The Rate and Timing of Revision Carpal Tunnel Release with Long-Term Follow-Up.

PURPOSE: Although carpal tunnel release (CTR) is the most common upper-extremity orthopedic procedure in the United States, revision surgery can be indicated when symptoms of carpal tunnel syndrome persist, recur, or worsen after primary surgery. Understanding the revision surgery profile after CTR is important for preoperative counseling and treatment decision-making, yet published rates of revision surgery vary widely. In our study, we aimed to establish the rate of revision CTR and describe temporal patterns of revisions after primary surgery for patients with long-term follow-up at our ins

Influence of ginger root extract supplementation on the microbiota-gut-brain axis in individuals with sciatica: Study protocol for a double-blind, placebo-controlled randomized trial.

Neuropathic pain (NP) is caused by damage to the peripheral or central nervous system and is associated with adverse complex sensory and affective symptoms. There are few current treatment options for NP, and opioid analgesics have severe side effects which can lead to opioid abuse. Therefore, the development of innovative, effective, and safe alternatives is urgently needed. This study will assess the effects of ginger root extract's anti-inflammatory and anti-oxidant properties on individuals with sciatica via the microbiome-gut-brain axis. Eighty participants (18-85 years) with chronic scia

Anatomical variations in the relationship between the sciatic nerve and the piriformis muscle: clinical implications.

OBJECTIVES: This study aims to contribute additional comparative anatomical and morphometric data to identify anatomical variations of the sciatic nerve (SN) and its relationship with the piriformis muscle (PM), clarify sex-related differences, and attempt to propose anatomical features relevant to understanding piriformis muscle syndrome. METHODS: Dissection of the deep gluteal region was performed on 80 specimens from 40 donors of both sexes. The frequency of SN anatomical variations in relation to the PM was recorded according to Beaton and Anson’s classification and analyzed by sex. Variou

An Acta Orthopaedica educational article: Treatment of adult patients with a humeral shaft fracture.

This educational review outlines the core principles of humeral shaft fracture (HSF) management and is designed for orthopedic trainees, general orthopedic surgeons, emergency physicians, and allied health professionals who participate in the acute or postoperative care of patients with HSFs. The content integrates the authors' expert opinion with the current evidence. Humeral shaft fractures account for 1-3% of adult fractures, most often resulting from low-energy falls in older adults or high-energy trauma in younger patients. Although open fractures and neurovascular injuries are rare, prim

Increased risk of cardiovascular diseases among patients with carpal tunnel syndrome in a multicenter global retrospective cohort study.

Carpal tunnel syndrome (CTS) is linked to cardiovascular outcomes such as cardiac amyloidosis, but prior studies with limited scope leave uncertainty about its role as an marker of other major cardiovascular diseases such as stroke and myocardial infarction. The aim of this study is to evaluate cardiovascular risks in CTS using large-scale global electronic health records. We conducted a retrospective cohort study using TriNetX Global Collaborative Network from a cohort of 619,538 patients with CTS and 8,630,919 CTS-free controls. To minimize confounding, a 1:1 propensity-score matching strate

Diagnostic utility of ultra-microangiography and shear wave elastography in pediatric carpal tunnel syndrome associated with mucopolysaccharidosis.

BACKGROUND: Carpal tunnel syndrome is rare in the paediatric population but is frequently associated with mucopolysaccharidosis. Owing to nonspecific symptoms and limited reliability of clinical examination, early diagnosis of carpal tunnel syndrome in patients with mucopolysaccharidosis remains challenging. Advanced ultrasonographic techniques may enable objective and early detection. OBJECTIVE: To evaluate the diagnostic performance of ultra-microangiography and shear wave elastography for the early detection of carpal tunnel syndrome in pediatric patients with mucopolysaccharidosis and to

Cannabis use disorder is associated with increased early postoperative opioid use and pain but no long-term differences after arthroscopic rotator cuff repair: a retrospective cohort study using TriNetX.

BACKGROUND: Cannabis use has become increasingly common in the general population, with cannabis use disorder (CUD) defined as clinically significant impairment and/or distress related to cannabis consumption. Prior studies have associated CUD with increased postoperative pain and opioid use, but its impact following arthroscopic rotator cuff repair (RCR) remains unclear. This study aimed to compare short- and long-term surgical outcomes following RCR between patients with and without CUD. METHODS: Using the TriNetX database, we conducted a retrospective cohort study of patients who underwent

Wrist Splinting versus a Placebo Soft Bandage for Carpal Tunnel Syndrome.

BACKGROUND: Wrist splinting is a common treatment for carpal tunnel syndrome (CTS), but there is little evidence supporting its efficacy. METHODS: In this parallel-group clinical trial, patients with primary idiopathic CTS with symptoms for at least 1 month and no previous treatment were randomly assigned to wear either a rigid wrist splint or placebo soft bandage (allowed full wrist motion) for 6 weeks. If symptoms persisted after 6 weeks, treatment was continued for 4 more weeks. If symptoms persisted after 12 weeks, surgery was considered. Co-primary outcomes were change in the six-item CT

Impact of Physiotherapy Intervention on Pain, Quality of Life, and Function in Low Back Pain Associated With Piriformis Syndrome: Protocol for Systematic Review.

BACKGROUND: Piriformis syndrome is a neuromuscular condition with hip and buttock pain and other symptoms, including referred pain towards the lower back and leg and radiating towards the foot's medial aspect. Similarly, low back pain caused by piriformis syndrome is undetected or difficult to diagnose because of similar symptoms of lumbar disc herniation, lumbar stenosis, or radiculopathy, as well as neurogenic pain. A study conducted in 2013 found 2910 patients experienced low back pain with sciatica, which is the most common cause of low back pain, because of piriformis muscle stiffness. Th

MRI Assessment of Median Nerve Size in Patients with Proximate Electrodiagnostic Studies.

BACKGROUND: Carpal tunnel syndrome (CTS) diagnosis has traditionally relied on electrodiagnosis (EDX) to confirm the diagnosis and to assess severity. Ultrasound has shown potential in measuring median nerve cross-sectional area (CSA) for CTS diagnosis, and magnetic resonance imaging (MRI) can be used for wrist soft-tissue evaluation. This study explored the correlation between CTS diagnosis and median nerve CSA measured on MRI at different wrist levels. METHODS: A retrospective review of an electronic medical record database identified patients who underwent both wrist MRI and EDX within a 9

More than just sciatica. Clinical presentation, surgical treatment and functional outcomes of sciatic nerve schwannomas.

Introduction: Peripheral nerve schwannomas are rare benign tumors. The onset of a schwannoma in the sciatic nerve is uncommon and can impair the nerve’s functionality. To this date, the literature on this topic is mainly composed of case reports, lacking large case series. Aim: To overcome the limitations of previous literature, we performed a retrospective study on surgical enucleation of sciatic nerve schwannomas, evaluating surgical treatment’s clinical and functional effectiveness. Materials and methods: We examined retrospectively all cases of sciatic nerve schwannomas that underwent surg

Preoperative tests before cubital tunnel decompression in 2025.

Despite the burden to patients and society, there are no diagnostic criteria for cubital tunnel syndrome. Our survey shows that 93% of surgeons order preoperative tests (mostly electrodiagnostics only, which is discordant with expert consensus). Moreover, six in 10 still operate when test results are normal. The diagnostic pathway for cubital tunnel syndrome must be standardized.Level of evidence: IV.

CCS-mediated mechanistic link between gestational diabetes mellitus and carpal tunnel syndrome: a multi-omics MR framework.

BACKGROUND: Carpal tunnel syndrome (CTS) is a common condition in pregnancy, yet reliable tools for identifying women at high risk-particularly those with gestational diabetes mellitus (GDM)-remain lacking. Although GDM shares metabolic features with type 2 diabetes, a recognised CTS risk factor, whether GDM itself causally increases CTS risk and through which molecular pathways has not been established. METHODS: We used linkage disequilibrium score regression and two-sample Mendelian randomization across FinnGen and UK Biobank to evaluate the genetic correlation and causal effect of GDM on C

Return-to-Work After Carpal Tunnel Release Across Surgical Techniques: A Narrative Review.

Carpal Tunnel Syndrome (CTS) is the most common upper-limb entrapment neuropathy and remains a major contributor of work-related disability. While Carpal Tunnel Release (CTR) reliably improves symptoms, functional recovery is less consistently reported. Return-to-Work (RTW) has emerged as a practical functional outcome, but definitions and reporting remain heterogeneous. We conducted a narrative review of English-language studies reporting RTW or work-absence outcomes following open, endoscopic, ultrasound-guided, ultraminimally invasive, and microinvasive CTR techniques. Due to variability in

Selenium attenuates paclitaxel-induced peripheral neuropathy by suppressing oxidative stress, inflammation, and apoptosis in rat sciatic nerve.

Paclitaxel (PTX) is a potent taxane widely used in the treatment of solid tumors and can cause dose-limiting peripheral neuropathy. This study evaluated the therapeutic potential of selenium in a paclitaxel-induced peripheral neuropathy model. A total of 30 male Sprague-Dawley rats were divided into five groups (n = 6): Control, SE1, PTX, PTX+SE0.5, and PTX+SE1. PTX (2 mg/kg, i.p., days 1-5) was administered followed by SE (0.5 or 1 mg/kg, i.g., days 6-15); sciatic nerve tissues were analyzed on day 16. In addition to molecular and histopathological analyses, behavioral assessments were perfor
