A transgenic rat expressing green fluorescent protein (GFP) in peripheral nerves provides a new hindlimb model for the study of nerve injury and regeneration.

BACKGROUND: In order to evaluate nerve regeneration in clinically relevant hindlimb surgical paradigms not feasible in fluorescent mice models, we developed a rat that expresses green fluorescent protein (GFP) in neural tissue. METHODS: Transgenic Sprague-Dawley rat lines were created using pronuclear injection of a transgene expressing GFP under the control of the thy1 gene. Nerves were imaged under fluorescence microscopy and muscles were imaged with confocal microscopy to determine GFP expression following sciatic nerve crush, transection and direct suturing, and transection followed by re

[Peroneal nerve palsy in patient with Ehlers-Danlos syndrome].

Ehlers-Danlos syndrome (EDS) is a hereditary generalized connective tissue disorder characterized by skin hyperextensibility, joint hypermobility and tissue fragility. Peripheral neuropathy is described sporadically. Although the exact mechanism of the neuropathy is not well-known, excessive stretch and abnormal connective tissue support are the suggested causative agents. To our knowledge, total peroneal nerve palsy occurring in conjunction with a growth spurt has not previously been described. We describe the case of a young male patient, who developed a total unilateral peroneal palsy durin

Femoral nerve palsy in Pavlik harness treatment for developmental dysplasia of the hip.

BACKGROUND: Transient femoral nerve palsy is a potential complication of the use of a Pavlik harness to treat developmental dysplasia of the hip. Our hypothesis was that patients who develop a femoral nerve palsy while undergoing Pavlik harness treatment for developmental dysplasia of the hip are more likely to have unsuccessful orthotic treatment and to require closed or open hip reduction. METHODS: We performed a retrospective review of all patients who underwent Pavlik harness treatment for developmental dysplasia of the hip within a seventeen-year period (1992 to 2008). All cases of femor

Is Velband still a safe and cost effective skin protection beneath the tourniquet in hand surgery?

Skin complications have been reported to be associated with tourniquet use. The UK Medical Devices Agency (MDA) recommends the use of a limb drape to act as underpadding to reduce these complications. Our hospital had traditionally used Velbands as an underpadding, but changed to limb drapes on the advice of the MDA. The purpose of this study is to establish whether Velbands are still a safe and cost-effective tourniquet underpadding. A prospective clinical study was performed on consecutive patients undergoing hand surgery over a four-month period. Patients were randomly allocated into one of

Insulin resistance increases risk of carpal tunnel syndrome: a case-control study.

Carpal tunnel syndrome (CTS) is one of the most common upper limb compression neuropathies. In only 50% of cases it is possible to identify a cause. Our objective was to determine the role of glucose metabolism abnormalities in idiopathic CTS. We identified 117 patients with idiopathic moderate or severe CTS and 128 controls. In all we evaluated glucose and insulin levels at fasting and after 2-h oral glucose tolerance test (2h-OGTT). In addition we determined insulin resistance (IR). Following OGTT the prevalence of glucose metabolism abnormalities was significantly higher in the CTS group (p

Acute paediatric compartment syndrome of the hand caused by hereditary angiooedema.

Compartment syndrome of the leg and forearm are well described in the literature. However, compartment syndrome of the hand is rare and in children it is even rarer. Atraumatic hand compartment syndrome has not to our knowledge been previously reported. We describe a case of an atraumatic compartment syndrome of the hand in a child who underwent an urgent fasciotomy. The child was diagnosed with hereditary angiooedema. We highlight a rare but serious complication of a hereditary disease not commonly seen by the surgical community. We hope that this report raises the awareness of this condition

Cross-cultural adaptation and validation of the Korean version of the Michigan hand questionnaire.

PURPOSE: The Michigan hand questionnaire (MHQ) is increasingly being used and has been adapted cross-culturally in some Western and Asian countries, but the validation process for an Asian translation of MHQ has not been well described. In this study, we translated and adapted the original MHQ cross-culturally to produce a Korean version, and then assessed the validity and reliability of the Korean version of the MHQ. METHODS: A total of 176 patients with common hand disorders completed the Korean version of the MHQ and the Disabilities of the Arm, Shoulder, and Hand questionnaire. We include

Treatment of high-energy supracondylar/intercondylar fractures of the distal humerus.

Operative treatment of high-energy intra-articular fractures of the distal humerus is challenging and controversial. We conducted a retrospective study at a Level I trauma center to evaluate the results of a specific treatment protocol. Seventy-one patients were treated through a posterior olecranon osteotomy. Capsulectomy was performed in patients who perceived their functional range of motion to be limited after an average of 10~months. Clinical followup consisted of physical examination, radiography, and completion of the Disabilities of the Arm, Shoulder and Hand (DASH) questionnaire. Aver

δ-Aminolevulinic acid dehydratase genotype predicts toxic effects of lead on workers' peripheral nervous system.

There is a wide variation in sensitivity to lead (Pb) exposure, which may be due to genetic susceptibility towards Pb. We investigated whether a polymorphism (rs1800435) in the δ-aminolevulinic acid dehydratase (ALAD) gene affected the toxicokinetics and toxicodynamics of Pb. Among 461 Chinese Pb-exposed storage battery and 175 unexposed workers, allele frequencies for the ALAD1 and ALAD2 alleles were 0.968 and 0.032, respectively. The Pb-exposed workers had a higher fraction of the ALAD1-2/2-2 genotype than unexposed workers (7.8% vs. 2.3%, p=0.01). The Pb levels in blood (B-Pb) and urine (U-

The six-item CTS symptoms scale and palmar pain scale in carpal tunnel syndrome.

PURPOSE: To evaluate measurement properties of 2 brief outcome measures for carpal tunnel syndrome: the 6-item carpal tunnel symptoms scale (CTS-6) and the 2-item palmar pain scale (measuring severity of pain in the scar/palm and pain-related activity limitation). Our hypothesis was that the CTS-6 is responsive to change in symptoms after surgical treatment and the pain scale is a valid measure of surgery-related pain. METHODS: This study followed 447 consecutive patients with carpal tunnel syndrome undergoing open release; 308 completed the CTS-6 and the Disabilities of the Arm, Shoulder, an

The effect of dividing muscles superficial to the transverse carpal ligament on carpal tunnel release outcomes.

PURPOSE: To test the hypothesis that division of muscle fibers lying over or within the transverse carpal ligament (TCL) in an open carpal tunnel release does not have an effect on outcomes in patients with carpal tunnel syndrome (CTS). METHODS: A total of 152 patients with a mean age of 57 years (range, 31-83 y) diagnosed with CTS were enrolled for intraoperative observation of the muscles overlying or within the TCL as seen through a 3-cm incision. These muscles when present were incised layer by layer in line with division of the TCL. Patients were divided into 3 groups according to the ex

Ipsilateral, cabled sural nerve for a sciatic nerve defect: an experimental model in the rat.

The 10 mm rat sciatic nerve defect model is commonly used to investigate new strategies to improve functional recovery with segmental nerve defects. However, a lack of standardization makes comparisons between studies difficult. The present study aims to evaluate a standardized experimental model that can minimize the number of animals required for obtaining valid results and simulates a current treatment for human peripheral nerve injury defects. Eighteen cadaveric Sprague-Dawley rats were utilized in the anatomic arm of the study and 18 living Sprague-Dawley rats were used in the experimenta

The basis for diminished functional recovery after delayed peripheral nerve repair.

The postsurgical period during which neurons remain without target connections (chronic axotomy) and distal nerve stumps and target muscles are denervated (chronic denervation) deleteriously affects functional recovery. An autologous nerve graft and cross-suture paradigm in Sprague Dawley rats was used to systematically and independently control time of motoneuron axotomy, denervation of distal nerve sheaths, and muscle denervation to determine relative contributions of each factor to recovery failure. Tibial (TIB) nerve was cross-sutured to common peroneal (CP) nerve via a contralateral 15 mm
