) can drastically impair one's ability to work and interferes with activities of daily living. We recently demonstrated that, in rodents, conditioning electrical stimulation (CES) delivered to the nerve 7 days prior to surgery imparts a conditioning lesion-like effect by accelerating the rate of regeneration along the entire length of the nerve. The goal of this study is to test the hypothesis that CES could accelerate nerve regeneration and improve function in patients with moderate or severe CTS. METHODS: Using a double-blind randomized controlled study

Clinical Role of Corticosteroids Versus Platelet Rich Plasma Injections in the Management of Carpal Tunnel Syndrome: a Narrative Review.

PURPOSE OF REVIEW: This review seeks to assess and compare the mechanisms, effectiveness, safety, and clinical applications of corticosteroid and platelet-rich plasma (PRP) injections in managing Carpal Tunnel Syndrome (CTS). RECENT FINDINGS: Recently, PRP therapy has gained attention as a promising alternative, offering a regenerative approach. PRP, which contains autologous growth factors, supports tissue repair and cellular regeneration, potentially enabling long-term recovery. While corticosteroids offer rapid symptom relief, repeated use carries risks, including tendon degeneration and sy

Combined Melorheostosis and Osteopoikilosis: Uncommon Presentation with Sciatic Nerve Neuropathy: A Case Report.

CASE: A 27-year-old man presented with an 8-year history of chronic right gluteal and posterior thigh pain, worsened by sitting and unresponsive to conservative treatment. Imaging revealed widespread melorheostosis and osteopoikilosis of the right lower extremity, with 2 gluteal soft tissue masses compressing the sciatic nerve. The masses were excised using a dual surgical approach, achieving successful sciatic nerve decompression. Postoperatively, the patient regained normal sitting and sleeping tolerance, with complete pain relief maintained over a 15-month follow-up. CONCLUSION: Melorheost

Role of Alpha-Lipoic Acid as a Co-adjuvant Treatment with Surgical Decompression of Carpal Tunnel Syndrome: A Retrospective Study.

Introduction: Surgical decompression remains the gold standard for managing severe carpal tunnel syndrome (CTS). However, perioperative pain and incomplete symptom resolution persist in some patients. Alpha-lipoic acid (ALA), a neuroprotective antioxidant, has shown potential benefits in neuropathic conditions. This retrospective study evaluates the clinical and electrophysiological effects of ALA as an adjunctive therapy in CTS management. Methods: We retrospectively analysed 164 patients with electrodiagnostically confirmed CTS who underwent open surgical decompression. Patients were divided

Comparison of Immunohistochemical and Histopathological Findings between Patients with Diabetic and Idiopathic Carpal Tunnel Syndrome.

Background: Carpal tunnel syndrome (CTS) is the most common nerve entrapment syndrome, caused by median nerve compression at the wrist. Its aetiology remains unclear, with idiopathic and diabetic-related forms. Diabetic patients (type 1 and 2) have a higher CTS risk, likely due to oedema, neo-angiogenesis and increased non-inflammatory fibrosis in the sub-synovial connective tissue (SSCT). Vascular endothelial growth factor (VEGF) A plays a key role in vascular permeability and angiogenesis, particularly in diabetic CTS. This study compared histopathological differences and VEGF expression in

Utilizing an ultrasound-guided nerve block for management of sciatica in the ED: A national simulation-based cost savings analysis.

OBJECTIVES: Acute sciatica is a frequent cause of emergency department (ED) visits and hospital admissions. We evaluated the potential national cost savings of using ultrasound-guided transgluteal sciatic nerve block (TGSNB) in patients with acute sciatica who would otherwise be admitted. METHODS: We performed a Monte Carlo simulation with 10,000 iterations to compare the costs of usual care versus TGSNB targeted to patients who would otherwise require admission. Model inputs included national ED visits for acute sciatica, pre-block admission rates, admission costs, and procedural costs. The

Short-term symptom aggravation after cubital tunnel decompression: Clinical features and mechanistic insights from a retrospective cohort study.

While surgical decompression for cubital tunnel syndrome (CuTS) generally leads to favorable outcomes, some patients experience early postoperative worsening marked by transient symptom flare-ups. The underlying mechanisms remain poorly understood. We retrospectively analyzed 127 patients who underwent anterior ulnar nerve transposition without corticosteroids. Clinical scores (VAS, PRUNE, two-point discrimination) and electrophysiological parameters (CMAP, SNCV, MNCV) were assessed preoperatively, at 2 weeks, and at 6 months. Early worsening was defined as deterioration in any clinical indica

The sciatic gliding space.

BACKGROUND: This study investigates the connective tissue structures that enable the sciatic nerve to glide through the deep gluteal and upper limb area. Characterizing the gliding space may improve the understanding of non-discogenic sciatica and support clinical approaches to its treatment. METHODS: Thirty limbs of alcohol- and Thiel-fixed specimens (21 female, 9 male; mean age 85.2 years) were examined using macroscopic dissection. Additional visualization was achieved through methacrylate corrosion casts. Ultrasound examinations were performed on specimens and two healthy volunteers to as

Early Diagnosis of ATTR-CM Using Carpal Tunnel Biopsy Examination: EDUCATE: A United Kingdom Prospective Multicenter Study.

BACKGROUND: Transthyretin amyloid cardiomyopathy (ATTR-CM) is an increasingly recognized but still underdiagnosed cause of heart failure in older people, typically causing substantial and irreversible cardiac damage before diagnosis. Specific therapies have lately become available that slow disease progression, early diagnosis now being a major priority to improving outcomes. Amyloid-associated carpal tunnel syndrome commonly precedes symptomatic cardiomyopathy, but histological examination at surgical decompression is seldom sought. This multicenter prospective study was conducted to determin

Surgically Oriented Ultrasound-Based AI of Median Nerve Morphology as a Decision Support for Carpal Tunnel Release: A Calibrated ConvNeXt-CBAM Framework With Supervised Contrastive Warm-Up.

AIM: Carpal tunnel syndrome (CTS) is a compressive neuropathy commonly encountered in hand surgery, and decisions regarding whether operative decompression should be conducted rely on accuracy and clinical interpretability of imaging findings. Ultrasound is commonly used to directly visualize the median nerve in this condition, but remains operator-dependent and is limited by inconsistent diagnostic thresholds. This study aimed to develop a surgically oriented ultrasound-based artificial intelligence (AI) model that provides strong discriminative power, reliable probability calibration for pre

Chronic constriction of the sciatic nerve does not induce mood-related comorbidities in female mice.

BACKGROUND: Neuropathic pain affects around 7-8% of the population and is associated with a high level of lifetime anxio-depressive comorbidities. Despite the higher incidence of the pathology in women, preclinical studies using rodents almost exclusively focused on males so far. METHODS: Sciatic nerve cuffing is a well-characterized model of neuropathy in which anxio-depressive comorbidities develop over time in male mice. In order to examine potential sex-differences in this model, we compared the alterations in the mechanical sensitivity using von Frey testing and used three ethologically
