In Patients with Dysimmune Motor and Sensorimotor Mononeuropathies, the Degree of Nerve Swelling Correlates with Clinical and Electrodiagnostic Findings.

In non-vasculitic immune-mediated neuropathies, imaging studies demonstrate an enlargement not only of clinically involved but also of clinically intact nerves. The present study aimed to present a pattern of nerve swelling and its relation to nerve function. In a group of patients with dysimmune motor and sensorimotor mononeuropathies, nerve cross-sectional areas (CSAs) were measured using ultrasonography (US) and compound muscle action potential (CMAP) amplitudes using electrodiagnostic (EDx) studies. Nerve CSAs were compared in (1) clinically involved, (2) swollen and clinically uninvolved,

The Diagnostic and Prognostic Role of Ultrasonography in Cubital Tunnel Syndrome: Results on a Consecutive Series of 100 Patients.

BACKGROUND AND OBJECTIVES: High-resolution ultrasonography (US) has become increasingly popular in cubital tunnel syndrome (CubTS). In this article, we confirmed the diagnostic role of US in a large and homogeneous cohort of patients. Our primary aim was to analyze the predictive role of US findings. By systematically re-evaluating neurosonographic abnormalities over time, we also sought to assess the role of US during follow-up. METHODS: Patients diagnosed with CubTS based on clinical, electrophysiological, and sonography findings and operated on for in situ decompression of the ulnar nerve

Distal Nerve Transfer for Refractory Ulnar Neuropathy After a Pediatric Elbow Fracture: Expanding the Role of AIN-to-Ulnar SETS.

Medial epicondyle fractures account for 11% to 20% of pediatric elbow fractures. While most are managed conservatively, ulnar nerve palsy, present in up to 16% of cases, is an accepted indication for surgical treatment. Neurologic symptoms may begin in a delayed fashion and, in rare cases, persist or progress despite surgical intervention. In adults, distal nerve transfers such as anterior interosseous nerve (AIN) to ulnar motor branch transfer have demonstrated promising outcomes, but their role in pediatric patients remains limited. We report the case of a 13-year-old, right-handed gymnast w

Risk of new-onset glaucoma in people with carpal tunnel syndrome: a global-federated, multicenter retrospective cohort study.

Background: Carpal tunnel syndrome (CTS) is a common entrapment neuropathy involving chronic inflammation, while glaucoma is an optic neuropathy linked to neuroinflammation and vascular insufficiency. Shared pathogenic mechanisms have been hypothesized, but large-scale epidemiologic evidence is lacking. This study aims to evaluate whether CTS patients present an elevated risk of glaucoma compared to CTS-free individuals. Materials and Methods: We conducted a retrospective cohort study using the TriNetX global research network. Adults diagnosed with CTS were matched 1:1 with CTS-free controls b

End-to-side anterior interosseous nerve transfer augmentation of ulnar nerve function in moderate and severe cubital tunnel syndrome; a randomised controlled trial protocol.

BACKGROUND: The anterior interosseous nerve (AIN) end-to-side (ETS) nerve transfer is a relatively recent treatment innovation in managing cubital tunnel syndrome (CuTS). Potential benefits must be balanced against the risks associated with the adoption of an innovative technique, evaluating procedure-specific risks, and considering additional costs in the safe delivery of care. The ETS technique has been proposed as a method of improving intrinsic muscle function in the hand in the setting of ulnar nerve compression at the elbow. The technique is controversial with emerging evidence from retr

Distal Nerve Transfers for Foot Drop: A Systematic Review and Meta-Analysis.

BACKGROUND: Foot drop from peroneal nerve palsy causes steppage gait and functional disability. Tendon transfer is reliable but sacrifices native biomechanics. Distal nerve transfer (DNT) reinnervates dorsiflexors, potentially restoring more physiologic function. This study aimed to systematically review and meta-analyze the clinical outcomes of DNT for foot drop and to identify key prognostic and technical factors that inform contemporary reconstructive decision-making. METHODS: Following PRISMA, we searched MEDLINE, EMBASE, and Cochrane (through September 2025) for clinical studies of DNT t

Radiologic and pathologic evaluation for a large schwannoma of the sciatic nerve: A neglected cause of sciatica.

Schwannomas are benign tumors originating from the peripheral nerve sheath. Although they rarely involve the sciatic nerve, their presence may result in sciatica, a condition characterized by pain radiating along the course of the sciatic nerve from the buttock or gluteal region. Despite being an uncommon cause of sciatica, sciatic nerve schwannomas present diagnostic challenges and management complexities. Their occurrence within the sciatic nerve is frequently overlooked, underscoring the need for accurate diagnosis and individualized therapeutic strategies. Surgical intervention is the prim

Differences in Diagnosing Tarsal Tunnel Syndrome Across the Literature: A Systematic Review and a Call for Standardization.

BACKGROUND: The lack of a gold standard in tarsal tunnel syndrome (TTS) diagnosis leads to diagnostic inconsistencies and variation in patient selection for treatment. Therefore, the aim of this review is to summarize the diagnostic criteria used in current studies on TTS based upon this best-evidence synthesis. METHODS: Three databases were searched to identify all studies on TTS. Studies were included when they included (1) diagnosis or treatment of TTS as the primary focus, (2) a description of the diagnosis of TTS, (3) an original data set of TTS cases, and (4) a minimum of 10 adult patie

Sciatic nerve compression as a cause of severe chronic pain after total knee replacement: a case report.

BACKGROUND: Persistent pain following total knee replacemnt (TKR) remains a challenging and often multifactorial issue. While common causes include infection, malalignment, or complex regional pain syndrome, less typical origins are easily overlooked. We report a unique case of chronic, disproportionate posterior thigh pain 1 year after TKR, caused by a complete proximal hamstring rupture leading to symptomatic sciatic nerve compression. To our knowledge, this is the first published case describing this specific postoperative complication and its successful surgical management. CASE PRESENTAT
